99mTc-aminohexylidendiphosphonate and 99mTc-pyrophosphate in the scintigraphic diagnosis of experimental myocardial infarction in dogs.

99mTc-aminohexylidendiphosphonate (99mTc-AHDP) is a new Czechoslovak pharmaceutical of the phosphonate line which contains the amino group NH2 in its molecule. This substance was tested in 5 dogs with experimentally provoked 48-h old myocardial infarction. The in-vivo scan and the radioactivity of tissue samples demonstrated that 99mTc-AHDP is as suitable for imaging acute myocardial infarction as is the commonly used 99mTc-pyrophosphate.

[Dual energy emission computed tomography with 99mTc pyrophosphate and 201T1 in evaluating coronary reperfusion therapy in patients with acute myocardial infarction].

To evaluate the clinical significance of scintigraphic overlap of thallium-201 (201T1) and technetium-99m pyrophosphate (99mTc), we observed 28 patients with acute myocardial infarction (AMI) in whom coronary reperfusion was attempted. All patients underwent dual energy emission computed tomography (D-ECT) on the third post AMI day which facilitated comparing 99mTc and 201T1 images from identical slices simultaneously. The scintigraphic results of the 28 patients were as follows: (table, see text) In conclusion, (1) significant 201T1 and 99mTc overlap on D-ECT suggest very early and successful reperfusion, and (2) none of the patients with unsuccessful reperfusion exhibits overlap.

[Evaluation of coronary reperfusion for acute myocardial infarction by emission CT using technetium-99m pyrophosphate].

Twelve patients with acute transmural myocardial infarction (AMI) were treated with percutaneous transluminal coronary angioplasty (PTCA) following intracoronary thrombolysis using urokinase, and underwent technetium-99m stannous pyrophosphate (Tc-99m-PPi) imaging 9.2 +/- 2.1 hours after the onset of chest pain. The imaging was performed with emission computed tomography (ECT). Compared to planar imaging, this allowed more accurate detection of small myocardial infarcts and accurate measurements of infarcts irrespective of their location was also made. Early Tc-99m-PPi images were obtained to test the hypothesis that an early, abnormal Tc-99m-PPi image suggest successful reperfusion. The results were presented for two groups of patients: three with unsuccessful reperfusion (Group A) and nine with successful reperfusion (Group B). Eight of the nine patients with successful reperfusion had positive acute Tc-99m-PPi images. On the contrary, all the three patients for whom reperfusion failed had negative acute Tc-99m-PPi images. We also examined the feasibility of estimating infarct size using positive Tc-99m-PPi images in patients with successful reperfusion during the early phase of AMI. The Tc-99m-PPi uptake score (Tc-US) was used to measure infarct size in this study. Areas of increased Tc-99m-PPi uptake within myocardial infarcts were threshold at 60% of the peak activity. The Tc-US of each patient was obtained to sum the scores of all myocardial segments using a scoring system with a maximum score of 108. Using this method, Tc-US ranged from 2 to 39. The correlation of Tc-US with the peak serum creatine kinase level was significant (r = 0.91).(ABSTRACT TRUNCATED AT 250 WORDS)

Usefulness of 99mTc pyrophosphate bone scintigraphy in the survey of dialysis osteodystrophy.

Fogelman's score (FS) was used to determine the usefulness of 99mTc pyrophosphate (Tc-PP) bone scintigraphy in the evaluation of dialysis osteodystrophy. FS correlated well with bone 47Ca accretion rate. It remained stable after six months in patients treated with 1 alpha (OH)D3 and increased significantly in a randomised group of untreated patients. It decreased after two years of 1 alpha (OH)D3 therapy while serum calcium increased and iPTH and alkaline phosphatases decreased. Patients with low FS, treated by 1 alpha (OH)D3, rapidly developed hypercalcaemia. In cases of spontaneous hypercalcaemia, parathyroidectomy did not normalise serum calcium in patients with low FS despite a significant decrease in serum iPTH. Lower FS were associated with a higher increase in serum aluminium after desferrioxamine (DFO) administration and in two cases of proven aluminium osteomalacia, DFO therapy was followed by a dramatic increase in FS.

