PURPOSE: We evaluate the results of tunical incision and venous patch grafting for correcting penile deformity in Peyronie's disease. MATERIALS AND METHODS: In 113 [corrected] patients with symptoms of Peyronie's disease for more than a year indications for surgery included penile shortening, persistent pain, severe curvature, penile narrowing or indentation and/or failure of previous surgery. Preoperative evaluation included determination of patient and partner expectation, potency status, circumcision status, measurement of penile length (short and long side) and saphenous vein, and color duplex ultrasonography to evaluate possible accessory vascular communication. Patients underwent plaque incision and venous patch grafting. The configuration, size and number of tunical incisions depended on the size and shape of the lesion. Lower and upper saphenous, and deep dorsal veins served as the graft materials. Postoperative followup was as long as 18 months. RESULTS: In 96% of patients the penis became straight, while residual curvature was 30 degrees in 3% and 15 degrees in 1%. In 94% of patients narrowing and indentation were absent and in 83% penile length was the same or longer postoperatively. Of the patients who were potent preoperatively 88% experienced the same or better erectile quality after surgery. In 10% of cases a change in sensation occurred lasting longer than 6 months. Overall satisfaction was expressed by 92% of men who believed that surgery improved the psychological state as well as the relationship with the partner. CONCLUSIONS: The results are satisfactory and this procedure offers a reasonable solution for correction of Peyronie's disease.
In this paper, we discuss what factors are important to realize an autonomous robot as a partner with humans. We believe that it is important to interact with people without boring them, using verbal and non-verbal communication channels. We have already developed autonomous robots such as AIBO and QRIO, whose behaviours are manually programmed and designed. We realized, however, that this design approach has limitations; therefore we propose a new approach, intelligence dynamics, where interacting in a real-world environment using embodiment is considered very important. There are pioneering works related to this approach from brain science, cognitive science, robotics and artificial intelligence. We assert that it is important to study the emergence of entire sets of autonomous behaviours and present our approach towards this goal.

Social theories of successful aging attempt to explain how individuals adapt to changes characteristically associated with aging and to predict whether older adults' adaptations will lead to successful aging. The death of a spouse and the accompanying bereavement process entail dramatic changes to personal networks and experience to which individuals must adapt to age successfully. Network text analysis (including word frequencies, cluster analysis, and multidimensional scaling) of advice for adjusting to, and coping with, the loss of a spouse given by a sample of 60 bereaved spouses (mean age = 68) at 6 points in time after the death of their marital partner (3-4 weeks to 24 months) reveal respondents' lay theories of successful aging. Thematic clusters address social positioning and qualifiers, activity, communication, time, and spousal characteristics. Results indicate respondents frame their advice as unique to their context of social relationships while providing support for activity theory and negatively addressing disengagement theory.
Sexual dysfunctions are differentiated according to the following symptoms: libido dysfunction, orgasmic dysfunction, vaginism, algopareunia and polysymptomatic dysfunction. In the out-patient service for sexual and other psychosomatic disorders of the University of Heidelberg's Women's Hospital 90 women with libido dysfunction were treated with conflict-centered psychotherapy after exclusion of organic diseases which could explain the reduction of libido and after exclusion of a depression where the loss of libido is a regular symptom. The success-rate was 63%. It was attempted to reinforce the ego of the patient which enables him to experience his surroundings with less conflicts. Neurotic anxieties and symptoms are decreased and loos their importance so that the patient is able to handle his sexual disturbance. Among the psychosocial factors, i.e. focal causes, responsible for the libido dysfunction we found: In 48% of the cases the relationship between parents and children had been disturbed, which caused difficulties in the identification process (lack of father or mother, cool mother, divorce, borken home). Problems with the partners were found in 42%. 2/3 of these women had not grown up in intact families, which seems to predestinate difficulties in communication later on. Psychodynamically the loss of libido means sexual resistance which can lead to aversion or disgust. When there is no excitement the vasocongestion and lubrication do not take place: cohabitation pain (algopareunia) results. On the other hand the reduction of libido can also be a result of chronic algopareunia, i.e. secondary. Psychogenic bleedings ("aversion-bleeding") during or after intercourse occasionally occur in combination with algopareunia and lack of libido.
