Ectopic expression of KCNE3 accelerates cardiac repolarization and abbreviates the QT interval.

Regulatory subunit KCNE3 (E3) interacts with KCNQ1 (Q1) in epithelia, regulating its activation kinetics and augmenting current density. Since E3 is expressed weakly in the heart, we hypothesized that ectopic expression of E3 in cardiac myocytes might abbreviate action potential duration (APD) by interacting with Q1 and augmenting the delayed rectifier current (I(K)). Thus, we transiently coexpressed E3 with Q1 and KCNE1 (E1) in Chinese hamster ovary cells and found that E3 coexpression increased outward current at potentials by > or = -80 mV and accelerated activation. We then examined the changes in cardiac electrophysiology following injection of adenovirus-expressed E3 into the left ventricular cavity of guinea pigs. After 72 hours, the corrected QT interval of the electrocardiogram was reduced by approximately 10%. APD was reduced by >3-fold in E3-transduced cells relative to controls, while E-4031-insensitive I(K) and activation kinetics were significantly augmented. Based on quantitative modeling of a transmural cardiac segment, we demonstrate that the degree of QT interval abbreviation observed results from electrotonic interactions in the face of limited transduction efficiency and that heterogeneous transduction of E3 may actually potentiate arrhythmias. Provided that fairly homogeneous ectopic ventricular expression of regulatory subunits can be achieved, this approach may be useful in enhancing repolarization and in treating long QT syndrome.

Principal Components Analysis of the General Purpose Abbreviated Battery of the Stanford-Binet, Fourth Edition, for Young Children

With increased emphasis on preschool assessment, the technology and theoretical basis of intelligence measures suitable for this age group is of increasing importance. One method of examining the appropriateness of preschool measures involves verification of the factor structure of the measure as an indication of construct validity. Factor analytic studies of the Binet 4 have not resulted in consistent findings, and more specifically, the number of factors identified have demonstrated variability across age levels. In that many clinicians use abbreviated forms of the Binet 4, this study conducted a principal component analysis of the General Purpose Abbreviated Battery (GPAB) for a sample of 107 children (ages 2-7 years) who were referred for evaluation to a community-based pediatric psychoeducational outpatient clinic. The structure of the GPAB resulted in a single general intelligence component that mitigates against Binet Area Score interpretation for this age group when the GPAB is used. Implications of previous research as well as the results of this study are discussed relative to the interpretation and utility of the GPAB with preschool and young children.

Abbreviated treatment for elderly, infirm, or noncompliant patients with limited-stage small-cell lung cancer.

PURPOSE: To evaluate the efficacy of an abbreviated treatment plan consisting of two cycles of chemotherapy plus thoracic irradiation in a population of limited-stage small-cell lung cancer (LSCLC) patients who were elderly, infirm, or noncompliant with standard-duration therapy. PATIENTS AND METHODS: Fifty-five LSCLC patients (median age, 73) were treated with one cycle of cyclophosphamide, doxorubicin, and vincristine (CAV) followed 3 weeks later by one cycle of etoposide and cisplatin (EP). Both regimens were administered at conventional full dose. Thoracic irradiation (20 to 30 Gy) was delivered concurrently with EP. RESULTS: Complete response occurred in 28 patients (51%) and partial response in 21 (38%). The median survival time was 54 weeks; the 2-year survival rate was 28% and the actual 5-year survival rate was 18%. Three patients died of toxicity. CONCLUSION: Elderly, infirm, or noncompliant LSCLC patients who are unable to receive standard-duration chemotherapy may have useful palliation and potential for long-term survival with abbreviated chemotherapy (two cycles) and thoracic irradiation.

Abbreviated course of radiation therapy in older patients with glioblastoma multiforme: a prospective randomized clinical trial.

