Unwanted pregnancies: understanding contraceptive use and benefits in adolescents and older women.

The highest percentages of unintended pregnancies and the largest increases in births reported in the United States in recent years were in adolescents and in women more than 35 years of age. Increasing numbers of these women will require contraception to avoid unintended pregnancy. In adolescents the combined oral contraceptive agents protect fertility indirectly and exert favorable actions on menstrual dysfunction and certain hormone-related disorders such as acne and hirsutism. To avoid sexually transmitted disease, barrier protection should be used along with oral contraceptives until mutually monogamous, stable relationships are established. Healthy older women who are nonsmokers may also safely use currently available contraceptives. These agents have little impact on metabolic parameters linked to the development of cardiovascular disease. In addition to providing reliable contraception, oral contraceptives offer noncontraceptive benefits to older reproductive-age women, including control of abnormal bleeding and a reduction in the incidence of ovarian and endometrial cancers and other gynecologic pathology. Intrauterine devices and progestin implants are safe, effective, and underused in the United States. Progestin implants may have an additional role in patients for whom estrogen preparations are contraindicated. Counseling is very important before insertion because of the high rate of nuisance side effects. The contraception selection process must consider the efficacy and acceptability of the specific method to avoid the probability of unintended pregnancy and the risk of sexually transmitted diseases.

Factors affecting the stability of the spike-triggered averaged force in the human first dorsal interosseus muscle.

The reproducibility of motor unit twitches obtained using spike-triggered averaging (STA) was examined in the human first dorsal interosseus. For each motor unit (30 total) a series of STA twitches was derived using a 30 s averaging window. Within each averaging window, eight independent measures characterizing motor unit discharge and whole muscle force properties were recorded. These included the mean and standard deviation (S.D.) of the interspike interval (ISI), the mean and S.D. of pre and post-trigger ISIs used in averaging, and mean and S.D. of whole muscle force. To determine the relative importance of the independent variables on twitch reproducibility, the variables were used in a multiple regression analysis performed on STA twitch peak force (PF), time to peak force (TTP) and time of half-force decay (HFD). It was found that PF was significantly correlated to the mean and S.D. of whole muscle force, and mean post-trigger ISI. TTP was significantly correlated to the S.D. of the post-trigger ISI and mean whole muscle force while HFD was related to the mean and S.D. of the pre-trigger ISI and the mean post-trigger ISI. It was concluded that by minimizing whole muscle force variability and the mean and S.D. of acceptable ISIs used in the STA process, the reproducibility of the STA twitch is improved.

Folding of the human immunoglobulin G3 Kus core hinge into the thirteenth globular domain.

Earlier, the electron microscopy and hydrodynamic studies revealed the transformation of the globule-like form of the human (h) IgG3 Kus hinge into a rod-like shape under non-denaturing perturbations [Eur. J. Biochem. 190 (1990) 393]. In this work, it is shown with the differential scanning calorimetry (DSC) that the melting of a globule-like form of the hIgG3 Kus hinge is a co-operative process. The 'two-state' model accepted for small globular proteins well describes the transition. Thus, in the hIgG3 Kus molecule, the core hinge folds into the thirteenth globular domain. The model of folding of four double poly-L-proline (PLP) helices of the core hinge into the compact tertiary structure similar to 'a folded camp bed' is suggested.

Exploratory data analysis using set operations and ordinal mapping.

Exploratory data analysis requires the ability to issue ad hoc queries to filter and summarise data sets. As the sizes of health data sets grow, traditional methods of processing data have difficulty in providing acceptable response times for such queries. An alternative method is described which combines complete vertical partitioning of data with set operations on ordinal mappings (SOOM). An initial implementation of the technique provides significantly better performance than a conventional SQL database on typical exploratory data analysis queries. The use of parallel, distributed computation to further increase the performance of the technique appears to be feasible.

Critical fat, menarche, and the maintenance of menstrual cycles: a critical review.