The technology of bar coding has been recommended because of the ability to increase efficiency. The relative widths of both the bars and spaces code the data stored in the bar code. The Health Industry Business Communications Council (HIBCC) is a non-profit organization aimed at developing appropriate standards to improve exchange among all health care trading partners. According to these recommendations all health care products should be labeled with a primary symbol identifying the labeler and the product code and a secondary one that is useful to distributors and providers. The efficacy of bar code technology depends on the ease of use and the incorporation of user workflow into the system. Despite problems depending on industry factors as well as the lack of standards, resource factors associated with costs for the implementation of the new technology and human factors for cultural exchange that facilitate adoption, it is outstanding that bar code technology improves patient safety and increases organization productivity reducing manual-consuming steps and allowing for more accurate and complete data collection.

OBJECTIVE: Based on the information acquired in the first two parts of this study and the data in the literature, to propose ethic codifications for solving moral dilemmas concerning fetal medicine. METHODS: Identification of situations where the object of the conflict is ethics: general methodology propositions for making individual decisions. RESULTS: In all cases a rigorous method is required, based on three main elements: a complete and well documented medical file; the opinion of the couple after appropriate participative communication; case by case collegial discussion. CONCLUSION: The methodology proposed underscores the roles played by health carers: obtain a medical file as complete as possible with prime emphasis on the infant; establish active communication with the parents giving greater value to their free examination of the situation; collegial discussion in order to be pertinent social partners.
Macrophages within the tumor microenvironment facilitate angiogenesis and extracellular-matrix breakdown and remodeling and promote tumor cell motility. Recent studies reveal that direct communication between macrophages and tumor cells leads to invasion and egress of tumor cells into the blood vessels (intravasation). Thus, macrophages are at the center of the invasion microenvironment and are an important drug target for cancer therapy.
Clients may appear compliant but may not fully adhere to recommendations made by the veterinarian or veterinary technician for many reasons. Client adherence is directly related to one's communication skills, which can be practiced by all members of the health care team in each client encounter. The four habits approach to enhancing communication skills has been tested in a large human health care organization and shown to result in greater satisfaction between patients and doctors as well as in improved adherence to recommendations. This model is easily adapted to small animal practice and provides the foundation for a relationship-centered approach to care.
Existing studies of the sexual content of television programming and advertising and the effects of this content on adolescent viewers are reviewed. Content studies show that the frequency of sexual references have increased in the past decade and are increasingly explicit. Studies of the effects of this content, while scarce, suggest that adolescents who rely heavily on television for information about sexuality will have high standards of female beauty and will believe that premarital and extramarital intercourse with multiple partners is acceptable. They are unlikely to learn about the need for contraceptives as a form of protection against pregnancy or disease. Suggestions for future research and trends in television programming policies are explored.
A growing body of research shows that gay men in primary relationships engage in higher rates of unprotected anal intercourse than single gay men. We sought to (a) describe patterns of HIV risk taking among young gay men in boyfriend relationships and (b) identify characteristics of relationships that were most associated with engaging in unprotected intercourse. The data come from the Young Men's Survey, an ongoing longitudinal survey of 416 gay men aged 18 to 27 in three medium-sized West Coast communities. Fifty-one percent of the men with boyfriends had engaged in unprotected anal intercourse in the past 2 months, compared with 21% of the single men. Forty-two percent of the respondents who had unprotected intercourse with their boyfriend did not know their own and their partners' HIV status. Factors associated with unprotected intercourse included greater relationship involvement and time spent together, sexual behavior patterns, less involvement with the gay community, poor sexual communication skills, and misperceptions about safe sex. The findings highlight the need for HIV risk reduction interventions designed specifically for young gay men with boyfriends and identify critical areas to target in such interventions.
