The postmigrational development of polymicrogyria documented by magnetic resonance imaging from 31 weeks' postconceptional age.

We report the case of a 27-week premature infant in whom magnetic resonance imaging (MRI) at 4 postnatal weeks (postconceptional age, 31 weeks), term, and 6 months of age documented the postnatal postmigrational evolution of bilateral perisylvian polymicrogyria. The polymicrogyria was readily detected by ultrafine 1.5-mm coronal slices on three-dimensional, Fourier-transformed, spoiled gradient-recalled and T2-weighted MRI sequences. These MRI sequences provide the first in vivo documentation of the postmigrational evolution of polymicrogyria. The likelihood that the polymicrogyria was related to an ischemic encephaloclastic mechanism is supported by the simultaneous occurrence of periventricular leukomalacia.

From conjecture and refutation to the documentation of occupational diseases in Taiwan.

The documentation of occupational diseases in a developing country like Taiwan is a challenge to an occupational physician. For lack of a system including material safety data sheet (MSDS), workers are usually not aware of what chemicals they are exposed to. Since many occupational diseases have long latencies and do not show any specific symptoms and signs, recognition and identification of the occupational origin are often very difficult. Using databases which provide a relatively complete list of industrial chemicals and a set of specific signs and/or symptoms, combined with the epidemiologic approach of conjectures and refutations--i.e., considering and ruling out all possible alternative explanations--we have documented eight kinds of occupational diseases and an outbreak of botulism. We recommend that a similar approach be applied to any other country with a similar situation, and that a system involving an identification sheet (e.g., MSDS) for each chemical be advocated and implemented in such countries as one means to enable prompt recognition and prevention of occupational diseases.

Bone marrow transplantation for severe combined immune deficiency in an infant with chimerism due to intrauterine-derived maternal lymphocytes: donor engraftment documented by chromosomal marker studies.

Chromosomal heteromorphisms defined by the quinacrine banding technique were used to identify the maternal origin of 46,XX lymphocytes present in the blood of a male infant with severe combined immune deficiency disease. These chromosomal markers were also used to document the engraftment by donor lymphocytes from the sister and the concurrent disappearance of maternal lymphocytes after a successful bone marrow transplantation. Donor lymphocytes were detected by this technique 6 days after transplantation, earlier than is usually possible with other marker systems and before definite evidence of immunoreconstitution. Maternal lymphocytes persisted in the patient's peripheral blood for a prolonged period of time, being detectable 172 days after transplantation. Analysis of T-lymphocyte- and B-lymphocyte-enriched populations after transplantation documented lymphoid chimerism with T-lymphocytes of donor origin and B-lymphocytes of both patient and donor origin, demonstrating prolonged persistence of patient B-lymphocytes and suggesting that the patient's immune defect is primarily at the T-lymphocyte level.

Environmental effects on skeletal versus dental development: Using a documented subadult skeletal sample to test a basic assumption in human osteological research.

This study examines the relationship between measures of skeletal and dental development and socioeconomic factors in a 20th century documented skeletal sample of children from Portugal. The skeletons are of known sex and chronological age, and include other biographic data, such as cause of death. Growth in the length of the long bone is used as a measure of skeletal growth, and schedules of tooth formation are used as a measure of dental development. These two measures of physiological age were compared to chronological age, to assess growth and developmental status. Socioeconomic indicators were obtained from the supporting documentation, and include the occupation of the father and the place of residence, which were used to build a socioeconomic classification based on two groups, one of low and the other of high socioeconomic status. Growth and development status was then compared in these two groups. Results show that socioeconomic differences are much more pronounced in skeletal growth than in dental development. This largely supports the assertion that dental development is buffered against environmental factors relative to skeletal development. However, in this study, skeletal maturation could not be assessed, and findings indicate that dental development can show significant delays at the lower end of the socioeconomic gradient.

Banff schema for grading liver allograft rejection: an international consensus document.

