[Prolonged anuria during bacterial endocarditis].

The authors report a case of anuria which lasted 3 months in a patient with sub-acute streptoccocal endocarditis. The investigations led to the discovery, at the level of the kidneys, of arterial aneurysms, renal infarction and diffuse endo-capillary proliferative glomerulonephritis, with deposits of complement and immunoglobulin and finally, interstitial nephritis, perhaps of metastatic origin, which was probably the lesion responsible for the renal failure. Renal function progressively improved and hemodialysis was stopped at the 6th month after correction of the mitral and aortic valve disease.

[Hypercalcemia during immobilization and prolonged anuria treated with disodium clodronate].

Severe hypercalcaemia was observed during prolonged anuria and prolonged immobilization in a previously healthy 34 years old male after a severe trauma caused by a traffic accident. Other causes of the hypercalcaemia were evaluated, and even though granulomas of unidentified nature were found in the liver after partial resection due to traumatic rupture the immobile state was concluded to be the major cause. A maximum calcium value of 4.44 mmol/l was seen after 10 weeks immobilization. The patient was treated with daily dialysis and, even though the calcium content in the dialysis fluid was reduced, only a minor effect was seen on the calcium level. After eight weeks of hypercalcaemia, the patient was treated with disodium clodronate intravenously 400 mg for daily five days. However, the calcium level was not normalized and the treatment was repeated with a further reduction in the calcium level. If immobilization is a major contributory factor to hypercalcaemia, disodium clodronate seems to be a safe and effective treatment.

Anuria due to lithiasis associated with septic shock.

During 18 months 38 patients with anuria due to lithiasis were admitted in our department, 8 of them developed septic shock. Twenty eight patients were cured using different procedures and were released from the hospital with normal urinary output. Ten patients died, 6 of them from septic shock, which has a high mortality rate. Emergency restoration of urinary flow, correction of fluid and electrolyte imbalance, massive antibiotherapy may improve results.

Bilateral renal papillary necrosis due to Candida infection in a diabetic patient presenting as anuria.

A 38 years insulin-dependent diabetic male, with nephropathy on antituberculous treatment presented with painless frank hematuria followed by anuria for a day which was associated with fever. Ultrasonogram of the abdomen showed bilateral hydroureteronephrosis. Necrotic papillae were retrieved after ureteroscopy which on histopathological examination and culture showed Candida albicans. This was successfully treated with fluconazole and ureteroscopic removal of necrotic papillae.

Urinary ascites and anuria caused by bilateral fungal balls in a premature infant.

A case is reported of anuria and urinary ascites secondary to bilateral ureteropelvic obstruction by fungal balls. Management consisted of bilateral nephrostomy drainage with local irrigation with amphotericin B, and systemic antifungal treatment without surgery. Aspiration by paracentesis was performed for the urinary ascites and continuous drainage through an 8 Fr pig tail catheter for the urinoma. The literature on renal fungus balls in neonates and infants is reviewed.

[Urogenital lymphoma presenting with obstructive anuria].

The authors report a case of urogenital lymphoma with multiple sites in a patient presenting with oligo-anuria. Clinical examination revealed a hard hypogastric and prostatic mass and an enlarged left scrotum. Abdominopelvic and scrotal ultrasound demonstrated a prostatic tumour, a hypogastric mass, hepatic nodular lesions and coeliac lymphadenopathy with bilateral ureterohydronephrosis and a heterogeneous intrascrotal mass in contact with the lower pole of the left testis. The laboratory assessment revealed severe renal failure. After a haemodialysis session and ultrasound-guided right percutaneous nephrostomy, pelvic magnetic resonance imaging (MRI) showed a very large pelvic mass between the bladder and the rectum and transrectal biopsy of the mass confirmed the diagnosis of high-grade malignant non-Hodgkin's lymphoma (NHL) with a type B lymphoblastic phenotype. Treatment consisted of chemotherapy according to the LMB 93 protocol. The course was favourable with return of normal renal function and complete remission 1 month after induction. The patient is currently in complete remission with a follow-up of 12 months. In the light of this case and a review of the literature, the authors discuss the diagnostic, therapeutic and prognostic aspects of this rare site of lymphoma.

[The condition of the oxidant-antioxidant system of plasma and of blood erythrocytes in patients with severe renal insufficiency, anuria stage, due to compression trauma].