PURPOSE: To prospectively compare standard radiation therapy (RT) with an abbreviated course of RT in older patients with glioblastoma multiforme (GBM). PATIENTS AND METHODS: One hundred patients with GBM, age 60 years or older, were randomly assigned after surgery to receive either standard RT (60 Gy in 30 fractions over 6 weeks) or a shorter course of RT (40 Gy in 15 fractions over 3 weeks). The primary end point was overall survival. The secondary end points were proportionate survival at 6 months, health-related quality of life (HRQoL), and corticosteroid requirement. HRQoL was assessed using the Karnofsky performance status (KPS) and Functional Assessment of Cancer Therapy-Brain (FACT-Br). RESULTS: All patients had died at the time of analysis. Overall survival times measured from randomization were similar at 5.1 months for standard RT versus 5.6 months for the shorter course (log-rank test, P =.57). The survival probabilities at 6 months were also similar at 44.7% for standard RT versus 41.7% for the shorter course (lower-bound 95% CI, -13.7). KPS scores varied markedly but were not significantly different between the two groups (Wilcoxon test, P =.63). Low completion rates of the FACT-Br (45%) precluded meaningful comparisons between the two groups. Of patients completing RT as planned, 49% of patients (standard RT) versus 23% required an increase in posttreatment corticosteroid dosage (chi(2) test, P =.02). CONCLUSION: There is no difference in survival between patients receiving standard RT or short-course RT. In view of the similar KPS scores, decreased increment in corticosteroid requirement, and reduced treatment time, the abbreviated course of RT seems to be a reasonable treatment option for older patients with GBM.

Abbreviated chemotherapy with fludarabine followed by tositumomab and iodine I 131 tositumomab for untreated follicular lymphoma.

PURPOSE: To evaluate the safety and efficacy of a sequential chemotherapy plus radioimmunotherapy (RIT) regimen in previously untreated follicular non-Hodgkin's lymphoma. PATIENTS AND METHODS: Thirty-five patients received an abbreviated course (three cycles) of fludarabine followed 6 to 8 weeks later by tositumomab and iodine I 131 tositumomab. RESULTS: After fludarabine, 31 (89%) of 35 patients responded, with three (9%) of 31 patients achieving a complete response (CR). After the full regimen of fludarabine and iodine I 131 tositumomab, all 35 patients responded; 30 (86%) of 35 patients achieved CR, and five (14%) of 35 achieved partial response. After a median follow-up of 58 months, the median progression-free survival (PFS) had not been reached (95% CI, 27 months to not reached), but it will be at least 48 months. The 5-year estimated PFS rate is 60%. Baseline Follicular Lymphoma International Prognostic Index (FLIPI) was significantly associated (P = .003) with PFS. Five of six patients with more than 25% bone marrow involvement at baseline achieved adequate bone marrow cytoreduction to receive standard-dose iodine I 131 tositumomab. Ten (77%) of 13 patients with baseline bone marrow Bcl-2 positivity demonstrated molecular remissions at month 12. Toxicities were manageable and principally hematologic. Two (6%) of 35 patients developed human antimurine antibodies (HAMA) after RIT. CONCLUSION: Use of abbreviated fludarabine before iodine I 131 tositumomab can reduce bone marrow involvement, when needed, to allow the use of RIT and can suppress HAMA responses. This sequential treatment regimen is highly effective as front-line therapy for follicular lymphoma, particularly for low- or intermediate-risk FLIPI patients.

Evaluation of the safety of an abbreviated course of injections of allergen extracts (rush immunotherapy) for the treatment of dogs with atopic dermatitis.

