Strengthening the capability of family health leaders for sustainable community-based health promotion.

The Family Health Leader (FHL) Project was initiated in 1997 to encourage self-care and health promotion. Since then, there has been no retraining. This study aimed to strengthen the FHLs' capability to sustain community-based health promotion and network establishment. The study, of a quasi-experimental design, was conducted in a village in Phan Thong district, Chon Buri Province, Thailand. The intervention emphasized enhancing the FHLs' knowledge, ability, leadership and motivation to advance health promotion activities. Before implementing the main intervention, 5 community health workers were trained as facilitators to strengthen 36 FHL capabilities. The curriculum and manual for training facilitators and FHLs were also developed. The intervention for strengthening FHLs' capabilities lasted for 7 months using participatory training. A within-subject repeated ANOVA was used to measure changes in the main outcome variables immediately, and at three and six months after the intervention. A qualitative methodology was utilized to assess network establishment. The results indicate that the FHLs' knowledge of self-care and health promotion, ability, leadership and motivation had increased significantly after the intervention (p < 0.001). The FHLs also sought to extend their network by involving both the community committee and the local authorities. The intervention appeared to be successful in strengthening the FHLs' capabilities to sustain health promotion within the community, and it established networks vertically and horizontally. The FHLs were the key persons to bring good health to family members. Periodical participatory learning and group empowerment are recommended for encouraging the FHLs to maximize their potential for family self-care and health promotion.

Social, economic and policy issues in the long-term control of HPAI.

Prevention and control of HPAI in Asia is a long term problem with important economic and policy consequences. The macro level impact of a single national outbreak is greatest for an exporting country, ranging in 2003-5 from dollars 65 million to over dollars 400 million. No estimates are available for the impact of market disruption if endemic disease changes the pattern of regional and international trade. In countries with minimal exports, the total financial impact may be much smaller, but there can be serious losses to vulnerable sectors of society at several stages of the market chain. The economies of the region are growing and some countries could finance recurrent costs of AI control, but substantial investment in veterinary services is required. National and regional financing structures need to be reviewed. AI control strategies should include a broad financial support system that addresses education, credit, compensation and social relief programmes. Some strategies may result in restructuring of the industry, or affect the wider development of rural areas and local food security.

Prevalence and factors related to smoking among secondary school students in Kota Tinggi District, Johor, Malaysia.

Smoking among adolescent is a public health concern in Malaysia. Multiple studies on smoking prevalence and its related factors have been conducted in Malaysia, however, they were specific to either urban or rural areas alone. Studies in mixed settlement areas (urban, rural, land development area) had not been intensively investigated. This study reports the prevalence, demographic and factors related to smoking amongst form four students in the district of Kota Tinggi, Johor. A cross-sectional study of 16-year old secondary school students in Kota Tinggi district was conducted using two-stage stratified, proportionate sampling in July 2005. The study instrument used was a validated structured questionnaire on smoking and its related factors. Smoking prevalence was found to be 29.7%. More than 50% of male students were smokers. Prevalence was highest in FELDA (Federal Land Development Authority) settlement areas. Smoking was associated with having a brother or friend who smokes and poor academic performance. The study revealed that smoking prevalence was high, especially among male students in land development schemes. This situation will contribute to high smoking-related health problems in the future if proper preventive measures are not taken accordingly.

[Selected psychosocial factors and the sexual development of girls].

In a group of 771 Czech girls aged 16 to 18 years who were examined by means of an interview, the influence of three psychosocial factors on their sexual development was studied. Through statistical analysis of data it was found that probands living in towns had their first sexual intercourse significantly earlier (p < 0.004) than village girls and further they had more experience with cunnilingus and fellatio. Their level of education and that of both parents was significantly higher than in probands living in villages. However, in the second study we ascertained that sexual development of students in comparison with girl-apprentices was highly significantly retarded (p < 0.004). In the third study a comparison of psychosexual development of 535 girls who lived in complete and stable families, and of 166 probands from divorced marriages was carried out. Sexual development in the later group was accelerated (p < 0.02). The emotional atmosphere in disrupting families and the incompleteness after divorce influenced negatively the development of girls during their adolescence in different directions.
