[2 cases of paucibacillary leprosy resistant to dapsone].

Two patients are presented with a primary resistant BT leprosy. Diagnosis was made on a clinical base: luck of improvement under supervised sulfone treatment and quick regression of the lesions under a short course of rifampicine (8 weekly doses). The two children lived together with their lepromatous mothers who are suspected of DDS resistance. Where secondary DDS resistance is found only in multibacillary leprosy, primary resistance shows up in all clinical forms of leprosy.

Social and personality factors in dapsone resistant patients.

A preliminary study in which a sample of 25 patients registered with SLR and TC, Karigiri, with proved secondary DDS-resistant leprosy were compared, with 25 patients not suspected to be DDS-resistant, for social and personality factors, which could have lead to non-compliance in treatment. A structured interview schedule enquiring into factors leading to irregular treatment and the psycho-social background of the patient was used. The Eysank's Personality Inventory (EPI) and Personality traits assessment on a linear analogue was also administered to them. Statistical analysis showed that DDS-resistant leprosy patients were as a group more irregular for treatment, showed personality traits of being casual and uncontrolled and scored high on the neuroticism scale of the EPI.

Mechanism of action of the folate blocker diaminodiphenylsulfone (dapsone, DDS) studied in E. coli cell-free enzyme extracts in comparison to sulfonamides (SA).

The antibacterial activity of DDS has been studied in whole cell (E. coli), cell-free folate synthesizing enzyme extracts and compared to effects obtained for sulfonamides (SA). It is shown that DDS acts as a synthetase inhibitor in the folate synthesizing enzyme system. DDS reacts with the substrate 7,8-dihydro-6-hydroxymethylpterinopyrophosphate to form a 7,8-dihydropteroic acid analog. Bacterial growth kinetic studies were performed to test for possible synergistic activity of the analog in combination with DDS. Possible reasons for the extremely large inhibitory power of DDS against M. leprae are discussed.