A panel of recognized experts in liver transplantation pathology, hepatology, and surgery was convened for the purpose of developing a consensus document for the grading of acute liver allograft rejection that is scientifically correct, simple, and reproducible and clinically useful. Over a period of 6 months pertinent issues were discussed via electronic communication media and a consensus conference was held in Banff, Canada in the summer of 1995. Based on previously published data and the combined experience of the group, the panel agreed on a common nomenclature and a set of histopathological criteria for the grading of acute liver allograft rejection, and a preferred method of reporting. Adoption of this internationally accepted, common grading system by scientific journals will minimize the problems associated with the use of multiple different local systems. Modifications of this working document to incorporate chronic rejection are expected in the future.

Allelic genes of blood group antigens: a source of human mutations and cSNPs documented in the Blood Group Antigen Gene Mutation Database.

In this report, we analyze data assembled in the Blood Group Antigen Gene Mutation Database (www.bioc.aecom.yu.edu/bgmut/index.htm), which describes sequence information on human genes associated with expression of the various serologically-determined blood group phenotypes. The database documents 38 genetic loci and a total of 624 alleles that together encode a large repertoire of proteins and constitute 27 serologically-defined blood group systems. Analysis of sequence variation patterns across alleles of a number of genes is focused on their molecular profiles, including mutational sites and recurrence, patterns of gene rearrangements in duplicated gene families, correlation of predicted location of epitopes in extracellular loops with sites of alterations, and effects of mutations on protein expression. That information, and the relative ease of identifying individuals bearing variant alleles, has led to the proposal that genes encoding blood group antigens are an important and unique resource for studies of human DNA variation. Another focus is on mutations in regions that encode the antigenic epitopes and on their occurrence in world populations. These mutations may be viewed as coding single nucleotide polymorphisms (cSNPs). We propose that one group of these cSNPs, which are known to occur with significant frequency in all world populations, could serve as well-validated genetic markers. In addition, specific mutations in a number of "low incidence" and rare alleles could serve as cSNPs specific for a given population. The allelic frequencies of these mutations and knowledge of their world-wide occurrence add a valuable dataset to the existing cSNP pools documented in SNP databases.

Patient discharge planning documentation in an Australian multidisciplinary rehabilitation setting.

This article, drawing on data from 17 case studies, addresses problems in documentation of discharge planning in an Australian multidisciplinary rehabilitation center and the ways these were, or could be, overcome. The problems identified in the case studies were not specific to one discipline; they were due to inadequately defined guidelines and responsibility and poorly designed forms, and they possibly reflected the tendency for members of practice-oriented disciplines to see documentation as low on their list of priorities.

An incident reporting system: documented at the point of service.

Many healthcare organizations maintain two systems for documenting incidents reports: A paper incident report system completed by staff and managers and A computerized system where clerical staff duplicate the same information in a database. This article discusses the way risk managers in hospitals and healthcare organizations can reduce paperwork and more rapidly address the implementation of a point of service computerized system to document incidents. The benefits of such a system include: Real-time information and trending of occurrences. Reduced liability by securing reports earlier. Maintaining a higher level of confidentiality. Earlier problem resolution. Providing a valuable link between the risk, quality, and safety functions of the organization.

Computed tomography of the abdomen and pelvis: documentation of tumor response and progression in disseminated prostate cancer.

Computed tomography of the abdomen and pelvis has been used to stage early prostatic cancer. We investigated its value in monitoring tumor response in more advanced disease. Serial computed tomography of the abdomen and pelvis was obtained along with multiple other staging tests prior to treatment and at 3- to 4-month intervals thereafter in 32 patients with stage D2 tumor treated initially with combination chemotherapy and with hormones at progression. Pretreatment lymphography with follow-up abdominal films was also performed. Initial computed tomography of the abdomen and pelvis showed evidence of node metastases in 35% of patients while lymphography was positive in 54%. Among 19 patients with tumor response and 25 with progression, the results of treatment were objectively documented by improvement or worsening, respectively, in a mean of 5.1 and 5.0 other staging tests, exclusive of computed tomography. Computed tomography of the abdomen and pelvis improved in 85% of responding patients examined. Progression was confirmed by worsening of computed tomography in 32%. Although lymphography was often abnormal prior to treatment, in our hands it was not useful in serial monitoring of tumor status. We conclude that serial computed tomography of the abdomen and pelvis, when initially positive, is a useful test to document objectively tumor response and progression in disseminated prostatic cancer.

