Estimated risk for altered fetal growth resulting from exposure to fine particles during pregnancy: an epidemiologic prospective cohort study in Poland.

The purpose of this study was to estimate exposure of pregnant women in Poland to fine particulate matter [less than or equal to 2.5 microm in diameter (PM 2.5)] and to assess its effect on the birth outcomes. The cohort consisted of 362 pregnant women who gave birth between 34 and 43 weeks of gestation. The enrollment included only nonsmoking women with singleton pregnancies, 18-35 years of age, who were free from chronic diseases such as diabetes and hypertension. PM 2.5 was measured by personal air monitoring over 48 hr during the second trimester of pregnancy. All assessed birth effects were adjusted in multiple linear regression models for potential confounding factors such as the size of mother (maternal height, prepregnancy weight), parity, sex of child, gestational age, season of birth, and self-reported environmental tobacco smoke (ETS). The regression model explained 35% of the variability in birth weight (beta = -200.8, p = 0.03), and both regression coefficients for PM 2.5 and birth length (beta = -1.44, p = 0.01) and head circumference (HC; beta = -0.73, p = 0.02) were significant as well. In all regression models, the effect of ETS was insignificant. Predicted reduction in birth weight at an increase of exposure from 10 to 50 microg/m3 was 140.3 g. The corresponding predicted reduction of birth length would be 1.0 cm, and of HC, 0.5 cm. The study provides new and convincing epidemiologic evidence that high personal exposure to fine particles is associated with adverse effects on the developing fetus. These results indicate the need to reduce ambient fine particulate concentrations. However, further research should establish possible biologic mechanisms explaining the observed relationship.

Selenium levels in human plasma and hair in northern Poland.

The aim of this study was to (1) estimate the concentration of selenium in the plasma of 146 residents (65 men and 81 women) and in the hair of 34 persons from the Gdańsk region in northern Poland, aged 19-70 and (2) compare the obtained results with data corresponding to healthy populations living in different European countries. Selenium in plasma was determined by atomic absorption spectrometry using the hydride generation method. The mean selenium concentration in plasma of the investigated persons was 73.3 +/- 14.1 microg/L, 76.7 +/- 13.2 microg/L in men, and 70.4 +/- 14.7 microg/L in women. No age - dependent differences in plasma selenium were found in the investigated population. In 20% of the investigated persons, the selenium level in plasma was lower than 60 microg/L. The mean selenium concentration in hair was 0.30 +/- 0.11 microg/g. A positive, statistically significant correlation between selenium concentrations in the plasma and hair of the investigated persons was found. The obtained results indicate that the selenium level in significant part of this population is suboptimal and should be elevated by supplementation with this element.

Arsenic content in the femur head of the residents of southern and central Poland.

Arsenic content was assayed in the samples of the femur head of the people living in southern and central Poland (Kraków, n=13; Silesian region, n=13; Lódz, n=12). The average age being 68.7+/-8.7 yr. Arsenic content in the femur head was determined applying the hydride generation atomic absorption spectrometry (HG-AAS) method after microwave mineralization. The average arsenic contents in the femur head of the residents of the Lódz, Kraków, and Silesian regions were 0.41 microg/g, 0.37 microg/g, and 0.18 microg/g, respectively. No correlation has been found between arsenic content in the femur head and the content of other metals. Neither the age nor sex of the people tested affected the arsenic content in the femur head.

Selenium concentrations in human renal cortex, liver, and hair in northern Poland.

The aim of this study was to (1) determine the selenium concentration in the renal cortex, liver, and hair in 64 residents from northern Poland (Gdańsk region) aged 17-81 yr, who died suddenly, and (2) assess whether a correlation between the selenium concentration in hair and in the renal cortex and liver occurs. Selenium was determined by atomic absorption spectrometry using the hydride generation method. The mean selenium concentration in the renal cortex, liver, and hair in the investigated persons was 0.791+/-0.191 microg/g (wet weight), 0.289+/-0.084 microg/g (wet weight), and 0.443+/-0.128 microg/g, respectively. No age-dependent differences in selenium level in the investigated tissues was found. Also, no correlation between the selenium concentrations in hair and in renal cortex and liver was assessed.

