Primary carcinoma of the fallopian tube in a 88 years old woman: review of the literature.

A woman of 88 years old with primary carcinoma of the fallopian tube, the oldest patient reported in the English literature according to the best of our knowledge, was presented. After debulking surgery four courses of chemotherapy including paclitaxel and carboplatin was performed. The patient was free of disease during two years follow-up.

Epithelioid trophoblastic tumour: report of a case in the fallopian tube.

A case of epithelioid trophoblastic tumour (ETT), occurring in a fallopian tube of a 39-year-old woman, is reported. The patient presented with a positive pregnancy test, but continued to have 'periods'. A palpable right adnexal mass was noted that was confirmed on ultrasound. The mass was removed together with the uterus, omentum and associated ovary. Careful examination of the uterus revealed no evidence of either an antecedent tumour or intra-uterine pregnancy. Histologically, the tubal mass displayed sheets and islands of large, relatively uniform, mitotically active polyhedral cells, with surrounding necrosis. The immunoprofile of the tumour was atypical in that alpha-inhibin and epidermal growth factor were weakly positive, but other results were consistent with the diagnosis of ETT. The patient received a foreshortened course of standard EMACO (etoposide, actinomycin-D, methotrexate, vincristine, and cyclophosphamide) combination chemotherapy for high-risk gestational trophoblastic disease. Serum beta-hCG fell from a pre-operative level of 52 000 U/mL to non-pregnant levels within two courses and she remains well and disease-free 12 months post-diagnosis.

[Carcinoma of the fallopian tube after prophylactic laparoscopic ovariectomy in a patient with a BRCAI mutation].

A 48-year-old woman with a distended abdomen appeared to have ascites and was admitted to the gynaecological ward. At the age of 31 years she had been diagnosed with breast cancer and had undergone surgical breast conservation of the right breast. There was a history of both ovarian cancer and breast cancer in her family. Genetic evaluation showed that she was carrying a BRCAI germline mutation. At the age of 42 years she underwent a prophylactic bilateral laparoscopic ovariectomy and 5 years later she underwent a complete mastectomy due to breast carcinoma of the left breast. Two months later she developed ascites, a raised CA125 level and on a CT scan carcinoma of the peritoneum. During the laparotomy a fallopian tube carcinoma was found. After the uterus, fallopian tubes and omentum had been surgically removed, chemotherapy took place. The patient tolerated this well and the CA125 value decreased. Recently, the first molecular evidence was found that linked fallopian tube cancer to germline mutations in BRCAI patients. Patients harbouring a BRCA germline mutation not only have an increased risk of ovarian carcinoma but also of fallopian tube carcinoma. Therefore, in patients with a BRCA mutation, prophylactic surgery should take the form of an adnexectomy, not an oophorectomy.

Immunohistochemical profile of serous papillary cystadenofibroma of the fallopian tube: a clue of paramesonephritic origin.

Cystadenofibromas of the fallopian tube are rare tumors of the female genital tract. These tumors are usually asymptomatic and are found incidentally. The authors present an incidentally found fallopian serous cystadenofibroma in a 48-year-old woman with leiomyoma uteri. The topographic localization of the lesion, histopathologic findings of müllerian-type epithelium, immunophenotypic profile of vimentin-cytokeratin coexpression, and diffuse apical epithelial membrane antigen (EMA) immunoreactivity suggested that the tumor was an embryologic remnant originating from the müllerian duct.

[A study of the psychosomatic aspects of endometriosis].

Based upon existing psychological studies of endometriosis, the hypothesis was tested if patients suffering from this disorder differ from other infertile females with respect to psychological characteristics. Using questionnaires as well as a semi-structured psychotherapeutic interview 38 patients with endometriosis and 18 females with tubal sterility were investigated. A comparison of both groups indicated a higher level of anxiety and some evidence for a somatic preoccupation in the endometriotic patients. The analysis of the interviews (which were judged by several independent raters) confirmed the hypothesis of gender role conflicts in combination with endometriosis. This conflict seemed to be characterized by a negative experience of menarche and puberty, early gynecological problems and negative sexual experiences. This constellation is illustrated using a typical case. Based upon these results, further studies of endometriosis using a psychosomatic perspective seem to be promising.

[Primary fallopian tube cancer].

Seventeen patients with a primary Fallopian tube carcinoma have been treated in the Landesfrauenklinik Magdeburg between 1961 and 1990. Initial therapy consisted of surgery alone. The adjuvant postoperative therapy was radiation or combined chemotherapy. The overall 5-year survival rate was 56%, of stage I 75%, of stage II 40% and 35% of stage III.

PCNA and laminin as prognostic factors in primary Fallopian tube carcinoma.

Primary Fallopian tube carcinoma (PFTC) is one of the rarest malignant tumours of the female genital tract. Staging of the disease according to FIGO scale is the most common prognostic indicator. Others, such as histological type, grading according to Hu classification, depth of tubal wall invasion, the presence of neoplastic cells in peritoneal leakage, invasion of the lymphatic and blood vessels, mitotic activity, DNA ploidy, Ki-67 expression, AgNOR level and p53 and c-erbB-2 immunoreactivity, are not widely accepted. 70 cases of primary Fallopian tube carcinomas were analysed with regard to clinicopathological data, survival and the expression of proliferating cell nuclear antigen (PCNA) and laminin. Histological classification of PTFC revealed 26 cases of the endometriod type, 16 undifferentiated, 15 serous, 8 urothelial, 3 clearcell and 3 of other types. A total of 70 cases revealed positive nuclear staining for PCNA. The index of PCNA (labelling index, LI, proportion of PCNA-positive cells relative to all neoplastic cells) was evaluated. PCNA LI values were classified as high, > 0.45, or low, </= 0.45. Intracellular expression of laminin was found in 46 cases and extracellular in 28 cases. There was no significant correlation between the expression and distribution of laminin and survival. The p value was statistically significant only for PCNA LI as an independent prognostic factor.

Laparoscopically assisted vaginal hysterectomy with bilateral salpingo-oophorectomy due to endometrial cancer in a heart transplant recipient. A case report.

INTRODUCTION: Transplant recipients have a higher incidence of cancer compared with the general population. This increased risk is related to the intensity and chronicity of immunosuppression that these patients receive. In this report, we present a case of a heart transplant woman with endometrial cancer who was diagnosed six months after transplantation. CASE REPORT: A 49-year-old woman who had undergone a heart transplant was referred to our department in May 2002 for final treatment. The diagnosis of endometrial cancer was established on the basis of the histopathology findings of the fractional curettage. Her heart transplant had occurred six months before, as a result of idiopathic restrictive cardiomyopathy. The patient received triple immunosuppression with cyclosporin, azathioprine and prednisone and she displayed no signs of acute graft rejection features. Laparoscopically assisted vaginal hysterectomy with adnexa was performed without any complications. Duration of surgery was 85 minutes. The patient was operated on under general anesthesia and intraabdominal pressure was automatically maintained at 10 mmHg with a carbon dioxide insufflator (AESCULAP, Germany). Immunosuppressive therapy was continued without modification. The postoperative course was uncomplicated in our patient. No significant changes in heart rate or blood pressure were observed. The patient was discharged from the hospital on the 11th postoperative day. Microscopic appearance revealed Stage I endometrial cancer. The patient is in good physical condition with normal heart performance and without disease recurrence. CONCLUSION: In our opinion LAVH was a justifiable form of surgical management in the treatment of a heart transplant recipient with an early-stage endometrial cancer.
