Paragonimiasis. Atypical appearances in two adolescent Asian refugees.

Paragonimiasis is a parasitic disease endemic to Asia, Africa, and South America. With the increasing number of Southeast Asian immigrants arriving in the United States, physicians should be aware of the various manifestations of this disease. We describe two adolescent Asian refugees who had atypical cases of paragonimiasis. One experienced acute abdominal pain mimicking appendicitis, and the other had asymptomatic left upper-lobe cavitation.

Refugee flow or brain-drain? The humanitarian policy and post-Tiananmen mainland Chinese immigration to Canada.

"The humanitarian policy that the Canadian government implemented in response to the 1989 Tiananmen Square crackdown changed a migration system primarily based on personal networks into a brain drain. Post-Tiananmen mainland Chinese immigrants (MCIs) were better educated than those arriving in Canada previously. Among the post-Tiananmen MCIs, those who landed under the policy were better educated than those landing in other categories. The analysis suggests that post-Tiananmen MCIs represented a brain-drain rather than a refugee flow, that the humanitarian policy implicitly contained ideological and human capital concerns in addition to humanitarian concerns, and that Canada benefited from the policy by obtaining human capital as well as satisfying its humanitarian obligations and ideological aspirations."

Tinnitus among Cambodian refugees: relationship to PTSD severity.

Consecutive Cambodian refugees attending a psychiatric clinic were assessed for the presence and severity of current tinnitus (i.e., at least one episode in the last month). Fifty percent (52/104) of surveyed patients had tinnitus. Among the tinnitus patients, posttraumatic stress disorder (PTSD) rates were significantly more elevated than among nontinnitus patients (OR=13.5; 95% CI=5.8 to 39.4), as were Clinician-Administered PTSD Scale (CAPS) scores. In a hierarchical regression among tinnitus patients (n=52), tinnitus-related trauma associations and catastrophic cognitions accounted for variability in CAPS severity beyond a measure of tinnitus severity. Among tinnitus patients, tinnitus-related trauma associations and catastrophic cognitions mediated the effect of tinnitus severity on CAPS severity.

Ethnopharmacologic analysis of medicinal plants used by Laotian Hmong refugees in Minnesota.

Laotian Hmong refugees in the Twin Cities of Minneapolis and St. Paul, Minnesota, cultivate many plants and employ them as a first line of defense against illness and disease. Thirty-seven medicinal plants have been identified, phytochemical components catalogued through literature research, and potential pharmacological activities correlated with Hmong medicinal uses. Using western biomedical criteria of efficacy, 92% of the medicinal plants being cultivated were found to be potentially efficacious. The frequent use of these plants in diet (81%) suggests that they have served to mediate against illness and disease states in the cultural and natural environment in which the practices evolved. A listing of 37 plants with Hmong names, uses and potentially active constituents is provided.

Posttraumatic stress disorder and psychiatric co-morbidity: symptoms in a random sample of female Bosnian refugees.

OBJECTIVES: This study investigated psychological symptoms in Bosnian women 3-4 years after their arrival in Sweden. SUBJECTS AND METHODS: A simple random sample of 163 Bosnian women aged 19-59 was drawn from the Swedish populations register in 1996. The control group consisted of 392 Swedish-born women. Data were collected in face-to-face interviews. The Hopkins Symptom Checklist 25 (HSCL-25) and the Posttraumatic Symptom Scale (PTSS-10) were used to measure psychological symptoms of depression, anxiety, psychological distress, and posttraumatic stress disorder (PTSD). Unconditional logistic regression was used to estimate odds ratios (OR) for psychological symptoms after adjustment for age, country of birth, education, marital status, economic difficulties, social network, and feeling secure. RESULTS: The prevalence of symptoms of PTSD was 28.3% among the Bosnian women. Bosnian women had significantly higher risks of symptoms of depression, anxiety, and psychological distress than Swedish-born women. For depression the odds ratio was 9.50 among Bosnian women. CONCLUSION: Psychiatric community interventions need to target Bosnian refugee women. Awareness among health-care workers who encounter these women in a clinical setting should be improved.

