Enabling care: working through obscurity and uncertainty a basic social process used in selected acute care settings.

Using grounded theory methodology this study explored the process that nurses used to determine, deliver, and communicate patient care in acute care hospital settings and to assess whether or not this was congruent with the nursing process structure (O'Connell, 1998). Data were obtained from semi-structured interviews, participant field observations and in-depth audits of patient records. Textual data were managed using NUD*IST and analysed using the constant comparative method. The findings of this study revealed a number of factors that hindered nurses from using the nursing process in its current form. Instead, nurses used a basic social process termed: Enabling Care: Working through obscurity and uncertainty. This paper describes a basic social process used in selected acute care settings and discusses its implications for nursing practice. An understanding of this process will assist the profession to critique some nursing theories and to construct ideas of nursing practice based on current clinical realities.

Nursing diagnosis: exploring the factors which may influence the reasoning process.

Nursing diagnosis is defined and discussed in relation to nursing's metaparadigm, the cognitive processes involved in problem-solving, and nursing's theoretical knowledge base. It is argued that each of these distinct yet related domains must be addressed by the clinician when applying the reasoning process. Factors are identified which can hinder and dramatically influence this diagnostic process, even to the detriment of the patient's welfare. Suggestions are offered throughout the discussion which may assist the nurse in developing more effective and accurate clinical decision-making strategies.

A post-modern nursing model.

For some time, nursing has been based on the structure of the nursing process and nursing models. However, in an age where the old order of science and medicine can no longer answer all of society's questions, can a nursing model, with its roots firmly based in the modernist structure of the nursing process, be post-modern?

A standards of care plan in the postanesthesia care unit.

Every patient is entitled to consistent, high-quality, individualized care. The PACU staff and former director of staff education at Southwestern Vermont Medical Center developed a documentation tool to achieve this end. The primary focus of the tool is on standards of care with an emphasis on outcome standards. Every patient received and cared for in the PACU is evaluated in terms of these outcome standards. If for any reason an outcome standard is not achieved, appropriate action is taken. The four-page bifold tool incorporates the nursing process. Potential nursing diagnostic problems are described and assessment and intervention data are integrated accordingly. The plan is further individualized vis à vis nursing progress notes. The tool has been in use for well over 1 year, and quality assurance studies demonstrate that outcome standards achieved and not achieved are consistently documented. The PACU staff believe this tool has enhanced professional practice and improved use of the nursing process and standards of care.

Education in nursing diagnosis: evaluating clinical outcomes.

This article describes the development, implementation, and evaluation of an educational program to update the knowledge and skills of nurses in the use of the nursing process and nursing diagnoses. Results 1 year later suggest improvement in participants' knowledge of the phases of the nursing process. Chart audits demonstrated this knowledge was more specifically applied to improved documentation of nursing diagnoses.

[Electronic data processing in nursing: specific needs related to rostering programs in nursing].

Electronic data processing is taking over in the nursing domain in German hospitals. This is due, not only to the fact that computer hard- and software are getting better and cheaper, but also due to the legal demands of the new health legislation. Neither nurse-managers nor the nurses themselves are adequately prepared for this technical innovation or its implications. The dominant influence of management-interests leads to the danger that criteria relevant to the content of nursing fail to be taken into account. Consequently the quality of care may suffer. The Ministry for environment, energy, youth, family and health has commissioned the Agnes Karll Institute for Nursing Research to identify software features specific for the requirement of nurses. The study focuses on rostering programs for nursing staff allocation, in relation to the workload and on conditions which have to be met to make software nurse-friendly. Selected, important results are reported.

Record-keeping and routine nursing practice: the view from the wards.

This paper examines general nurses' views of the nursing record and its routine usage in contemporary hospital practice. It draws on ethnographic data generated on a surgical ward and a medical ward in a single District General Hospital in the United Kingdom (UK). A key research finding was ward nurses' equivocal attitudes to the nursing record. On the one hand, because of its links with the nursing process, the nursing record was highly valued as a symbol of professionalism and ward staff were loathe to criticise it directly. On the other hand, however, the nurses in this study clearly found it difficult to reconcile their professional ideals with the ways in which the nursing record was routinely employed on the wards. In this paper it is suggested that at one level, nurses' ambivalence towards the nursing record reflected the distortion of its founding philosophy by the new managerialism in the contemporary UK health care context. At another level, however, there are also difficulties which arise from the tension between the assumptions about the nature of nursing work which underpin the nursing record and the workplace reality on hospital wards.

Conceptual and theoretical approaches to patient care: associate versus baccalaureate degree prepared nurses.

The purpose of the study was to determine if there is a relationship between types of entry-level preparatory nursing programmes in which a nurse receives basic nursing education and conceptual and theoretical approaches to patient care. A secondary purpose of the study was to determine if there is a relationship between entry-level basic preparatory nursing education and nursing leadership, the ability to make nursing diagnosis, and implementation, as well as evaluation of the nursing process. Three nursing practice categories were identified and included: professional, all-nurse and technical. Useable data collected by short-essay questionnaire from 343 out of 344 sample subjects were computed to determine the relationship, if any, between basic preparatory nursing education and conceptual and theoretical approaches to patient care. Decisions about three null hypotheses were made at the 0.05 level of significance utilizing analysis of covariance and the 0.01 level of significance utilizing chi-square analysis. For Hypothesis 1, the main effect for degree work when covaried with Verbal Scholastic Aptitude Test scores, Quantitative Scholastic Aptitude Test scores, time and age indicated a significance of 0.001 for all item associations in the professional, all-nurse and technical categories utilizing analysis of covariance, and 0.001 utilizing chi-square analysis. For Hypotheses 2 and 3, the main effect for degree work when covaried with the aforementioned variables indicated a significance of 0.001 utilizing analysis of covariance, and 0.001 utilizing chi-square analysis. In conclusion, the findings from the study suggest that graduates of baccalaureate degree programmes do vary in conceptual and theoretical approach to nursing care in specific nursing care situations.(ABSTRACT TRUNCATED AT 250 WORDS)

Making nursing models relevant for the practising nurse.

In this article the authors examine the background to the development of the nursing process and nursing models. The latter, it is argued, provide the theoretical framework for nursing actions. The descriptive study described here by the authors, took place at Michigan State University. The aim was to determine how useful nurses perceived nursing theory to their practice--in a setting where nursing theory had been taught for ten years. Nursing models were found to be useful to practising nurses in the clinical setting.

Decentralized unit orientation: a model for professional practice.

Facilitating use of professional practice models is a goal of nursing administrators and educators. A plan for decentralized unit orientation designed to foster use of nursing process and nursing standards was developed within the division of surgical nursing. Conceptual frameworks, topic selection, behavioral objectives, content outline, teaching strategies, operationalization of the plan, and methods to evaluate learning are described in this article. Staff development educators can use the information to assist unit managers in developing a decentralized unit orientation plan that emphasizes professional practice.