The lipid peroxidation processes as well as the activity of the antioxidant system of plasma and of blood erythrocytes were analyzed in severe renal insufficiency due to acute compression trauma. The levels of diene conjugates, malonic dialdehyde and shiff bases were found to be reliably higher in plasma in the anuria stage; as for erythrocytes, only the level of the final peroxidation products, i.e. shiff bases, turned out to be higher in their case. The antioxidant system reaction was as follows: a reliable decrease of the ceruplasmin level was registered in blood plasma, and a reduced activity of superoxide dismutase was detected in erythrocytes. No changed catalase activity was registered either in plasma or erythrocytes versus the controls. The occurred lipid-peroxidation activation had, in the erythrocyte membranes, a negative effect on the protein-lipid contact zones. According to the obtained results, it is necessary to use antioxidants and activation of the non-enzyme system within the complex therapy for the purpose of preventing the secondary damage to cellular membranes.

[Inflammatory aneurysms of the abdominal aorta, complicated by hydronephrosis and anuria. Surgical management].

In this paper the clinical condition of two male patients, aged 58 and 65 years are presented, after being admitted as a consequence of a rare complication of an inflammatory aneurysm of the abdominal aorta, which is an ureteral compression, with hydronephrosis, anuria and acute renal failure. After having an urgent haemodialysis session, the etiology of the process was diagnosed by echography and abdominal CT-scans, followed by ureteral catheterization, restoration of diuresis and normalization of renal function. Conventional surgery was performed later, in elective conditions, and the post operative course was normal, without complications. The main features of this clinical entity, its diagnosis and multidisciplinary management are presented and discussed.

Anuria following reduction of a giant omphalocoele in a neonate: an unusual complication.

Giant omphalocoele in the neonate is a challenging surgical emergency and requires individualized approaches to operative repair. Post-operative complications and associated congenital anomalies still continue to exert a high morbidity and mortality in spite of intensive and improved post-operative care. This case report describes an unusual mechanism which produced anuria in a neonate immediately after operative reduction of a giant omphalocoele. In retrospect, this complication was related to the unique anatomy of the neonate and the type of closure utilized.

Cocaine and indomethacin: fetal anuria, neonatal edema, and gastrointestinal bleeding.

A case is reported in which exposure to cocaine and indomethacin was associated with development of fetal anuria, anasarca, and neonatal gastrointestinal hemorrhage. Cocaine and indomethacin may act synergistically to adversely affect renal, cardiovascular, and platelet function. It may be prudent to obtain a drug history and urine screen for cocaine before instituting indomethacin therapy for preterm labor or polyhydramnios.

[Nondilated obstructive anuria complicating urethrocystopexy].

Anuria complicating urethrocystopexy is generally an obstructive uropathy. No urinary tract dilatation had been observed in these two following cases. The rupture of the collecting system, with urinary extravasation, is due to acute ureteral obstruction and furosemide associated. The first exam to be done is the ultrasonography. However, this exploration can fail to demonstrate obstruction. Urgent percutaneous nephrostomy appears to be the best curative method. Surgical liberation of the trapped ureters is almost necessary.

[Calculous anuria in children. Apropos of 12 cases].

The authors report their experience of calculus anuria in children. They based their study on a series of 12 cases. They point out, in particular, the diagnostic difficulties and prognostic risks which could be encountered. They also emphasize, the role of prompt renal drainage in the management of such an emergency and recommend percutaneous nephrostomy followed by cold nephrolithotripsy.

[Complication during the period of anuria after cadaveric transplantation--with special reference to wound bleeding].

Some period of anuria is usually expected after cadaveric renal transplantation, and hemodialysis is necessary during the period. Postoperative bleeding in the anuric period is not uncommon and usually can be easily controlled with adequate conservative treatment. However, it becomes uncontrollable in some cases and causes a series of serious complications. Out of 22 patients having undergone cadaveric renal transplantation in our hospital, six patients had postoperative bleeding. On investigation of these cases, we found that not only a hemorrhagic tendency due to uremia but also hemodialysis is a risk factor of bleeding in the wound.

[Anuria in an infant caused by an intrapyelic mycelial bezoar in a solitary kidney].

A 4 month-old premature infant having received prolonged intensive care for necrotizing enterocolitis and Pseudomonas infection, developed anuria with intrapyelic fungus ball developed in a congenital single kidney. Review of the literature found 19 similar cases and emphasized the difficulty of an early diagnosis and the usefulness of renal ultrasonography for its detection. Moreover the opportunity to treat premature neonates carrying candida albicans with IV amphotericin B and flucytosine before the occurrence of an uneasy treatable urological obstacle in discussed.
