Angioplasty for the treatment of buttock claudication caused by internal iliac artery stenoses.

A case of buttock claudication due to isolated internal iliac artery stenoses is presented. Although ankle systolic pressure at rest was within normal limits and distal pulses were palpable, an angiogram demonstrated severe stenoses. A computed tomography scan showed no spinal stenosis. The patient was successfully treated with angioplasty. This diagnosis may be elusive if ankle pressure or distal pulses are normal, thereby directing the clinician's suspicion away from vascular pathology.

Late-onset fistula presenting as buttock abscess in two patients with ileo-anal J-pouches for ulcerative colitis.

Two cases are described of patients who presented with a buttock abscess 3 and 5 years after restorative proctocolectomy for ulcerative colitis. In both the abscess was secondary to a fistula, which was shown on further investigation to be a track traversing the greater sciatic notch. Both patients had had an ileo-anal J-pouch after a proctectomy which had included a mesorectal excision. The possibility that these unusual fistulae might be more common following removal of the mesorectum rather than a close rectal dissection is explored.

The value of prostaglandin E1 and selective angiography in the territorial definition of axial pattern flaps of the buttock.

The definition of the cutaneous vascular territory in axial pattern flaps has been studied by intravital injection of dye or fluorescein, or by post mortem dissections and injection. This paper illustrates the use of selective angiography and the intra-arterial injection of Prostaglandin E1 in the definition of axial pattern flaps in the buttock. The technique is safe, with no side effects and can be repeated. Its clinical application in the discovery and selection of axial pattern flaps is obvious.

Stress distribution in a physical buttock model: effect of simulated bone geometry.

Mechanical stresses developed in the tissue during sitting or reclining could cause bedsores in paralyzed individuals. Cushions are usually prescribed to redistribute the stresses. Two two-dimensional physical models of the buttock were developed and used to study whether the stress distribution is different with round- and flat-base bone core geometries and to find out whether the relative cushion responses are dependent on loading direction and bone core geometries. In these models, PVC gel simulated the soft tissue and a wooden core simulated the bony prominence. One model had a round-base core and the other had a flat-base bone core. A grid etched on the model allowed strain measurements, and stress calculations. The sharp-base bone core model generated large regions of high shear stress during vertical and inclined loading. However, the round-base core produced maximum compressive stress during vertical loading. The relative cushion responses were dependent on bone core geometry and loading direction.

Leiomyosarcoma of the buttock.

A 56-year-old man had a deeply invasive leiomyosarcoma of the right buttock. Leiomyosarcomas of the skin and subcutaneous tissues are rare, and most occur on the extremities. Wide local excision is the therapy of choice.

Buttock and back pressure distribution tests on seats of mobile agricultural machinery.

Multidisciplinary effort is usually required in the evaluation of comfort problems in the working environment of mobile agricultural machinery workers. Comfort can be partly assessed from the study of the pressure distribution of the human-seat interface. Four combine foam seats and a new air-based seat were compared with regards to static buttocks and back support pressures. Within and between subject variability and the issue of measurement repeatability were addressed. Significant differences were found in the maximum pressure profiles of the four seating systems with seats 2 and 3 performing better than seat I (seat 4 showed no significant difference when compared to the other seats). There is an almost linear relationship between mean pressure and body mass index. In all cases the air seating system performed better with regards to the static pressure gradients.

Isolated buttock burns: epidemiology and management.

This is a retrospective study of the epidemiology and management of isolated buttock burns presenting to the Welsh Regional Burns Centre from January 1996 to December 1999. A total of 36 cases have been treated of which 31 are included in this study. Approximately, 50% are in the paediatric age group and the sex distribution is equal for both adults and children. Contact burns form the largest group, and in children resulted in superficial burns requiring dressings only. The adult population is more likely to sustain deeper burns that require skin grafting, and approximately 50% will have a contributing premorbid condition. Despite difficulties in dressing and positioning of the patients, grafting of full thickness burns is appropriate without recourse to faecal diversion.

[Misleading "hip" or buttock pain: proximal arteritis or lumbar spinal stenosis?].