OBJECTIVE: To evaluate the safety of an abbreviated course of injections of allergen extracts (rush immunotherapy) for the treatment of dogs with atopic dermatitis. ANIMALS: 30 dogs with atopic dermatitis examined at a veterinary dermatology referral practice for treatment with allergen-specific immunotherapy. PROCEDURE: A catheter was placed in a vein in each dog. Dogs were constantly observed throughout the procedure. Allergen extracts were administered in increasing concentrations every 30 minutes for 6 hours to a maintenance concentration of 20,000 protein nitrogen units/ml. Epinephrine, oxygen, and emergency treatment were available as needed. RESULTS: In 22 (73%) dogs, rush immunotherapy safely replaced the prolonged induction period (15 weeks) of weekly injections that consists of increasing concentrations of allergen extract. In 7 (23%) dogs, the induction period was abbreviated to 4 weeks. Of the 8 dogs that developed problems during rush immunotherapy, increased pruritus necessitated premature cessation of rush immunotherapy in 7, and 1 developed generalized wheals. Oral administration of prednisolone (1 mg/kg of body weight) resulted in resolution of adverse effects in all 8 dogs. CONCLUSION AND CLINICAL RELEVANCE: Rush immunotherapy performed by personnel at a veterinary hospital is a safe method for treatment of dogs with atopic dermatitis.

The efficacy of an abbreviated course of nimodipine in patients with good-grade aneurysmal subarachnoid hemorrhage.

OBJECT: Nimodipine therapy has become a standard component of the treatment regimen used in patients with aneurysmal subarachnoid hemorrhage (SAH). Its prescribed use at 60 mg every 4 hours for 21 days is based on reputable, randomized prospective studies. However, because only 20 to 30% of patients with SAH suffer clinical cerebral vasospasm, it is clear that most patients do not actually need the drug. Of course, this fact is not evident until several treatment days have passed. It is common practice, without well-documented consequences, to terminate nimodipine therapy before 21 days in certain clinical circumstances. The aim of this study was to evaluate the effectiveness of abbreviating the duration of nimodipine treatment in the setting of a good-grade aneurysmal SAH. METHODS: A retrospective clinical review was made of 90 consecutive patients who experienced a Hunt and Hess Grade I through III aneurysmal SAH and were treated with nimodipine for 15 days or less. CONCLUSIONS: None of the patients studied suffered a delayed neurological deficit as a result of the abbreviated course of nimodipine.

An abbreviated task-oriented assessment (Bay Area Functional Performance Evaluation).

The purpose of this study was to explore development of an abbreviated version of the Task-Oriented Assessment component of the Bay Area Functional Performance Evaluation (BaFPE). The BaFPE is widely used by occupational therapists practicing in mental health, but therapists have requested an instrument that could be administered and scored more quickly. Both a subjective and objective analysis support the development of an abbreviated version of the Task-Oriented Assessment.

Using the absolute neutrophil count as a stand-alone test in a hematology/oncology clinic: an abbreviated test can be preferable.

More is not always better. We hypothesized that clinicians prefer an abbreviated but focused test to one that provides more--but not totally essential--information. We performed a retrospective analysis of physicians' ordering practices after introducing the absolute neutrophil count (ANC) as a stand-alone test in a hematology/oncology clinic. Previous studies have demonstrated a strong correlation between the ANC, as determined by the Coulter STKS instrument, and the manual neutrophil count. In contrast to the more extensive five-part differential the ANC is a one-part differential that focuses exclusively on the neutrophil count and provides abbreviated information. The hematology laboratory's records from 2 months before and 4 months after introducing this new test were analyzed. Total numbers of ANCs, automated complete differentials, and manual complete differentials performed were tabulated. After its introduction, the ANC comprised more than 75% of differentials performed in this clinic. Manual complete differentials dropped from 69.4% to 18.8%. The turnaround time (TAT) for all differentials taken together dropped by 75% (p = 0.042). We conclude that the ANC can be used widely in the hematology/oncology clinic as a separate, stand-alone test. For clinicians, its popularity over the five-part differential may be based on the fact that the ANC is a focused test with a relatively short TAT. Our experience suggests that it is the test result's relevance and not the volume of information provided that determines clinicians' use.

[Abbreviated laparotomy in patients with hemorrhagic shock].

