Recurrent gigantic pyogenic granuloma disturbing speech and mastication: a case report and literature review.

A case of gigantic pyogenic granuloma with three recurrences in the lower anterior gingiva is presented. Surgical wide excision of the lesion is the treatment of choice. The tumour must be excised down to the periosteum and the irritants around it removed to avoid recurrence. A contributing factor to the gigantic lesion is hormonal changes during pregnancy. Long-term review for 18 months after the third surgery showed no evidence of recurrence at the surgical site.

Trans-surgical restoration of extensive Class IV defects in the anterior dentition.

Direct bonding is the most commonly utilized treatment for conservative aesthetic restoration of the anterior dentition. Class IV defects require special attention due to their high incidence, particularly in young patients. One of the major challenges for the clinician in treating defects in this category is selecting the appropriate restorative composite resin material that emulates the physical and optical characteristics of dentin and enamel and conceal the fracture line at the tooth/composite interface. Class IV defects with margins violating the biologic width present another concern, since this violation often impedes the attainment of the correct anatomic contours. The learning objective of this article is to describe a technique for achieving a correct integration of the periodontium, the tooth structure, and the restorative composite resin material to achieve a high level of aesthetic excellence.

Laser-assisted restorative dentistry.

A case of rampant decay influenced by the use of a high-sugar, high-caffeine, citrus drink was presented. A pulsed Nd:YAG was used to achieve a clear operative area by removal of redundant tissue. An argon curing laser was used to effectively cure a sandwich technique of glass ionomer and microfill-resin restoration. While lasers increased the effectiveness and efficiency of routine dental care, diet control of sugar intake is crucial for long-term success.

Idiopathic gingival enlargement--report of two cases.

Idiopathic gingival enlargement is a rare condition of undetermined etiology, although some cases have definite hereditary basis. It affects entire gingivae in the form of small to large masses. It causes diastema, displacement and malocclusion of teeth. In severe cases it leads to poor esthetics, restricted movements of tongue, interference with speech, and difficulty in mastication.

[The characteristics of the choice by each of 6 periodontists of the site to be operated on on periodontal tissues].

Periodontal pockets deeper than 4-5 mm require surgical treatment. Records of surgical journals of a periodontology room over 6 years are reviewed and 2183 operations performed by six periodontologists are analyzed. The results of surgery are determined by patient's status, site of anesthesia, type of secondary adentia, convenience of surgical access, terms between operations. Individual features of surgical technique of each of the six surgeons are shown.

Achieving optimal gingival esthetics around restored natural teeth and implants. Rationale, concepts, and techniques.

The role of prosthetic restorations in the final appearance of the surrounding soft tissues has long been recognized. Innovative prosthodontic concepts as described should be used to enhance the biologic as well as the esthetic data of the supporting tissues, in natural teeth and implants alike. Combined dental treatment modalities of different kinds (i.e., orthodontics, periodontal treatment) are often required for optimal results. Meticulous care and attention to the delicate soft tissues should be given throughout all phases of the treatment, with a view to achieving a functional, healthy, and esthetic oral environment.
