Vasomotor symptoms, serum estrogens, and gonadotropin levels in surgical menopause.

Hormonal parameters of young women who developed vasomotor symptoms in the immediate postoperative period following castration are compared to those who remained asymptomatic. Only 37.5 per cent of 16 premenopausal women developed "hot flushes" after operation. Perimenopausal women with vasomotor symptoms and elevated follicle-stimulating hormone levels demonstrated normal luteinizing hormone and estrogen values preoperatively. There were no statistically significant differences in total serum estrogen, follicle-stimulating hormone, or luteinizing hormone concentrations between the group of patients with symptoms and the group withoyt symptoms. The results of the study indicate that rising gonadotropin or declining estrogen values appear to have no direct correlation to the onset of vasomotor symptoms in the immediate postoperative period. Thus, the precise etiology of the "hot flush" remains to be elucidated.

Uterine tissue development in healthy women during the normal menstrual cycle and investigations with magnetic resonance imaging.

OBJECTIVE: High-resolution magnetic resonance imaging (MRI) was used to monitor both uterine endometrial and junctional zone morphometry during the normal menstrual cycle. STUDY DESIGN: Twenty-four healthy, ovulating women were studied during a single menstrual cycle. Three scans were performed to prospectively coincide with the follicular, periovulatory, and luteal phases of the cycle. RESULTS: MRI data showed a significant increase in endometrial and junctional zone volume, between the follicular and periovulatory phases, with a significant decrease in endometrial volume observed from the periovulatory to luteal phases. The regularity index, which is a novel subjective assessment of junctional zone structure, varied significantly and demonstrated a less regular junctional zone in the luteal phase. CONCLUSION: This study has quantified the normal developmental changes of uterine tissue during the menstrual cycle with MRI. Junctional zone data from MRI may play a major role in future studies that investigate menstrual disorders, subfertility, and pathologic changes.

Women's health advocacy: its growth and development in oncology.

Women's health has become a topic of national importance. Advocacy initiatives by consumers, scientists, government officials, health care professionals, industry, and the media have played a role in helping to set this agenda. Much of the current interest in women's health is the result of the women's movement and its interaction with science, medicine, and health care. Emerging consumerism and increasing public knowledge of medical and scientific topics has led to the emergence of patients as individuals seeking to actively make decisions regarding health care options. Nurses should embrace the advocacy movement and, whenever possible, work with patients and their advocates toward their many shared goals.

Evaluation of a simplified vitamin supplement inventory developed for the Women's Health Initiative.

OBJECTIVE: To evaluate the accuracy of a simplified inventory procedure for assessing nutrient intake from vitamin and mineral supplements. DESIGN: Participants brought their supplements to a clinic. An interviewer conducted the supplement inventory procedure, which consisted of recording data on the type of multiple vitamin and single supplements used. For the multiple vitamins, the interviewer recorded the exact dose for a subset of nutrients (vitamin C, calcium, selenium). For other nutrients, we imputed the dose in multiple vitamins. The dose of all single supplements was recorded. Labels of the supplements were photocopied and we transcribed the exact nutrient label data for the criterion measure. Spearman correlation coefficients were used to assess precision of nutrient intakes from the simplified inventory compared to the criterion measure. SETTING/SUBJECTS: Data are from 104 adult vitamin supplement users in Washington state. RESULTS: Correlation coefficients between nutrient intake estimated from the simplified inventory compared to the criterion measure were high (0.8-1.0) for those nutrients (vitamin C, calcium, selenium) for which the interviewer recorded the exact dose contained in multiple vitamins. However, for nutrients for which imputations were made regarding dose in multiple vitamins, correlation coefficients ranged from good (0.8 for vitamin E) to poor (0.3 for iron). CONCLUSIONS: The simplified inventory is rapid (4-5 min) and practical for large-scale studies. The precision of nutrient estimates using this procedure was variable, although excellent for the subset of nutrients for which the dose was recorded exactly. This study illustrates many of the challenges of collecting high quality supplement data.

Use and evaluation of health services by women in a developing country: is age important?

Today, the majority of the world's older persons live in less developed countries, where they represent an increasing proportion of the population. Using interviews conducted with 136 female heads of households in rural Sri Lanka, we considered the importance of age relative to other factors (e.g., marital status, education, employment, diet, and health practices) in accounting for use and evaluation of health services. Marital status figured more importantly than age in the use of services. As unmarried heads of households increase in less developed places, their demands for health care at the village level likely will escalate.

