Diagnosis and treatment of shoulder injuries in the throwing athlete: the role of thermal-assisted capsular shrinkage.

The treatment of the throwing athlete is complex. Many factors enter into the decision regarding the nature and timing of appropriate intervention. Because of the nature of the mechanical aspects of the throwing motion, increased external rotation (overrotation) is often necessary to throw at a highly competitive level. This increased motion is associated with acquired increased glenohumeral laxity which, by itself, is generally not problematic. However, in the athlete with an overuse-type shoulder injury such as a SLAP lesion or a partial-thickness undersurface tear of the rotator cuff (internal impingement), it is perhaps the increase in glenohumeral laxity that allows such pathology to occur. A careful history and physical examination are important to determine the nature of the injury and the amount of laxity that may be present. Currently, we believe that addressing the capsular laxity arthroscopically at the same time that the intra-articular pathology is addressed is the best form of treatment for these athletes, and affords them the best chance of returning to competition at the same or higher level. Obviously, further follow-up is necessary to determine the long-term results of such treatment.

The effect of chromium picolinate on muscular strength and body composition in women athletes.

Fifteen women softball athletes were randomly divided into 2 groups, the chromium treatment group (n = 8) and the placebo control group (n = 7) to examine the effect of chromium, in the form of chromium picolinate (CrPic) supplementation, on muscular strength, body composition (body weight, percent body fat, and lean body mass), and urinary excretion. The CrPic supplementation consisted of a 500 ug dosage taken once per day. All participants trained 3 times per week with 2-3 sets of 8-12 repetitions at 80% of 1 repetition maximum (1RM) using variable resistance machines and free weights. No significant (p < 0.05) differences in muscular strength or body composition were found after 6 weeks of resistance training. In addition, chromium excretion (microg per 24 every hours) was examined and increased significantly with the treatment group after the 6-week period.

Rehabilitation programs and prevention strategies in adolescent throwing athletes.

A specific and early diagnosis must be made in the injured skeletally immature throwing athlete. A well-outlined program of rest, rehabilitation, and proper throwing techniques should be implemented and continued. Overuse injuries are preventable when biomechanics are sound and pitch counts are done with the limits enforced. Guidelines for inning limits, number of pitches, rest intervals, and throwing programs should be followed for adolescent pitchers because adolescents differ from adults. Information is included for specifications of the ball and helmet, as well as chest protective equipment. The goal should be for the Little League players to have fun and be injury free as they are competing, which gives them the best experience and allows continuation of athletic activities for a lifetime.

Long thoracic neuropathy from athletic activity.

Four cases of long thoracic mononeuropathy associated with sports participation are presented. Each patient developed shoulder pain or dysfunction after an acute event or vigorous activity, and demonstrated scapular winging consistent with serratus anterior weakness. The diagnosis was confirmed with electromyography in each case. It is suggested that the athletic activity caused a stretch injury to the long thoracic nerve. Conservative management, consisting of range of motion exercises for the shoulder and strengthening of the serratus anterior muscle, resulted in a favorable outcome in all patients.

Effects of creatine loading on muscular strength and endurance of female softball players.

AIM: The purpose of this study was to determine the effects of Cr supplementation on muscle strength and endurance of the trained females, the effect of anaerobic exercises performed before the supplementation and disappearance of the effects on the performance. METHODS: Twenty-six collegiate female softball players were divided into the Cr1, 2, Control (Cont.)1 or 2 group, randomly. They performed maximal 2-isometric and 30-isokinetic knee extensions at the angular velocities of 180 degrees and 60 degrees /s. Thereafter, the subjects of the Cr2 and Cont.2 groups performed resistance exercises with 10 RM and cycling with 0.01 kg x kg-1 body weight, alternately during the 1(st) week. During the 2(nd) week, the subjects of the Cr1 and 2 groups ingested 20 g Cr a day for 1 week. Thereafter, the subjects in the Cr2 group continued 3 g Cr ingestion a day for 2 weeks while the Cr1 group ingested the same amount of placebo during the 3(rd) and 4(th) weeks. The subjects of the Cont.1, 2 groups ingested the same amount and taste of placebo during the 1(st) to 4(th) weeks. This study was undergone in double blind fashion. The similar measurements, to which the subjects performed the 1(st) day, were carried out on the final day of the 2(nd), 3(rd) and 4(th) weeks. RESULTS: Though maximal isometric strength did not differ in intra- and intergroups, the isokinetic peak torque in the Cr1 group was significantly decreased in the 4(th) measurement (p<0.01). The mean torque of the 1(st) to 10th knee contractions at 60 inverted exclamation mark /s was significantly decreased from the 2(nd) measurement in the Cont.1, 2 groups, compared with that in the 1(st) measurement (p<0.01). In the Cr groups, the significant decrease was detected from the 3(rd) in the Cr1 and in the 4(th) measurement in the Cr2 group. The mean torque of the latter 20 contractions at 180 inverted exclamation mark /s was significantly increased in the Cr2 group (p<0.001). In the 1(st) measurement, the mean torque in the Cr1 group was significantly higher than that in the Cr2 group (p<0.05). However, the differences between Cr1 and 2 groups were not detected from the 2(nd) measurement. CONCLUSION: Twenty g Cr supplementation a day for a week to the trained females improves not the maximal static strength and dynamic peak torque but the mean strength and endurance of repeated contractions. However, the effects on females are not so great as that ever found in males. The effects are enhanced by anaerobic exercises performed before the supplementation and would disappear in a week if the supplementation were stopped.

Little league shoulder: case report and literature review.

When compared with adults, children have unique injury patterns secondary to their anatomical differences. The susceptibility of the growth plate to injury is well-documented. "Little Leaguer's shoulder" is the term used for injury to the open proximal humeral epiphysis in the Little League pitcher. We present a case report and literary review. Discussed are the possible etiologies, patient presentation, physical exam, radiographic findings, and treatment recommendations.

Olecranon stress fractures in throwers. A report of two cases and a review of the literature.

The evaluation and treatment of elbow injuries in throwing athletics has been the topic of numerous articles. The intense and repetitive stresses placed on the elbow may, in rare instances, produce undisplaced olecranon stress fractures. Two cases are presented and the literature is reviewed to propose that the fractures can be successfully treated by conservative methods.
