[Adenoid cystic carcinoma of the anterior segment of the tongue].

The authors describe a rare case of adenoid-cystic carcinoma localized in the anterior tongue. The neoplasia is made up of epithelial and myo-epithelial cells, organized frequently in islands with cribriform aspect, was circumscribed by a thin fibrous capsule that was focally infiltrated in the proximity of the deep margin. At the physical examination no local regional lymph nodes were absent. The surgical excision and radiotherapy constituted the only therapeutic protocol. Absence of local relapse and/or distant metastasis after eight years from the operation confirm the low aggressiveness of the neoplasia when localized in the anterior part of the tongue.

Adenoid cystic carcinoma of salivary and lacrimal gland origin: localization, classification, clinical pathological correlation, treatment results and long-term follow-up control in 84 patients.

AIM: Adenoid cystic carcinoma (ACC) is a malignant tumor arising from glands. In the maxillofacial region, the salivary glands are particularly affected by ACC and, less frequently, the lacrimal glands. The aim of this study was to determine the outcome of patients with ACC in the maxillofacial region classified according to an internationally accepted staging system. MATERIALS AND METHODS: Over a period of more than 31 years, 84 patients with ACC underwent surgical treatment or a combined treatment in a University Hospital (primaries: 70; local recurrence: 13; distant metastasis: 1). RESULTS: In salivary glands the major glands were affected in 20 individuals, the minor glands in 50, while the glands of the maxillary sinus gave rise to ACC in 8. The lacrimal glands were affected in 6 patients [45 females (53.6%), 39 males (46.4%); age: 13 to 93 years, mean 55.9 years]. Reclassified TNM-stage (UICC, 1987) at the time of diagnosis varied (T0:1, T1:14, T2:13, T3:12, T4:30, NX:1, N0:53, N1:10, N2:5, N3:0, MX:5, M0:59, M1:5). The history of symptoms ranged from 1.82 to 7.3 years, depending on the localization, without any specificity of features. However, swelling and pain were the most frequently recorded findings (> 73%). The therapy of choice has to be the resection of the primary tumor with safety margins. The resection status is important for determining the local control. The resection of the related efferent lymphatics has to be included in the therapeutic concept in patients suspected of having metastasis of the regional lymph nodes. However, hematogenic spread was frequently recorded in our patients, even after several years. An excellent prognosis is only in ACC which is detected early and radically resected. Neither localisation nor the histological subtypes, but tumor stage, had statistically significant impact on prognosis. The differences in survival between surgically-treated patients and those who received radiotherapy as an adjunct were not significant. CONCLUSION: The TNM classification and the analysis of histopathological subtypes provide some information on the tumor biology. The prognosis is uncertain in ACC. Improvement of the current diagnostics and therapy hopefully will come from the development of tumor markers and future molecular genetic investigations.

[Adenoid cystic carcinoma of the breast. Case report].

We report a case of adenoid cystic carcinoma of the breast in a 62-year-old lady. This tumor is a rare histologic form of breast cancer, associated with a favorable prognosis, and a special clinical behavior. It is mainly diagnosed on morphological grounds, by identifying two populations of neoplastic basaloid myoepithelial and epithelial cells with extracellular deposits of eosinophilic basement membrane-like substance. Lymph node metastases are rare. Partial or simple mastectomy is an adequate treatment. Axillary lymph node dissection may not be necessary.

[Primary adenoid cystic carcinoma of the nasopharynx and its relation to Epstein-Barr virus infection].

