Important computer competencies for the nursing profession.

Nursing requires computer competencies. This study aimed at identifying those competencies required for the nursing profession in Taiwan. The Delphi technique was deployed in this study. In the Delphi questionnaires, computer competencies were sorted into seven domains: concepts of hardware, software, and networks; principles of computer applications; skills of computer usage; program design; limitations of the computer; personal and social issues; attitudes toward the computer. In three Delphi questionnaires, nursing informatics experts gave us their opinions on the importance of each computer competency for the nursing profession. The experts also designated when the competency should be cultivated. This study provides a comprehensive list for nursing professionals to check on their computer competence. The results of this study should also serve as good references for teachers and schools in designing related curriculums.

De novo protein design: towards fully automated sequence selection.

Several groups have applied and experimentally tested systematic, quantitative methods to protein design with the goal of developing general design algorithms. We have sought to expand the range of computational protein design by developing quantitative design methods for residues of all parts of a protein: the buried core, the solvent exposed surface, and the boundary between core and surface. Our goal is an objective, quantitative design algorithm that is based on the physical properties that determine protein structure and stability and which is not limited to specific folds or motifs. We chose the betabetaalpha motif typified by the zinc finger DNA binding module to test our design methodology. Using previously published sequence scoring functions developed with a combined experimental and computational approach and the Dead-End Elimination theorem to search for the optimal sequence, we designed 20 out of 28 positions in the test motif. The resulting sequence has less than 40% homology to any known sequence and does not contain any metal binding sites or cysteine residues. The resulting peptide, pda8d, is highly soluble and monomeric and circular dichroism measurements showed it to be folded with a weakly cooperative thermal unfolding transition. The NMR solution structure of pda8d was solved and shows that it is well-defined with a backbone ensemble rms deviation of 0. 55 A. Pda8d folds into the desired betabetaalpha motif with well-defined elements of secondary structure and tertiary organization. Superposition of the pda8d backbone to the design target is excellent, with an atomic rms deviation of 1.04 A.

[An experimental study of the computer-controlled equipment for delivering volatile anesthetic agent].

Our newly-designed computer-controlled equipment for delivering volatile anesthetic agent uses the subminiature singlechip processor as the central controlling unit. The variables, such as anesthesia method, anesthetic agent, the volume of respiratory loop, age of patient, sex, height, weight, environment temperature and the grade of ASA are all input from the keyboard. The anesthetic dosage, calculated by the singlechip processor, is converted into the signals controlling the pump to accurately deliver anesthetic agent into respiratory loop. We have designed an electrocircuit for the equipment to detect the status of the pump's operation, so we can assure of the safety and the stability of the equipment. The output precision of the equipment, with a good anti-jamming capability, is 1-2% for high flow anesthesia and 1-5% for closed-circuit anesthesia and its self-detecting working is reliable.

GRAFTER: a computational aid for the design of novel proteins.

The GRAFTER suite of programs provides geometric search and evaluation functions that simplify and automate the process of identifying the best scaffolds for a particular structural motif. Three application of the GRAFTER suite are presented. Potential grafts between lambda repressor and 434 repressor were identified that should change the DNA binding specificity of these repressors. These results are compared with site-directed mutagenesis experiments that have been shown to alter repressor-DNA binding specificity. Next, 26 loops from antibody structures were grouped into families of similar structure. Grafts of antibody loops onto a pre-existing scaffold are an essential component of antibody humanization. Finally, interleukin (IL)-4 was searched as a scaffold that might accept the graft of a five residue epitope from human growth hormone (hGH). The existence of a crystal structure of the hGH-hGH receptor complex, extensive mutagenesis studies of the hGH residues that contribute to the energetics of ligand-receptor interactions and the gross structural homology between hGH and IL-4 make this an appealing computational target. The approach presented here could aid the development of novel enzymes and binding proteins.

Computer simulation in allocation design of a health-care delivery system.

With the introduction of the concept of progressive patient care in the health-care delivery system, most hospitals now encounter a heterogeneous flow of patients through the various service units. This complicates the problem of allocation of beds and other facilities and equipment. In this paper, a simulation model has been developed by considering the flow of patients in a hospital as a multi-population immigration-birth-death process. Based upon this model, an algorithm has been developed for allocation of beds to various service units using a search process combined with total systems simulation.

A computer-based assessment of structural and displacement asymmetries of the mandible.

