The survival of ingested lactoferrin in the gastrointestinal tract of adult mice.

Lactoferrin is an 80 kDa major protein component of mammalian colostral whey. The antimicrobial active centre of lactoferrin, lactoferricin (Lfcin), may also be an important determinant of the interaction between lactoferrin and specific receptors on lymphocytes. We have documented the survival in vivo of ingested lactoferrin in the gastrointestinal tract of adult mice by surface-enhanced laser desorption/ionization affinity MS. Various kinds of degraded lactoferrin fragments were detected as molecular-ion peaks corresponding to Lfcin after being captured by an affinity capture device, hydrolysis in situ and laser desorption/ionization. No evident molecular-ion peaks of Lfcin were observed upon analysis of faeces from mice fed commercial milk, whereas lactoferrin fragments containing the Lfcin region were detected at concentrations in the order of at least pmol/g in the faeces of mice fed milk enriched with lactoferrin at 40 mg/ml. These results suggest that ingested lactoferrin would survive transit through the gastrointestinal tract as partially degraded forms containing the receptor-binding region(s) as well as the antimicrobial active centre.

Corynebacterium equi in the gastrointestinal tract of ruminants.

Corynebacterium equi has been recovered from the gastrointestinal tract of sheep and cattle. It can be found in all parts of the gut, and around 80% of animals have the organism in one or more intestinal sites. C. equi could be detected in the faeces of sheep which were kept caged and free from recontamination by the organism.

Sources of heterogeneous contrast enhancement in the gastrointestinal tract.

Water soluble gadolinium chelates demonstrate heterogeneous enhancement of the gastrointestinal tract (GI) when administered orally. To investigate the causes, ProHance (2.0 mM) was administered orally to rats. There was a dramatic enhancement of rat GI lumen signal intensity in T1-weighted MR images which provided increased contrast relative to the adjacent abdominal tissues. Heterogeneity of MRI signal enhancement along the rat GI tract was investigated by sampling rat GI fluid at various times post-ingestion and at different locations along the GI tract. The corresponding T1 and T2 relaxation times, Gd concentrations, and viscosities of each GI fluid sample revealed that changes in each of these parameters contribute to the observed heterogeneity of MRI signal enhancement.

Innovative drugs and strategies in the treatment of upper gastrointestinal tract carcinomas.

The results of the standard therapy of upper gastrointestinal tract cancer remain unsatisfactory, mainly because still the majority of cases are diagnosed at a late stage of disease, which implicates a poor prognosis. Innovative strategies focus on neoadjuvant chemotherapy in locally advanced gastric cancer and chemo- or radiochemotherapy in advanced oesophageal cancer. Results of phase II studies published so far have been promising. There is rising evidence that adjuvant radiochemotherapy in gastric cancer may be of benefit for curatively resected patients, whereas the results of trials including a systematic lymphadenectomy have to be awaited. New substances like the taxanes or irinotecan in the palliative setting did not prove to be superior to platinum-based standard chemotherapy protocols so far.

Pinealectomy reduces melatonin levels in the serum but not in the gastrointestinal tract of rats.

Melatonin levels were determined in serum and gastrointestinal tract (GIT) tissues of control (C), sham-pinelaectomized (SPx) and pinealectomized (Px) rats sacrificed in mid scotophase. Serum melatonin concentrations of Px rats exhibited the significantly lowest values (8.6 pg/ml), followed by SPx (20.1 pg/ml) and C (37.5 pg/ml) rats. In C, the ileum (542 pg/ml) and jejunum (531 pg/ml) exhibited the highest average GIT concentrations, followed by the colon (362 pg/ml), stomach (359 pg/ml) and cecum (164 pg/ml). However, only jejunum and ileum samples had significantly higher melatonin levels than cecum samples. There were no major differences between GIT melatonin levels in Px and C rats (range: 169-247 pg/ml). Statistically, pinealectomy did not influence melatonin levels in the GIT of rats. The findings support the hypothesis that melatonin concentrations in the tissues of the GIT are independent of pineal production.

Effect of taurine on drug absorption from the rat gastrointestinal tract.

Effect of taurine on drug absorption from the rat gastrointestinal tract was investigated by using in situ loop method for the stomach and in situ recirculation method for the small or large intestine. Aspirin absorption from the stomach or the small intestine was enhanced by the presence of taurine, but not from the large intestine. The absorption enhancement effect of taurine was not only site-specific, but also substrate-specific. Taurine increased the absorption of aminopyrine from the small intestine, but not from the stomach. On the other hand, the absorption of salicylamide, o-methoxybenzoic acid and o-ethoxybenzoic acid was enhanced in the stomach, although they were not influenced in the small intestine. Furthermore, among the taurine analogues investigated only homotaurine showed the taurine-like action in the small intestine. Glycine also increased the gastric absorption of aspirin. These effects were observed neither by the pretreatment of the intestine with taurine, nor by i.v. administered taurine. Taurine at the site of drug absorption, even at a low concentration, seems to influence the drug absorption due to its effect on the permeability characteristics of the mucosal membrane.

