Cosmetic breast surgery.

Appropriate follow-up care, including early postoperative serial mammography, is essential for women receiving cosmetic breast surgery. Common procedures include augmentation or reduction mammoplasty and breast reconstruction following mastectomy for cancer. Postoperative complications include infection, poor wound healing and pain. Hypertrophic scar formation, which may disrupt the normal breast contour, may be reduced by gentle and frequent massage of the scar. Avoiding sun exposure and using sun screens may prevent hyperpigmentation of the scar. Reduction mammoplasty is associated with sensory loss, which is occasionally permanent. Special complications associated with augmentation mammoplasty include diffusion of silicone through the implant and unfavorable breast appearance secondary to capsular contracture. Capsular contracture may be prevented by regular implant exercises. Preoperative and early postoperative serial mammograms are essential to differentiate surgical from malignant changes.

[Cosmetic aspects of dental treatment].

Communication between the dentist and his patient is an absolute condition for successful cosmetic dentistry. The patient must have possibilities to explain his wishes and expectations to the dentist (and if an indirect technique is used, to the dental laboratory). In this article methods are described and hints are given to arrange this in dental practice.

[Cosmetic dentistry and adhesive prosthodontics].

A better chemical bonding to metals offers the opportunity to bind composites onto metal and metals onto tooth structures. This may lead to an increase of adhesive prosthetic and cosmetic treatments. In this article some applications of this modern treatment method are described, anterior and posterior, in solitary elements as well as multiple bridge constructions.

[The planning of cosmetic plastic repair of the face and neck].

The author describes a method for layout before cosmetic plasty of the face and neck. Three types of layout are described according to age-associated changes of the female face and neck. Positive effect of this method was achieved in 157 women.

Cosmetic repair of a facial deformity by use of a silicone implant in a horse.

A heat-vulcanized silicone implant was used for cosmetic repair of a facial deformity in a horse. The deformity had resulted from fracture of the frontal bone into the frontal sinus 6 months earlier. Normal facial contour was restored by suturing the sculpted implant to the periosteum over the defect. Results 1 year after surgery were excellent.

Laser applications in cosmetic surgery.

The CO2-laser has proved to be an effective tool in the exciting field of cosmetic surgery. The ability to use the CO2-laserbeam either for vapourization or incision and its haemostatic effect makes it outstanding in many conditions in comparison with conventional methods. Teleangiectasias, portwine stains, decorative tattoos, scars, ageing skin and blepharoplasties are discussed. To obtain good results, experience, theoretical and practical understanding of the technique is a must, otherwise results will reflect poorly on the method.

[Carcinoma of the lip. Therapeutic results and cosmetic assessment].

The results of treatment of 283 patients with histologically verified infiltrating squamous cell carcinoma of the lip are presented. The cosmetic results after interstitial implantation of cesium needles, surgery or external irradiation are assessed. In tumours with the maximal surface diameter between 1 and 2 cm, fewer visible scars were found after implantation (6.9%) than after surgical excision (23%). The recurrence rate after interstitial cesium implantation is 12% and after surgery 15%.

Photodamaged skin: a medical or a cosmetic concern?

A well-tanned skin is currently regarded as a sign of fitness, youth and health, with people increasingly exposing their skin to the sun and using tanning accelerators, such as psoralens, but exposure to ultraviolet B (UVB) radiation from the sun causes extrinsic ageing of the skin. The minimal erythema dose is rarely exceeded but the UVB dose that brings about irrepairable DNA damage leading to photodamage is lower. Growing numbers of patients, therefore, have prematurely aged skin with pigmented spots, dryness, itchiness and wrinkles. These symptoms are more than just of cosmetic concern. It is known that epitheliomas frequently develop in photodamaged skin; consequently, this skin requires medical treatment. Prophylactic measures should be taken from childhood, but an effective treatment for already damaged skin is topically applied tretinoin, which should be used with a mild skin cleansing and skin care programme.

Pseudostrabismus secondary to use of cosmetic contact lenses.

The DuraSoft 3 Colors cosmetic contact lens is manufactured with a pattern of opaque dots applied to its front surface in order to produce a change in apparent iris color. We fit ten subjects with the DuraSoft 3 Colors contact lens, and all were found to have pseudoexotropia. The deviation ranged from 10 to 22 delta (prism diopters) (average: 16 delta). The cause of the pseudoexotropia was creation of a pseudopositive angle kappa from lateral displacement of the contact lens. Physicians fitting the DuraSoft 3 Colors contact lens should pay special attention to centration characteristics during the fitting session in order to avoid creation of pseudostrabismus.