The purpose of this article was to assess and quantify the different components that can lead to mandibular asymmetry in a person during or at the end of growth and to investigate the extent to which improvement can be obtained in the different situations. Three boys and 17 girls, 8 to 21 years of age, with facial asymmetry and chin deviation were selected. Posteroanterior cephalometric radiographs were taken in intercuspal position. On each radiograph, three mandibular points (menton, gonion or antegonion, and articular point) were selected to define a mandibular area. A vertical axis of reference was also determined. Computer-aided design was employed to develop two systems (A and B) to assess a symmetry degree of the mandible. With system A, the left mandibular area was rotated around the vertical axis of reference, and the degree of the superimposition between the left and right areas was plotted. With system B, the left area was rotated around an axis that ran through menton and was perpendicular to a line connecting the two articular points of the mandible. Also, in this case, the degree of superimposition between the two areas was plotted. With system A, the degree of superimposition was a function of mandibular position and mandibular symmetry. With system B, it was a function of mandibular symmetry only. Thus through comparative examination of the data, assessment of displacement asymmetry and structural asymmetry was possible. In two patients, only displacement asymmetry was present, whereas 14 patients showed various features of structural asymmetry. The patients were treated with orthopedic splints to keep the mandible in a position of symmetry. Orthodontic treatment followed so that the intercuspal position would finally coincide with the position of symmetry. Computer-aided design analysis was performed again after a mean observation period of 41.1 months. In 11 patients an improvement in symmetry was observed with both systems. The patients in whom no improvement of structural asymmetry was observed showed a relatively high degree of symmetry at the beginning or had a considerably higher age than the mean age of the whole group. The clinical implications of the data are discussed.

The VA-Cyberware lower limb prosthetics-orthotics optical laser digitizer.

Characterization of the residual limbs and limb segments of patients for prosthesis and orthosis design has principally been a subjective process, highly dependent upon the skill, level of training, and experience of the prosthetist/orthotist involved. Even with the application of computer-aided design (CAD) and computer-aided manufacturing (CAM) technologies in prosthetics and orthotics, residual limb/limb segment characterization has remained substantially subjective and dependent upon prosthetist/orthotist skill, training, and experience. To eliminate the variations and errors that frequently occur because of this dependence, and to further quantify the patient measurement process, rehabilitation engineering researchers at the New York Department of Veterans Affairs Medical Center developed an optical laser digitizer for quantitative characterization of patients' residual limbs'/limb segments' spatial geometry and surface topography. The optical digitizer developed is described, and results of laboratory and clinical tests with the digitizer are presented. Examples showing the capability of the digitizer to accurately, rapidly, repeatably, and consistently capture the contours over the entire surfaces of the residual limbs of patients with below-knee (BK) and above-knee (AK) amputation, the lower limbs of orthotics patients, and the feet and ankles of pedorthics patients, are given. In addition, results of a comparative clinical study of optical digitization and standard prosthetics CAD plaster wrap cast electromechanical digitization of the residual limbs of subjects with BK and AK amputation are presented. The enhanced accuracy, repeatability, and consistency afforded by optical digitization are shown. Finally, areas for refinement of the optical digitizer's design, identified in the project's laboratory and clinical tests, are discussed.

Clinical aspects and strategy for biomaterial engineering of an auricle based on three-dimensional stereolithography.

At the present time, the partial and/or complete reconstruction of an auricle from autologous rib cartilage is one of most widely published techniques. In the field of tissue engineering, different techniques have been described to generate cartilage tissue using isolated chondrocytes. The basis of these tissue-engineering techniques is bioresorbable or non-bioresorbable biomaterials, which serve as a three-dimensional cell carrier. Tissue engineering of an auricle requires preformed bioresorbable biomaterials designed to fit the form of a patient's auricular defect. Three-dimensional imaging acquired from computed tomography scans or laser surface scanning has become an important tool in modern medicine. This study represents the preoperative procedures for the reconstruction of an auricle through tissue engineering in accordance with the clinical aspects. Hyaff 11, a hyaluronic acid derivative, was used as a three-dimensional cell carrier for isolated human nasoseptal chondrocytes. The chondrocytes were amplified in a conventional monolayer culture before the cells were seeded on a hyaluronic non-woven mesh and cultured in vitro for 4 weeks. The chondrogenic potential of human nasal chondrocytes in Hyaff 11 was investigated by confocal laser scanning microscopy, histology (toluidine blue) and immunohistochemistry (collagen type II). Computer-aided design (CAD) and manufacture of an auricle model with stereolithographical methods were used for the prefabrication of a bioresorbable three-dimensional cell carrier designed in the form of a patient's auricular defect. The cell carrier used was Hyaff 11, a fully benzyl-esterified hyaluronic acid derivative. Confocal laser scanning microscopy has shown good cell attachment, a homogenous distribution of amplified chondrocytes and a viability of more than 90%. After 4 weeks in vitro culture the human nasoseptal chondrocytes synthesized new cartilage with the expression of cartilage-specific collagen type II. In order to shape a patient's designed scaffold the auricle model was fitted exactly and symetrically to the contralateral side. Subsequently, the mirror image patient-specific model was used to prepare an identical scaffold model made of a fully benzyl-esterified hyaluronic acid derivative. The bioresorbable scaffold that was produced gave a satisfactory representation of auricle structure. Bioresorbable preformed biomaterials in the form of a patient's auricle defect represent an important prerequisite for the tissue engineering of autologous auricle grafts. Hyaff 11 seems to be a promising material for tissue engineering of cartilage transplants, and the application of this approach will improve conventional reconstructive surgery in the future.