[Digital radiocardioangiography as an auxiliary method for the precise localization of zones of myocardial concentration of 99mTc-Sn(II)-pyrophosphate. Preliminary report].

A new method is introduced for the superposition of 99mTc-pyrophosphate (PiF) myocardial images on the isolated images of the radioangiocardiographic dextrophase and levophase, in order to include cardiac anatomical references for the accurate topographic localization of the PiF concentration areas. Results in two different patients are presented. In one of them, the PiF concentration was evident at the inferoseptal, inferoapical and posterolateral regions of the left ventricle. In the other, the PiF concentration was evident at the same areas than in the former plus the inferoanterior region of the right ventricle. Our project is to validate this procedure by correlating its results with those obtained through other clinical methods that detect right ventricular infarction.

[Clinical importance of 99mTc-pyrophosphate scintigraphy in the bicycle ergometry test].

The clinical value of myocardial scintigraphy with 99mTc-pyrophosphate in conjunction with bicycle ergometry was assessed through the correlation of the latter's results with the clinical course of coronary heart disease over 1 year in 21 patients with first myocardial infarction during its early healing stage and 17 anginal patients without a history of myocardial infarction. The disease ran a stable course at the time of the testing in all the patients; later on, 11 of those developed various complications associated with coronary heart disease, or died. Three scintigraphic recordings were made: prior to, at the peak of, and after the exercise. Thirteen scintigraphic patterns were identified. They are interpreted on the basis of presumed variation in the degree of myocardial damage and blood supply to affected areas under the effect of exercise; a relation is also noted with predicted risk of complications. The method has proved reliable in identifying patients at low risk for complications. Providing indirect evidence of the quality of blood supply to damaged myocardial areas during rationed exercise, it furnishes additional criteria for the assessment of the patients' working capacity.

[Negativity of myocardial scintigraphy with technetium-pyrophosphate in a case of primary cardiac amyloidosis].

We describe herein the case of a 62-year-old man affected by congestive heart failure of obscure origin, in which the clinical history, the electrocardiographic and echocardiographic findings were compatible with the diagnosis of amyloid cardiomyopathy. Cardiac catheterization disclosed a restrictive physiology. Tc-99m-pyrophosphate myocardial scintiscan was negative despite the markedly increased left ventricular wall thickness on echocardiographic examination. A definite diagnosis was obtained by means of endomyocardial biopsy.

[A clinical study of calcium pyrophosphate dihydrate crystal deposition disease].

Clinical features of calcium pyrophosphate dihydrate crystal deposition disease (CPPD c.d.d.) were studied and the following results were obtained. The prevalence of CPPD c.d.d. was 9.7% in 300 persons aged 50 or older, who stayed or worked at an old-age home. The most common type of CPPD c.d.d. was the asymptomatic type. Comparative studies between the patients with CPPD c.d.d. and subjects without it revealed that scoliosis of the lumbar spine, osteoarthritis-like changes of the knee, subchondral bone cyst and patella wrapped around the femur were statistically more frequent in the former. The clinical study of 50 patients with CPPD c.d.d. revealed calcification not only in the articular cartilage but also in periarticular tissues and ligamentum flavum. The histological study demonstrated frequent destructive changes of the tissues around the site of CPPD crystal deposition. Formation of CPPD crystals appeared to be initiated by degenerating chondrocytes and metaplastic chondrocytes.

[Prospective study of the value of echocardiography and myocardial scintigraphy with Tc 99m pyrophosphate during the acute stage of right ventricular infarction].

The aim of this study was to compare the clinical values of 99mTc pyrophosphate scintigraphy and M mode and 2D echography in the diagnosis of right ventricular infarction and in the predicting of some of its complications. Fifty-two patients were prospectively studied by echocardiography and scintigraphy at the acute stage of inferior wall infarction. Scintigraphy was performed in the antero-posterior and 45 degrees left anterior oblique incidences during the first 3 days of infarction. Right ventricular infarction was diagnosed if the right ventricular fixation was separated from the left ventricle by fixation at the base of the septum. Echocardiography was performed at an early stage by the usual 3 incidence. Dilatation of the right ventricle on the parasternal and submitral incidences; abnormal right ventricular contraction was searched for in all the incidences. The following results were obtained: scintigraphy showed a localised fixation allowing a topographic study in 40/52 patients (77%); satisfactory echocardiographic studies were obtained in 46/52 patients (88.5%). Signs of right ventricular infarction: scintigraphy showed signs of necrosis of the right ventricle in 12/40 patients (30%) who had a localised fixation; echocardiography showed dilatation (greater than 25 mm) of the superior part of the right ventricle with a right/left ventricle ratio greater than 0.5 in 16/46 patients (37%) with interpretable studies and obvious abnormalities of right ventricular wall motion in 7 patients (15.2%), less obvious in 10 other patients (22%).(ABSTRACT TRUNCATED AT 250 WORDS)