Proximal arterial stenosis (bifurcation of the aortic, common iliac and especially hypogastric or hip arteries) can cause misleading 'hip' or buttock pain (two thirds of these cases involve the area of the trochanter, groin, or thigh). Their essential characteristic is that they cause patients to stop walking, improve in a minute or two and reappear when the patient starts walking again. Neither articular complications nor periarthritis of the hip fit this pattern, and physical examination and imaging exclude them. Neurogenic intermittent claudication is more difficult to rule out: it is 'often accompanied by paresthesia and anteflexion of the trunk to relieve the pain. A radiologic image of lumbar canal stenosis alone does not indicate that it is responsible for the pain: the stenosis may be asymptomatic and only fortuitously associated with the pain. The usual reduction in femoral pulse or ankle pressure index is absent if the stenosis is located on the hypogastric or gluteal artery and there is not substantial damage to the aorta-iliac axis. Arterial stenosis is confirmed by Doppler ultrasound. Arteriography is an examination performed during treatment, to guide angioplasty. It is indicated when medical treatment fails, as it frequently does.

Compartment syndrome of the buttock following a total hip arthroplasty.

An elevation of the interstitial pressure in a closed osseofascial compartment results in compartment syndrome. This may be caused by a decrease in compartment volume or an increase in compartment contents. The gluteal compartment syndrome is rare. Because the gluteal region has a large volume, this compartment requires a massive increase in content to cause a compartment syndrome. Also, this compartment blends anatomically with the muscles of the thigh, allowing extravasation of blood outside the compartmental envelope. This case report is of a patient whose medial circumflex femoral artery was severed during a total hip arthroplasty through a posterior approach leading to compartment syndrome of the buttock. Although this clinical presentation has not been described as compartment syndrome, one similar presentation was described in the literature.

False aneurysm of the inferior gluteal artery following penetrating buttock trauma: case report and review of the literature.

The case of a 28-year-old man who presented with a false aneurysm of the inferior gluteal artery 3 months after a penetrating wound to the buttock is reported. Percutaneous transcatheter embolization was unsuccessful. At operation, abdominal access was used for control, with a gluteal approach to provide exposure of the aneurysm. The presentation and management of patients with this pattern of injury are discussed, and the relevant literature reviewed.

Solitary cutaneous metastasis on the buttock: a disclosing sign of pancreatic adenocarcinoma.

A 48-year-old man, previously healthy and asymptomatic, showed a unique skin lesion located on the right buttock. The histopathological study of the lesion disclosed an adenocarcinoma metastatic to the skin. The primary tumour was found at the head of the pancreas, and was confirmed by biopsy. Six months after the beginning of treatment with Gemcitabine the patient is still asymptomatic, and the cutaneous lesion has disappeared. Carcinomas of the pancreas represent less than 5% of human malignant neoplasms, skin involvement is rare, and metastasis generally multiple and situated in the periumbilical area. To our knowledge, very few patients have been reported with a solitary cutaneous lesion disclosing a pancreatic adenocarcinoma. The atypical location, the absence of related symptoms or analytical disorders, and the good progress of the patient make this case especially interesting.

Anogenital and buttock ulceration in infancy.

Rashes in the anogenital and buttock region are some of the commonest dermatological problems occurring in infancy. The most frequent causes seen in clinical practice are ulcerating haemangiomas, bullous impetigo and severe irritant contact dermatitis. Other causes include nutritional deficiencies, bullous diseases, trauma, Langerhans cell histiocytoses and inflammatory disorders such as pyoderma gangrenosum and Crohn's disease. This review presents a brief overview of these causes and outlines the recommended management strategies.

A cerebriform mass on the right buttock.

BACKGROUND: Nevus lipomatosus cutaneous superficialis of Hoffmann-Zurhelle (NLCS) is a rare disorder that is characterized by tumors of mature fat cells situated within the dermis. OBJECTIVE: To illustrate by a case report the clinical presentation and management of this rare condition. METHODS: Wide excision of the right thigh and buttock mass was performed, and pathological material was submitted. The defect was repaired with a split-thickness skin graft. RESULTS: The largest measured nevus lipomatosus, congenital type (20 x 30 cm), was removed from this patient. CONCLUSION: Physicians should be aware of this rare tumor, which can become extremely large if untreated.
