A description of the relationship between breastfeeding experiences, breastfeeding satisfaction, and weaning in the first 3 months after birth.

This longitudinal study describes the relationships among breastfeeding experiences, maternal breastfeeding satisfaction (measured by the Maternal Breastfeeding Evaluation Scale [MBFES]), and weaning in the first 3 months postpartum. Postal surveys were used to collect data antenatally and at 2 weeks, 6 weeks, and 3 months postpartum from 365 women recruited from 3 public hospitals in Australia. Breastfeeding problems were associated with a reduction in MBFES subscale scores (P < .05) but not with weaning. Infant breastfeeding satisfaction subscale score was negatively associated with perceived inadequate milk supply (P < .05) and positively associated with engorgement (P < .05). Significant predictors of weaning at all points were total MBFES score in the lowest tertile (adjusted odds ratios [ORs]: 2 weeks, 21.9; > 2-6 weeks, 14.6: > 6 weeks to 3 months, 6.4) and leaking milk (adjusted ORs: < or = 2 weeks, 0.23; > 2-6 weeks, 0.19; > 6 weeks to 3 months, 0.15). Perceived inadequate milk was a significantpredictoronly for weaning > 2 to 6 weeks (adjusted OR = 4.2).

Plasma catecholamines and plasma renin activity at birth and during the first days of life.

1. Plasma noradrenaline and adrenaline concentrations and plasma renin activity were measured in 21 mothers at delivery and in their babies at birth (umbilical cord blood) and on days 1 and 5 of extrauterine life. 2. At birth plasma renin activity was significantly higher in the newborn than in mothers. Plasma renin activity increased further, but not significantly, on day 1 of life and significantly decreased on day 5. On day 5, 10 min head-up tilting caused no change in plasma renin activity. 3. Plasma noradrenaline in the newborn was higher than in mothers at birth and significantly decreased thereafter. Plasma adrenaline levels at birth were similar in the newborn and their mothers and significantly lower in the newborn in subsequent days. Tilting caused no increase in either plasma adrenaline or noradrenaline levels. 4. No correlation was found between plasma noradrenaline and adrenaline levels and plasma renin activity, or between noradrenaline, adrenaline or plasma renin activity and blood pressure.

Determinants of induced abortion and their policy implications in four counties in north China.

A retrospective survey conducted in four counties in North China in 1991-92 shows that the probability of aborting a pregnancy is strongly related to parity. No induced abortions are found prior to the first live birth, and almost universal abortion is shown after the second. Women had a high risk of undergoing abortion after their first live birth because most (82 percent) had become pregnant again without meeting official requirements for late second births with long spacing between births. The likelihood that a pregnancy will be aborted is strongly determined by official family planning policy and regulations. Individual and household socioeconomic status plays a relatively insignificant role. Great variation in the prevalence of induced abortion exists at the county and village levels. In recent years, the incidence of induced abortion has increased among women with one living child. Even a two-child policy, with late childbearing and spacing, can have high social and health costs in a country where childbearing is universal and begins relatively early.

[Growth of full term low and adequate birth weight infants during the first two years of life].

OBJECTIVE: To assess the growth pattern of full term low and adequate birth weight infants during the first two years of life and to identify the determinants at the time of the greatest growth deceleration. METHODS: A prospective cohort study was conducted with 148 full term infants in five small towns of the state of Pernambuco, Northeastern Brazil. Newborns were recruited from maternities between January 1993 and January 1994 and their anthropometric measurements were taken at one, two, four, six, 12 and 24 months of life. Risk factors were analyzed using multivariable linear regression. RESULTS: The increment of mean weight-for-age and length-for-age were more evident for low birth weight when compared with adequate weight infants, especially during the first two months after birth. From this point onward it was observed a progressive mean growth deceleration in both indexes up to 12 months of life. All infants had similar weight and length growth patterns. However, adequate birth weight infants remained in an upper level. Socioeconomic variables explained 23% of variation for weight-for-age, followed by 4% for birth weight. Socioeconomic condition was also the factor mostly affecting length-for-age, explaining 28% of its variation, followed by birth weight, maternal height and diarrhea. CONCLUSIONS: The study results suggest that interventions aiming to adequate growth should focus on prenatal care and social and environmental factors during childhood as a way of ensuring full expression of the genetic potential of this population.