The critical weight (fat) hypothesis has generated interest as a mechanism for the onset and maintenance of normal reproductive cycles in human females. It postulates that menarche is triggered by the attainment of a critical percentage fat and that the maintenance of menstrual cycles requires the persistence of a minimal level; each level is argued to be universal for a particular population or race and, by implication, for the species as a whole. However, an examination of the evidence reveals the hypothesis to have a number of serious methodological and empirical shortcomings which may be grouped under three headings. First, there are no acceptable measures of body fatness, and workers rely instead on estimates from height and weight which have been shown to be intolerably erroneous for individuals. Second, using either these estimates, or more reliable ones using appropriate methods, it may be shown that critical levels do not apply: numerous exceptions involving fat levels above and below the suggested threshold may be shown. Finally, where specific conditions involving reduced fatness and menstrual delay or dysfunction are used to support the hypothesis, it may be shown that other confounding factors are equally plausible. Consequently, based on available evidence, the critical weight (fat) hypothesis cannot be accepted. Hypotheses based on normal maturational processes, especially of the central nervous system, currently provide better explanations.

Mentorship: the students' views.

Whilst the concept of mentorship is not without its critics it is generally regarded as a valid way of supporting student nurses. There is much academic literature concerning mentorship but little recorded empirical evidence to support its use. This study goes some way to correcting this deficit by looking at mentorship from the students point of view. Following on from a pilot study 19 third-year student nurses were asked to complete a 10 item questionnaire concerning their experiences of mentorship. Analysis of the data suggested that the students found mentorship to be a valid means of support, particularly in the early stages of their training. Mentors were seen as fulfilling a socialising role as they passed on norms of behaviour and ward routines. Nevertheless, the students did not become dependent on their mentors and as training progressed they noticed the relationship became more equal. Whilst the varied support offered to pre-registration students was seen to be of value, finding an acceptable name to describe this eclectic process is problematic, given that the support included aspects of preceptorship and clinical supervision as well as mentorship.

Psychophysical tuning curve and critical band determined by masking in the presence of FM sounds.

Frequency modulation (FM) sounds were used as simultaneous maskers to estimate the psychophysical tuning curve (PTC). Maskers (800 ms duration) were FM sounds sweeping upward (1.2-->2.8 kHz) in Experiment I and downward (2.8-->1.2 kHz) in Experiment II. The signal was a pure tone of 2 kHz and the signal levels were 15-, 20-, 30-, and 35-dB SL. Masked PTCs were obtained as a function of masker frequency. The results were as follows: (i) When an upward-sweeping FM sound is used for high signal level, the slope of the PTC is shallower on the low frequency side and steeper on the high frequency side, and the 10-dB bandwidth of PTC coincides with that of the restricted PTC. (ii) When a downward-sweeping FM sound is used for the high signal level, the above relation for the PTC slope is reversed, and the 10-dB bandwidth for PTC coincides with that of unrestricted PTC. (iii) PTCs determined by masking in the presence of FM sounds apparently restrict the subject to listening to off-frequency, and it is shown that the bandwidth of PTC properly reflects the critical band. (iv) The properties of these PTCs can be easily explained by a neural network model of lateral inhibition, so we can accept a hypothesis that the hierarchical processing mechanism is incorporated within the inferior colliculus for parallel signal processing. (v) We conclude that the frequency resolution and the critical band filtering are, therefore, realized in a psychophysically relevant way in the auditory midbrain level.

Frequency, characteristics, and child and parent risk factors for nonfatal injury in rural children.