Evaluation of the efficacy of hand-held computer screens for cardiologists' interpretations of 12-lead electrocardiograms.

BACKGROUND: Newly designed computer-based applications and the development of wireless technology have allowed the transmission of 12-lead electrocardiogram (ECG) waveforms from remote locations to the hand-held computers of cardiologists. If these computer ECGs can be reliably interpreted, then the time to treatment for cardiac patients may be reduced. METHODS AND RESULTS: Twenty classic examples of cardiac abnormalities were chosen to test the efficacy of the hand-held computer's liquid crystal display (LCD) screen in the interpretation of 12-lead ECGs. Ten cardiologists interpreted these 20 ECGs on the hand-held computers and then twice later on traditional printed paper. The control intraobserver agreement between the sets of paper-displayed ECGs was measured against the agreement between each of the paper sets and the LCD-displayed set of ECGs. Eighty-nine percent (178/200) of the ECGs were interpreted identically by the participants between the 2 paper sets. When comparing the interpretations of the LCD-displayed ECGs with those of each of the paper sets of ECGs, 88.0% (176/200) and 87.5% (175/200) of identical diagnoses were noted. These differences of 1.0% and 1.5% in intraobserver agreement between paper-to-paper and each of the 2 paper-to-LCD comparisons were not significant (P =.75 and P =.88, respectively). CONCLUSIONS: The strong intraobserver agreement shows that cardiologists make the same diagnoses when viewing LCD-displayed ECGs as they do when viewing paper-displayed ECGs. A study to measure the intraobserver agreement of the decision regarding administration of reperfusion therapy after interpretation of ECGs of patients with acute chest pain is now underway.

[Design of geometric models of the removable partial denture framework].

PURPOSE: The aim of this study was to introduce a method on computer aided design (CAD) of removable partial denture(RPD) framework based on domestic CAD/CAM software system, which contributes to the further research to develop the domestic software system applied in restorative dentistry. METHODS: Point cloud data of a partially edentulous cast, a mandibular Kennedy Class II modification 2 arch, was captured by an optical scanning system with projective grating and high-resolution digital camera. Using domestic CAD/CAM software system, the above point cloud data was reduced, digital surveying line and inserting path were determined. In terms of the principle of clinical design, the tissue surfaces and polishing surfaces of every component of RPD were built such as clasp, rest, lingual bar, and mesh construction, et al. As well as the characteristic structures of the framework, including the half-pear shaped cross section of lingual bar, the internal and external finish lines, the tissue stop at the tissue surface of mesh construction and so on, were fabricated on the base of reduced data model. 3-D surface model of the RPD framework was created. RESULTS: Geometric model of the RPD framework was fabricated successfully, which took on good fitting, high visibility and editing conveniently. And the data converted to STL file format that could be read by any other CAD/CAM software system and was in preparation for subsequent computer aided manufacture (CAM) of RPD framework. CONCLUSIONS: It is suggested that this method based on domestic software in CAD of RPD framework be feasible.

Computer-aided rehabilitation engineering--CARE.