The risk of human immunodeficiency virus-1 infection in twin pairs born to infected mothers in Africa.

We examined birth order and delivery route as risk factors for mother-to-child transmission of human immunodeficiency virus (HIV)-1 in 315 twin pairs born in Malawi during 1994-1998. No antiretroviral drugs were administered to these subjects. Infections were detected by polymerase chain reaction and were stratified as having occurred either in utero, perinatally, or postnatally. Risk of in utero infection for 630 infants (39 infections) did not differ by birth order (first born, 6.3%; second born, 6.0%). Similarly, in 260 vaginally delivered infants evaluated for perinatal infection (45 infections), risk did not differ by birth order (first born, 15.9%; second born, 18.7%); risk of perinatal infection was significantly lower in cesarean-delivered infants (odds ratio, 0.19 [95% confidence interval, 0.02-0.78]). There was no effect on postnatal transmission rates. Thus, in contrast to the authors of earlier studies, we did not find birth order to be an important risk factor for infection in twins. These findings indicate that birth-canal exposure is not a major contributor to a newborn's risk of HIV-1 infection.

Changes in the cortisol responses of lambs to tail docking, castration and ACTH injection during the first seven days after birth.

Cortisol responses of Dorset and Scottish Blackface lambs were investigated at six ages between birth and seven days. There were four treatments: control handling and blood sampling (n = 52), tail docking (T) (n = 57), castration plus tail docking (CT) (n = 54), and intravenous adrenocorticotrophin (ACTH) injection (n = 60). Most lambs exhibited transient rises in plasma cortisol concentrations after treatment and the results in individuals were expressed as the area under the cortisol curve (integrated response). Postnatal changes in the integrated responses of control, T and CT lambs differed significantly between the two breeds; in Dorsets they increased markedly between four hours and one day and then decreased, whereas in Scottish Blackface lambs there were no significant changes. The cortisol responses of ACTH lambs decreased markedly between four hours and seven days in both breeds. The results shed light on postnatal changes in the activity of the hypothalamic-pituitary-adrenal axis and suggest for both breeds that the average increments in ACTH secretion caused by noxious stimuli increased markedly during the first one to three days after birth. Whether this reflected parallel increases in the levels of distress experienced by the lambs remains to be clarified.

Maternal and social factors associated with abortion in India: a population-based study.

CONTEXT: A cultural preference for sons may be a factor driving recourse to abortion in India, as women carrying female fetuses may decide to terminate their pregnancies. To assess this hypothesis, more information on the incidence of abortion, and on maternal and social correlates of the procedure, is needed. METHODS: Birth order-specific abortion ratios were calculated using the birth histories of 90,303 ever-married women aged 15-49 who participated in India's 1998-1999 National Family Health Survey. For the first four births, the association between abortion and various maternal and social variables, including the sex of the respondent's last child, was assessed using logistic regression. RESULTS: The overall abortion ratio was 17.0 per 1,000 pregnancies. The ratio increased from 5.3 per 1,000 pregnancies for first-order births to 25.8 per 1,000 pregnancies for third-order births and then declined. The strongest predictor of abortion was maternal education: Women with at least a primary education were more likely than those with no education to have had an abortion (odds ratios, 1.9-6.7). Rural residence was associated with a reduced likelihood of abortion (0.6). There was no association between the sex of a woman's previous child and the odds that she subsequently had an abortion. CONCLUSION: At the national level, it is likely that unintended pregnancy, rather than the sex of the previous child, underlies demand for abortion in India. Rising educational attainment among women may lead to an increase in the demand for abortion.

The post-natal changes in minimal metabolic rate in the pig.

