[The active detection of tumors of the large intestine by using automated screening].

A system for automated screening for early detection of proctologic pathologies is described. Questionnaire-based data, formation of high risk groups using a computer and results of examination of 1660 office and professional workers of an industrial enterprise according to a given program are discussed. After filled questionnaires had been processed using a computer, 134 (8%) subjects were selected to group 1, 454 (27%) subjects-to group 2, 383 (23%) subjects-group 3 and 689 (42%) subjects-to group 4. Proctologic pathologies were detected in all persons of group 1, 83% of those in group 2 and in 30% cases of group 3. Asymptomatic proctologic pathology was established in 630 (37.8%) screenees of whom 10 revealed large bowel tumors, 5 early cancers included, whereas 49-large bowel polyps.

[Methods for study of soluble antigens in pathological anatomy].

The authors restate their personal researches about several antigen systems, viz. ACE, neurospecific antigens. They point out the complementarity of immunochemistry and immunohistochemistry which implicates a very precise methodology and utilization of rigorously monospecific immune serums with regard to the studied antigen. This conduces to reproducible, responsive and reliable techniques in optic microscopy but much less easily so in electron microscopy. These techniques are still partly experimental; but their application to the study of pathological tissues and especially of tumors may however be considered. The identification of histological types and the correlated evolution is suggested on hand of concrete examples.

Pain in older persons.

Chronic pain is more prevalent in older persons than in young adults. In this review the physiological, pathological, and psychological reasons for altered pain sensibility in older persons are explored and strategies for the management of pain in older persons described. The evidence suggests that altered physiology of peripheral and central pain mechanisms combine with psychological attitudes, such as stoicism and reluctance to confirm the presence of pain, to raise pain threshold. However, once pain is experienced, older persons describe the same severity, quality, and psychological disturbance as younger persons. There is some evidence to suggest that the complaint of pain in the presence of pathology is reported less often in older persons. On the other hand, the presence of persistent or recurrent clinical pain may have a greater impact on the psychological, social, and physical function of older adults. It is also clear, however, that further empirical studies are required in order to delineate the age-related differences and similarities in the chronic pain experience. Management of chronic pain in the elderly requires meticulous diagnosis of the causal pain mechanisms as well as a holistic approach which gives due regard to psychological and social consequences of pain.

The Shatin community mental health survey in Hong Kong. II. Major findings.

A large-scale community survey in Shatin, Hong Kong, is presented with a modified two-phase design using flagged and nonflagged subsamples. A modified Self-Reporting Questionnaire and the Diagnostic Interview Schedule (version III) were used as the screening and diagnostic instruments, respectively. Lifetime rates for 19 Diagnostic Interview Schedule/DSM-III diagnoses are presented. The most common diagnoses in Shatin were tobacco dependence, generalized anxiety disorder, alcohol abuse and/or dependence, all phobias, and dysthymic disorder. The male-predominant disorders were tobacco dependence, alcohol abuse/dependence, pathological gambling, and antisocial personality. The female-predominant disorders were generalized anxiety disorder, all phobias, dysthymic disorder, major depressive disorder, obsessive-compulsive disorder, and bereavement.

Areas of convergence and discordance between the MMPI-168 and the Reiss Screen for Maladaptive Behavior in mentally retarded clients.

The Reiss Screen for Maladaptive Behavior and a modified version of the MMPI-168 were administered to 65 residents of a state mental institution diagnosed with mild or moderate mental retardation. Possible associations between the two screening instruments were then examined. Significant correlations were obtained between four scales of the Reiss Screen and Scales F, K, 1, 6, and 7 of the MMPI-168. These results support the clinical efficacy of both instruments in screening for behavioral pathology in mentally retarded persons. However, some scales purported to assess similar psychological constructs failed to show expected positive associations.

A fluorophotometric study on the aqueous humor dynamics in primary open angle glaucoma.

One hundred and ten eyes in 40 normal persons with no ocular pathology, 20 with ocular hypertension and 50 with primary open angle glaucoma were studied. The value of the aqueous humor flow was obtained and then compared with the different samples and with general (age, sex) and ocular factors (intraocular pressure, tonographic outflow facility, campimeter and disk/cup ratio, among others).

Evaluating a perimetric expert system: experience with Octosmart.

When evaluating expert systems to be used in clinical perimetry, various aspects of their performance as compared with that of human interpreters must be considered. In this investigation, the results produced by the new Octosmart diagnostic program have been compared with the performance of three interpreters with various amounts of experience in visual field analysis. The evaluations were based on 27 visual fields with glaucomatous damage, which had been examined with the Octopus program G1. It is shown that in borderline cases (i.e., neither clearly normal nor clearly pathological) where strict statistical criteria must be employed in order to distinguish between possible pathology and artifacts, the "personal styles" of human interpreters, more than standardized decision criteria, implicitly guide the decision process, resulting in unpredictable, non-standardized interindividual differences. A standardized expert system, based on constant, explicit, and logical criteria is therefore considered to be superior to unaided human interpretation. It is pointed out that the influence of the implicit decision criteria of human interpreters must be controlled carefully if expert systems are to be evaluated with reference to human interpreters.