Computer-aided design (CAD) and related technologies are having a dramatic impact upon manufacturing industry. The advantages of these technologies over traditional methods include improved accuracy and quality, reduced product development time and ease of product modification and hence customization. The resultant increases in productivity, whilst maintaining flexibility of design, have obvious potential in the rehabilitation equipment industry. Recent applications of these technologies to the analysis of prostheses and wheelchairs and to the design and production of sockets for amputees, prosthetic implants and custom seating are reviewed. Computer-Aided Rehabilitation Engineering (CARE) is in its infancy. However, given the rapidity of developments in this field, CARE has the potential to radically alter rehabilitation engineering over the next decade.

Computer-aided simulation and design of nanofiltration processes.

The modelling of membrane filtration processes is often performed by applying black-box models or short-cut methods, because of the complexity of the molecular interactions on and inside the membrane. The assumptions made for short-cut methods can be applied with accuracy to reverse osmosis processes, whereas the simulation of nanofiltration can lead to unreliable results that sometimes deviate from real conditions to a great extent. A steady-state process simulation, NF-PROJECT, based on input information from membrane characterization, was developed (isothermal operation). The individual separation characteristics of each membrane element are calculated in an iterative sequence, illustrating the successive reduction in permeability and rejection between the elements arranged inside the pressure vessel. The simulation provides information on the increasing feed concentration and osmotic pressure, the hydraulic pressure loss, the deterioration of the flow conditions in the vessel, and the joint performance of the membrane elements to be analyzed. Taking an example from a practical application, a two-stage nanofiltration pilot plant was simulated, the results of which are presented in this article. Examples of optimization potentials are illustrated for the target criteria of economic efficiency (specific energy costs), permeate quality, and flow.

Design and structure-based study of new potential FKBP12 inhibitors.

Based on the structure of FKBP12 complexed with FK506 or rapamycin, with computer-aided design, two neurotrophic ligands, (3R)-4-(p-Toluenesulfonyl)-1,4-thiazane-3-carboxylic acid-L-Leucine ethyl ester and (3R)-4-(p-Toluenesulfonyl)-1,4-thiazane-3-carboxylic acid-L-phenylalanine benzyl ester, were designed and synthesized. Fluorescence experiments were used to detect the binding affinity between FKBP12 and these two ligands. Complex structures of FKBP12 with these two ligands were obtained by x-ray crystallography. In comparing FKBP12-rapamycin complex and FKBP12-FK506 complex as well as FKBP12-GPI-1046 solution structure with these new complexes, significant volume and surface area effects and obvious contact changes were detected which are expected to cause their different binding energies-showing these two novel ligands will become more effective neuron regeneration drugs than GPI-1046, which is currently undergoing phase II clinical trail as a neurotrophic drug. Analysis of volume and surface area effects also gives a new clue for structure-based drug design.

Individual augmentation of the atrophic mandible based on CAD/CAM-manipulated computed tomography data--in vitro results.

The problems of shaping a stable alveolar ridge form favorable for prosthodontic treatment by the various methods of ridge augmentation raise the question of whether computer-aided design (CAD) may be used for geometric modeling of the implant. Helical computed tomography (CT) data were used to create a three-dimensional (3-D) model of the atrophic mandible. This numerically based model served as the basis for a free-form-surface design of the implant's form, height, and position. The idealized data required for this geometric modeling were also derived from numerically based serial sections of 104 mandibles of dried skulls. The modeling procedure followed the formula "idealized mandibular geometry minus patient's geometry", yielding the geometric data of the implant. These numerically control (NC) a milling machine to fabricate the individual implant. The augmentation of an atrophic mandible of a dried skull is presented as the first in vitro trial of this new method.

Computation of digitally reconstructed radiographs for use in radiotherapy treatment design.

The increasing use of 3-dimensional radiotherapy treatment design has created greater reliance on methods for computing images from CT data which correspond to the conventional simulation film. These images, known as computed or digitally reconstructed radiographs, serve as reference images for verification of computer-designed treatments. Used with software that registers graphic overlays of target and anatomic structures, digitally reconstructed radiographs are also valuable tools for designing portal shape. We have developed radiograph reconstruction software that takes full advantage of the contrast and spatial detail inherent in the original CT data. This goal has been achieved by using a ray casting algorithm which explicitly takes into account every intersected voxel, and a heuristic approach for approximating the images that would result from purely photoelectric or Compton interactions. The software also offers utilities to superimpose outlines of anatomic structures, field edges, beam crosshairs, and linear scales on digitally reconstructed radiographs. The pixel size of the computed image can be controlled, and several methods of interslice interpolation are offered. The software is written in modular format in the C language, and can stand alone or interface with other treatment planning software.
