The significance of bacteriologically positive ventriculoperitoneal shunt components in the absence of other signs of shunt infection.

The purpose of this study was to determine the significance of "asymptomatic bacteriological shunt contamination" (ABSC), defined as a positive bacteriological culture found on a ventricular shunt component in the absence of bacteria in the cerebrospinal fluid (CSF) culture and/or clinical evidence of infection. Of 174 ventriculoperitoneal shunt revisions, 19 cases of ABSC were identified and reviewed retrospectively. In all but one case, no antibiotic medications were instituted because of the positive bacteriological culture. The most common infecting organisms were coagulase-negative staphylococci (seven) and propionibacteria (eight). A comparison of the 19 study cases with the authors' overall shunt experience, as documented in the British Columbia's Children's Hospital shunt database for the time period of the study, lead the authors to suggest that ABSC was not of significance in causing the shunt failure at which contamination was identified and, more importantly, did not increase the risk of future shunt malfunction. The results of this study indicate that in the absence of clinical evidence of shunt infection or a positive bacteriological culture from CSF, bacteria in a shunt component removed at revision in a child almost always represents a contaminant that may be ignored. Therefore, the authors advise that routine culture of shunt components removed at revision of a shunt is not indicated.

Water quality for aquaculture development in Bahía Blanca estuary, Argentina. Bacteriological studies.

The present bacteriological report is part of an institutional multidisciplinary research project designed to study the factibility of using a tidal channel, located in the southern part of the Canal Principal in Bahía Blanca Estuary for aquaculture. Three sampling stations were located. Samples were taken in six opportunities from both water and sediments. Bacteriological studies from water included quantification of heterotrophic marine bacteria, heterotrophic terrestrial bacteria, total coliforms, Escherichia coli and Pseudomonas aeruginosa. Escherichia coli and hydrocarbon-degrading bacteria were quantified from sediments. It is important to include the latter physiological group since their size reflects the presence and fluctuations of this type of pollutant in the environment. The evaluation of faecal indicator values leads to the conclusion that the organic matter present in the studied area, does not come directly from the domestic sewage. The bacteriological data suggest that the studied area would be suitable for aquaculture. Nevertheless, further monitorings contributing to detected modifications in the present bacteriological conditions are recommended.

The bacteriological changes after re-using a vaginal continence guard.

BACKGROUND: Involuntary loss of urine is a major health care problem, especially in women. The Conveen Continence Guard is a non-surgical treatment of stress incontinence. A possible risk of vaginal devices is introduction of changes in bacteriological flora of the vagina, especially after re-use of the guard. Moreover, vaginal devices can induce magnesium ion deficiency and thereby facilitate growth of Staphylococcus Aureus. This can lead to the production of toxin-1, eventually resulting in toxic shock syndrome. METHODS: The first aim of this study was to investigate the capacity to absorp magnesium ions in an in-vitro study. Secondly, 14 patients with stress incontinence were examined for bacteriological changes in the vagina after re-using the continence guard for four days. After completing this study the patients were asked whether re-using the continence guard was as convenient as single use. RESULTS: The continence guard had no absorption capacity for magnesium ions. The vaginal bacteriological changes after re-use showed in 11% changes in bacterial growth and in 20% increased bacterial growth. Only one patient developed a vaginal colonization with Staphylococcus Aureus but had no symptoms of disease. The continence guard showed itself to be less effective in preventing stress incontinence after re-use. CONCLUSIONS: Although re-use of the continence guard is feasable from a bacteriological point of view, re-use renders the device less effective.

[Bacteriological control of blood preservation, production of infusion solutions and dry human plasma under conditions of aseptic work and possible sources of their contamination].