Increased fetal erythroblasts in women who subsequently develop pre-eclampsia.

In pregnancies complicated by pre-eclampsia (PET) and/or intrauterine growth restriction (IUGR) there is an increased number of fetal cells in the maternal circulation. The aim of this study was to investigate whether this increase in fetal cells precedes the onset of these pregnancy complications. Doppler ultrasound studies at 24 weeks gestation have shown that increased impedance to flow in the uterine arteries identifies pregnancies with impaired placental perfusion that subsequently develop PET and/or IUGR. We obtained maternal blood from 18 pregnancies with abnormal Doppler results at 22-24 weeks gestation and from 10 normal controls. Fetal erythroblasts were enriched from maternal blood by triple density gradient centrifugation and magnetic cell sorting with CD71 antibody, and the percentage of these erythroblasts was determined. The median proportion of fetal erythroblasts in the group with abnormal Doppler results was 4.5% (range 1-12%), which was significantly higher than in the control group [median 1% (range 0-3%; P < 0.001)]. Furthermore, within the group with abnormal Doppler the median proportion of fetal erythroblasts was higher in the 10 cases which subsequently developed PET and/or IUGR [median 5.5% (range 3-12%)], than in those with normal pregnancy outcome [median 2% (range 1-5%; P < 0.01)]. These findings suggest that impaired placental perfusion is associated with an increase in feto-maternal cell traffic, which precedes the onset of PET and/or IUGR by several weeks.

Lost and found possible selves, subjective well-being, and ego development in divorced women.

Divorced women, who had been married for an average of 22 years, wrote narrative descriptions of their best possible future selves before the divorce (retrospectively) and after the divorce, rated the salience of these narratives, and completed measures of SWB and ED. Independent raters coded the narratives for amount of elaboration (i.e., vivid detail). The salience of the lost possible self was negatively related to SWB while the salience of the current possible self was positively related to SWB. Elaboration of the found possible self was associated with concurrent ED as well as ED two years later. Lost self elaboration interacted with time since divorce to predict ED, controlling for age and Time 1 ED. Results are interpreted as indicating that, while happiness may require us to avoid thinking about what might have been, maturity might require an awareness of the losses and sacrifices of adulthood.

Is the prophylactic administration of magnesium sulphate in women with pre-eclampsia indicated prior to labour?

OBJECTIVE: To determine whether prophylactic magnesium sulphate is necessary to prevent eclampsia and associated complications among women with pre-eclampsia prior to labour. DESIGN: Case series. SETTING: Tertiary referral centre. POPULATION: Three hundred and eighteen women with pre-eclampsia (blood pressure > or = 140/90 mmHg and > or = 2+ proteinuria) who were not in labour or for planned induction thereof and had not received magnesium sulphate during transfer. METHODS: Clinical evaluation of the pregnant women with careful blood pressure control. Magnesium sulphate was withheld even in the presence of imminent eclampsia. During labour, the option of magnesium sulphate prophylaxis was left to the clinician, but magnesium sulphate was administered in cases of eclampsia. MAIN OUTCOME MEASURES: Eclampsia and related complications. RESULTS: Five women (1.5%) developed eclampsia, although none developed related complications. Women presented at an early gestational age (mean 30 weeks), with high blood pressure, often suffering from headaches. Twenty pregnancies were terminated prior to viability, of which half were terminated for maternal reasons. Ten intrauterine deaths occurred. Most often fetal distress (38.6%) initiated the delivery process, which was mainly by caesarean section (68.5%). With the exception of epigastric discomfort, symptoms and signs of imminent eclampsia decreased after admission. Blood pressure values were significantly lower at delivery although biochemistry results deteriorated from admission to delivery. CONCLUSION: In women with pre-eclampsia prior to labour, where blood pressure control was carefully applied but magnesium sulphate not given, the eclampsia rate was low and eclampsia did not appear to worsen the existing prognosis for mother or fetus.

Pregnancy and delivery characteristics of women whose infants develop child cancer. A study based on registry information.

Data on all cancers among children born 1973-1984 were extracted from the Swedish Cancer Registry and linked to data in the Medical Birth Registry (1,268 cancer cases). For each case, two controls were selected, matched for maternal age, parity, county, and month of birth. A marginally increased risk of childhood malignancy was seen in the offspring of teenage mothers and at parity 1 and 4+. There was an excess among cases of the diagnosis physiological icterus. No statistically significant associations were found between childhood malignancy and infant sex, twinning, birthweight, non-chromosomal malformations, maternal smoking, or complications during pregnancy.