OBJECTIVE: To investigate the clinicopathologic characteristics of primary nasopharyngeal adenoid cystic carcinoma (NPACC) and its relation to Epstein-Barr virus (EBV) infection in Guangzhou where is a high-incidence area of EBV-associated nasopharyngeal carcinoma (NPC). METHODS: 17 cases of NPACC with clinical record and biopsy samples were collected in Guangzhou and their clinical manifestations were reviewed. Besides HE, Alcian blue and PAS, LSAB immunohistochemistry was performed for detecting the expression of a variety of epithelial markers, CD21 and EBV encoded LMP1. EBV encoded early RNAs (EBER) was detected by using in-situ hybridization. Nested PCR was applied for studying the presence of EBV W-fragment in tissues. RESULTS: The ratio of male to female was 7:10. The patients' age ranged from 30 to 63 years, and the median age was 46 years. 14 out of 17 tumors showed markedly local aggressive growth, presenting as T3 or T4. However, only 1 patient had metastasis of an ipsilateral cervical lymph node. The majority of neoplastic cells were basal-cell like in shape and with scanty cytoplasm and a deeply stained nucleus. Intercellular hyaline or mucinous substance was always present in between the carcinoma cells. Cribriform structure formed by the neoplastic cells could be found in 16 out of these 17 biopsies. The NPACC always express the wide-spectrum cytokeratin and the epithelium membrane antigen. Only a few or a small number of carcinoma cells showed nuclear EBER-signals in 9 cases (9/17). Concurrently, these 9 NPACCs showed a 192 bp W-fragment positive band on electrophoresis gel by nested PCR. LMP1 expression had been found in 5 out of the 9 NPACCs (55.6%) accompanying with EBER-positive carcinoma cells. The EBER-positive infiltrating lymphocytes could also be found in the stroma of 3 out of the 9 EBER-stained NPACC slides. All the tumor cells, including the EBER-positive cell of the 17 NPACCs showed no CD21 expression. CONCLUSIONS: The female is predominant over the male in development of NPACC, which often accompanied with a markedly invasive capacity at the nasopharynx and its neighboring sites. Only a small number of tumor cells, nearly a half of the studied cases were infected with EBV. Therefore, it's postulated that there seems no close relation present between NPACC and EBV infection.

[ Ulcerated adenoid cystic carcinoma of the lacrimal gland--case presentation].

The authors present the case of a 63 years old patient, admitted in the Neurosurgical Department, presenting exulcerated adenoid cystic carcinoma of the right lacrimal gland. The tumor was extended medially to the superolateral part of the posterior orbit and laterally in the temporal fossa. The patient presents one and a half years history of left exophthalmos, the tumor exulcerated in the left temporal region one year ago, meantime the tumor extended in the superior half of the posterior orbit. The tumor was completely removed using superolateral orbital approach, with very good postoperative course. Complete recovery of the visual function and satisfactory aesthetic aspect was obtained, without the risks of a craniotomy. Good outcome without recurrence in the past two years.

[Cystic adenoid carcinoma of the larynx: two cases].

Adenoid cystic carcinoma is the most common malignant tumor of the minor salivary glands in the head and neck. Laryngeal localization is rare due to the poor distribution of the accessory salivary glands in this area. It arises exceptionally before 20 years and no risk factor is known. Diagnosis is a histological finding because symptoms do not differ greatly from other laryngeal tumors. Histological features, stage, quality of tumor resection and nerve invasion are prognostic factors. These tumors grow slowly and local recurrence or systemic metastasis are exceptional. Surgery is the treatment of choice but outcome is always fatal, after a long clinical course in many patients.

Fine needle aspiration biopsy diagnosis of adenoid cystic carcinoma of the breast. A case report.

A case of adenoid cystic carcinoma arising in the breast is presented. The diagnosis was established on a fine needle aspiration (FNA) biopsy sample with histopathologic corroboration. The aspirate yielded a typical smear composed of round, multilayered clusters of uniform epithelial cells arranged around cores of homogeneous, acellular material. Both the conclusiveness of FNA biopsy in diagnosing this rare mammary neoplasm and its resulting in immediate treatment are stressed.

[Adenoid cystic carcinoma of the maxillary sinuses].

OBJECTIVE: To investigate characteristic clinical features and outcomes for patients with adenoid cystic carcinoma of the maxillary sinuses (ACC). METHOD: Thirteen patients with ACC were treated with a combination of surgery and radiation. Prognostic factors were retrospectively analyzed. RESULT: The most frequent cause of failure was local recurrence at the primary site. There was no neck node failure, 1 patient experienced distant metastases after the treatment. The overall disease-free survival rate at 5 years and 10 years were 72.5% and 54.4%, respectively. CONCLUSION: The symptoms of the ACC occurred in the maxillary sinuses are not specific. The early diagnoses are usually difficult. Surgeries combined with postoperative radiation are recommended for optimal local control and survival. The prognostic factors include the clinic stages of the tumor and the resection margins.

