[Sodium regulation disorder, hypothermia, and circadian rhythm disturbances of the body temperature and sleep-wakefulness as sequelae of acute subdural hematoma].

We reported an 11-year-old boy who suffered from transient hypernatremia, hypothermia, and circadian rhythm disturbances of sleep-wakefulness and body temperature from the age of 4 years, as sequelae of acute subdural hematoma. T1-weighted magnetic resonance imaging (MRI) of the brain revealed low intensity consistent with necrotic change in the whole left cerebral hemisphere, hypothalamic region, and the right-sided brain stem including tegmentum, while the pituitary structure was well preserved. Anterior pituitary function was almost normal. ADH (antidiuretic hormone) was neither stimulated by hyperosmolality nor suppressed by hyposmolality but continued to be secreted at almost constant level approximating the normal basal state. This pattern seemed to be due to complete destruction of the osmoreceptor located in the anterior hypothalamus. He exhibited a dispersed-type sleep with differentiated stages of NREM (non-rapid eye movement), although the percentage of sleep was higher at night than in the daytime. It is suggested that circadian rhythm of sleep-wakefulness and differentiation of NREM sleep stages are regulated by different neuromechanisms. Brain stem lesion on MRI may be connected with the pathogenesis of the dispersed-type sleep with special respect to amplitude reduction of sleep-waking circadian rhythm. Circadian rhythm of body temperature (BT) was irregular in amplitude, phase, and period without synchronization with sleep-wakefulness rhythm. Hypothermia was also demonstrated at the basal state, while BT increased when he suffered from respiratory infection. It is likely that hypothermia in our case is caused by the BT shift to the lower side due to malfunction of BT integrating system including preoptic area and anterior hypothalamus.(ABSTRACT TRUNCATED AT 250 WORDS)

[Risk factors and prognostic variables in transient ischemic attacks and in infarctions with minimal sequelae. A multivariate comparative study].

BACKGROUND: There are no clinical differences accounting for a distinction between transient ischemic attack (TIA) and cerebral infarct with minimal sequelae (IMS). The aim of the present study was to evaluate whether there are diverse risk factors involving a different prognosis in these two groups. METHODS: 144 patients with TIA and 110 with IMS of atherothrombotic origin were prospectively controlled during a mean period of 21 months. Risk factors in both groups were compared with the calculation of the odds ratio (OR) and its 95% confidence intervals (95%CI). The prognosis of the series was evaluated with survival curves, considering as end points the development of recurrences or cardiovascular death. RESULTS: In the TIA group there was a higher rate of previous TIA (OR = 3.6; 95%Ci = 1.5-8.6) and a smaller prevalence of hypertension (OR = 0.45; 95%CI = 0.27-0.76). There were no significant differences between both groups regarding the probability of survival without TIA or cerebral infarction. Cox's proportional risk analysis selected the stenosis of supraaortic arterial trunks and hyperglycemia as the only factors with independent prognostic significance. CONCLUSIONS: These results do not support the classification of reversible ischemic attacks into TIA and IMS. Both groups have similar risk factors with a similar prognosis.

[Cerebral tomodensitometry (scanner) and accident sequelae: its medico-legal value].

The authors report their experience of one years use of a cerebral scanner in the investigation of the sequelae of head injuries, and the medico-legal interest of this discovery. It permits one to visualise the pathological processes responsible for the symptoms following head injuries and its is thus of major intetrest in legal medicine where the subjective factor in these symptoms is difficult to assess. Increasing considerably the proportion of definite etiologies in this pathology, the authors believe that this new method will become standard in the medico-legal assessment of head injuries.

Neuropsychological sequelae of brain tumors.

Investigation of the neuropsychological sequelae of brain tumors is extremely complex largely because the neurobehavioral consequences of brain tumors depend upon complex interactions among disease and treatment variables as well as patient characteristics. To illustrate some of these complexities, we present case studies of two patients in whom the behavioral outcome was not easily predictable on the basis of our current understanding of brain-behavior relationships in tumor patients. The case studies do illustrate how neuropsychological evaluation aids in identifying cognitive deficits which have implications for subsequent quality of life. Recommendations for future experiments and statistical analyses of neurobehavioral data of this population are given.

Skeletal sequelae of radiation therapy for malignant childhood tumors.