In premises for blood conservation, production of dry human plasma and infusion solutions "notwithstanding the permanent measures for desinfection, new bacterial contamination occurs from time to time and whose source are the casings and material originating from non-sterile environment. Bacteriological control, which has primarily a preventive character, enables a due forecast for measures to be undertaken by the appropriate desinfection of the working surfaces and air, satisfactory conditions of aseptic work can be maintained. General hygiene should be paid attention to as well as mechanical cleansing of premises, avoidance of groups for lunch-time etc., since the treatment by desinfectors would not be sufficient for maintenance of aseptic working conditions. In order to prevent the transmission of bacterial contamination, premises for blood conservation should be strictly separated from other operations and also prevent the unnecessary movements of personnel through corridors. The results of the bacteriological control of the personnel show that greater attention should be paid to their health care since the workers there work in closed aseptic systems and thus avoid them as a bacteria transmittors in respect to danger of blood and dry human plasma contamination. It is also necessary to efficiently educate the personnel for work in aseptic conditions and also increase their elementary knowledge from bacteriology and hygiene. The bacterial skin-flora on the spot of donor's venepuncture also presents a certain danger for blood contamination. Therefore, it is necessary to establish the most optimal manner of skin desinfecate together with the most appropriate means having a fast bactericidal and fungicidal action. It would also be useful, on the basis of further test, to suggest certain standard for an allowed number of conditionally pathogenic and saprophytic microorganisms which would be used by the instutions performing the blood transfusion and production of the intravenous solutions during their bacteriological controls.

Bacteriological monitoring after burn injury.

The most frequent and serious complication after burn injury is infection. Bacteriological monitoring of patients after burn injury is part of complex care. The algorithm of bacteriological checkups is set. Burned areas are microbiologically monitored by semi quantitative imprint method. Colonization is proportional to the length of patient's hospital stay. Resistance of the bacteria to antibiotics is usually higher in burn units than in other departments. That is why it is important to consider antibiotic treatment and strictly observe sensitivity by obtaining bacteriological results and current epidemiological situation. Burned patients are in danger of infection from the burned areas, respiratory tract, urinary tract and central venous catheters. Attending doctor should daily be in contact with the bacteriological laboratory.

The role of nasal cavity disinfection in the bacteriology of chronic sinusitis.

OBJECTIVES: Several factors may influence the results of bacteriological studies in chronic rhinosinusitis. We investigated the potential role of nasal cavity disinfection in the bacteriology of the bulla ethmoidalis in patients suffering from chronic sinusitis. MATERIAL AND METHODS: Bacteriology of the bulla ethmoidalis was studied in 176 consecutive adult patients presenting a chronic sinusitis refractory to standard medical treatment who underwent functional endoscopic sinus surgery. Two different techniques were used: (A) a technique with nasal vestibule and facial disinfection with chlorhexedin (N = 89 patients and 165 samples) vs. (B) a technique with facial, nasal vestibule and nasal cavity disinfection with a povidone-iodine solution followed by a cleansing of the nasal cavity (N = 87 patients and 166 samples). RESULTS: Culture rate was 89.6% (183 bacterial isolates) for technique (A) vs 76.5% (164 bacterial isolates) for technique (B) (p < 0.001). Major bacteria encountered in the (A) group and in the (B) group were respectively: Coagulase Negative Staphylococcus: 77 vs 40 isolates (p < 0.001); Coagulase positive Staphylococcus: 44 vs 30 isolates (p = 0.061); Streptococcus pneumoniae: 4 vs 5 isolates; Others: Streptococcus sp.: 12 vs 16 isolates; Haemophilus influenzae: 8 vs 6 isolates; Enterobacteriacea: 33 vs 53 isolates (p = 0.013) and others Gram Negative Bacilli: 3 vs 7 isolates. CONCLUSIONS: The standard (A) technique to study the bacteriology of the bulla ethmoidalis in patients with chronic sinusitis yielded a higher percentage of positive culture and of bacterial isolates than a more advanced (B) technique. This is mainly due to the higher percentage of contaminant bacteria such as Coagulase Negative Staphylococcus recovered with the standard technique. Enterobacteriacea and others Gram Negative Bacilli were more often encountered into the bulla ethmoidalis with the technique where disinfection of the nasal cavity was performed.

[Bacteriological profile of infectious endocarditis in the area of Sfax (Tunisia)].

