INTRODUCTION: The price and complication rates of pump models applied in the treatment of intractable ascites are significantly high. The development of a more simple, cheaper technique--using the biologically given venous valves--the saphenoperitoneal shunts became essential. TECHNIQUE AND RESULTS: In authors' department 11 saphenoperitoneal shunts have been performed on 9 patients from 01.01.2001. to 01.04.2002. After operation significant reduction in abdominal girth and increase in diuresis with diuretics in unchanged doses were observed in 6 patients decreasing their dyscomfort. In one of the patients the ligation of fistula have been needed due to peritoneal reflux, in another one severe hypoproteinaemia have developed. In 3 cases shunt occlusions have appeared: one of them have got Denver-shunt, in other case contralateral saphenoperitoneal shunt have been created with PTFE prosthesis implantation, in the third patient desobliteration have been performed with silicone drain. CONCLUSIONS: Significant improvement of quality of life would be caused by saphenoperitoneal shunts with minimal negative operative effects in successful cases. Besides this method is cost-effective, and has moderate and well corrigable hemodynamic effects.

Intensive saluresis, intended as an exceptional though real and practicable therapeutic approach, has made unimaginable progress possible in different sectors of Internal Medicine relating to hydrosaline balance alterations such as chronic and acute renal failure, nephrosic syndrome, refractory heart failure, acute pulmonary edema and ascites. We are proud to be the first who followed a path that assured patients compensation beyond "classic" therapeutic possibilities, and undoubtedly better quality of life, as well.

The effect of a 20-min infusion of atrial natriuretic factor (ANF) 99-126, 0.4 microgram/kg per min, on both urinary albumin and beta 2-microglobulin excretion was examined in nine normal male subjects during stable water diuresis. ANF caused a rise in urinary albumin in excretion (from 4.19 +/- 0.66 to 13.49 +/- 3.07 ng/min, P less than 0.01) without any significant change in either creatinine clearance of beta 2-microglobulin excretion. These findings suggest that ANF may enhance glomerular permeability to albumin in man.

The influence of bumetanide on renal excretion of water and electrolytes was measured in saline loaded rats of various ages. The relative increase in sodium excretion following bumetanide administration is more pronounced in 5 and 15 day old rats than in older animals. The lower natriuretic potency of bumetanide in adult rats is not limited by the administered sodium dose since an additional sodium load does not provoke a more pronounced sodium excretion in these animals.
The pathogenetic treatment of acute renal insufficiency (ARI) of the type of "shock kidney" started early in the disease changes the clinico-anatomical picture of ARI. Therapeutically conditioned pathomorphosis of ARI may exist. According to the data of the literature and the author's data based on morphological analysis of 400 autopsy cases of ARI, the ARI pathomorphosis consists of the following : (1) emergence of reversible ARI forms (when the treatment is started early and nephron is not deeply affected); (2) disappearance of some morphological signs of ARI (reaction of the renal tissue to cyclinders, tubulorexis); (3) appearance of unusual regeneration of nephroepithelium in deep affection of the kidneys. Renal changes associated with therapy should be regarded as pathology of ARI therapy.
A comparative test on the efficacy of bumetanide and furosemide was made in ten healthy individuals. The effect on diuresis and urinary excretion of ions and metabolites after the acute oral administration of 1 mg bumetanide and 40 mg furosemide was observed. From the results of the experiment it can be concluded that the effectiveness of bumetanide is about 15 percent superior to that of furosemide with a later but more sustained effect. The excretion of Na and K was similar with both compounds but the chloruresis was more intense with furosemide. Although the excretion of uric acid was increased too, it was proportionally less than the increase of the diuresis obtained with both drugs. A noticeable increase of the urinary excretion of calcium caused by bumetanide stands out. The authors suggest its possible therapeutic usefulness in patients with hypercalcemia as well as its possible mechanism of action on the reabsorption of calcium. In every case tolerance of both drugs was good and there were no signs of toxicity or adverse side effects. Generally speaking the doses of 1 mg bumetanide and 40 mg furosemide were judged to be equally powerful.
