Vaccine adverse events reported in New Zealand 1990-5.

AIM: New Zealand monitors vaccine safety through vaccinator reports of adverse events following immunisation. The rate of reporting for the commonly used vaccines during 1990-5 are presented. During this time new vaccines were added to the immunisation schedule, enabling comparison of reporting rates. METHOD: The number of events were obtained from the CARM database and a rate calculated based on estimated vaccine use. RESULTS: Injection site reaction with adult tetanus-diphtheria vaccine was most commonly reported (68/100,000); a rate five times higher than with tetanus vaccine. There were also more reported reactions from diphtheria-tetanus-pertussis-Haemophilus influenzae type b (DTPH) than DTP vaccine, with 'abnormal crying' the commonest reaction reported after 29/100,000 doses of DTPH and 3/100,000 doses of DTP. The next commonest reaction for DTPH was injection site reaction (25/100,000) which compares to 17/100,000 doses of DTP. For the other inactivated vaccines, more reports were made for fever following Hib (16/100,000), than for hepatitis B (2/100,000) or influenza (1/100,000). The most common reports following measles-mumps-rubella (MMR) vaccine were rash (17/100,000), fever (12/100,000), and injection site inflammation (5/100,000). There were very few reports following polio vaccine, with rash, fever and headache all reported at less than 1/100,000 doses. CONCLUSION: Although only a proportion of events are reported, the picture presented here confirms the overall safety of vaccines and the value of the adverse event monitoring system. Monitoring vaccine adverse events is an essential part of the immunisation programme.

Scanning laser ophthalmoscope-evoked multifocal ERG (SLO-mfERG) in patients with macular holes and normal individuals.

AIMS: A scanning laser ophthalmoscope (SLO) has been used for multifocal electroretinography (mf ERG) measurements under simultaneous fundus monitoring. The aim of this study was to prove if the SLO-mfERG measurement reflects reliably the clinically registered underlying disease, and to demonstrate the importance of its main advantage, fixation monitoring. METHODS: In all, 10 patients with macular hole stage II/III were included in the study, and 19 normal individuals served as the control group. The mf ERG device was combined with an SLO, which was used both as a stimulus and trigger unit as well as a fundus-monitoring system. Monitoring of the fundus was guaranteed by an infrared laser (780 nm). The stimulus matrix consisted of 61 hexagonal elements, covering 24 degrees of the posterior pole. We examined both, patients with macular holes and healthy individuals. RESULTS: Compared to normal controls, patients with a macular hole (Gass stage III) showed a significant decrease in response density in the centre of the stimulus array, which correlated well with the morphological alteration observed by clinical examination. However, variation of response density of the central hexagonal area has been proved to be high. CONCLUSIONS: SLO-mfERG is a feasible and reliable new technique to investigate macular function under simultaneous fundus control. The main advantage is that control of fixation can be used in order to obtain more reliable results that correlate well with visible fundus abnormalities such as in patients with macular holes. However, further investigations have to be performed in order to overcome sufficiently the problem of fixation instability.

[Influence of intraschool factors on pupils' mental performance and health status].

The study of the intraschool environment in the examined educational establishments has indicated that the sanitary and hygienic conditions of pupils' school stay are characterized by a diversity of disadvantages. The leading factors that have the greatest influence on schoolchildren's morbidity are those associated with the organization of a teaching-and-educational process (36.9%) and with the intraschool environment (28.1%). All the foregoing suggests that a hygienic monitoring system should be set up in each school for the systemic monitoring of pupils' health and for its forming factors.

[Development and utilization of the integral system of computer monitoring in anesthesiology in clinical obstetrics and gynecology].

An original computer monitoring system for preoperative monitoring was developed at the department of anesthesiology and intensive care of the Research Center for Obstetrics and Gynecology of the Russian Academy of Medical Sciences. The system includes a bedside monitor Athena (S&W + Artema, Denmark) and two special monitors ABM-100 (Datex, Finland) and NCCOM (Bomed, USA). The potentialities of the system are discussed; physiological parameters were monitored and estimated in the real time mode. Variants of graphic interface of the system are presented, including an original multisystem integral nomogram. Utilization of the system demonstrated the advantages of objective physiological information recorded by the system, a high informative values of integral estimated parameters in the real time mode. The use of the multisystem integral nomogram helps the anesthesiologist rapidly and unambiguously to interpret the clinical situation and essentially accelerates the decision-making on further treatment strategy. The work with the system does not require any special knowledge and skills, the system is simple and easy in use.