We performed a study to analyze epidemiological characteristics and bacteriological profile of infectious endocarditis (I.E) in the area of Sfax (Tunisia). We analyzed, retrospectively, all cases of I.E, according to Duke Criteria, hospitalized in the CHU Hédi Chaker of Sfax between January 1997 and December 2000. Bacteriological investigation included blood culture, cardiac valve culture and serology. Seventy-two cases of I.E were diagnosed. The average of age was 32.3 years. 47.3% of the patients did not have cardiac disease, 25% had a valvular prosthesis, 20.8% a native valvulopathy and 6.9% a congenital cardiopathy. Antecedent of acute rheumatic fever was noted in 66% of I.E on native valvulopathy and in 55.5% of I.E on prosthesis. The mitral valve was involved in 39%, the aortic in 27.5% and the two in 26% of the cases. The origin of bacteremia was found or supposed in 55.5% of the cases and was commonly dental (33 % of EI). The bacteriological diagnosis was positive in 51 cases (70.8%). Staphylococci were isolated in 17 cases (23.6%), Streptococci in 17 cases (23.6%) and dominated by oral streptococci (12 cases). Chlamydial serology was positive in 8 cases (11.1%). Diagnosis of infectious endocarditis due to Chlamydia pneumoniae was confirmed in a case by genomic amplification (PCR) and in situ hybridization on the valve. Endocarditis in Tunisia remains frequent. It reaches with predilection the young person in particular with rheumatic heart diseases. The bacteriological profile remains dominated by Streptococci and the Staphylococci.

[Extent of tuberculin skin testing spot in bacteriologically verified tuberculosis].

BACKGROUND: Tuberculin skin test was evaluated in patients with bacteriologically verified tuberculosis notified in the Czech Republic during 2003 and 2004. METHODS AND RESULTS: Out of 1172 patients with bacteriologically verified tuberculosis altogether 28.8% were tuberculin-negative. The average value of tuberculin reaction in this cohort was 11.6 mm. Among 30-year-old patients with bacteriologically verified tuberculosis the number of tuberculin-negative individuals was statistically lower (14 %) in comparison with 70-year-old and older patients, where the prevalence of tuberculin-negative individuals was 42 %. Higher rate of tuberculin-negative individuals with bacteriologically verified tuberculosis was found among patients who were diagnosed with liver disease (42 % of tuberculin-negative) or malignant tumor (47 % of tuberculin-negative). Presented results show that in circumstances of the Czech Republic the tuberculin skin test can be used to follow up the spreading of tuberculosis infection in younger age groups without accompanying serious disease only. CONCLUSIONS: The improvement of the diagnostics of latent tuberculosis infection can bring the routine introduction of new tests, e.g. Quantiferon-TB Gold or other methods.

Bacteriological safety of closed enteral nutrition delivery system.

One of the most commonly reported side effects of enteral tube feedings is diarrhea, at times attributed to the bacterial contamination of tube feedings. A closed enteral delivery system has recently been devised. It consists of a cardboard Tetrapack containing the sterile enteral nutrition formula and and independent sterile administration set; together these constitute a closed Tetrapack-administration set enteral delivery system. The bacteriological safety of this system was evaluated in vitro under controlled laboratory conditions in a series of studies. There was no or bacteriologically insignificant bacterial contamination of the enteral nutrition formula, even with repeated use of one administration set for multiple containers over 24 h. Bacteriological growth in the enteral nutrition formula was directly related to the duration of the hanging time of a Tetrapack container. No bacterial growth occurred with hanging times less than 18 h; insignificant bacterial growth occurred by 24 h. A progressive time-related increase in bacterial growth occurred between 24 and 48 h. Our data indicate that the newly developed closed Tetrapack-administration set enteral delivery system is, and will remain, bacteriologically sterile if each Tetrapack container is allowed to hang for no longer than 24 h.

[Bacteriologic control of total parenteral nutrition formulations at the Bellvitge Hospital].

Septic complications often lead to halting nutritional parenteral therapy. In general, the source of infection lies in the mixture, the connection and/or point of catheter insertion. The objective of this study is to show the need for bacteriological control throughout the parenteral nutrition process in order to assess the efficacy of the methods used, discover the source of infection and establish an in-house quality control. Different methods for bacteriological control of parenteral nutrition mixtures are studied and are compared with the method used at our hospital which is based on routine culture sampling after the mixture is prepared and before it is perfused, on repeating bacteriological control cultures and on performing bacteriological assessments of the catheter. In order to achieve this goal, 28.501 nutritional parenteral samples from 1,782 patients were studied. Only 185 (0.65%) samples were originally positive, and only 59 of them (0.21%) remained positive following a verification culture. Of the 6 culture-verified septic cases, 5 corresponded to Enterobacter cloacae and 1 to Klebsiella pneumoniae.

