OBJECTIVE: To investigate the clinical activity and toxicity of a combination chemotherapy consisting of cyclophosphamide (C), adriamycin (A) and cisplatin (P) for patients with primary adenocarcinoma of the Fallopian tube having FIGO stage III-IV disease. METHODS: The CAP-regimen consisted of cyclophosphamide 600 mg/m2, adriamycin 45 mg/m2, and cisplatin 50 mg/m2 administered intravenously on day one every 28 days. RESULTS: Twenty-four eligible patients with histologically-confirmed Fallopian tube adenocarcinoma were entered in the trial. Fourteen patients had FIGO stage III, and ten had stage IV disease. The median number of CAP cycles was six. Ten patients had a complete and six had a partial response (response rate: 67%, 95% confidence limits: 45-84%). WHO grade III-IV side-effects included haematological toxicity, nausea/vomiting and alopecia. Furthermore, mild signs of cisplatin-related peripheral neurotoxicity were observed. At a median follow-up of 40 months, nine patients were alive and 15 had died due to malignant disease. The median time to progression was 13 months for all patients. The median overall survival was 24 months and the 1-, 3- and 5-year survival and their 95% confidence limits were 73% (54-92%), 25% (4-46%) and 19% (0-38%), respectively. CONCLUSION: The present data confirm the therapeutic activity of the CAP-regimen in primary Fallopian tube adenocarcinoma. The response rate is moderate and the toxicity profile is acceptable.
The differential diagnosis of abdominal pain in women during their childbearing years is still a challenge. Abdominal pain caused by isolated tubal torsion is extremely rare. In the report, we presented a case of isolated tubal torsion caused by benign serous cystadenoma. A 28-year-old married woman (G0) having lower abdominal pain and nausea was referred to our Emergency Room with suspicion of ovarian mass. Physical examination, transvaginal ultrasound and computed tomography showed a 4-cm right adnexal cystic mass. Other laboratory data were all within normal limits. She was managed by laparoscopic examination due to a lack of improvement in her clinical symptoms and inclusive diagnosis after 48-hours' conservative treatment. Laparoscopy showed isolated torsion of right fallopian tube but the right ovary was normal without torsion. Initially, detorsion was performed but necrotic change of the fallopian tube persisted because of permanent darkened color tube without blood flow redistribution, so salpingectomy was performed 30 minutes later. Final pathology showed benign serous cystadenoma of fallopian tube. The present case is the first case of benign serous cystadenoma with resultant necrotic tubal torsion. We reported this case to emphasize the possible value of early performance of laparoscopy in aiding an accurate diagnosis.
Morphological examination of 452 Fallopian tubes removed together with the uterus for uterine cervix cancer revealed in 221 patients at the time of operation and in 5 dead on autopsy, metastatic cancer in 8 females (3.5%) in 12 tubes (right-side involvement 2 cases, left-side 2 cases, ambi-lateral 4 instances). In 37 patients there were metastases in inner organs; of these diseased 21.6% were affected by metastatic cancer of the Fallopian tube. Macroscopically, metastatic tumors were of different form in 4 cases; in the remaining instances they were only discovered on microscopical examination. In two cases the tubes were affected in their whole length, in the other case the tumor was located in the ampullary portion. The cyto- and histological structures of the metastatic cancer were the same as those of the primary uterine cervix tumor. The predominating route of tumor dissemination has proved to be lymphogenic propagation (5 cases). In two cases lymphohematogenic dissemination, in one case propagation by implantation were observed.

BACKGROUND: Since telomerase activity is detectable in cancer cells but not in some normal somatic cells, it has been considered as a potential diagnostic marker as well as a target for possible anticancer strategies. The purpose of this study was to assess the value of telomerase activity determination in some gynecological tumors and to compare it with the CA 125 tissue and serum profile. PATIENTS AND METHODS: The telomerase activity was determined in 11 gynecological tumors: 7 ovarian carcinomas, 2 carcinomas of the fallopian tube and 2 cervical carcinomas, and compared to the activity in the normal peritoneal tissue of the same patients. Additionally, the levels of CA 125 were measured in the tumor and normal peritoneum tissue samples as well as in the patients' sera. RESULTS: In ovarian tumors, the telomerase activity was detected in 71.4% (5 out of 7), while in the carcinomas of the fallopian tube and cervical carcinomas such activity was not observed. Negative for telomerase activity were also all samples of peritoneum. The range of CA 125 in the tumor tissue was 99 U/g-803667 U/g of tissue and in the normal peritoneum 71 U/g-4925 U/g of tissue. CONCLUSION: In conclusion, telomerase activity could be detected in some of the gynecological tumors, but for clinical use as a diagnostic marker it is inferior to CA 125.
OBJECTIVE: To observe the therapeutic effect of palitaxel. METHODS: Of 37 cases, 33 were treated when tumor recurrence occurred after surgery or chemotherapy. There were 27 cases with ovarian cancer, 6 with fallopian tube cancer. 3 with cervix cancer, and 1 with endometrium cancer. In all cases, diagnosis was confirmed by histopathology. Palitaxel (135-150 mg/m2) was administered i.v. or i.p. once every three werks in combination with cisplatin (PDD) 60 mg/m2 and cyclophosphamide (CTX) 60 mg/d. Antihistaminics and antiemetic were given prior to palitaxel. The results of treatment were compared with those in 37 cases treated in the some time period with PAC or VBP regimen. RESULTS: The overall median survival time of the control group was 4 months, and 8.5 months in the palitaxel treated group (P < 0.001). The response rate was 18.9% and 48.6% respectively (P < 0.005). The major toxic effects was myelosuppression and alopecia. CONCLUSION: Palitaxel is effective in the treatment of advanced gynecological tumors.
We report a rare case of primary carcinoma of the fallopian tube. A 72-year-old post-menopausal woman presenting with abnormal secretion of blood-stained liquid, underwent surgery because of findings of atypical squamous cells (ASCUS) on a routine Papanicolaou smear. The histological diagnosis on cervical biopsy was of CIN 3. Adenocarcinoma of the Fallopian tube was incidentally found.

A 51-year-old woman consulted for pelvic pain, metrorrhagia and leukorrhea. Physical examination revealed a renitent and mobile mass in the pelvis. A right lateral uterine mass with hydroxalpinx was found at ultrasonography. Pathology examination of the right annexectomy specimen provided the definitive diagnosis: fallopian tube cancer with polymyositis. No residual tumor was found at total hysterectomy with total bilateral annexectomy. The patient was lost to follow-up for three years without complementary treatment then consulted later for functional disability of the upper then lower limbs with myalgia, swallowing disorders and left supraclavian node enlargement resulting from pelvic relapse of the right fallopian tube adenocarcinoma and left supraclavian metastasis with paraneoplastic polymositis. The patient was given 6 courses of chemotherapy with radiotherapy (45 Gy) centered on the left clavian region. The patient exhibited a spectacular response, and remains in complete remission 50 months after diagnosis. The association of a fallopian tube tumor with polymyositis is exceptional, requiring rapid anticancer treatment effective against the cancer and the paraneoplastic polymyositis.
The cancer of the fallopian tube is one of the most rare malignant diseases of the female genital tract. The authors present a case of a 66-year old women, that was difficult for diagnosis and interpretation of the clinical appearance and the results of biopsic curettage. The discussion of the case is accompanied by a literature review for the last five years concerning diagnosis, prognosis and treatment of this rare disease.