Risk of colorectal cancer from alcohol consumption at lower vitamin intakes. A hospital-based case-control study in Poland.

The purpose of the study was to assess the relation between the simultaneous exposure to alcohol and consumption of micronutrients that have protective properties against colorectal cancer. A hospital-based case-control study of colorectal cancer was carried out between January 1998 and November 1999 at the University Hospital in Krakow, Poland. In total, 180 incident cases of colorectal cancer, confirmed by histopathology, were recruited and an equal number of controls, individually matched by gender and age (+/-5 y), were chosen amongst patients with no history of cancer. A food-frequency questionnaire for 148 food items, combined with the quantity of foods eaten, was used to assess the normal dietary pattern. The data confirmed the reported inverse association between the intake of retinol, thiamine, or antioxidant micronutrients (vitamins C, E) and the occurrence of colorectal cancer. Alcohol intake was found to be an important risk factor for this cancer site, and the risk escalated in parallel with increased intake of retinol, carotene, vitamins C and E, but with high consumption of alcohol ran a noticeably high risk of colorectal cancer (OR= 6.79; 95% CI: 2.08-22.18). The risk was markedly lower, however, among alcohol drinkers who reported a high intake of micronutrients (OR = 1.35; 95% CI: 0.39-4.67). The data suggest that a combination of high consumption of alcohol and low intake of retinol and antioxidant vitamins may considerably increase the risk of colorectal cancer.

Alcohol consumption and alcoholic psychoses in Poland.

In Poland, analysis of partial correlations showed a close connection of the incidence of alcoholic psychoses with total alcohol consumption and consumption of distilled spirits but not with consumption of wine or beer. It is suggested that the incidence of alcoholic psychoses can serve as an indicator of the incidence of alcohol-related problems.

Scabies and head-lice infestations in different environmental conditions of Lower Silesia, Poland.

A comparison of external parasitic infestations among inhabitants of Legnica, Wałbrzych, and Wrocław districts, in the Lower Silesia region of Poland showed a direct relationship between the high incidence of scabies and low standard ecological indices, as well as social economic setting of the communities. In the years 1990-1997, the highest mean incidences of scabies per 100,000 people (80 and 46) were noted, respectively, in the Legnica and Wałbrzych districts, compared to only 7.9 in the Wrocław district. Infestation was correlated with percentages of the population with higher education (4.8; 4.2, 10.1, respectively) and the number of patients per physician (795, 632, 288, respectively), and the percentages of degraded land/and land threatened by degradation (10/37, 5/16, 0.7/10, respectively), forest stands damaged by gases and particulates (99.4, 99.4, 58.8, respectively), and air pollution emission indices in the towns of Legnica and Wałbrzych (30 and 21 tons/km2) and Wrocław (16). Scabies infestation was highest in children and teenagers (0-19) and was gender-associated (in all age groups, women were more often infested than men). A decreasing rate of scabies infestation, especially from the mid-1990s, was noted for both scabies and pediculosis in Wałbrzych district; in the 0-19-yr-old inhabitants, it varied from 0.75% in 1994 to 0.41% in 1996.

Molecular evidence of coinfection of Borrelia burgdorferi sensu lato, human granulocytic ehrlichiosis agent, and Babesia microti in ticks from northwestern Poland.

To assess the potential risk for tick-borne agents, Ixodes ricinus were collected from 2 sites in northwestern Poland. The ticks were tested by polymerase chain reaction for coinfection with Borrelia burgdorferi sensu lato (s. l.), human granulocytic ehrlichiosis (HGE) agent, and Babesia microti. Of the 533 processed ticks, 16.7% were positive for B. burgdorferi s. l., 13.3% for B. microti, and 4.5% for the HGE agent. Twenty ticks were coinfected with 2 or 3 of the pathogens.

Genetic variability among freshwater mussel Anodonta woodiana (Lea, 1834) (Bivalvia: Unionidae) populations recently introduced in Poland.