PURPOSE: We sought to identify child and parent risk factors for nonfatal unintentional injury among rural children.METHODS: Design: cross-sectional health interview survey. Setting: a completely rural midwestern county not adjacent to a metropolitan area. Participants: stratified, random sample of farm, town, and nonfarm, nontown households, including parents of all children aged 0-17 years. Outcome measures: parent-reported injury episodes that occurred in the past 12 months in children that required professional care, restricted normal activities for at least four hours, or resulted in blacking out or losing awareness or memory. Potential risk factors included nine child factors and six parent factors.RESULTS: Of the 621 children in participating households, 137 or 22.1% were injured. Children on a sports team were 1.88 times (95% confidence interval: 1.07 to 3.31) more likely than other children to be injured. Track team members were more likely than non-members to have an injury (54.5% vs. 23.0%, chi square test, p = 0.001), and volleyball team members were more likely than non-members to have an injury (50.0% vs. 29.0%, chi square test, p = 0.009). Unrelated to injury status were the child's gender, rural stratum, emotional health, behavioral problems, impairment or medications. Also unrelated to child injury were the parent's marital status, alcohol dependence, binge drinking, antisocial personality, depression symptoms or fear of child abuse by spouse.CONCLUSIONS: Our findings illustrate the difficulty of discovering clues with potential to aid design of effective childhood unintentional injury prevention policies, programs, and practices. The instruments and processes in our study were of acceptable quality, therefore we failed to asses the true risk factors for childhood injury or our sample size was too small to detect associations with the factors that were included.

Hugh sinclair lecture: the regulation and remodelling of HDL by plasma factors.

High density lipoproteins (HDLs) originate as lipid-free or lipid-poor apolipoproteins that acquire most of their lipid in the extracellular space. They accept phospholipids from cells in a process promoted by the ATP binding cassette A1 transporter to form prebeta-migrating discoidal HDL that are efficient acceptors of cholesterol released from cell membranes. The cholesterol in discoidal HDL is esterified by lecithin:cholesterol acyltransferase (LCAT) in a process that converts the prebeta-migrating disc into an alpha-migrating, spherical HDL. Spherical HDL are further remodelled by cholesteryl ester transfer protein (CETP) that transfers cholesteryl esters from HDL to other lipoproteins and by hepatic lipase that hydrolyses HDL triglyceride in processes that reduce HDL size and lead to the dissociation of prebeta-migrating, lipid-poor apolipoprotein (apo)A-I from the particle. Prebeta-migrating, lipid-poor apoA-I is also generated as a product of the remodelling of HDL by phospholipid transfer protein. Thus, apoA-I cycles between lipid-poor and lipid associated forms as part of a highly dynamic metabolism of HDL. The other main HDL apolipoprotein, apoA-II is incorporated into apoA-I-containing particles in a process of particle fusion mediated by LCAT. Extracellular assembly and remodelling of HDL not only plays a major role in HDL regulation but also provides potential targets for therapeutic intervention. One example of this is the development of inhibitors of CETP.

Foot-and-mouth disease virus leader proteinase: specificity at the P2 and P3 positions and comparison with other papain-like enzymes.

The foot-and-mouth disease virus Leader proteinase (L(pro)) frees itself from the growing viral polyprotein by self-processing between its own C-terminus and the N-terminus of the subsequent protein VP4. The ArgLysLeuLys*GlyAlaGlyGln sequence is recognized. The proteinase subsequently cleaves the two isoforms of host cell protein eukaryotic initiation factor (eIF) 4G at the AlaAsnLeuGly*ArgThrThrLeu (eIF4GI) and LeuAsnValGly*SerArgArgSer (eIF4GII) sequences. The enzyme does not, however, recognize the sequence on eIF4GII (AlaAspPheGly*ArgGlnThrPro) which is analogous to that recognized on eIF4GI. To investigate the basis for this specificity, we used site-directed mutagenesis to show that the presence of Phe at the P2 position or Asp at the P3 position severely compromises self-processing. Furthermore, these substitutions also give rise to the production of aberrant cleavage products. As Leu is the preferred amino acid at P2, the specificity of L(pro) is reminiscent of that of cathepsin K. This cellular proteinase can also process collagen through its ability to accept proline at the P2 position. Investigation of the L(pro) substrate specificity showed, however, that in contrast to cathepsin K, L(pro) cannot accept Pro at P2 and does not cleave collagen. Subtle variations in the arrangement of the S2 binding pockets on the enzymes are responsible for these differences in specificity.

