On the differences in psychotropic drug use between the three major urban areas in Sweden.

Marked differences in the utilisation of psychotropic drugs between the three major urban areas in Sweden were recorded from four sources of information: drug supplies from wholesalers to pharmacies, drug supplies to hospitals for in-patient use, drugs sold on prescription for out-patient use, and out-patient consultation and drug prescribing as recorded by physicians. The total sales of psychotropics in the counties of Gothenburg (110.8 defined daily doses per 1000 inhabitants per day) and Malmö (102.1) were much higher than in the county of Stockholm (73.4), with about 25% of the difference being accounted for by diazepam. Differences in the total sales of psychotropics were not explained by any differences in hospital sales, which amounted to about 10% in all counties. Prescription sales differed due to the higher average number of DDD (defined daily doses) per prescription in Gothenburg and Malmö than in Stockholm (total psychotropics 8 and 15%, respectively), and especially because of the higher number of prescriptions per inhabitant (about 40 and 30-35%, respectively). There was no substantial difference in the pattern of diagnoses between areas, but there was a noticeable difference with regard to prescriber category, as psychiatrists accounted for more of the prescriptions in Stockholm than in Gothenburg and Malmö. The results raise questions about over- and under-treatment of mental disorders and about abuse of drugs. In order to explain the geographical differences in psychotropic drug sales morbidity patterns and prescribing practices should be further explored.

Incidence of colorectal cancer and all cause mortality in non-selected patients with ulcerative colitis and indeterminate colitis in Malmö, Sweden.

Cancer morbidity and all cause mortality were studied prospectively in all patients with definite and probable ulcerative colitis and indeterminate colitis diagnosed from 1958 to 1982, in the city of Malmö, Sweden. The follow-up to Jan. 1, 1990 was complete for all but ten patients. Nine of the 471 patients with ulcerative colitis and three of the 100 patients with indeterminate colitis developed colo-rectal cancer. The incidence of colorectal cancer in ulcerative colitis was 1.4 per 1000 person-years. The observed number of cases was 2.1 times higher than expected; (95% C.I. 1.0-4.1), based on the age- and sex-specific cancer incidence in the city during the study period. Indeterminate colitis was associated with a higher colorectal cancer risk than ulcerative colitis; 2.4 per 1000 person-years; (SMR 8.6, 95% C.I. 1.8-25.1). Both conditions were associated with a slight increased mortality rate, for ulcerative colitis 12.6 per 1000 person-years; (SMR 1.3, 95% C.I. 1.0-1.5), and for indeterminate colitis 11.7 per 1000 person-years; (SMR 2.7, 95% C.I. 1.6-4.4). Complications of colitis were the main cause of death in both groups. The cancer risk was related to extent of disease, duration of disease and female gender. Ten out of the 12 cases with cancer had or developed total colitis. However, only seven of the 134 cases with total ulcerative colitis and two of 87 cases with total indeterminate colitis developed cancer.(ABSTRACT TRUNCATED AT 250 WORDS)

Silicosis in the iron-ore mine in Kiruna, Sweden, and the future need for silicosis control.

From 1931 to 1977 a total of 144 cases of silicosis have been diagnosed in the iron-ore mine in Kiruna, Sweden. Drilling, loading and tapping caused all cases of silicosis. In 24% of the cases, the disease had progressed after the diagnosis. The progression of the disease after diagnosis showed no significant correlation to the length of the exposure but a dose-response relationship was present between the cumulative quartz exposure and the stage of silicosis 20 years after the diagnosis. Signs of tuberculous infection were found in 17%. There was a significant relationship between tuberculosis and progression of the silicosis. Mortality was increased in association with silicosis stage II-III and in stages with concomitant tuberculosis infection. Half of the cases of silicosis had been diagnosed before 1942, and after 1960 no more cases in stages II or III were diagnosed. The mean concentration of respiratory quartz was approximately 0.8 mg/m3 in the 1950s and early 1960s and decreased progressively to below 0.05 mg/m3 in the late 1970s. The cumulative incidence rate, with respect to the decade in which the exposure began, was 0.021% in 1951-1960. With an unaltered dose-response, less than one case of silicosis per 500 workers may be expected among those who start working underground in the 1980s. Evidently silicosis is no longer a major health risk among those who start working underground in this mine today, and it is recommended that the 40-year-old regulations for the medical prevention of silicosis be revised.