[Bacteriological tests in periodontal practice].

The specificity, sensitivity and predictive value of bacteriological tests in periodontology are reviewed. It is stated that the bacteriological aim in periodontal therapy is the change of the subgingival flora in a facultative flora which is compatible with the host. Bacteriological tests can be used to identify indicator bacteria. It is discussed how a microscopical or a bacteriological identification test can support periodontal diagnoses and indication of periodontal therapy.

[Mastitis in general practice. Is bacteriologic examination useful?].

For a period of 22 months, postpartial women in Oslo were asked to consult one of several specific general practitioners in the event of mastitis. Clinical symptoms, bacteriological findings in breast milk and treatment were recorded in 43 patients. Patients with a favourable (n = 35) and with an unfavourable outcome (n = 8) defined as abscess, relapse and/or relief of symptoms after more than seven days, were compared. Unfavourable outcome was characterized by higher score of clinical symptoms and a higher isolation frequency of Staphylococcus aureus. The occurrence of fever did not differ between the groups. Bacteriological findings in milk from both breasts were compared with the findings from 100 milk donors. Staphylococcus aureus was more frequently isolated in milk from affected breasts than from unaffected and control breasts (17/40 versus 4/40 versus 4/100). Most of the Staphylococcus aureus strains (70%) were betalactamase producers. Coagulase negative staphylococci were a frequent finding in all milk samples, whereas Gram-negative bacteria were frequent only in the controls. The presence of pathogenic bacteria, as well as high bacterial counts, were associated with a higher number of symptoms. However, the predictive value of the bacteriological examination was low. Our study indicates that bacteriological examination of breast milk is justified only in patients with severe, acute symptoms and recurrences when betalactamase producing Staphylococcus aureus are suspected.

To the etiology of whooping cough syndrome. Part I: Bacteriological examinations.

A total of 7530 cases of whooping cough syndrome was bacteriologically examined from January 1978 through June 1983. Bordetella pertussis was confirmed by cultivation in 19, B. parapertussis in 284 and H. influenzae in 285 examinees. The total percentage of bacteriologically elucidated cases was 7.81%. Routine bacteriological screenings conducted during the same time period among the preschool children from Prague 10 (a total of 3651 children of 4-6 years of age were examined after the first or before the second revaccination) gave no positive isolation of B. pertussis, isolates of B. parapertussis and H. influenzae were obtained from 43 and 107 children, respectively. This comprehensive bacteriological examination helped identify the causative agent of whooping cough syndrome and reduced thus somewhat the number of cases of uncertain etiology.

[Microbiological study via occluded telescopic catheter in patients under mechanical ventilation and suspected of pneumonia. Clinico-bacteriological follow up of the cases].

Pneumonias related to mechanical ventilation pose a serious diagnostic challenge and are responsible for elevated mortality. Conventional diagnostic methods are of little help. The introduction of invasive techniques such as bronchial catling via an occluded telescopic catheter (OTC) has proven to be an important advance in diagnosis and therapy. We have compared the bacteriological results obtained using OTC with those using conventional diagnostic methods such as tracheobronchial pumps (TBP) and hemocultures, and we have undertaken a bacteriological, clinical follow-up of the cases. Seventy-four patients under mechanical ventilation and suspected of pneumonia were under study. The study consisted to two consecutive phases of collecting bacteriological samples throughout the patient's evolution. A total of 121 fibrobronchoscopies were performed. The sensitivity obtained with OTC was 76%, and the specificity was 100%. The two techniques, OTC and TBP, coincided in 38% of the cases. According to the bacteriological results obtained with OTC, the antibiotic treatment was modified in 44.5% of the patients. The sensitivity of the OTC was significantly less in those patients were the last doses of antibiotic was administered within two hours of performing the procedure. The procedure of gathering secretions from the inferior respiratory tract with OTC possesses greater sensitivity and diagnostic specificity in patients with mechanical ventilation and pulmonary infiltrates and improves the diagnosis performed by other conventional and routine techniques such as TBA and hemocultures.

[Results of initial bacteriology of pulmonary tuberculosis among male NTT employees in the Tokyo area--annual transition and influence of fiberoptic bronchoscopy].