Vaccinia virus-based expression of gp120 and EGFP: survey of mammalian host cell lines.

Production of recombinant proteins with the vaccinia virus expression system in five mammalian cell lines (HeLa, BS-C-1, Vero, MRC-5, and 293) was investigated for protein yield and proper posttranslational modifications. Regulatory acceptance of the host cell line was taken into consideration, where Vero, MRC-5, and 293 were considered more acceptable to the regulatory authorities. Relevant process knowledge for ease of scale-up with the particular cell type was also considered. Two proteins were expressed, enhanced green fluorescent protein (EGFP) in the cytoplasm and gp120, an HIV envelope coat protein that is secreted into the culture medium. HeLa cells produced the most EGFP at 17.2 microg/well with BS-C-1 and 293 following. BS-C-1 produced the most gp120 at 28.2 microg/mL with 293 and Vero following. Therefore, of the three most appropriate cell lines (Vero, MRC-5, and 293) for production processes, the best results were obtained with 293 cells. Although MRC-5 had a very high productivity on a per cell basis, the low cell density and slow growth rate made the overall production insufficient. Because gp120 contained a significant amount of posttranslational modification, this protein, produced by the different cell lines, was further analyzed by PNGase digestion suggesting N-linked glycosylation modifications in all cell lines tested. On the basis of these results and overall process considerations, 293 cells are recommended for further production process optimization in a serum-free suspension system.

1,4-asymmetric induction in methoxyselenenylation of double bonds at the nitrogen side of chiral perhydro-1,3-benzoxazines promoted by nonbonded Se...N interactions.

Regio- and diastereoselective methoxyselenenylation of cinnamylamines attached to a chiral perhydrobenzoxazine occurs in high yields by reaction with benzeneselenenyl chloride in dichloromethane-methanol. The diastereoselection is dependent on the temperature of the reaction and the structure of the substituent at C-2 and can be rationalized by accepting a 1,4-asymmetric induction process after coordination of the selenium to the nitrogen atom of the allylamine system.

Working Pasteur's Quadrant: harnessing science and action for community change.

Community psychology in general and the field of prevention in particular has unquestioningly accepted the assumption that the research process should proceed in a linear fashion from a search for basic knowledge to application in the community context. This ignores the compelling insight offered by Stokes (1997) that the drive for new knowledge and the pursuit of application can be combined in a single effort. If research in community psychology pursues the drive for application without an equal commitment to the development of knowledge about underlying community processes of social cooperation and change, it will become a field less capable of innovative and enduring contributions to community well-being and effectiveness Opportunities abound in community psychology for the simultaneous pursuit of new knowledge and more effective practice. We offer the example of a community leadership development program to promote collective efficacy as a case in point.

Verification of Donders' subtraction method.

In an experiment on 6 young adults advance (precued) information of the correct choice response was utilized completely: (a) For precue-to-stimulus intervals (PSIs) clearly shorter than the difference between mean choice and simple reaction time (RT), median response latency (L) measured from precue onset was invariant; (b) For clearly longer PSIs, median RT was very near the value for simple RT. This precue-utilization effect would be expected if response actualization had been delayed until the response had been selected and if the requirement for discrimination and selection had no adverse effect on readiness to respond. Donders' (1868/1969) hypothesis in his initial application of the subtraction method, that choice and simple reactions are identical except for the serial insertion of discrimination and selection operations in the former, is thereby strongly supported. If this formulation is accepted, models that hold that response processing can overlap other processing stages may be considered valid only for response selection, not response actualization.

Shape from shadows.