Surgically documented responses to paclitaxel and cisplatin in patients with primary peritoneal carcinoma.

Intra-abdominal carcinomatosis indistinguishable from ovarian cancer may occur after removal of the ovaries or in association with surface ovarian involvement. Because its histologic pattern and behavior approximate those of ovarian cancer, this entity, known as primary peritoneal carcinoma, has been treated in a similar fashion--cytoreductive surgery followed by systemic chemotherapy. This review was undertaken to assess the efficacy of combination chemotherapy with paclitaxel and cisplatin, the current front-line chemotherapeutic regimen for ovarian cancer, in patients with primary peritoneal carcinoma. Sixteen patients diagnosed between January 1989 and July 1994 with primary peritoneal carcinoma were treated at the Hospital of the University of Pennsylvania. The records of the three patients whose initial chemotherapeutic regimen included paclitaxel and cisplatin were reviewed. An additional case from the Robert Wood Johnson Medical Center, Camden, New Jersey, was included. Pathologic review of all cases was conducted at the time of clinical management and again as part of this study. Reassessment laparotomy was performed in all patients after the completion of chemotherapy. Complete clinical information was available on all patients. All four patients presented with intra-abdominal carcinomatosis, and large volume (> 1 cm) residual disease was present following initial cytoreduction. Following chemotherapy, second-look laparotomy documented one complete pathologic response and three partial (>50% tumor volume reduction), but marked, responses. Combination chemotherapy with paclitaxel and cisplatin produces surgically documented responses in patients with primary peritoneal carcinoma.

PADS (Patient Archiving and Documentation System): a computerized patient record with educational aspects.

Rapid acquisition and analysis of information in an Intensive Care Unit (ICU) setting is essential, even more so the documentation of the decision making process which has vital consequences for the lives of ICU patients. We describe an Ethernet based local area network (LAN) with clinical workstations (Macintosh fx, ci). Our Patient Archiving and Documentation System (PADS) represents a computerized patient record presently used in a university hospitals' ICU. Taking full advantage of the Macintosh based graphical user interface (GUI) our system enables nurses and doctors to perform the following tasks: admission, medical history taking, physical examination, generation of problem lists and follow up notes, access to laboratory data and reports, semiautomatic generation of a discharge summary including full word processor capabilities. Furthermore, the system offers rapid, consistent and complete automatic encoding of diagnoses following the International Classification of Disease (ICD; WHO, [1]). For educational purposes the user can also view disease entities or complications related to the diagnoses she/he encoded. The system has links to other educational programs such as cardiac auscultation. A MEDLINE literature search through a CD-ROM based system can be performed without exiting the system; also, CD-ROM based medical textbooks can be accessed as well. Commercially available Macintosh programs can be integrated in the system without existing the main program thus enabling users to customize their working environment. Additional options include automatic background monitoring of users learning behavior, analyses and graphical display of numerous epidemiological and health care related problems. Furthermore, we are in the process of integrating sound and digital video in our system. This system represents one in a line of modular departmental models which will eventually be integrated to form a decentralized Hospital Information System (HIS).

[ICPM-independent documentation of new reimbursement forms--method and 1995 results].