1. Minimal metabolic rate was measured as oxygen consumption in new-born pigs when environmental temperature was rising and passing through thermal neutrality.2. Three different periods were observed in 20 days after birth. The first period included the time from birth to 82 hr and was characterized by a continuous rise in minimal metabolic rate from 9.4 to 16.0 ml. O(2)/kg.min (70% increase). The second period extended from 82 hr to 9.5 days with values near to 16 ml. O(2)/kg.min. The third period beginning on the eleventh day lasted until observations ceased on the twentieth day and was characterized by a 25-30% fall in minimal metabolic rate when compared with the second period.3. Minimal metabolic rates from the second and third period were proportional to body weight and not to the surface area. Exponential factors for body weight were 0.93 and 0.96 for the second and third periods respectively.4. Rectal temperature rose from 38.6 degrees C on the first day to 39.3 degrees C on the second and 39.5 degrees C on the fourth day after birth.5. Environmental temperature associated with minimal metabolic rate showed a general tendency to fall from 36.5 in the first 4 days to between 33 and 34 degrees C in the following days.6. Age rather than body weight is the decisive factor influencing changes in minimal metabolic rate in the new-born pig.

Associations between ghrelin levels in serum of preterm infants and enteral nutritional state during the first 6 months after birth.

BACKGROUND: Previous studies have suggested a possible influence of ghrelin on foetal growth. After birth, the regulation of this newly discovered orexigenic peptide is largely unknown. OBJECTIVE: To study the relationship between circulating levels of ghrelin and enteral nutritional state in preterm infants during the first months of life. METHODS: Ghrelin levels were measured in a cross-sectional study by radioimmunoassay on the second day after birth (n = 51), at 3 months (n = 63) and at 6 months (n = 53) of corrected postnatal age. Complete data sets of auxological parameters, biochemical values, perinatal diseases, nutritional management and therapy were determined. RESULTS: All infants showed levels of ghrelin in postnatal serum. In correlation analyses and multivariable linear regression models, ghrelin was strongly related to enteral caloric intake on the second day after birth (all P < 0.01). At 6 months of corrected postnatal age, infants who were exclusively breastfed/formula fed had significantly lower ghrelin levels than infants with solid foods (Mann-Whitney U-test: P = 0.027). CONCLUSION: Ghrelin levels were positively correlated with the enteral nutritional state in preterm infants on the second day after birth. The introduction of solid foods increased the ghrelin levels in a group of preterm infants at 6 months of corrected postnatal age.

Developmental changes in astrocyte density in the macaque perifoveal region.

We studied astrocyte density both in the perifoveal region and in extrafoveal regions within the same distance of the optic disc (OD) over a time period from foveal pit formation (embryonic day E112) until 2 months after birth. The study was prompted by earlier observations that the adult macaque displays an almost astrocyte-free region around the fovea which, however, at birth is occupied by astrocytes. Thus, we wanted to determine if the perifoveal region is invaded by astrocytes during early development to the same degree as other regions in the central retina, and how the reduction in density can be explained. From the earliest age we studied (embryonic day 112), less astrocytes were found in the perifovea than in other regions equidistant from the OD. In addition, the number of astrocytes steadily declined both in the perifovea and outside until birth. During the first week after birth, there was a further dramatic decline in perifoveal astrocyte density. Double-labelling with glial fibrillary acidic protein (GFAP) immunocytochemistry and the TUNEL method showed that during the whole observation period astrocytes undergo DNA fragmentation and presumably die. However, the rate of TUNEL-positive astrocytes did not significantly differ between perifovea and other regions equidistant to the OD, and at no time did we find a significant peak of apoptosis rate. Thus, the reduction in perifoveal astrocyte density cannot be explained by missing invasion or by selectively elevated apoptosis rates in the foveal and perifoveal regions. Alternative hypotheses are discussed.

A modified stochastic model for closed birth interval.

"Based on some simple assumptions regarding [the] human reproduction process, a continuous time stochastic model for describing the variation in any closed birth interval of a woman of marital duration (t) has been developed. The model incorporates the possibility of the variation of the amenorrhea period among the women under observation. For illustration, the model is applied to an observed closed birth interval between first and second births of the women with marital duration of seven years."

Risk factors for breast cancer: pooled results from three Italian case-control studies.