Five cases of severe hemorrhagic shock of abdominal origin treated with abbreviated laparotomy due to the onset of hypothermia, acidosis and consumption coagulopathy are described. The abdomen was temporarily closed and the patients were transferred to these Intensive Therapy Unit for the treatment correction of these disorders. Patients were then re-operated on 30 hours after the reestablishment of vital functions. Clinical complications of hemorrhagic shock, in which the "Abbreviated Laparotomy" technique is applicable, are presented.

[Reference values in oral health-related quality of life for the abbreviated version of the Oral Health Impact Profile].

Questionnaires measuring oral health-related quality of life (OHRQoL) need reference values for their interpretation. Population-based norms for a questionnaire allow a comparison of the level of OHRQoL observed for an individual or a group of subjects with the level of that construct in the general population. The aims of this study were to establish normative values for abbreviated instruments of the German version of the Oral Health Impact Profile (OHIP-G). Data of the long OHIP-G (53 items) were collected in a national survey (N=2050, age: 16-79 years). Three short instruments with 5 (OHIP-G5), 14 (OHIP-G14) and 21 items (OHIP-G21) were derived from the long OHIP. Norms were computed for the OHIP-G summary score (simple sum of all item responses). The OHIP-G summary score distribution was described by empirical cumulative distribution functions. Because denture usage was strongly associated with OHIP-G summary scores, percentile-based norms were stratified by this variable. Among subjects without dentures, 50% of the population had OHIP-G5 or OHIP-G14 scores 0 and OHIP-G21 scores < or = 2. Summary scores for the three abbreviated OHIPs were < or = 2, < or = 4, and < or = 7 for subjects with removable dentures and < or = 3, < or = 6, and < or = 8 for subjects with complete dentures. Among subjects without dentures, 90% of the population had < or = 4 OHIP-G5, < or = 11 OHIP-G14, and < or = 16 OHIP-G21 points. For subjects with removable dentures, the figures were < or = 7, < or = 17, and < or = 26 points for OHIP-G5, OHIP-G14 and OHIP-G21, respectively; and for subjects with complete dentures, < or = 10, < or = 25, and < or = 38 points, respectively. The presented norms will provide a frame of reference in future cross-sectional studies in the general population or samples with specific oral conditions when OHRQoL is measured by OHIP-G5, OHIP-G14 or OHIP-G21.

Improving the sensitivity and specificity of the abbreviated injury scale coding system.

The Abbreviated Injury Scale with Epidemiologic Modifications (AIS 85-EM) was developed to make it possible to code information about anatomic injury types and locations that, although generally available from medical records, is not codable under the standard Abbreviated Injury Scale, published by the American Association for Automotive Medicine in 1985 (AIS 85). In a population-based sample of 3,223 motor vehicle trauma cases, 68 percent of the patients had one or more injuries that were coded to the AIS 85 body region nonspecific category external. When the same patients' injuries were coded using the AIS 85-EM coding procedure, only 15 percent of the patients had injuries that could not be coded to a specific body region. With AIS 85-EM, the proportion of codable head injury cases increased from 16 percent to 37 percent, thereby improving the potential for identifying cases with head and threshold brain injury. The data suggest that body region coding of all injuries is necessary to draw valid and reliable conclusions about changes in injury patterns and their sequelae. The increased specificity of body region coding improves assessments of the efficacy of injury intervention strategies and countermeasure programs using epidemiologic methodology.

A comparison of three methods for abbreviating G1 examinations.

The G1 glaucoma program is now widely employed in Octopus automated perimetry in order to detect early visual field damage and as an aid in regular follow up. For some patients, however, the examination duration, about 20 minutes, can be excessive. Three methods which could be used for an abbreviated initial screening phase were studied: 1) examining 30 test locations with full 2 dB minimum step size bracketing; 2) using all 59 test locations, but with 4 dB minimum step size bracketing; and 3) using 30 test locations and 4 dB minimum step size. Two indices derived from these abbreviated examination procedures were compared to those obtained from the first phase of the G1 program. The study was based on data of 1008 visual fields of both normal and pathological eyes. It was found that the 30 test location full bracketing method yielded the best agreement with the results obtained with the first phase of the G1 program. Recommendations concerning the optimal utilization of these methods in possible future versions of the G1 program are included.