Animal consumption of antibiotics and chemotherapeutic drugs in Sweden during 1980, 1982 and 1984.

The total animal consumption of antibiotic and chemotherapeutic drugs in Sweden during 1980, 1982 and 1984 has been studied. The study also included coccidiostatic, antiparasitic and antimycotic drugs. The total animal consumption during the years mentioned was 66.1, 68.8 and 72.6 tons of active substance, respectively, and the amount of antibacterial drugs for therapy was 28.7 (43.4%), 29.1 (42.5%) and 34.1 (47.0%) tons, respectively. Tetracycline, penicillin, sulphonamide and aminoglycoside were the four major groups of antibiotics used for therapy of bacterial infections in animals. The consumption was related to the metabolic bodyweights of animals which were the potential consumers. A comparison was also made with the corresponding human consumption during 1980. The data on antibiotic consumption can be divided by county and compared to the animal population and disease situation in each county. This study thus offers a good base for further investigations on different aspects of the use of antibiotics.

Male predominance of type 1 (insulin-dependent) diabetes mellitus in young adults: results from a 5-year prospective nationwide study of the 15-34-year age group in Sweden.

The incidence of diabetes mellitus in Sweden in the 15-34 year age group was prospectively studied on a nationwide basis, beginning 1 January 1983. A total of 1,214 male and 720 female cases of newly-diagnosed (excluding gestational) diabetes were reported over a 5-year period. This corresponds to an incidence of 20.5 per 100,000/year in male subjects and 12.7 per 100,000/year in female subjects. Most cases were classified as Type 1 (insulin-dependent) diabetes, with an incidence of 15.9 in males and 8.6 in females. The incidence of Type 1 diabetes decreased gradually with age, while the incidence of Type 2 (non-insulin-dependent) diabetes increased. A male predominance was found in all age groups, with a male-to-female ratio of 1.8:1 for Type 1 diabetes and 1.3:1 for Type 2 diabetes. Maximum blood glucose concentration at diagnosis was significantly higher in males than in females in both Type 1 and Type 2 diabetic subjects. In contrast, the percent desirable weight was significantly higher in females, both in Type 1 and Type 2 diabetic subjects. The difference in diabetes incidence therefore cannot be attributed to any methodological error. The present finding of a marked male predominance after puberty in Type 1 diabetes in an ethnically quite homogeneous population supports the hypothesis that environmental risk factors and life-style are important for the development of the disease.

Recording of drug prescriptions in the county of Jämtland, Sweden. III. Drugs presented for blood donors in a 5 year period.

Drug prescriptions in the county of Jämtland, Sweden, have been examined in order to estimate drug exposure of blood donors. During the period 1970-74 a donor group (n = 292) purchased significantly more drugs than non-donors matched for age, sex and residential area in the county. This was true for most drug categories, except for cardiovascular and endocrine drugs and iron preparations. The actual consumption of the drugs obtained was not investigated. Prospective studies are required to assess the clinical consequences of the surprisingly high drug exposure of blood donors.

Over-the-counter sales of drugs in Sweden 1976-1983.

The over-the-counter (OTC) sales of drugs were estimated by subtraction of sales on prescription and to hospitals from total drug sales in Sweden during the period 1976-1983. Increased sales of antitussives and decreased sales of expectorants characterized cough remedies. Making topical nasal decongestants available as OTC drugs immediately resulted in a large and increasing volume of OTC sales. Among analgesics, sales of acetaminophen and acetylsalisylic acid + caffeine combinations increased, while those of other groups decreased. Drugs for disorders of the alimentary tract were second to analgesics in the numbers of packs sold; amongst them antacids and laxatives dominated. Laxative sales decreased due to decreased sales of stimulant preparations. Vitamin C and multivitamin preparations dominated OTC vitamin sales. The sales of topical corticosteroids, only available as OTC drugs during the last quarter of 1983, were substantial. The OTC drug sales statistics reflect patterns of self-medication and may be used to monitor this important part of health care.