Joint scintigraphy using technetium-99m pyrophosphate in experimental hemarthrosis.

To determine the validity of a method for induction of experimental hemarthrosis in dogs and for the nuclear imaging of hemarthrosis, serial technetium-99m pyrophosphate [( 99mTc]PYP) flow and blood-pool scans were performed monthly in eight dogs who received bi-weekly injections of autologous blood into their femoro-tibial joints (also called stifle joint). In four control dogs, one joint was injected with saline while the other joint received only a sham injection. In addition, two dogs received intra-articular injections of autologous blood into their right stifle joint and saline into their left stifle joint. These dogs were studied with 99mTcO4 joint scintigraphy at monthly intervals. The dogs were periodically taken out of the study and explored surgically. Pathologic examination of synovial tissue was performed. Serial radiographs were also obtained and correlated with the scan and surgical findings. There was a striking abnormal increase in blood-pool activity of [99mTc]PYP in the treated stifle joints, commencing at the first examination after 1 mo of blood injections and continuing for the length of the study. All radiographs showed only minimal joint space widening and some soft-tissue swelling. On pathologic examination, both grossly and microscopically, there was profuse pannus formation, with intense inflammatory infiltrate replacing much of the subsynovial fat. The scintigraphic findings correlated well with these pathologic findings. This study not only validates this method for simulating hemophilic hemarthrosis but also suggests that [99mTc]PYP joint scintigraphy is a simple, and noninvasive method for monitoring the early changes in hemophilic arthropathy and is superior to pertechnetate imaging for this disease process. Instead of the previously recommended delayed bone images, we recommend, in addition, flow studies to assess joint hypervascularity and immediate static images to visualize the synovium and joint capsule.

[New knowledge on the calcium pyrophosphate dihydrate (CPPD) crystal deposition disease in the cervical ligamentum flavum].

A case of 64-year-old Japanese woman with cervical radiculomyelopathy caused by the deposition of both calcium pyrophosphate dihydrate (CPPD) and hydroxyapatite crystals in the cervical ligamentum flavum is reported. The patient developed paresthesia in her both hands and gait disturbance in the previous six months. Neurological examination revealed generalized hyperreflexia and hypesthesia in the C7 region. Neuroradiologically, two nodular opacifications were shown in the C3-4 and C5-6 level. CT scan revealed linear and symmetrical calcifications in the C3-4 and nodular calcifications in the C5-6 level. En bloc laminectomy was performed and the patient was discharged from the hospital with improvements of neurological symptoms. Specimens of the spinal laminae and ligamenta flava were studied by light microscopy, scanning electron microscopy (SEM), energy dispersive X-ray microanalysis (X-MA) on SEM, and an X-ray diffraction study was carried out on the crystals. The results showed A. CPPD crystal depositions throughout the ligamenta flava together with a minimal thickening of the ligaments. The deposition was seen in the midzone of the ligaments forming a plate-like distribution and forming lines in the cut surface, the latter coincided well with the CT finding at the C3-4 level. B. The nodule (7 X 6 X 6 mm in size) at the C5-6 level was composed of two crystals, CPPD and hydroxyapatite, the latter being encased by the former. The thickness of the CPPD layer was quite thin varying from 10 to 100 mu. Myelographically, the spinal cord was most severely compressed by the C5-6 nodule. C. Because of the absence of family histories and predisposing factors, the case was considered one of primary or idiopathic CPPD crystal deposition disease. The origin of the deposition of mixed crystals was discussed and it appeared likely that the central part of CPPD crystal deposition had transformed to hydroxyapatite at the C5-6 level. In the literature a considerable number of cases have been reported under the title, "Calcification of the Ligamentum Flavum", and some of them have been shown to have hydroxyapatite deposition. Considering the great similarities in clinical, radiological and histological findings between the so-called "calcification of the lig. fl." and CPPD crystal deposition disease, the present case was the first to clearly indicate that these two conditions belong to a single category, i.e. CPPD crystal deposition disease.(ABSTRACT TRUNCATED AT 400 WORDS)

Pyrophosphate scintigraphy and other non-invasive methods in the detection of cardiac involvement in some systemic connective tissue diseases.