Corrective cosmetic supramalleolar osteotomy for valgus deformity of the ankle joint: a report of two cases.

Valgus deformity of the ankle joint can cause pain in walking and a significant gait disturbance because of the altered ankle mechanics and the secondary planovalgus deformity of the foot. A technique of supramalleolar osteotomy that is cosmetically better than a closing wedge osteotomy for correction of this condition is described in two patients with hereditary multiple exostoses. The osteotomy is inherently stable, and minimal or no internal fixation is needed.

Cosmetic orchiectomy for prostate cancer.

With several effective therapeutic methods available for achieving androgen ablation, we must respect the psychological needs of our patients. The choice should ultimately be made by the patient after a thoughtful discussion of the advantages and disadvantages of various approaches. Cosmetic orchiectomy has the advantages of having the minimal morbidity of a simple operation, achieving the reliable result of androgen ablation and preserving the physical male organ without the risk of drug side effects and the dependence on patient compliance. In our experience, the operation is satisfactory both in achieving the therapeutic goal of androgen deprivation and in meeting the psychological needs of patients.

[Short-acting anesthesia in cosmetic surgery of the breasts].

Fourteen patients subjected to short-term cosmetic operations on the mammary glands comprise the study group. Anesthesia was performed with ketamine 0.07 mg/kg/min, applied against the background of droperidol 0.1 mg/kg or diazepam 0.2 mg/kg and spontaneous ventilation with nitrous oxide and oxygen in proportion 3:1. At the end of the ketamine anesthesia 1.0 g pyracetam was applied. There was negligible decrease in the systolic arterial pressure and decreased occurrence of psychotic reactions during the early postoperative period.

[Skin transplantation from the cosmetic and functional viewpoint].

With regard to the covering of large skin defects, split skin flaps and mesh grafts are of first consideration. Split skin flapping is preferably applied in cosmetically and functionally sensitive skin areas, whereas the rather unpretending mesh grafting is indicated in large defects on the trunk and extremities, in intertriginous regions, as well as in case of unfavorable conditions of the wound. The application of a fibrin sticker has considerably facilitated the procedure of free grafting and reduced the postoperative risks regarding the graft. We report on the final results of split skin flapping and mesh grafting in 7 patients with large skin defects of varying origin.

[The results of cosmetic prosthesis in patients in relation to the method of formation of a moveable base for the prosthesis following removal of the eyeball].

The paper analyses remote results of cosmetic prosthesis (more than 4 months) in 107 patients with anophthalmos. For formation of the stump after enucleation different methods were used: simple enucleation; enucleation with suturing of rectus muscles to the conjunctiva of the eye ball; enucleation followed by introducing a lavsane implant into the cavity of the Tenon's capsule; evisceration followed by introducing a lavsane implant into the cavity of the scleral disc. The best results were achieved in groups of patients when enucleation was followed by plastic formation of a mobile volume stump using a vascular wicker, crimped lavsane prosthesis.

[Conservative treatment of breast cancer. Carcinologic and cosmetic results of combination quadrantectomy and irradiation at the Centre Alexis-Vautrin].

We studied the association of quadrantectomy and irradiation in 238 patients with a breast carcinoma treated from 1974 to 1984 in the Alexis-Vautrin Center. Five and ten year survival rate are respectively 89 and 77%. Local relapse rate at five and ten year are 2, and 9%. Local relapse rate is correlated with a young age, a high tumor grade and an internal site of tumor. We studied the cosmetic and functional results and compared objective results according to subjective self-evaluation by the patients.

New developments in genioplasty: functional versus cosmetic.

Since the orthodontist is frequently the first clinician to be consulted for dentofacial deformity, an awareness of potential surgical procedures available to correct such deformities is imperative. Despite the fact that isolated genioplasty is becoming rare, the role the procedure plays in corrective jaw surgery is not diminished. Functional and cosmetic aspects must be considered in case planning and the flexibility of the procedure lends itself well to deformity correction in all three dimensions.

Cosmetic extended wear of oxygen permeable hard contact lenses: one year follow-up.

The use of oxygen permeable hard contact lenses (Menicon EX) as a cosmetic extended wear lens was investigated in 44 patients (86 eyes). Extended wear of these lenses for a one year period was successfully accomplished by 31 patients (60 eyes). Visual acuity and wearing comfort were good, and severe complications such as corneal edema and ocular infection were not observed through the test period. In some patients with relatively high-grade astigmatism, the degree of corneal astigmatism showed a considerable reduction.