[The patient with refractory functional disorders of the gastrointestinal tract].

A group of patients with functional disorders of the gastrointestinal tract, manifest symptoms that are chronic, severe and invalidating. Their refractoriness to different treatments is a great challenge for physicians. They normally have a severe psychological and social dysfunction and an abnormal behavior towards their disease. All these factors lead these patients to perpetuate their illness condition. On the other hand, physicians usually have a negative reaction to their demands, leading to a non-satisfactory patient-physician relationship and to the failure of the healing process. Training in the systemic, biopsychosocial model of diseases and in interpersonal communication skills is required for an adequate management of these patients.

Brushing cytology of the upper gastrointestinal tract. Obsolete or not?

The merit of brushing cytology of the upper gastrointestinal tract has been questioned since it appears to duplicate biopsy. To determine its value, the reports on all endoscopic biopsy and cytology specimens from a one-year period were reviewed. The 683 procedures included 481 in which only a histologic biopsy sample was obtained, 47 in which only a cytologic brushing sample was obtained and 155 in which both types of samples were obtained ("combined specimens"). Among the 155 combined specimens, 4 of the confirmed malignancies were detected by histology only while 2 were detected by cytology only. A diagnosis of suspicious or positive for malignancy was made for 20% of the combined specimens as compared to less than 5% of the biopsy-only or brushing-only specimens. While 15% of the specimens that included a brushing sample (either alone or with biopsy) showed fungal infections, only 1.2% of the biopsy-only specimens did. It appears that the clinicians at this hospital tend to use brushing in combination with biopsy when either a malignancy or a fungal infection is suspected. Other empiric advantages of endoscopic brushing cytology include its rapid turn-around time, minimal invasiveness and good recognition of lymphoid cells. The selective use of brushing cytology should increase the probability of detecting malignancies and fungal infections without any increased risk or discomfort to the patients.

[Enteral feeding of patients after operations on the gastrointestinal tract].

Jejunostomy was put in 21 patients operated on the gastrointestinal tract, and on the 3-4 day after operation it was used for entero-probe nutrition (amino acid mixtures, fat emulsions, glucose solutions meant for parenteral nutrition, 3 1 a day at a speed 30-50 drops per minute). Clinical observations have shown high therapeutic effect, safely and technical simplicity of this entero-probe nutrition.

Endocrine cells in the gastrointestinal tract of a stomachless teleostean fish.

Endocrine cells in the gastrointestinal tract of the stomachless teleostean fish, Notemigonus crysoleucas, were studied using electron microscopy. Located between the absorptive cells of the intestinal epithelium, the enteroendocrine cells were very few in number. While some of the cells had their secretory granules located basally and a long narrow part extending toward the lumen, many appeared rounder and the plane of the section did not indicate that they extended to the lumen. Based upon size and shape of secretory granules, there appear to be several different types of cells: those with the smallest granules distributed throughout the intestine, those with intermediate sized granules more commonly found in the middle and distal segments and a few with large granules seen most often in the distal intestine.

[Current significance of polypectomy of the upper gastrointestinal tract].

Author's experience about 70 endoscopic polipectomy of high gastrointestinal tract, realized on 53 patients during 1978-1983 period was evaluated. Esophaguel polyps incidence was 0.06% among 7.900 endoscopic studies; gastric polyps was 0.8% and duodenal was 0.03%. Diagnosis was radiologically suspected in 58%. Gastric antrum was the predominant localization (45.7%) and macroscopic Yamada types IV (32.8%) and III (30.0%) were found more frequently. Most of patients presented solitary polips (86.7%). 63.2% of pieces were under 10 mm in size and 35.2% between 10 - 20 mm. High frecuency of adenomas (23.5%) and a low one of hyperplasiogenic (7.3%) was the prominent finding; only one case of early gastric cancer was founded (1.5%). Neither adenoms or hyperplasiogenic polyps were founded on gastric fundus, and no direct relation between size and histopatologic type was observed. Only one hemorragic complication (1.4%) was present. Polips incidence on 800 consecutive auptopsy protocols in adults was 0.12%.

[Pharmacological actions of alkylaminoalkyl-phenylbenzisothazole compounds on the gastrointestinal tract].