The structural health care law (known as the Gesundheitsstrukturgesetz or GSG) of the Federal Republic of Germany has been enacted to replace the covering cost prices by various forms of payment. On the basis of the Godesberg Diagnosis and Therapy Catalogue, a new system has been developed that assesses all new implications of the care benefit law. With the help of intelligent plausibility tests, a particular case (PC) or a special rate (SR) is suggested. The documentation system works without ICPM numbers. The conversion of diagnosis and therapies to ICD-10/ICPM numbers is possible. The results of the first half of 1995 and the second half of 1995 show that only two-fifths of operations can be assessed with PC or SR. Neither emphasis on certain operations nor the more differentiated illnesses are recognised as extra costs by the GSG regulations. Complete and correct documentation is achieved by a strict and stratified control system.

[The polytraumatized patient. One year's documentation with special reference to the prognostic significance of individual immune status (cell-mediated immunity)].

Fifty-four patients with multiple trauma have been investigated in a prospective one-year documentation. The following features were documented and evaluated: circumstances of the accident, condition on admission, course under special consideration of the cell-mediated immunity with Merieux multitest, course after indoor treatment.

Carola, a computer system for automatic documentation in anesthesia.

A computer system has been designed for documentation and data acquisition during open heart surgery. This computer system (called 'Carola') processes all patient data during cardiac surgery. More than 50 analogue or digital signals are scanned. These are derived from a monitoring rack, a Siemens Servo 900B ventilator with its accessory devices and a heart lung machine. All these values are plotted as well as offline data, such as medications, fluids, laboratory results and user comments, on an A3 format anesthetic record using an eight pen flat bed plotter. Simultaneously all data is written onto a cassette tape. These tapes are then transferred to a database for storage and statistical processing. The sampling frequency is every 10 seconds, averages being calculated over one minute periods. The chart is updated once a minute normally or every 15 minutes for slowly changing signals e.g. temperatures. Hardware and software of the computer have modular design. The hardware consists of two Motorola 6809 based microprocessor systems. The software is entirely written in Pascal. The user interface is implemented on a menu driven basis. A terminal with a keyboard is used for the communication with the users, namely anesthetic nurses and anesthesiologists. The system was readily accepted by the users. The menu structure proved to be easy to learn and allowed fast entries, even when the users were not previously accustomed to the use of a keyboard. The clear and detailed presentation of the data on the plotted chart helped to detect trends early and facilitated therapeutic decisions. From december 1983 the first prototype was used on a routine basis, followed by a second unit in June 1984 and a third in December 1985. Up to now more than 12.500 anesthetic hours have been recorded. Since then almost 100% of all anesthetics performed in our cardiothoracic unit have been documented by the computers, including all short procedures without invasive monitoring and all emergencies.

Comparison of technetium 99m Q12 and thallium 201 for detection of angiographically documented coronary artery disease in humans.

BACKGROUND: 99mTc-labeled Q12 (99mTc-Q12) is a new imaging agent that produces myocardial visualization in humans. This study examined the hypothesis that a 100-minute rest-exercise tomographic imaging protocol after injection of 99mTc-Q12 can be used to detect the presence or absence of coronary artery stenoses. METHODS AND RESULTS: Imaging with 201Tl and 99mTc-Q12 was performed in 20 patients with angiographically documented coronary artery disease and 10 "normal" subjects including two patients with chest pain and normal coronary arteriograms and eight subjects with a very low likelihood of occlusive coronary disease. 99mTc-Q12 was imaged beginning 15 minutes after injection at rest and with exercise. In the 20 patients, a corresponding myocardial defect was detected in blinded fashion in 18 with 201Tl and 17 with 99mTc-Q12 (difference not significant). Of 10 patients without evidence of coronary disease, nine had a normal 201Tl scan and eight had a normal 99mTc-Q12 scan (difference not significant). Agreement of 99mTc-Q12 and 201Tl imaging for detection of regional myocardial perfusion defects was excellent (kappa = 0.88). Identification of the presence or absence of angiographically documented coronary disease in individual coronary artery distributions was 80% and 82% for 201Tl imaging and 73% and 87% for 99mTc-Q12 (difference not significant). CONCLUSION: 99mTc-Q12, used in a rest-exercise sequence that can be completed in 100 minutes, provided identification of regional myocardial perfusion defects similar to that of 201Tl.