The alien Chinese mussel Anodonta woodiana was first reported in Poland in the system of heated lakes near Konin in 1993. Genetic studies with use of three molecular techniques (isoenzyme electrophoresis, PCR-RFLP and sequence analysis of a COI gene fragment) were carried out on the Polish first populations of A. woodiana. The studies have revealed low genetic variation between the populations (Nei's genetic distance for 12 loci ranged 0.000 to 0.007) as well as their considerable polymorphism. Each population averaged 2.28 alleles per locus, 2.72 alleles per polymorphic locus, and 75% polymorphic loci. Restriction analysis of the COI gene fragment have not revealed variability between the analysed specimens, including males and females. Restriction enzymes, ScrFI, Csp6I, and EcoRI used in the COI gene fragment PCR-RFLP generate distinct restriction patterns, which can be molecular markers for A. woodiana. The sequence obtained for COI fragment was the same in the examined female and male specimens and represents F mitotype (DNA was isolated from somatic tissues). The divergence between A. woodiana F and M mitotypes is high (34%), however it remains within the range of the general character of the DUI (doubly uniparental inheritance) phenomenon in freshwater bivalves (Unionidae).

Experimental and model-based evaluation of the role of denitrifying polyphosphate accumulating organisms at two large scale WWTPs in northern Poland.

The capabilities of denitrifying Polyphosphate Accumulating Organisms (DPAOs) in two large-scale plants in northern Poland performing enhanced biological phosphorus removal (EBPR) were evaluated in this study. A series of batch tests with the process biomass aimed at the measurements of phosphate release (with artificial substrate and real wastewater) and subsequent phosphate uptake under anoxic/aerobic conditions. The process kinetics were predicted using ASM2d implemented in the GPS-X ver. 4.0.2 simulation package. The results from one experimental series (summer) were used for the model calibration, whereas the results from another series (spring) were used for the model validation. The model parameters were also accurately confirmed by predictions of the accompanying field measurements in the full-scale bioreactors. The experimental and simulation results revealed that a relatively small fraction of PAO could denitrify (eta(NO3,PAO) = 0.32). The denitrification rates associated with the anoxic storage of PP and the anoxic growth of PAO only constituted 16.0-21.0% of the denitrification rates associated with the anoxic activity of "ordinary" heterotrophs.

Drug use and policy in Poland in the 1980s.

In this article, the author describes the recent changes in patterns of drug use in Poland in the light of available data. A much higher incidence of use has been seen in the 1980s, and there are indications that this can be accounted for by increased use among young people from the manual working class. Social deprivation may play a part in the etiology of the 1980s "epidemic," and evidence is put forward in support of this view. Both patterns of use and the policies that have been introduced to deal with them are viewed in terms of the political, economic, and social context of post-Solidarity perestroika.

Apparent epidemic of insulin-dependent diabetes mellitus in Midwestern Poland.

There is no information concerning the risk of developing insulin-dependent diabetes mellitus (IDDM) in eastern Europe. An IDDM registry has been developed in Midwest Poland for 1970-1984. The risk of developing IDDM in Polish children was determined through utilization of the registry. The incidence of IDDM in Polish children was very low compared with other Caucasian populations. There was a major increase in risk beginning in 1982; the incidence almost doubled from 3.5/100,000 in 1970-1981 to 6.6/100,000 in 1982-1984. The pattern of IDDM in the high-risk period was different from that in the low-risk period, with an altered seasonal pattern and unusual increased incidence in younger children. The rapid increase in incidence as well as altered epidemiologic patterns during this period suggest that major alterations of environmental factors were responsible for the change in risk.

Occupational diseases in the period of socioeconomic transition in Poland.