In the present paper pharmacological properties studied on the gastrointestinal tract of two new alkylaminoalkylphenylbenzisothiazole derivatives, 4-dimethylamino-2-phenyl-2-(1,2-benzisothiazole-3-yl)butyramide (PM2) and N,N-dimethyl-3-phenyl-3-(1,2-benzisothiazole-3-yl)propylamine (PM3) have been reported. Both drugs showed antispasmodic effects on gastroduodenal junction of the anaesthetized rat stimulated by Caerulein, according to previous results obtained on guinea-pig isolated ileum. On this substrate they were different in the potency being PM3 more active than PM2. On the contrary, the hypersecretion induced by Histamine or Gastrin-17 in rat perfused stomach was potentiated by PM2 and inhibited by PM3. Similar effects were observed on guinea-pig "in vitro" stomach preparation where PM2 and Papaverine were ineffective in modifying Histamine dose-response curves and PM3 reduced significantly maximal peak effects of Histamine, behaving as a non-competitive antagonist. The significant differences observed on gastric secretion but not on other substrates, from compounds structural analysis, appear scarcely justified and seem to us important to be further investigated.

[The endocrine cells of the gastrointestinal epithelium and the metabolism of biogenic amines in the gastrointestinal tract (author's transl)].

After a review on the historical development of morphological investigations of entero-endocrine cells, dating back to 1870, a detailed synoptical review of the current stage of findings in this field is given. At the present time nine different endocrine cell types can be distinguished in the epithelium of the gastrointestinal tract. Criteria for this differentiation are properties concerning specific staining methods, aldehyde-induced fluorescence, immunohistochemistry, and ultrastructure. From present results it is obvious that distinct cell types are responsible for the synthesis of defined polypeptide hormones (e.g. gastrin, secretin, enterogastrone). The metabolism of amines, in relation to the endocrine cells of the gastrointestinal tract is of particular interest here. Points investigated include the uniqueness of endocrine cells, with regard to the metabolism of biogenic amines ("APUD-cells") and the possibility of serotonin synthesis by a definite cell type, i.e. by the EC-cell ("enterochromaffin" cell). In our experimental animal, male Wistarrats, seven different entero-endocrine cell types can be discerned by ultrastructural means: EC-, ECL-, G-, AL-, EG-, D- and D1-cells. The I-cell (found in other species) can hardly be distinguished from the AL-cell by ultrastructural means and the S-cells, as found in other species, are not to be found at all. Only some of the cited cell types can be seen by fluorescence microscopy. After formaldehyde-treatment of the tissue, the "enterochromaffin" cell shows a yellow, serotonin-specific fluorescence. This cell corresponds in shape, number and distribution to the ultrastructurally defined EC-cell. EC-cells are found predominantly in the pyloric region and the duodenum and less frequently in the middle- and hindgut and the cardiac region; seldomly EC-cells are encountered in the oxyntic gland area of the stomach. In the rat gastro-intestinal tract, number and fluorescent intensity of EC-cells does not always correspond with the serotonin content of a certain region--sometimes the level of serotonin is largely determined by the mast cells, which in the rat also contain serotonin. For example, the high serotonin content of the oxyntic gland area, which contains very few EC-cells, has to be contributed nearly exclusively to mast cell serotonin. Mast cells can be domonstrated by fluorescence microscopy, due to their histamine content, after treatment of the tissue with o-phthalaldehyde (OPD). It seems likely that the histamine content, especially that of the so-called "atypical mast cells" of the mucosa, is inversely related to their respective serotonin content. --In addition to mast cells, OPD-treatment leads to a fluorescence in some of the entero-endocrine cells of the gastrointestinal epithelium. In the gastric epithelium these fluorescing cells should be regarded as histamine-containing ECL-cells and glucagon-containing AL-cells while in the colonic epithelium they are considered to be glucagon-containing AL-cells...

Peritoneal collagen type IV concentration in adenocarcinoma of the gastrointestinal tract and its relationship to histological differentiation, metastasis, and survival.

To determine if peritoneal collagen type IV levels could serve as a parameter for predicting metastasis and the subsequent course of disease, the concentration of collagen IV in the peritoneal fluid of 85 patients with adenocarcinoma of the gastrointestinal tract, including 50 with gastric cancer and 35 with colorectal cancer, was measured radioimmunologically. The peritoneal collagen type IV levels were elevated in 13 (26%) of the patients with gastric cancer, in 8 (23%) of those with colorectal cancer, and in none of the control subjects. The mean concentration of collagen type IV in tumors characterized by peritoneal dissemination was significantly higher than that in those without metastasis; however, there were no significant differences in the collagen type IV levels between tumors with and those without liver metastasis, or between those with and those without lymph node metastasis. There was a significant correlation between the peritoneal collagen type IV level and survival time in patients with clinically evident peritoneal dissemination. A positive correlation was also found between collagen type IV and carcinoembryonic antigen levels. In conclusion, the levels of peritoneal collagen type IV provide evidence of peritoneal dissemination, and can aid in the prediction of life expectancy in patients with adenocarcinomas of the gastrointestinal tract.