The role of menstrual and reproductive factors, family history, and body weight in the epidemiology of breast cancer has been reassessed in a meta-analysis of three large case-control studies of breast cancer from several Italian regions, for a total data set of 4,072 cases and 4,099 controls. Multiple logistic regression equations were used to obtain relative risks adjusted for study, center, age, and various combinations of risk factors considered. Relative to women with menarche at age 15 or over, those with earlier menarche had a 20-30% higher breast cancer risk. However, there was no tendency for the risk to increase with lower age at menarche, and the association with menarche was stronger at younger age. The risk of breast cancer was directly related to age at menopause (relative risk (RR) = 0.7 for less than 45 years vs. greater than or equal to 50 years), age at first live birth (RR = 1.8 for greater than or equal to 28 years vs. less than 22 years), and family history of breast cancer in first-degree relatives (RR = 2.0). The effect of these factors was similar in various age strata. After allowance for age at first live birth, the risk of breast cancer did not differ among women with one to four live births, but it was significantly below unity (RR = 0.6) for those with five or more live births. Furthermore, there was a clear modifying effect of age at diagnosis on parity-related risk, since parous women had elevated breast cancer risk below age 35 and reduced risk above age 40.(ABSTRACT TRUNCATED AT 250 WORDS)

Aquaporins in complex tissues. I. Developmental patterns in respiratory and glandular tissues of rat.

Developmental expression of aquaporin water transport proteins is not well understood in respiratory tract or secretory glands; here we define aquaporin protein ontogeny in rat. Expression of aquaporin-3 (AQP3), AQP4, and AQP5 proteins occurs within 2 wk after birth, whereas AQP1 first appears before birth. In most tissues, aquaporin protein expression increases progressively, although transient high-level expression is noted in distal lung (AQP4 at postnatal day +2) and trachea (AQP5 at postnatal day +21 and AQP3 at postnatal day +42). In mature animals, AQP5 is abundant in distal lung and salivary glands, AQP3 and AQP4 are present in trachea, and AQP1 is present in all of these tissues except salivary glands. Surprisingly, all four aquaporin proteins are highly abundant in nasopharynx. Unlike AQP1, corticosteroids did not induce expression of AQP3, AQP4, or AQP5 in lung. Our results seemingly implicate aquaporins in proximal airway humidification, glandular secretion, and perinatal clearance of fluid from distal airways. However, the studies underscore a need for detailed immunohistochemical characterizations and definitive functional studies.

Minor physical anomalies and family adversity as risk factors for violent delinquency in adolescence.

OBJECTIVE: Minor physical anomalies are considered indicators of disruption in fetal development. They have been found to predict behavioral problems and psychiatric disorders. This study examined the extent to which minor physical anomalies, family adversity, and their interaction predict violent and nonviolent delinquency in adolescence. METHOD: Minor physical anomalies were assessed in a group of 170 adolescent boys from low socioeconomic status neighborhoods of Montréal. The boys had been enrolled in a longitudinal study since their kindergarten year, when an assessment of family adversity had been made on the basis of familial status and the parents' occupational prestige, age at the birth of the first child, and educational level. Adolescent delinquency was measured by using self-reported questionnaires and a search of official crime records. RESULTS: Results from logistic regression analyses indicated that both the total count of minor physical anomalies and the total count of minor physical anomalies of the mouth were significantly associated with an increased risk of violent delinquency in adolescence, beyond the effects of childhood physical aggression and family adversity. Similar findings were not found for nonviolent delinquency. CONCLUSIONS: Children with a higher count of minor physical anomalies, and especially a higher count of anomalies of the mouth, could be more difficult to socialize for different and additive reasons: they may have neurological deficits, and they may have feeding problems in the first months after birth. Longitudinal studies of infants with minor physical anomalies of the mouth are needed to understand the process by which they fail to learn to inhibit physical aggression.

Timing of pregnancy and the risk of epithelial ovarian cancer.

Recent animal studies suggest that progestagen-induced apoptosis of transformed ovarian surface epithelial cells may underlie the observed protective effect of pregnancy on the risk of ovarian cancer. Assuming that increasing numbers of cells are transformed with advancing age, we postulated that the benefits of pregnancy would be greater for older than younger women and tested this hypothesis in a population-based case-control study. We conducted interviews with 620 parous women, ages 18-79 years, with histologically confirmed incident ovarian cancer and 723 parous controls of the same age. Detailed information was collected on reproductive history, as well as hormonal exposures, smoking, medical history, and other factors. We estimated the relative risk of ovarian cancer associated with births at different ages through multiple logistic regression models. After adjusting for parity, older age at first and last births, and shorter time since last birth were all associated with significantly reduced risks of ovarian cancer. Age at first birth and time since last birth were not associated with ovarian cancer when adjusted for each other, whereas age at last birth remained strongly protective [odds ratio (OR), 0.57; 95% confidence interval (CI), 0.36-0.90] among women >35 years versus women less than 25 years. The effect was independent of total parity (per year of age among women with one birth: OR, 0.93; 95%CI, 0.87-1.01; among women with four or more births: OR, 0.96; 95%CI, 0.90-1.02). Our finding that ovarian cancer risk is reduced by pregnancy at older ages is further evidence that pregnancy confers a benefit beyond anovulation and is consistent with the theory that ovarian surface epithelial cell apoptosis induced by pregnancy hormones may be the underlying protective mechanism.