[Adenoid cystic carcinoma of lower lobe of right lung: report of a case].

A 69-year-old male was admitted to the hospital for further examination of an abnormal shadow in the right lower lung fields. He was previously under medical treatment for right thoracic empyema. Chest computed tomography (CT) showed a solitary mass, 4.5 cm in diameter and broncofiberscopy evidenced a tumor in the right lower bronchus. The biopsy was performed and the tumor was diagnosed as a pleomorphic adenoma. Intraoperativefinding showed the tumor was 6 cm in gross, extended to the left atrium, and a daughter tumor was palpable in the middle lobe. The middle and lower lobe were resected. The tumor was located in S9, S10, 6 x 4 x 3.5 cm in size, 2 daughter tumor was found in the middle lobe, the pulmonary vein was thickened by tumor invasion. Pathohistologically, main tumor and daughter tumor showed malignant feature, were compatible with adenoid cystic cancer. Four years after operation, he is still now alive with home oxygen therapy.

Adenoid cystic carcinoma of the breast.

This study presents a series of eight patients with adenoid cystic carcinoma of the breast, a rare variant of breast cancer with special clinical and radiological characteristics that may lead to clinical misdiagnosis. Diagnosis may be made by core biopsy or fine needle aspiration cytology. Treatment by wide local excision followed by radiotherapy or mastectomy is necessary but axillary dissection can be avoided. Most of these patientshave an excellent prognosis.

Postoperative adjuvant radiotherapy for adenoid cystic carcinoma of the head and neck: treatment results and prognostic factors.

BACKGROUND: To analyze the treatment results and prognostic factors of patients with adenoid cystic carcinoma (ACC) arising from the major and minor salivary glands of the head and neck, and who received postoperative radiotherapy after radical tumor resection. METHODS: Between October 1987 and December 1999, 25 patients with ACC of the head and neck were treated with radical surgical resection and postoperative adjuvant irradiation without chemotherapy. Using appropriate radiation techniques to the tumor bed, total doses ranged from 44 to 68.4 Gy. All patients had a minimum of 2 years of follow-up. RESULTS: The 5-year overall survival rate, local control rate, and distant metastasis-free rate were 75%, 83%, and 66%, respectively. To the present, 2 patients had experienced recurrence at the primary sites, and 1 had regional lymph node metastasis. Distant metastases developed in 7 patients (28%), of whom 6 (86%) were disease-free at the primary site. The lung was the most common distant metastatic site. In multivariate analysis, the only statistically significant prognostic factor for a distant metastasis-free rate was the stage at presentation (p = 0.009). CONCLUSION: Patients with an advanced stage of disease had higher distant metastasis rates even when receiving postoperative radiotherapy. Distant metastasis is still the main problem in the management of ACC of the head and neck. However, more-effective treatment for this problem is still lacking.

[The study of apoptosis of salivary adenoid cystic carcinoma in nude mice].

OBJECTIVE: To study the feature of apoptosis of salivary adenoid cystic carcinoma (SACC) induced by recombined human tumor necrosis factor-alpha (rhTNF-alpha) in nude mice, and to evaluate the related genes expression of apoptosis. METHODS: Twelve SPF grade 4 approximately 5 weeks old female Balb/c nude mice were selected in this study. SACC-83 cells were collected to 6 x 10(7) per milliliter and injected subcutaneously. Group A and B were experimental group which was given 100 x 10(4) IU/kg TNF-alpha or 10 x 10(4) IU/kg TNF-alpha respectively. Group C was only given normal saline and used as normal control. The investigations were adopted by using both light and transmission electron microscope (LM and TEM), flow cytometer and In Situ Cell Death Detection Kit. The evaluations of bax and bcl-2 expression were utilized by immunohistochemistry. RESULTS: The percentage of apoptosis of transplanted tumors was much higher than that of the control (P<0.01). Apoptotic cells were calcified and grit bodies were formed. Apoptotic cells expressed and contained significantly higher proportions of both bax and bcl-2 proteins (P<0.05). CONCLUSIONS: It is suggested that calcification may be the obvious feature and the last outcome of the apoptosis of SACC transplanted tumors. Apoptosis induced by TNF-alpha can increase the expressions of bax and bcl-2.