[Deficits in management of patients with colorectal carcinoma in Germany. Results of multicenter documentation of therapy algorithms].

BACKGROUND: Adjuvant chemotherapy for patients with UICC III (Dukes C) colorectal cancer (consensus statements NIH 1990, German Cancer Society 1994) and palliative chemotherapy for metastatic disease have long been recognized to provide a survival benefit in colorectal cancer. However, it remains unclear if these concepts have made inroads into clinical practice. PATIENTS AND METHODS: Therefore, we asked 74 institutions treating colorectal cancer patients in Germany to document the treatment algorithms of all patients with colorectal cancer seen in the third quarter of 1998. Clinical careers of 1,001 patients (m/f 465/536; median age 62.9 years [28-93]; colon 596, rectum 405; UICC I 117, II 206, III 407, IV 218) were documented. RESULTS: Only 63.4% of patients with UICC III colorectal cancer received adjuvant therapy with a significant difference between hospitals with (67.1%) and without (42.6%) oncological departments (p < 0.01). Higher age appeared to be the most important factor for withholding treatment since 196 of 286 (68.5%) patients < 70 years, but only 57 of 121 (47.1%) > 70 years underwent adjuvant therapy. 78.4% of patients with UICC IV colorectal cancer (91.8% university hospitals, 76.8% hospital with, 50% without oncological departments, 66.7% rehabilitation clinics, 82.4% private practices) received palliative chemotherapy (first line: 5-FU/FA bolus 57%, 5-FU/FA infusion 20%, 5-FU mono 15%). CONCLUSION: Considering an annual incidence of colorectal cancer in Germany of 52,000 with 30% UICC III, discounting patients > 80 years or ECOG status > 2, and estimating a survival benefit of 10% after adjuvant chemotherapy, approximately 530 lifes are lost annually in Germany due to insufficient treatment of UICC III colorectal cancer based on our survey. In addition, substantial financial demand is generated by the subsequent palliative treatment of potentially curable patients.--In conclusion, survey-based analysis of treatment algorithms can provide valuable insights into clinical practice in oncology and can disclose deficits in patient care as demonstrated here in colorectal cancer.

[Hepatic resections. Analysis of data from the Tumor Documentation Center in the state of Brandenburg, Germany, focusing on liver metastases of colorectal carcinoma].

BACKGROUND: Data from the Brandenburg Tumor Documentation Center (TDCB) in Germany were analyzed for an overview of the current treatment standards of liver surgery in that state. MATERIAL AND METHODS: The analysis was based on prospective data from a total of 37,165 patients diagnosed with malignant tumors between 1 January 1999 and 31 December 2004. Of these patients, 3,986 were diagnosed with liver metastases and 554 had primary tumors of the liver or bile duct. Liver metastases of colorectal carcinoma were reported in 1,299. RESULTS: Analysis confirmed that resection of colorectal metastases (51%) and primary liver or bile duct tumors (23.1%) is by far the most frequent indication for liver surgery. Liver metastasis was developed by 29.2% (n=1299) of patients with colorectal carcinoma. Of the patient total, 71.5% showed evidence of liver metastasis already present when colorectal carcinoma was diagnosed. Of 248 patients who had received liver surgery after diagnosis of liver metastases of a colorectal carcinoma, 114 (46%) underwent hepatic segment resection, which was thus performed in only 8.8% (n=114) of patients with liver metastases after colorectal carcinoma (n=1299). CONCLUSIONS: Since only 8.8% of those with liver metastases underwent curative hepatic segment resection, we can conclude that if patients and doctors were provided with adequate information on the curative potential of this surgical method along with regular consultations with surgeons experienced in liver surgery, the result on resection rates would be positive. Data from tumor documentation centers enable selective analysis of the oncological situation of specific diseases.