Electrocardiographic electrodes provide the same results as expensive special sensors in the routine monitoring of anesthetic depth.

UNLABELLED: The Bispectral Index (BIS) is a mathematically derived electroencephalographic (EEG) derivative that has been introduced to monitor depth of anesthesia (1,2). The A-2000 BIS monitoring system (Aspect Medical Systems, Inc., Newton, MA) is currently the only commercially available system to monitor depth of anesthesia. In several studies, its propensity to optimize the use of hypnotics to maintain and achieve a certain depth of anesthesia has been described (3,4). Some studies have even proposed that the routine use of the monitoring system can decrease awareness (1,5), an increasing factor in malpractice claims. The cost-benefit calculations for BIS monitoring suffer from the fact that like its predecessor, the 1000-A BIS monitor, the A-2000 BIS monitoring system demands the use of expensive, special electrodes (6). Although the application of the single-use BIS sensor is very comfortable and easy to use, its high price of approximately $10-20 US prevents many anesthesiologists from using it. Furthermore, whereas the former model of the monitor (1000-A BIS monitor; Aspect Medical Systems, Inc.) used standardized connectors, which allowed the use of other electrodes such as electrocardiogram (ECG), the new monitoring system makes this very difficult because of special connectors that match the equivalent connector at the proximal BIS sensor site. The purpose of this prospective study was to compare BIS values derived from the original BIS sensor with BIS values derived from commercially available ECG electrodes. This comparison was made possible by designing and manufacturing a connector allowing the use of ECG electrodes. IMPLICATIONS: The Bispectral Index (BIS) monitor adequately monitors depth of anesthesia. The routine use of this monitor has been hampered by the benefit-cost equation because only special expensive electrodes can be used. We examined the agreement of BIS values obtained by original sensor electrodes and commercial electrocardiogram (ECG) electrodes. These ECG electrodes can replace more expensive BIS sensors.

A system for simulation and monitoring of robot-assisted and navigation-assisted surgical interventions. (Part 1).

Location monitoring systems for surgical applications are getting more and more common in clinical daily life. Navigation treatment systems are also a form of a location monitoring system. These systems always consist of an optical, US-, electromagnetic or mechanical navigation system. If two or more navigation systems are combined or one navigations system is combined with a robot system the kinematic model of the entire system consists of closed kinematic loops. In this paper, an approach is presented to gain more information from the closed kinematic chains instead of breaking the kinematic loops.

An automated spectrophotometric system for monitoring buffer capacity in anaerobic digestion processes.

Anaerobic digestion is a suitable method for the treatment of wastewater and organic wastes, yielding biogas as a useful by-product. A common way of preventing instability problems and avoiding acidification in anaerobic digesters is to keep the organic load to the digester far below its maximum capacity. An improved way of operating digesters would be to use monitoring and control systems for increased organic load under controlled conditions such that the digester performance is improved. The partial alkalinity, which indicates the bicarbonate concentration, has in many cases been found to be a suitable parameter to monitor. Here, an automated monitoring system for alkalinity measurements is described. It is shown to be applicable for measuring a wide range of bicarbonate concentrations. The system shows potential for monitoring anaerobic digesters as it responds to the alkalinity of digester effluent, as well as being stable over a relatively long time span with few maintenance requirements.

A system for ambulatory monitoring and computerised analysis of oesophageal motility and luminal pH.

It is often desirable to monitor physiological events from subjects free to participate in their normal daily activities. This is especially so where clinical symptoms are elusive or precipitated by specific activities. Through ambulatory monitoring, evidence of a causal relationship between recorded parameters and the spontaneous onset of symptoms may be revealed. We describe an ambulatory monitoring system which permits continuous 24-hour recording of oesophageal motility from three levels within the oesophagus together with luminal pH. Computerised analysis of the recorded data addresses many of the problems associated with processing the information accrued through long-term monitoring by providing an automatic comparison of patient data against either preselected values or those of the program's algorithm. Motility is quantified in terms of wave frequency, amplitude, duration and temporal relationship together with an integration of the wave area and mean pressure baseline. The corresponding pH data is quantified as frequency, duration and integrated area of pH excursions throughout a range of pH1-8. This system provides access to the entire patient record which may either be viewed on screen, analysed or output to a printer. Analysis yields a tabulated breakdown of motility and pH events from any section of the patient's record. Selective analysis readily permits statistical comparison of discrete time-matched samples corresponding to different patient circumstances.