Thirteen patients with systemic lupus erythematosus, 8 patients with polymyositis, and 6 patients with spondylitis ankylopoetica (Bechterew's disease) underwent clinical cardiologic examination and scintigraphy of the myocardium (99mTc-pyrophosphate), ECG, echocardiography, polygraphy, and their blood pressure was taken. The aim of the study was to ascertain how such a combination of non-invasive examinations can help in recognizing a cardiac involvement. In systemic lupus erythematosus cases one or more positive findings were revealed in 9 patients (69%), in 4 patients all examinations were negative (31%). Four patients (50%) with polymyositis had positive findings. In patients with spondylitis ankylopoetica positive findings occurred in 2 cases (33%). The study has shown that a combination of non-invasive cardiologic methods increases the probability of detecting cardiac involvement in systemic connective tissue diseases.

[Diagnostic significance of electrocardiography and myocardial scintigraphy with 99m Tc-Sn-pyrophosphate and 99m Tc-Sn-phosphone in patients with stenocardia].

Altogether 123 angina patients were examined using electrocardiography and scintigraphy of the myocardium with two 99m Tc radiopharmaceutical complexes: pyrophosphate and phosphone. A comparative analysis of the results obtained with different methods of investigation before and after an attack of angina pectoris was made. The authors have shown some peculiarities of radionuclide distribution in the myocardium and the diagnostic value of each method of investigation in patients with different clinical forms of angina pectoris.

Massive solitary tophus containing calcium pyrophosphate dihydrate crystals at the acromioclavicular joint.

A massive solitary tophus containing calcium pyrophosphate dihydrate (CPPD) crystals was resected from the distal clavicle and proximal acromion of a 62-year-old man who presented with a painful shoulder mass. The diagnosis was confirmed histologically and by X-ray diffraction. CPPD crystal deposition was found within bone, however, the articular cartilage was not involved. Other unusual features of the case include the size of the lesion, the monoarticular presentation and a peculiar discoloration of the overlying skin. The clinical presentation and radiographic features were initially consistent with a malignant process; thus the case demonstrates that CPPD arthropathy should be included in the differential diagnosis of periarticular tumors.

Localization and stability of technetium-99m-Sn-pyrophosphate in rat neutrophils.

Technetium-99m has been suggested as an alternative radiolabel for white cells, and while its physical characteristics are nearly ideal, its stability and site of localization in this procedure are unclear. We examined these parameters by radiolabeling 10(8) neutrophils from rat peritoneum with 74 to 370 MBq technetium-99m-Sn-pyrophosphate. We found that the percentage of initial activity bound to neutrophils was quite variable, possibly because the radiolabel associated with several subfractions: 19.8 +/- 11.5% (mean +/- s.d.) with nuclei and plasma membranes, 25.6 +/- 3.9% with mitochondria, 26.6 +/- 9.8% with microsomes, and 29.2 +/- 6.9% with cytosol. Approximately 80-90% of the radioactivity associated with neutrophils was not bound to protein and only about one-half of the activity localized to cell membranes was removable over 4 hr by pepsin digestion. We concluded that the variable labeling efficiency was due to the radiolabel's rather loose association with several cellular subfractions rather than specific binding to a unique substrate.

[Thiamine pyrophosphate-dependent enzymes in the regenerating liver of albino rats under different regimens of oxythiamine administration].

Activities of the thiamine pyrophosphate-dependent enzymes (pyruvate dehydrogenase, oxoglutarate dehydrogenase, transketolase) were increased in regenerating rat liver tissue as compared with their activities in the intact tissue. After administration of oxythiamine (20 mg/kg, within 10 days, subcutaneously) into the animals the enzymatic activity studied was decreased.