A study was made on the annual changes in the positive rate for acid-fast bacilli (AFB) at the initial bacteriological examination of newly detected pulmonary tuberculosis among male NTT (Nippon Telegraph and Telephone Corporation) employees in the Tokyo area during a period of 32 years from 1960 through 1991. The influence on the changes of recent introduction of fiberoptic bronchoscopy for obtaining the specimen was also analyzed, and the following results were obtained. 1. Results obtained by the conventional bacteriological method using expectorated sputum or aspirated gastric juice as the specimens. (1) The annual incidence of bacteriologically positive (smear and/or culture positive) cases constantly decreased and reached the lowest in 1981, then ceased to decline and remained at the level of 1981. (2) The annual ratio of AFB positive by smear and the ratio of smear and/or culture positive cases to the newly detected cases of pulmonary tuberculosis were constant during the entire observation period. (3) The ratio of AFB positive by smear to the newly detected cases annually increased but did not increase when the observation period was limited to within 1976 to 1991. 2. Results obtained by fiberoptic bronchoscopy in combination with conventional bacteriological methods. (1) Both the annual incidence of smear and/or culture positive cases and the ratio of these cases to the newly detected cases of pulmonary tuberculosis showed a tendency to increase in both the 80's and 90's.(ABSTRACT TRUNCATED AT 250 WORDS)

Diagnostic value of certain mastitis markers in following up the clinical and bacteriological changes in pharmacotherapeutic studies.

Trends of certain mastitis markers were studied in udder quarters (n = 201) showing clinical symptoms of acute mastitis. Besides the clinical examination, before the first treatment (baseline sample), and about 3 weeks later, 17 to 24 days following the last treatment (control sample) milk samples were collected for bacteriological identification of the mastitis pathogens and for the determination of certain inflammatory markers: somatic cell count (SCC), N-acetyl-beta-D-glucosaminidase (NAGase) and alpha 1-antitrypsin (ATR) activities, as well as bovine serum albumin (BSA) and chloride (Cl-) concentrations. Based upon the clinical and bacteriological status as well as the SCC recorded at the control investigations, 6 groups were established (recovered, latently infected, subclinical mastitis: bacteriologically positive and negative, as well as clinical mastitis: bacteriologically positive and negative). As compared to the baseline samples, all parameters decreased in the case of recovered udder quarters, as well as in those with abated latent infection or subclinical mastitis at the time of control examination. Comparing the control samples of the different categories, characteristic differences were found in NAGase activity, indicating the grade of cytodamaging effect of mastitis. Of the other markers, ATR and Cl- proved to be more adequate for the differentiation than BSA. It can be concluded that, in addition to SCC, first of all NAGase can be recommended for use as an inflammatory parameter in pharmacodynamic studies. Besides these two parameters, ATR and Cl- can also be chosen as a possible third marker.

Improvement of semen quality in infected asymptomatic infertile male after bacteriological cure.

Bacteriological etiology was investigated in 29 infected asymptomatic infertile males. The localization of the infection and the effect of a long term antibiotic therapy on semen parameters were evaluated. The most frequent etiological agent isolated was Enterococcus faecalis. Positive bacteriological culture was obtained in prostatic fluid in 16 patients and in semen in 13. Bacteriological cure was achieved in 24 cases and it was associated with improved seminal parameters: sperm concentration, viability and total motile sperm per ejaculate. In 5 patients without bacteriological cure there was no change in semen analysis after antibiotic therapy. In 45% of the infected patients there were less than 0.5 x 10(6)/ml seminal polymorphonuclear leukocytes. In view of these findings granulocyte concentration seems to be a poor marker to predict infection.

Bacteriological analysis of pyogenic infections of the brain.

Pyogenic infections of the brain are divided according to the anatomic structures they involve. Brain abscess and subdural empyema are the main phenomena. This retrospective study analyses case records of the Neurosurgical University Teaching Hospital at Münster from a bacteriological standpoint and compares them with other reports in the literature. Various kinds of bacteria depending on etiology and pathogenesis are described. It is found that with improved bacteriological techniques the number of anaerobic infections increases and, at the same time, the number of sterile empyema decreases. A significantly higher mortality of patients with sterile suppuration is also found. It must be assumed that insufficient bacteriological evaluation leads to faulty diagnoses and ineffective antibiotic therapy and that genuine sterile empyema is rare.