Test characteristics of the Hopkins Symptom Check List-25 (HSCL-25) in Sweden, using the Present State Examination (PSE-9) as a caseness criterion.

The Hopkins Symptom Check List-25 (HSCL-25) is a screening instrument designed to identify common psychiatric symptoms. It has been widely used in different settings outside Sweden and also compared to assessment of psychiatric illness made by general practitioners. The aim of the present study was to validate the HSCL-25 against a psychiatric interview using the Present State Examination (PSE-9) in a Swedish sample of patients in general practice. Validity coefficients of the HSCL-25 were calculated for two different thresholds of caseness, > or = 1.55 and > or = 1.75, respectively. When > or = 1.75 was chosen as a cutoff point, the validity coefficients obtained by the HSCL-25 in this study were comparable to those obtained in other studies.

Metal contents in liver tissues of non-fledged goldeneye, Bucephala clangula, ducklings: a comparison between samples from acidic, circumneutral, and limed lakes in south Sweden.

The risk of increased exposure to metals for a vertebrate predator foraging on aquatic insects in acidified lakes was investigated through analyses of the content of Al, As, Ca, Cd, Cu, Fe, Hg, Mn, Pb, Ru, Se, and Zn in the liver tissue of 42 non-fledged Goldeneye, Bucephala clangula, ducklings from acidic, circumneutral, and limed lakes in South Sweden. No indications of significantly different concentrations in samples from acidic lakes in comparison with circumneutral lakes were detected for any metal, but among individual birds from acidic lakes there were high contents of Hg. Liming probably affects the exposure to some metals; lower contents of Hg were recorded from limed lakes in comparison with acidic lakes, while the reverse trend was recorded for Cd and Mn.

Production of tremorgenic mycotoxins by isolates of Aspergillus fumigatus from sawmills in Sweden.

One hundred and six strains of A. fumigatus were isolated from 21 sawmills in Sweden, and 73 of these strains were examined for production of fumitremorgen B and verruculogen (tremorgenic mycotoxins) on YES-medium using thin layer chromatography (TLC). Twenty-three strains (32%) were tremorgen producers and 50 strains (68%) were non-producers. Tremorgenic mycotoxins were detected in conidia of seven A. fumigatus strains. The amount of toxin varied between 0.6-8.0 microgram/10(8) conidia (mean value 2.3 micrograms/10(8) conidia, equivalent with 0.18%). No production of the mycotoxin gliotoxin was detected in 6 strains of A. fumigatus. No tremorgens were detected during mould growth on wood substrates, in spite of the use of different wood species (Scots pine, Pinus sylvestris: Norway spruce, Picea abies and birch, Betula spp.), dried versus non-dried wood, bark (pine), leached wood, and wood after various sterilization methods.

A reconstruction of the population of north Italy from 1650 to 1881 using annual inverse projection with comparisons to England, France, and Sweden.

"North Italy annual population and vital rates are reconstructed from 1650 to 1881 using series of vital event indices from many rural parishes and cities. Inverse projection is applied to the reconstructed series of vital events and population to generate annual age distribution, gross reproduction rate, net reproduction rate, life expectancy at birth, and infant mortality rate. The results are compared with official sources and detailed demographic rates produced by annual inverse projection using data from England, France, and Sweden. Over the long term, North Italy is generally characterized by stagnant and relatively high mortality. Fertility and nuptiality are relatively high at the beginning and at the end of the period." Human fatalities caused by wasp and bee stings in Sweden.