Difference in degree of A-V block and QS2 abbreviation of ouabain digoxin and digitoxin in cats.

Six trained domestic cats received a spectrum of polar to nonpolar cardiac glycosides: ouabain, digoxin or digitoxin with medication free intervals of four weeks. Significant (P less than 0.01) rate reduction (-19 +/- 8%) and prolongation of PR interval (+13 +/- 5%) occurred with ouabain and with digoxin (-18 +/- 5% and +13 +/- 3%) but not with digitoxin (-6 +/- 4% and +6 +/- 5%). However, rate corrected QS2 abbreviation was greater with digitoxin (-13 +/- 3%) than with ouabain (-8 +/- 3%) or digoxin (-10 +/- 5%). The findings indicate that in cats the polar cardiac glycosides ouabain and digoxin produce greater prolongation of A-V conduction while the nonpolar glycoside digitoxin produces greater abbreviation of QS2. Previously this finding was reported by the authors in guinea-pigs and in rabbits and has since been reported in intact rats, dogs, and in man by other authors. Investigators using nonintact subjects have not been able to demonstrate the finding. The differences may have clinical significance and suggest new insight into structural/activity relationships within the cardiac glycoside family.

Abbreviated laparotomy for damage control: a case report.

During the initial operation on victims of multisystem trauma, life-threatening hypothermia, metabolic acidosis and coagulopathy occasionally develop. Without the immediate control of active bleeding and correction of these abnormalities, the intraoperative death rate is high. A patient with severe abdominal trauma was successfully managed with staged laparotomies. The patient's initial surgery was abbreviated to allow the aggressive correction of hypothermia and coagulopathy before definitive reconstruction of bowel injuries. Abbreviated laparotomy for damage control should be a part of the surgical armamentarium in the management of severe abdominal trauma.

Abbreviated method for urea kinetic modeling in continuous ambulatory peritoneal dialysis patients.

Urea kinetic modeling (UKM) is an established method for quantitating hemodialysis, with target values clearly defined. Precise methods for measuring continuous ambulatory peritoneal dialysis (CAPD) prescriptions are less well-defined, and 24-hour collections of dialysate effluent are logistically impractical. UKM parameters derived from an abbreviated (4-hour) collection period were compared with simultaneously obtained 24-hour collections of urine and dialysate effluent in 22 CAPD patients. Daily Kt/V was calculated from total (residual renal and peritoneal) urea clearance and an anthropometric-derived total body water volume. Results yielded from the 24-hour collection included a mean Kt/V of 0.29 +/- 0.09, and mean protein catabolic rate (PCR) of 0.84 +/- 0.24 g/kg/day. Daily Kt/V values were calculated from each individual dialysate cycle. The first morning cycle after an overnight dwell correlated best with results obtained using 24-hour collections (r = 0.921; p < 0.0001) with no significant differences in Kt/V found (p = 0.454) between the short and 24-hour methods. Daily Kt/V values converted by exponential transformation to a thrice-weekly hemodialysis value yielded a Kt/V equivalent of 1.02 +/- 0.40. UKM using an abbreviated collection period is an accurate and practical tool for quantitating CAPD adequacy in a routine clinical setting.

[Packing and abbreviated laparotomy in the injured--a life-saving procedure].

Packing of abdominal viscera and abbreviated laparotomy are gaining favor among surgeons for cases with multiple injuries. During a 2-year period, 10 patients required abbreviated exploratory laparotomy for abdominal trauma. The 6 records which were complete form the basis of this study. 3 patients were hypotensive at the scene of injury and in the emergency department. Major vascular injury was encountered in 4. The only procedures attempted at the initial operation were packing bleeding solid viscera and controlling bleeding and fecal spillage. Hypothermia, acidosis and need for massive blood transfusion were indications for terminating exploration without attempting definitive surgery.