A TLD-based personal dosemeter system for aircrew monitoring.

Evaluation of LiF:Mg,Ti thermoluminescence dosemeters (TLDs) according to the high-temperature ratio (HTR) method enables the determination of the dose-average linear energy transfer (LET), the mean quality factor and the dose equivalent in mixed radiation fields of unknown composition. The neutron contribution is assessed by the Extended Pair method calibrated in the CERN-EU High-Energy Reference Field (CERF). The advantages of the small passive detectors as an easy-to-handle monitoring system for in-flight surveillance are demonstrated by measurements on-board north-bound and trans-equatorial flights. The experimental results are compared with calculations by the well-established CARI code.

A computerized system for remote monitoring of subterranean termites near structures.

Sensors comprising wooden stakes painted with conductive circuits of silver particle emulsion were inserted in Sentricon stations in soil near structures. Sensors were wired to a datalogger that was programmed to test for circuit breakage every 2 h and store the data in its memory. A host computer was programmed to access the datalogger through telephone communication lines for data download every 4 d. The computerized monitoring system was tested in three remote sites, and site visits were conducted monthly for 6 mo to examine system accuracy in detecting termite activity. The mean monthly accuracy for the system to correctly report the presence (true positive) or absence of termites (true negative) in the stations was 85%, but the accuracy at 6 mo after system installation ranged from 41 to 79%. Mean sensor longevity, defined as the time for a sensor circuit to break in the absence of termites, was approximately 4.4 mo.

Technical note: Validation of a system for monitoring feeding behavior of dairy cows.

An electronic system has been designed that allows for passive monitoring of feeding behavior of individual cows housed in a free-stall barn. The objective of this study was to validate the data generated by this GrowSafe feed alley monitoring system. Twelve lactating cows were each monitored for 24 h using both the GrowSafe system and time-lapse video. The GrowSafe estimation of number of meals consumed by each cow showed perfect agreement with meal frequency identified using the video recordings. The duration of these meals, as estimated by GrowSafe, was highly correlated with the meal duration derived from the video (R2 = 0.98). Despite the excellent agreement for these meal-based measures, for each cow we found some instances in which the video showed that a cow was present at the feed alley but GrowSafe failed to detect cow presence (12.6% of observations) and a few instances in which the reverse was true (3.5% of observations). However, all the missed or extraneous data from the GrowSafe system were closely associated in time with known periods of feeding. These results indicate that this feed alley monitoring system can provide very good measures of meal frequency and meal duration and reasonable estimates of instantaneous feed alley attendance for loose-housed dairy cattle.

The Canadian Hemophilia Registry as the basis for a national system for monitoring the use of factor concentrates.

BACKGROUND: Canada's publicly funded blood system has recently introduced high-purity concentrates as the standard treatment for individuals with hemophilia. The added cost and the need to document patient outcomes have prompted the consideration of a national blood product monitoring system. STUDY DESIGN AND METHODS: This study investigates the suitability of the Canadian Hemophilia Registry (CHR) as the basis of such a monitoring system by assessing the degree to which it represents users of factor concentrates. RESULTS: Currently, there are 1978 individuals registered with the CHR, of whom 1594 (81%) have hemophilia A and 384 (19%) have hemophilia B. The total prevalence is 7.2 per 10(5) population, with the prevalence of severe cases being 2.3 per 10(5). This overall prevalence is similar to that seen in other countries with national registries. The CHR national prevalence also compares favorably with that in the province of Quebec, where registration of users of blood products is compulsory. The CHR figures indicate that the number of persons currently infected with human immunodeficiency virus, both alive and dead, is 652, which is similar to the number of applicants (658) to the federal government's assistance program. The registry is stable, and the number of persons with severe cases, other than young children, newly registered or lost to follow-up during the last 2 years is very small. CONCLUSION: The CHR includes the vast majority of factor concentrate users and is therefore ideal as the basis for a national monitoring system.

Recording vagal nerve activity for the control of an artificial heart system.