Bee and wasp stings represent an uncommon cause of death. In a retrospective study of all fatalities due to venomous animals in Sweden during a 10-year-period, we found 19 fatalities due to wasps, 1 due to bees but none due to snake bite. This gives an annual incidence of 0.2 per million inhabitants. Most of the deceased were older than 50 years and had an underlying cardiovascular disease while previous severe reactions after insect stings were uncommon. The number of stings was not a factor of importance. Symptoms and death occurred within minutes after the sting. The autopsy findings were in most cases non-specific. Although uncommon, the possibility of a fatal insect sting should be considered in unwitnessed deaths occurring outdoors in summertime.

Seroconversion to hepatitis C virus antibodies in patients with acute posttransfusion non-A, non-B hepatitis in Sweden.

Seventy-four patients in 1978 and 316 in 1986, all transfused during open-heart surgery in Stockholm, Sweden, were studied prospectively for the development of posttransfusion non-A, non-B (NANB) hepatitis, seroconversion to hepatitis C virus antibodies (anti-HCV) (C-100), time lag to seroconversion to anti-HCV and outcome of posttransfusion NANB/C hepatitis. Anti-HCV was tested up to six months after transfusions in patients from 1978 and up to one year after transfusions in patients from 1986. Fifty-four percent of the patients who developed posttransfusion NANB hepatitis seroconverted to anti-HCV, 7/15 (47%) in 1978 and 8/13 (62%) in 1986. Four (27%) of the 15 patients who seroconverted to anti-HCV were anti-HCV reactive within one week, 12 (80%) within eight weeks and all within 18 weeks after the onset of hepatitis. The ELISA optical density/cut-off (OD/CO) ratio was above 4.0 in all patients with hepatitis C who seroconverted. One transfused patient with normal serum aminotransferase levels throughout follow-up seroconverted after six months. He had a temporary positive anti-HCV reactivity with a maximal ELISA OD/CO ratio for anti-HCV of only 1.2, which became negative three years later. Development of chronic hepatitis was noticed in 9/15 (60%) patients who seroconverted to anti-HCV and in 5/13 (38%) patients with posttransfusion NANB hepatitis who did not seroconvert.

Determinants of short-term fluctuations in nuptiality in Sweden, 1751-1913: application of multivariate ARIMA models.

"This study examines the determinants of short-term fluctuations in nuptiality in Sweden from 1751 to 1913, using ARIMA [Auto Regressive Integrated Moving-Average] models.... The following general patterns prevailed throughout the period 1751-1913: both better economic conditions (good harvests and lower rye prices) and higher mortality (leading to more remarriages, and enabling first marriages through inheritance) were followed by higher nuptiality. In addition, emigration of single persons after 1850 was inversely related to nuptiality." (SUMMARY IN FRE)

Age-specific mortality and short-term changes in the standard of living: Sweden, 1751-1859.

"The response of mortality to short-term changes in real wages is analyzed here not just in its own right but more particularly as an indicator of long-term shifts in the general standard of living. It is hypothesized that the response would have been stronger the lower the standard of living. The relationship between age-specific mortality levels and real-wage series for Sweden 1751-1860 is analyzed using a distributed-lag model and spectral analysis. The results suggest a real shift in the material standard of living during the period." (summary in FRE)

Reduced risk of breast and endometrial cancer among women with hip fractures (Sweden).

To test the hypothesis that osteoporosis, which results partly from long-term estrogen deficiency, is associated with a lowered risk of breast and endometrial cancer, a population-based cohort study was performed in Sweden. Some 18,000 women were followed through record-linkages after a first hip fracture, on average for almost six years, to ascertain the incidence of breast and endometrial cancer. Expected numbers were derived from the general population. Overall, 253 cases of breast cancer were observed cf 300 expected (standardized incidence ratio [SIR] = 0.84; 95 percent confidence interval [CI] = 0.74-0.95). Forty-eight cases of endometrial cancer were found cf 55 expected (SIR = 0.87, CI = 0.54-1.16). There was no clearcut pattern of breast cancer risk by age at diagnosis of hip fracture, follow-up time, or calendar period. Our results are consistent with the a priori hypothesis that long-term estrogen deficiency is associated with a reduced risk of developing breast cancer as well as endometrial cancer.