Satellite observations of Lava Lake activity at Nyiragongo volcano, ex-Zaire, during the Rwandan refugee crisis.

In June 1994 the summit crater of Nyiragongo volcano, located in the Great Lakes region of central Africa, began to fill with new lava, ending nearly 12 years of quiescence. An earlier eruption of the volcano in 1977 had culminated in the catastrophic draining of a lava lake through fissures in the crater wall, feeding highly mobile lava flows which reached the outskirts of Goma and killed more than 70 people. By July 1994, as many as 20,000 Hutu refugees were arriving in Goma every hour, only 18 km south from the summit of Nyiragongo. The exodus brought more than one million people to the camps near the town raising fears of a repeat of the 1977 eruption. This paper examines the role that satellite remote sensing could have played in surveillance of the volcano during this time, and demonstrates the potential for monitoring this and other volcanoes in the future. Images recorded by the spaceborne Advanced Very High Resolution Radiometer (AVHRR)--freely available over the Internet--provide semi-quantitative information on the activity of the volcano. The aim of this paper is to promote the wider use of readily available technologies.

Pleural paragonimiasis in a Southeast Asia refugee.

We report a Laotian patient with pleural paragonimiasis who did not have the usual diagnostic triad for this parasitic disease. He did not have chronic hemoptysis (considered by many to be an "invariable" finding), there were no pulmonary infiltrations, and stool and sputum examinations did not yield Paragonimus ova. The diagnosis was made on the basis of ova found in the pleural fluid. Paragonimiasis pleural effusion did not resolve with bithionol, the drug of choice for pulmonary paragonimiasis, and, as a result, chest tube drainage was required. The difference between pleural paragonimiasis and pulmonary paragonimiasis is that the classic clinical presentation of the latter (hemoptysis, ova in sputum and stools, lung infiltration, etc.) requires an intrapulmonary location on the parasite. A search for ova in the pleural fluid may be the only diagnostic tool for patients suspected of pleural paragonimiasis. With the influx of Southeast Asia refugees, this case report may be of relevance to U.S. physicians involved in the care of patients in whom not all chronic pleuropulmonary diseases are tuberculous.

Pediatric tumors in north west Pakistan and Afghan refugees.

All patients referred to the Institute of Radiotherapy and Nuclear Medicine in Peshawar (IRNUM) during 1990 to 1994 were analyzed. There were 1655 children with biopsy-proven cancers; 1290 were from the North West Frontier Province (NWFP), and the remaining 365 were Afghan refugees. Male children from the NWFP were 67% and females were 33%. Among Afghan children, 69% were males and 31% were females. Patients whose histopathologies were doubtful or not available were excluded from the study. The most common tumors in children in the NWFP were lymphoid leukemia, lymphoma, myeloid leukemia, Wilms tumor, tumors of the central nervous system (CNS), soft tissue sarcoma, bone tumors, retinoblastoma, neuroblastoma, and testicular tumors. Among Afghan children the most common cancers were lymphoma, lymphoid leukemia, myeloid leukemia, Wilms tumor, retinoblastoma, tumors of soft tissue, bone tumors, CNS tumors, testicular tumors, and neuroblastoma.

[HIV-positive refugee from Africa with low compliance. Interview about reasons for treatment failure].

Strict compliance is required for a sustained response to antiretroviral therapy for HIV. Several studies from the US have found that African-Americans are less compliant and are more liable to treatment failure than caucasians. We describe a young, male refugee from Africa with treatment failure after non-compliance. A qualitative interview shows that his main reason for non-compliance is fear of disclosure and stigmatization. There seems to be a need for research into the reasons for non-compliance among vulnerable groups and targeted interventions.

Types of alpha+ thalassemia in Southeast Asia refugees.

We have determined alpha+ deletional thalassemia among 143 Southeast Asia refugees (Cambodians, Laotians, and Vietnameses). Gene frequency of alpha+ deletional thalassemia in Vietnameses (0.035) was found lower than in Cambodians and Laotians (0.11). Bam H1 and Bg1 II analysis indicated that both rightward and leftward thalassemias are encountered, the -alpha 3,7 form is being by far more frequent than the -alpha 4.2 one. Only type I cross-over was found by Apa I digestion on -alpha 3.7 chromosomes. The Rsa I polymorphism, 5' to Z alpha 2 block, is associated with -alpha 3.7 type I haplotype and the site is present in 12 out of 23 chromosomes. All these data suggest at least three origins of alpha+-thalassemia in Cambodia and Laos.