Monitoring cardiovascular control system information is important in considering the quality of life (QOL) of patients with artificial hearts. Natural heart circulation is controlled by an autonomic nervous system. Therefore, it is desirable to record autonomic nerve activity for the control of artificial heart systems. We directly recorded vagal nerve activity in long-term animal experiments. Six healthy adult goats were anesthetized with halothane inhalation, and thoracotomy was performed with the fourth rib resection during mechanical ventilation. Arterial blood pressure and right and left atrial pressures were continuously monitored with an inserted catheter. Cardiac output was measured by an electromagnetic flow meter attached to the ascending aorta. After the chest was closed, an incision was made in the left neck, and the left vagal nerve was separated. Stainless steel electrodes were inserted into the vagal nerve and fixed by a plasticizer. After the incision was closed, the goats were transferred to a cage and extubated after waking. Vagal nerve activity was measured using hemodynamic parameters when the animals were awake. Our results show that clear observation of autonomic nerve discharge was made through this experimental system for over 1 month. The tonus of the vagal nerve was significantly altered before body motion with hemodynamic changes, suggesting the possibility of prediction. These results suggest that information from autonomic nerves may help to control implantable artificial hearts or ventricular assist devices.

Real time remote monitoring of air pollutants and their online transmission to the web using internet protocol.

Recent advances in both information and sensor technologies have enabled the development of Real Time Remote Monitoring (RTRM) capabilities for environmental management. An online and real time remote monitoring system for air pollution has been designed and installed at a traffic police station at Punjagutta in Hyderabad, India. The system is optimized using electrochemical sensors and a real time particulate matter analyzer. The system also monitors meteorological parameters such as temperature, humidity, rainfall, barometric pressure, wind speed and wind direction. The system periodically monitors both pollution and meteorological parameters at pre- programmed intervals of [Formula: see text] hr during peak periods and 1 hr during non peak periods of the day and continuously uploads to a predestinated web site (www.appcb.org/home.htm) using File Transfer Protocol. The web site renders a quick, simple and graphical display of air pollution levels and meteorological parameters and their significance to humans. The present paper highlights design considerations of a pollution monitoring system, system hardware and software requirements and practical limitations and future directions for real time remote monitoring of air pollution.

Quality assurance applied to animal disease surveillance systems.

Monitoring and surveillance systems (MOSS) are essential activities for official Veterinary Services. In addition, the increased trade in animals and animal products over recent years has increased the importance of international disease reporting. A reliable surveillance system is the key to early warning of a change in the health status of any animal population. Such a system is also essential for providing evidence about the absence of diseases or in determining the extent of a disease which is known to be present. The authors discuss a set of methods and approaches for evaluating the quality of surveillance and survey systems. Certain steps are required when assessing the quality of a service or product. Various approaches for quality assessment are available and the suitability of each method depends on the objective of the evaluation. An essential basic requirement is, however, to use an objective, transparent and systematic approach. The evidence collected and the analyses used to reach conclusions must be of such high quality that the results are acceptable to both the management of the MOSS and the assessor. Repeated discussions and negotiations may be necessary to reach consensus, particularly if the judgement affects activities between trading partners. Well-documented MOSS with specified objectives and integrated quality assurance mechanisms are likely to be easier to evaluate.

Intraoperative monitoring for tethered spinal cord syndrome.

Functions of the lower extremities and bowel and bladder must be monitored during releasing surgery for tethered spinal cord syndrome, because neural elements are embedded in lipoma or anomalous tissues. Evoked muscle action potentials of the external anal sphincter muscle, external urethral sphincter muscle, and lower extremity muscles can indicate promptly whether or not neural elements are involved before the surgeon releases or cuts the tissue for cord release. It is also important to monitor vesical pressure because of different types of innervation from the external urethral and anal sphincters, in spite of slow reaction times of vesical pressure elevation. Evoked muscle action potentials indicate only motor function; therefore, it is more appropriate to record evoked spinal cord potentials from the spinal cord above a lumbosacral operative field, because sensory function can also be monitored. Evoked muscle action potentials are an easy and extremely sensitive monitoring system; nevertheless, monitoring of vesical pressure and evoked spinal cord potentials should also be done to achieve optimum monitoring. For the past 5 years, 10 patients have undergone cord-release surgery with such a monitoring system, and the results indicate no exacerbating case.