Parturition control in sows with a prostaglandin analogue (alfaprostol).

The objective of this study was to determine the dose of alfaprostol (18, 19, 20-trinor-17-cyclohexyl-13, 14 didehydro-PGF(2)alpha-methylester) and the day of administration most effective in inducing sows to farrow during normal working hours (0700 to 1700). One-hundred forty multiparous crossbred sows, taken from a herd whose mean gestation length was 114.3 days, were assigned to one of five treatment groups: 1) control vehicle-propylene glycol, 2) 0.5 mg alfaprostol (AP). 3) 1 mg AP, 4) 2 mg AP and 5) 3 mg AP. Sows received an intramuscular injection of AP between 0800 and 0830 on either day 111, 112 or 113 of gestation. Parameters studied included interval from injection to birth of first pig, farrowing interval, total number of pigs born, number born alive, average birth weight, percent stillborn, interval from weaning to next estrus and number born alive next litter. The mean intervals from injection to the birth of the first pig were 55.2 +/- 7.1; 41.1 +/- 5.1; 29.6 +/- 4.0; 24.3 +/- 1.1; 24.8 +/- 0.9 h for groups 1, 2, 3, 4 and 5, respectively (P<0.05). The percentage of sows that farrowed during normal working hours (23 to 33 h after injection) for groups 1, 2, 3, 4 and 5 were 15, 53, 60, 77 and 70%, respectively. Average birth weights (kg) for treatment groups 1, 2, 3, 4 and 5 were 1.54 +/- 0.05; 1.65 +/- 0.05; 1.59 +/- 0.05; 1.54 +/- 0.05 and 1.42 +/- 0.05, respectively (P<0.05). Mean differences in total number of pigs born and number born alive were also statistically significant (P<0.05) but stillbirth rate was not different (P>0.05) among treatment groups. These results indicate that a single injection of 1, 2 or 3 mg of alfaprostol will successfully induce parturition the following day in a majority of the treated sows.

Increased plasma concentrations of vasopressin, oxytocin, cortisol and the prostaglandin F2alpha metabolite during labour in the dog.

AIM: This study investigated if the plasma vasopressin concentration increases during labour in the dog and whether the change in vasopressin correlates with that of oxytocin, 15-ketodihydro-PGF2alpha and cortisol. METHODS: Five beagle dogs each delivered three to seven puppies. Blood samples were taken from a catheter inserted into the cephalic vein during labour and by venepuncture during the other periods. RESULTS: Vasopressin concentration increased from 2 +/- 0 pmol L-1 (anoestrus) to 26 +/- 11 pmol L-1 at the birth of the first puppy, remained high at the birth of the second puppy and then decreased. Oxytocin increased from 63 +/- 5 pmol L-1 (anoestrus) to 166 +/- 19 pmol L-1 at the birth of the first puppy and remained elevated throughout labour. The PGF2alpha metabolite concentration increased from 0.2 +/- 0.0 nmol L-1 (anoestrus) to 66 +/- 17 nmol L-1 at the birth of the first puppy and remained elevated 1 h after the completion of parturition. The cortisol concentration increased from 49 +/- 9 nmol L-1 (anoestrus) to 242 +/- 35 nmol L-1 at the birth of the first puppy, remained high during the birth of the second puppy and then declined. CONCLUSIONS: The plasma level of vasopressin was strongly correlated with that of cortisol but less with that of the PGF2alpha metabolite, and not significantly with the concentration of oxytocin. This indicates that the four hormones play different roles during labour in the dog.