One hundred forty-three patients who received radiation therapy for childhood tumors, and survived to the age of skeletal maturity, were studied by retrospective review of oncology records and roentgenograms. Diagnoses for the patients were the following: Hodgkin's lymphoma (44), Wilms's tumor (30), acute lymphocytic leukemia (26), non-Hodgkin's lymphoma (18), Ewing's sarcoma (nine), rhabdomyosarcoma (six), neuroblastoma (six), and others (four). Age at the follow-up examination averaged 18 years (range, 14-28 years). Average length of follow-up study was 9.9 years (range, two to 18 years). Asymmetry of the chest and ribs was seen in 51 (36%) of these children. Fifty (35%) had scoliosis; 14 had kyphosis. In two children, the scoliosis was treated with a brace, while one developed significant kyphosing scoliosis after laminectomy and had spinal fusion. Twenty-three (16%) patients complained of significant pain at the radiation sites. Twelve of the patients developed leg-length inequality; eight of those were symptomatic. Three patients developed second primary tumors. Currently, the incidence of significant skeletal sequelae is lower and the manifestations are less severe than reported in the years from 1940 to 1970. The reduction in skeletal complications may be attributed to shielding of growth centers, symmetric field selection, decreased total radiation doses, and sequence changes in chemotherapy.

[Selected problems of diagnosis, natural course and prognostic significance of borderline arterial hypertension. II. Sequelae of borderline arterial hypertension].

In 490 working men, aged 40-59 years the sequelae of borderline arterial hypertension (BH) were evaluated. The patients with BH (n = 253) were matched with normal controls (n = 237). At 13 years the cardiovascular state, ability to work and mortality were assessed. A significantly more frequent incidence of left ventricular hypertrophy and overload was found when hypertension developed. In the group of initial BH less men continued work (36%) than in the control group (48.1%, p less than 0.01), despite the same age. A relation between the ability to work and blood pressure becomes more prominent while taking into account final diagnosis; i.e. 65.7% of actual normotensives, 56.4% of the patients with BH and 47.5% with hypertension continue their work. The mortality rate was higher (15.4%) in the group with BH than in the controls (8.4%, p less than 0.01). The highest death rate was found when BH was combined with other coronary risk factors. The results confirm the presence of both indirect (through the development of arterial hypertension) and harmful effects of BH. They also show the need of devoting special attention to patients with BH and concommitant coronary risk factors.

[The sequelae of tuberculosis].

During the 20th century, tuberculosis has been the most prevalent and most harmful disease in Japan. Enormous medical researches have ever been performed to conquer the disease. Nevertheless tuberculosis has left various somatic and psychological residues on vast convalescents. On the other hand, researches to conquer tuberculosis have made considerable contribution to other fields of medicine. 1. Somatic and psychological residues on convalescents from tuberculosis. Chest x-ray findings, cardio-pulmonary disturbance, secondary infection, serum-hepatitis due to mass transfusion during the chest surgery, streptomycin-deafness and psychological disorder. 2. Sequelae of phthisiology. a. In the field of basic medicine. Respiratory physiology, immunology and genetic pharmacology. b. In the field of epidemiology. Methodology to control the disease. c. In the field of clinical medicine. Chest x-ray diagnostics, bronchoscopy, thoracoscopy, randomized controlled trial, regimens of chemotherapy, open chest surgery, anesthesiology, treatment of respiratory failure, informed consent, terminal care and cooperative study system. d. In the field of rehabilitation. Medical, vocational and social rehabilitation of the handicapped. e. In the field of public health. Comprehensive control system of the chronic disease. Smallpox has been eradicated, but the elimination of tuberculosis is still far away. Studies as excellent as past ones should intensively be carried out.

[Surgery in sequelae of chemical and thermal burns of the eye].

The treatment of sequelae of chemical and thermal burns of the eye is sometimes very difficult. Previous to a event, keratoplasty all symblephara have to be removed. If there is no conjunctiva, transplantation from the second eye or from a donor is necessary. A method of transplantation of donor conjunctiva with a corneal lamella has been worked out as preparation for keratoplasty. Epithelium than can migrate to the transplant. In quantitative or qualitative alterations of the tears, the condition can be improved by soft contact lenses. In about one third of serious burns longtermed good results can be gained.

[Surgical treatment of the sequelae of ischemic postdysplasia femur head necrosis in children. II. Surgical technique].

The author presents its concept concerning the possibilities of the surgical treatment of the sequelae of the postdysplastic necrosis of the femoral head originating from the treatment of congenital dislocation or subluxation of the hip joint. The classification is based on the division according to Bucholz and Ogden into 4 types. Indicated for the surgical treatment are hips with the injury of the type II and III. In Grade II type the joint congruence is not affected and the method of operation is the redirection pelvic osteotomy. In children up to the age of 10 the author applies Salter osteotomy and in elder children and adolescents he performs a triple osteotomy of pelvis. Grade III type is the most severe impairment because of the concurrent defect of the joint congruence, limb discrepancy and extensive anatomic changes of the proximal end of femur. Up to the age of 10 he recommends Salter innominate osteotomy with the simultaneous valgus osteotomy, in elder children he recommends successively a triple pelvic osteotomy and a double intertrochanteric valgus osteotomy. This facilitates a considerable improvement of most hip joints affected in this way and at the same time the compensation of the limb length.