Interprofessional exchanges such as workshops are common activities in international development programs. The authors describe processes used to select, implement, and evaluate six collaborative workshops held in Tianjin, China, between 1990 and 1995. These workshops targeted curricular change for a baccalaureate nursing program in Tianjin Medical University. Our experience highlights the importance of understanding the assumptive base for change, developing an institutional memory in both partner universities, introducing interactive teaching methods congruent with Chinese value systems, and using a formative evaluation strategy to foster cross-cultural dialogue and mutual understanding among project partners.
Serotonergic Retzius (R) neurons of the leech form a Cl-dependent synapse with pressure-sensitive (P) neurons both in vivo and in vitro. However, P cells show an extrasynaptic, cationic response to application of 5-hydroxytryptamine (5-HT) which is reduced upon contact between the neurons in culture. We have examined the cellular specificity of the selection of 5-HT responses in the P cell by pairing it in culture with a variety of identified neurons. Non-synaptic sensory cells, non-serotonergic pre- and postsynaptic partners and serotonergic neurons that do not form chemical synapses with the P cell failed to alter its responses to 5-HT. The selective reduction of the extrasynaptic response to 5-HT in the P cell therefore appears to be induced specifically by contact with its only known serotonergic partner during neuronal recognition leading to synapse formation.
As we move into the next millennium, we anticipate a rapid introduction of Information and Communication Technologies (ICTs) into the medical environment. In applying these technologies in Health Care Environments, the basic goals are to improve: (a) the collaboration between the different partners in the health sector, (b) the access to care and (c) the medical education, and to enhance overall quality (by increasing the availability of some applications, improve the quality of others and facilitate some completely new ones), and all these at affordable cost. This requires close interaction between health care practitioners and information technologists to ensure that the proposed technologies satisfy current user's needs, and anticipate future. The public awareness and the acceptance of the new medical environments by the users are key factors for successful implementations.
CONTEXT: Understanding young women's sexual assertiveness is critical to developing effective interventions to promote sexual health and reduce sexual risk-taking and violence. Young women's perception of their sexual rights may vary according to demographic characteristics, sexual health behaviors and victimization history. METHODS: Data were collected from 904 sexually active 14-26-year-old clients of two family planning clinics in Texas, reflecting their perceptions of their right to communicate expectations about or control aspects of their sexual encounters. Logistic regression analysis was used to assess which characteristics were independently associated with believing that one never has each specified sexual right. RESULTS: Almost 2096 of women believed that they never have the right to make their own decisions about contraception, regardless of their partner's wishes; to tell their partner that they do not want to have intercourse without birth control, that they want to make love differently or that their partner is being too rough; and to stop foreplay at any time, including at the point of intercourse. Poor grades in school, sexual inexperience, inconsistent contraceptive use and minority ethnicity were independently associated with lacking sexual assertiveness. CONCLUSIONS: Many sexually active young women perceive that they do not have the right to communicate about or control aspects of their sexual behavior. Interventions to prevent sexually transmitted diseases, unwanted pregnancy and coercive sexual behaviors should include strategies to evaluate and address these perceptions.
The mode by which cytolytic T-lymphocytes (CTL) deliver the lethal hit to their targets has been studied extensively. The CTL exhibits a characteristic polarization, with the attacking end formed by the uropod (containing the Golgi complex and associated granules). The effect on the target cell (TC) has been considered as a form of apoptosis or of both necrosis. In this communication we describe the particular morphological steps that relate to the participation of the target renal epithelial cell to its own destruction. Thus, the TC forms tight membrane contacts with the CTL that are invested by a dense network of stress fibers and microtubules within the TC cytoplasm in a manner similar to the areas of attachment between the epithelial cells or of the latter to the underlying surface. The TC mitochondria at the site of the CTL attachment are markedly swollen, in contrast to the more distally located ones that appear initially normal. The TC plasma membrane surrounding the CTL attachment site shows focally intense blebbing. In later stages, associated with overt apoptosis, global TC blebbing is present. In summary, the TC shows a polarization pattern, that corresponds roughly to the polarity exhibited by the attacking cell (uropod-protopod), and to some degree treats the latter cell as a junctional partner.