OBJECTIVES: Current situation in the epidemiology of occupational diseases is a resultant of many contributing factors, such as occupational exposures of the working population, social and economic conditions, medical measures, legislation, and ongoing changes in the national economy. MATERIALS AND METHODS: This work is based on the information compiled from reporting forms on occupational diseases gathered in the Central Register of Occupational Diseases run by the Nofer Institute of Occupational Medicine, Lódź, Poland. RESULTS: A systematic downward trend in the number of occupational diseases has already been observed over several years. In 1998, as many as 12 017 cases were recorded (117.3 per 100,000 workers), while in 2005, this number was reduced to 3249 (34.8 per 100 000). In 2005, the highest incidence rates were noted for chronic voice disorders, pneumoconioses, infectious and parasitic diseases, hearing loss, and skin diseases. The observed decrease in occupational pathology is associated with continued transformations in the national economy. Changes in the nature and level of occupational exposures have influenced the profile of occupational pathologies. Medical preventive measures, such as vaccinations covering high-risk groups have contributed to the reduced incidence of occupational diseases. For example, the number of cases of occupation-related viral hepatitis was seven times lower in 2005 than in 1993. The 2005 data show that over 50% of occupational diseases were diagnosed in workers employed outside industry. CONCLUSIONS: Changes taking place on the labor market, new technologies and most of all transfer of the workforce from industry to the service and administration sectors, will certainly affect the profile of occupational and work-related pathologies. In the present-day environment, the work-related stress has become a dominant factor.

Prevalence of antibodies against tick-borne encephalitis among residents of north-eastern Poland.

In 37 out of 613 (6%) residents of north eastern Poland, IgG antibodies to tick-borne encephalitis virus (TBEV) were detected at levels exceeding the diagnostic value of 60 VIEU/ml. The prevalence of the antibodies was not related to sex or place or residence. However, significantly higher antibody levels were found in the group of forest workers than in individuals not professionally connected with forestry.

A three-year training programme for primary health care physicians in Poland.

A three-year training programme was developed and implemented by the Medical Centre of Postgraduate Education in Poland with the aim of improving the knowledge and skills of primary health care physicians working in the State Health Services' community centres. The programme creates several learning and self-assessment opportunities with the general principle of not distracting a doctor from his daily duties in a community health centre for more than one day a month. Preliminary assessment of the programme revealed its positive impact on both the health services system and primary health care physicians themselves. It increased health authorities' activity in creating new learning opportunities and doctors' motivation to learn. Self-assessment made by the first group of trainees who completed the three-year programme (results obtained by means of a questionnaire) revealed substantial increase in professional competence resulting from the participation in the programme.

Evaluation of certain risk factors for lung cancer in Cracow (Poland)--a case-control study.

The relationship of lung cancer risk to cigarette smoking, occupational exposure, air pollution at permanent residence and usual frequency of consumption of fruits and vegetables was analysed in a case control study in Cracow, Poland. The cases were 176 male lung cancer patients diagnosed in 1992-1994 with histological confirmation of the diagnosis, obtained from the population-based Cracow Cancer Registry, and 341 controls randomly selected from the general population. Classification of exposure to outside air pollution was based on measured level of total suspended particular matter and sulphur dioxide by particular districts of town (from 1973 to 1980). Except calculation of odds ratios for male lung cancer associated with exposures among all study subjects, the association between lung cancer risk and frequency of consumption of fruit and vegetables was examined among drinkers of vodka above average. Risk of lung cancer was increased significantly with increasing number of pack-years of smoking (OR = 18.7 for more than 40 pack-years). The significant inverse association of frequent usual consumption of boiled vegetables was present both among all subjects and among vodka drinkers (OR = 4.6 and 12.5, respectively, for a rare consumption). The risk of male lung cancer was negatively associated with the level of air pollution but positively with the percentage of occupationally exposed. Our study provides no evidence of a significant harmful effect of air pollution and found a strong inverse association between frequent fruit and vegetables consumption and lung cancer risk.

Absent latissimus dorsi muscle and anhidrotic axilla in Poland's syndrome. Case report.

The case of an 18-year-old female with absence of the right breast, sternocostal part of the major pectoral muscle and total absence of the minor pectoral muscle and the latissimus dorsi muscle, is presented. She also had decreased sweating capacity and absence of terminal hairs in the central part of her right axilla. Histological examination of skin specimens from this area of the axilla demonstrated hypoplasia of the apocrine sweat glands, but normal eccrine sweat glands, indicating an apocrine dependent axillary sweating function. The breast was constructed using a direct subcutaneous prosthesis implant, giving a final result that was good in regard to shape and softness. This provides grounds for reconsidering the use of the latissimus dorsi muscle flap in breast constructions in Poland's syndrome.