[Diffuse peritonitis and its late sequelae].

The author gives an account of the late complications in patients operated at the surgical department of the District Institute of National Health Michalovce on account of diffuse peritonitis in 1966-1975. A total of 258 patients were examined who attended from a total number of 505 invited, operated during the ten-year period under investigation. The high incidence of chronic sequelae and their serious character must be the reason for seeking steadily reserves in surgical treatment of diffuse peritonitis as well as during postoperative care.

[Usefulness of demand oxygen delivery system for patients with chronic respiratory failure due to mainly tuberculosis sequelae].

To evaluate the usefulness of a new oxymatic conserver, Demand Oxygen Delivery System (DODS), we compared DODS breathing with Standard steady flow (SF) breathing in thirteen subjects with chronic respiratory failure due to mainly tuberculosis sequelae. The value of the DODS (Oxymatic) is that it delivers oxygen only during early inspiration, so as to minimize loss from delivery during expiratory phase. Improvement of SaO2, measured by BIOX 3740, and PaO2 were observed at rest and on exercise. The oxygen consumption ratio of the DODS to the SF method was between 0.5 and 0.3, favoring the DODS over the SF method. In some patients DODS hardly ran at rest. But no problems are observed during exercise. The results indicate the effectiveness of DODS in advancement of quality of life patients with chronic respiratory failure.

[Neonatal meningitis: observations on its etiology, mortality and sequelae].

A retrospective study was carried out on all cases diagnosed with neonatal meningitis at the Hospital Infantil de México Federico Gómez. A rate of 9.6 cases per 1,000 discharged patients was found as well as 6.7% association with sepsis. In 50 of the cases analyzed, an etiologic agent was identified in 23 children; the main bacteria identified were Escherichia coli (24%), Klebsiella pneumoniae (14%), Enterobacter (4%), Proteus mirabilis and Pseudomonas sp (2%), respectively. The mortality rate was 60% and sequelae were seen in 13 of the surviving 20 patients. The unfavorable prognosis of neonates with meningitis forces us to establish an early diagnosis, make every effort to identify the causing agent and try new medications as well as co-adjuvant treatments.

Hemorrhagic sequelae of immune thrombocytopenic purpura in human immunodeficiency virus-infected hemophiliacs.

Clinical bleeding tendency and tests of immune function were studied prospectively in 11 human immunodeficiency virus (HIV)-infected hemophiliacs with immune thrombocytopenic purpura (ITP) and a platelet count less than 50,000/microL. These 11 patients represented 13% of a well-characterized cohort of 87 HIV + hemophiliacs. ITP developed a mean 3.5 years after seroconversion, mean platelet count at presentation was 36,000/microL (range 15,000 to 49,000/microL), and the mean age at seroconversion was 37.1 years. Nine patients (82%) suffered bleeding complications, including four with intracranial hemorrhage, which was fatal in three. At the onset of ITP, five had AIDS and six were asymptomatic. Mean T4 lymphocyte count at onset of ITP was 126 +/- 32/microL (range 5 to 267/microL). Sustained treatment responses occurred with intravenous gammaglobulin (2 of 2), one of whom spontaneously remitted, and with zidovudine (1 of 2), but not with steroids (0 of 6) or danazol (0 of 3). In conclusion, 13% of a cohort of HIV + hemophiliacs has developed ITP with platelets less than 50,000/microL, a significant proportion of whom (82%) have experienced bleeding complications. It is recommended that treatment for ITP in HIV + hemophiliacs be instituted once the platelet count falls below 50,000/microL in order to avoid serious hemorrhagic sequelae.

[An extragonadal retroperitoneal endodermal sinus tumor--results of treatment and treatment sequelae in a 3-year-old boy].

A case of an extragonadal endodermal sinus tumour in a three-year old boy is presented. Complete resection of the retroperitoneal tumour was possible after initial chemotherapy. Combined radio-chemotherapy after tumour removal led to complete remission now lasting for more than 5 years. There was unilateral renal atrophy and diverticulum of the stomach as treatment related complications. The favourable prognosis of this tumour due to intensified treatment is discussed in relation to treatment sequelae. The interdisciplinary approach in treatment of cancer in children is emphasized.