The colors, textures, and shapes of shadows are physically constrained in several ways in natural scenes. The visual system appears to ignore these constraints, however, and to accept many patterns as shadows even though they could not occur naturally. In the stimuli that we have studied, the only requirements for the perception of depth due to shadows were that shadow regions be darker than the surrounding, nonshadow regions and that there be consistent contrast polarity along the shadow border. Three-dimensional shape due to shadows was perceived when shadow areas were filled with colors or textures that could not occur in natural scenes, when shadow and nonshadow regions had textures that moved in different directions, or when they were presented on different depth planes. The results suggest that the interpretation of shadows begins with the identification of acceptable shadow borders by a cooperative process that requires consistent contrast polarity across a range of scales at each point along the border. Finally, we discuss how the identification of a shadow region can help the visual system to patch together areas that are separated by shadow boundaries, to identify directions of surface curvature, and to select a preferred three-dimensional interpretation while rejecting others.

A justification-suppression model of the expression and experience of prejudice.

The authors propose a justification-suppression model (JSM), which characterizes the processes that lead to prejudice expression and the experience of one's own prejudice. They suggest that "genuine" prejudices are not directly expressed but are restrained by beliefs, values, and norms that suppress them. Prejudices are expressed when justifications (e.g., attributions, ideologies, stereotypes) release suppressed prejudices. The same process accounts for which prejudices are accepted into the self-concept The JSM is used to organize the prejudice literature, and many empirical findings are recharacterized as factors affecting suppression or justification, rather than directly affecting genuine prejudice. The authors discuss the implications of the JSM for several topics, including prejudice measurement, ambivalence, and the distinction between prejudice and its expression.

2-Methylhopanoids as biomarkers for cyanobacterial oxygenic photosynthesis.

Oxygenic photosynthesis is widely accepted as the most important bioenergetic process happening in Earth's surface environment. It is thought to have evolved within the cyanobacterial lineage, but it has been difficult to determine when it began. Evidence based on the occurrence and appearance of stromatolites and microfossils indicates that phototrophy occurred as long ago as 3,465 Myr although no definite physiological inferences can be made from these objects. Carbon isotopes and other geological phenomena provide clues but are also equivocal. Biomarkers are potentially useful because the three domains of extant life-Bacteria, Archaea and Eukarya-have signature membrane lipids with recalcitrant carbon skeletons. These lipids turn into hydrocarbons in sediments and can be found wherever the record is sufficiently well preserved. Here we show that 2-methyl-bacteriohopanepolyols occur in a high proportion of cultured cyanobacteria and cyanobacterial mats. Their 2-methylhopane hydrocarbon derivatives are abundant in organic-rich sediments as old as 2,500 Myr. These biomarkers may help constrain the age of the oldest cyanobacteria and the advent of oxygenic photosynthesis. They could also be used to quantify the ecological importance of cyanobacteria through geological time.

GENERATIVE CELL SPECIFIC 1 is essential for angiosperm fertilization.

The double fertilization process in angiosperms is based on the delivery of a pair of sperm cells by the pollen tube (the male gametophyte), which elongates towards an embryo sac (the female gametophyte) enclosing an egg and a central cell. Several studies have described the mechanisms of gametophyte interaction, and also the fertilization process - from pollination to pollen tube acceptance. However, the mechanisms of gamete interaction are not fully understood. Cytological studies have shown that male gametes possess distinct cell-surface structures and genes specific to male gametes have been detected in cDNA libraries. Thus, studies of isolated gametes may offer clues to understanding the sperm-egg interaction. In this study, we identified a novel protein, designated GCS1 (GENERATIVE CELL SPECIFIC 1), using generative cells isolated from Lilium longiflorum pollen. GCS1 possesses a carboxy-terminal transmembrane domain, and homologues are present in various species, including non-angiosperms. Immunological assays indicate that GCS1 is accumulated during late gametogenesis and is localized on the plasma membrane of generative cells. In addition, Arabidopsis thaliana GCS1 mutant gametes fail to fuse, resulting in male sterility and suggesting that GCS1 is a critical fertilization factor in angiosperms.