The cardiac pathology of sudden, unexplained nocturnal death in Southeast Asian refugees.

Sudden death during sleep has occurred among previously healthy Southeast Asian male refugees, but routine autopsies have not determined the cause of death in any of these cases. We report the first systematic attempt to define the cardiac abnormalities associated with this syndrome. Among 18 hearts examined, 14 showed slight to significant cardiomegaly, characteristic of increased cardiac work load. The reasons for the cardiomegaly remain unexplained. Conduction system anomalies were present in all but one heart. These included persistent fetal dispersion of the atrioventricular node and/or bundle of His, present in 14 hearts; accessory conduction fiber connections, found in 13 cases; and congenital heart block, observed in one case. These abnormalities were associated with variations in the structure of the cardiac base, suggesting a common aberrant developmental process. Although the functional significance of these findings has not been established, the conduction system anomalies may be the substrate for sleep-related cardiac arrhythmias and sudden death.

Pleuropulmonary melioidosis in a Cambodian refugee.

A 47-year-old Cambodian refugee presented with an acute respiratory illness that featured consolidation of the lower lobe of the left lung and progressive involvement of the adjacent pleura caused by Pseudomonas pseudomallei. Initial difficulty in identifying the organism resulted in an inadequate duration of therapy. Chronic pleural disease followed, and the organism became resistant to many antibiotics during therapy. A diagnosis of pleuropulmonary melioidosis should be entertained and the microbiology laboratory alerted when patients with pneumonia who are from endemic areas are encountered, so that a diagnosis can be made early and the appropriate treatment begun.

Freshwater snails survey in and around the Cambodian refugee camp, Sakaeo District, Prachinburi Province, Thailand.

The freshwater snails surveys were carried out during September 15, 1981 to December 15, 1981, especially for the intermediate or possible intermediate host of Schistosoma mekongi in and around the Cambodian refugee camp at Ban Kaeng Holding Centre, Sakaeo District, Prachinburi Province. A total of 30 localities were visited and 15 species of freshwater snails were collected. The snails did not shed any cercaria of S. mekongi and none of these was the intermediate host of S. mekongi.

[Hemoglobin abnormalities and glucose 6 phosphate dehydrogenase deficiency (G6PD) in 101 refugees from southeast Asia].

Hematological studies were performed systematically in 101 refugees from South-Eastern Asia living in the Grenoble area at the present time. We found: --4 haemoglobin E homozygosity, --29 haemoglobin E heterozygosity, --2 beta-thalassemia heterozygosity, --4 alpha-thalassemia heterozygosity, --14 glucose 6 phosphate dehydrogenase deficiencies.

[Lupus vulgaris. Cutaneous tuberculosis in a Vietnamese refugee].

Since 1990 there has been a slight increase in the number of cases of tuberculosis in Norway. Tuberculosis can affect the skin with or without apparent involvement of internal organs. A case of lupus vulgaris in a 13 year-old male Vietnamese refugee is described. The Pirquet test was strongly positive. However, no involvement of lungs or other internal organs was found. Clinical remission was achieved using isoniazide and rifampicin.

Adaptation of a strain of Plasmodium falciparum from a Montagnard refugee to Aotus monkeys.

A strain of Plasmodium falciparum from a Montagnard refugee was shown to produce large numbers of gametocytes in culture. Attempts were made to establish this strain in Aotus monkeys via trophozoite and sporozoite inoculation. The Montagnard S-1 strain was readily adapted to A. l. griseimembra monkeys via trophozoite inoculation. Other species of Aotus failed to support the development of high density parasitemia. None of 12 attempts to transmit the infection via sporozoites from Anopheles freeborni or An. dirus mosquitoes was successful; however, developing exoerythrocytic stages were demonstrated in hepatocytes of an A. lemurinus griseimembra monkey.