Lymphoepithelial cyst of the pancreas. Report of two cases and review of the literature.

CONCLUSION: Lymphoepithelial cyst of the pancreas (LC) is a very rare benign lesion and preoperative diagnosis is difficult. Conservative surgery seems to be the appropriate therapy in symptomatic patients or when a precise preoperative diagnosis is not achieved. The benign behavior of all reported cases suggests that the asymptomatic patients with a certain morphological preoperative diagnosis might be clinically followed up. The histogenesis of LC remains to be elucidated. BACKGROUND: LC of the pancreas is a cyst that is histologically characterized by a fibrous tissue, a lymphoid component and a lining squamous epithelium. METHODS: Clinical and pathological findings of two personal cases are reported with review of the literature. RESULTS: A 56-yr-old man, complaining of epigastric pain, and a 47-yr-old man, with a history of alcohol abuse, were admitted to hospital. In both cases the lesion was detected with abdominal ultrasound but a certain diagnosis was obtained only after histological examination of the resected cysts.

Effects of maternal ethanol infusion during pregnancy on the growth and development of children at four to seven years of age.

Twenty-five children whose mothers had been given ethanol for the arrest of preterm labor were evaluated at 4 to 7 years of age and compared with matched control subjects. The group as a whole showed no major differences from control subjects. However, when a subgroup of children born during or within 15 hours of termination of the infusion was investigated, significant pathology in developmental and personality evaluations were noted. It is concluded that if ethanol infusion is to be used, the patient should be carefully evaluated during the infusion. If it appears that labor is progressing, the infusion should be discontinued so as to allow a maximal drug-free interval before delivery.

An automated system for the analyses of temporal speech patterns: description of the hardware and software.

A system (WELMAR II) is described for analyzing time patterns of speech on small computers. Temporal speech patterning refers to the pacing of the sounds and silences that make up a stream of speech. The system is particularly useful for research involving clinical populations since it has been shown that speech rhythms are sensitive to interpersonal influence, to dimensions of personality, and to psychological pathology.

Evoked potentials in patients with Huntington's disease and their offspring. II. Visual evoked potentials.

Visual evoked potentials (VEPs) to full-field and half-field stimulation were recorded in a large sample of patients with Huntington's disease (n = 36) and subjects at risk (first-order offspring, n = 55) using a checkerboard pattern reversal stimulus. Whereas a clear diminution of the amplitudes found confirmed earlier findings in patients with Huntington's disease, further alterations of VEP wave forms consisted in the absence of asymmetric hemisphere lateralization to half-field stimulation (i) and severe distortion with amplification of the early NPN/PNP complexes (ii). Considering at least two different abnormal parameters necessary to represent a reliable indicator of the disease, 24 patients (67%) and 14 (25%) of the persons at risk exhibited pathological results. In contrast, only 2 among 36 patients (5%) but 30 (55%) among the offspring exhibited normal results. The diagnostic and predictive value of the investigation of VEPs in Huntington's disease is discussed.

Assessment of patients' reporting of pain: an integrated perspective.

A common assumption about pain is that it always results from the presence of underlying organic pathology. In the absence of objective pathology, an individual's report of pain may be ascribed to psychological causes. There is a wide variation in patient's experience of pain and organic factors alone cannot explain individual differences in patients' reports. Assessment of patients who report pain requires attention to psychosocial, behavioural, and organic factors. We describe a comprehensive approach to the assessment of psychological and behavioural variables that affect patients' reports of pain. We counter the duality of the somatogenic versus psychogenic perspective and suggest a more integrated assessment that encompasses not only the severity of pain and related physical pathology but also the person who is reporting the presence of pain.

Utilization of screening mammography as a preventive practice prior to initiating a population-based breast cancer screening program.

Factors associated with the level of utilization of screening mammography as a preventive practice were analyzed prior to initiating a population-based breast cancer screening program. A questionnaire was used to collect information about screening mammography utilization during the last 4 years among women invited to enroll in the program. Sociodemographic and health factors, and health services utilization were studied. A screening mammography in the last 4 years had been performed on 59.3% of the 8867 women interviewed. The utilization of mammography was higher among women who were younger, had a higher education level, or had previous visits to a physician (P < 0.001). The variables more strongly associated with use of screening mammography were: visit to a gynecologist and performance of a pap smear. Also associated with the utilization of screening mammography were: personal history of breast pathology, family history of breast cancer, breast self-examination, and recent visit to a general practitioner. Self-perceived health presented an inverse relation with screening mammography use (P < 0.001). The use of screening mammography appears to have increased in recent years, although inequalities persist which the breast cancer screening programs help to modulate. Mammographic frequency only would be deemed appropriate by the European guidelines in fewer than one-half of the cases.