[Cystic adenoid carcinoma of the trachea].

Cystic adenoid carcinoma (CAC) of the trachea, also called cylindroma, is a rare malignant tumor accounting for 1 per thousand of all respiratory tract cancers. We report three cases of cylindroma in patients hospitalized for inspiratory dyspnea due to a tracheal tumor. Bronchoscopy using a rigid tube revealed a smooth tumor of the trachea. Pathology analysis of the biopsy specimen diagnosed CAC. The three patients underwent resection with end-to-end anastomosis. Adjuvant radiotherapy was given in one patient. The short-term outcome was good but one patient developed local recurrence and lung metastasis at six years.

[Gene expression differences between high and low metastatic cells of adenoid cystic carcinoma].

OBJECTIVE: To investigate the complex differences between high metastatic and low metastatic cells of the Adenoid cystic Carcinoma. METHODS: Gene expression patterns were examined in high metastatic cell ACC-M strain and low metastatic ACC-2 strain with the method SSH (suppression subtractive hybridization). RESULTS: although extensive similarity was noted between the expression profiles, twelve genes were highly expressed of, in low metastatic cell ACC-2 tester, compared with driver, high metastatic cell ACC-M. These genes were cysteine-rich angiogenic-inducer protein (cyr61), chromosome7 clone RP11-52501, G protein, was family member Iferritin heavy polypeptide I, jumping translocation breakpoint, eukaryotic translation elongation, folate receptor, ribosomal proteins L7a, S21, P0 and other two novel genes-ACC metastasis-associated RNH and ACC metastasis-associated suspected protein. GenBank accession number were AF522024 and AF522025 respectively. CONCLUSIONS: the result suggests that the obtainment of the ability of metastasis is related to the low expression or mutation of these genes. These data provide insight into the extent of expression differences underlying metastasis-related genes that may prove useful as diagnostic or prognostic markers.

Adenoid cystic carcinoma with hyperostosis after stereotactic radiosurgery.

Adenoid cystic carcinoma (ACC) of the head and neck is a relatively rare tumor characterized by slow evolution, recurrences, and protracted clinical course. We performed stereotactic radiosurgery in order to palliate symptoms for intraorbital recurrence of ACC that had been treated with total resection of the left parotid gland, radical neck dissection, and conventionally fractionated radiotherapy. We experienced interesting hyperostosis in the area irradiated by the stereotactic radiosurgery.

[Combining of TNP-470 and 5-Fu in inhibition of adenoid cystic carcinoma in nude mice model].

OBJECTIVE: To study the effect of angiogenesis inhibitor and its combine with chemical drug in suppressing the growth of adenoid cystic carcinoma (ACC). METHODS: Acc-M cells were inoculated subcutaneous into BABL/C nu/nu mice. The mice were divided into control, different dose of TNP-470 treatment groups, 5-Fu treatment group and TNP-470 plus 5-Fu treatment group. Treatments were given 48 hours after inoculation. The mice were sacrificed on the 22nd day and excised tumors were weighted. Tumors were also investigated by immunohistochemistry and ultrastructural observations. RESULTS: TNP-470 100 mg/kg/qod efficiently inhibited the growth of Acc-M tumors. TNP-470 30 mg/kg/qod combined with 50 mg/kg/week 5-Fu also resulted in significant growth inhibit of the tumors. TNP-470 suppressed tumor growth by inhibiting neovascularization, therefore inducing apoptosis of Acc-M cells. All experimental groups had different degrees of VEGF and bFGF express. CONCLUSION: Since ACC is a slow developing tumor, blood supply is not so sufficient as sarcomas. Angiogensis inhibitor may inhibit its growth in high dosage. Combining medium dosage of angiogensis inhibitor with chemical drug may have synergistic result in inhibiting ACC growth.
