[The efficacy of using metal ceramic dentures fixed on pulpectomized teeth].

A total of 432 patients aged 34 to 60 were examined, 226 of them aged 31 to 59 were admitted for treatment. Out of 1980 examined cermet crowns fixed on depulped teeth complications were detected round 412 (20.88%). Inflammations of the gingiva were found near 218 crowns, apical chronic periodontitis in 98 cases, functional over-exercise of the periodontium in 61, ceramic fracture in 18, failure of cement fixation in 17 cases. When all prophylactic measures are taken, apical periodontitis was properly treated, and dental channels filled during pulp removal, if indicated, complications develop almost three times less frequently, occurring in 7.4% of cases.

Periodontal status following trapezoidal and semilunar flaps in apicectomy.

Apicectomy is offered where routine endodontics cannot resolve periapical inflammation. This study compared the influence on periodontal attachment level of two surgical procedures, the trapezoidal (TF) and the semilunar (SF) flaps. Twenty patients aged 16-44 years (mean 23.2 years), were randomly assigned to either flap procedure. Prior to surgery the patients received periodontal prophylaxis and oral hygiene instruction. Records were made of gingival indices, pocket depth and the distance from the cemento-enamel junction to the bottom of the gingival pocket. The measurements involved buccal surfaces of 13 to 23. The TF flap extended from 14 to 24 between the interdental papillae, whereas the SF flap was carried in a semi circle from 14 to 24 about 2mm from the attached gingiva. The surgical wounds were closed with black silk sutures. Antibiotics and analgesics were prescribed. Sutures were removed after one week. On recall after 6, 12 and 24 weeks the same measurements as before surgery were made. No statistically significant change was observed in pocket depth or attachment level between TF and SF flaps (p > 0.05). However, the TF flap produced less noticeable scarification than the SF flap.

[Prospective comparative randomized study of tooth apex resection and orthograde root filling with Guttapercha and titanium pins].

The prospective randomised study on the root apex resections with orthograde root filling comparing Guttapercha with Titan pins encompasses 500 patients with 633 operations on all front teeth and premolars. The patients were divided into three preoperative stages so that for this broadly defined diagnoses comparable collectives could be built. After an average postoperative interval of 21 months, 258 patients with 298 resected teeth could be evaluated. From these, 188 root apex resections had been treated with Guttapercha and 110 with Titan pins. 73.5% of the root apex resections were successful, whereby the group treated with Guttapercha was more successful by 7%. The comparatively high failure rate of 26.5% is attributable to the broadly defined diagnosis and the long follow-up period.

Guided bone regeneration in the treatment of a large periapical lesion: a case report.

A large (19 mm x 24 mm) periapical lesion associated with tooth #10, perforating both the labial and palatal cortical plates, was treated with decalcified freeze-dried bone allograft and guided bone regeneration. The membranes were removed after a seven-month healing period. Complete osseous reconstruction of the cortical plates was noted. Biopsies demonstrated healing with mature bone and hemopoietic marrow. Few particles of decalcified freeze-dried bone could be observed. Graft particles that remained were surrounded by new bone. The learning objective of this article is to present the regeneration procedure utilized along with the histologic biopsy results to the clinician.

Survival rates of hemisected teeth: an attempt to compare them with survival rates of alloplastic implants.

Use of hemisection in an attempt to preserve multirooted teeth that have furcation involvement has increased in recent years. However, there are few long-term studies of the success of this treatment. Furthermore, it is difficult to compare results of different studies because of differences in the length of follow-up and the criteria for failure. Results of various studies were reduced to a common denominator to allow comparison. The compiled results of studies on hemisection revealed an average reported failure rate of 13.1%. This rate was compared with results of studies on implants; the failure rates of the two treatment alternatives are not substantially different. Thus, because hemisection is a relatively simple, inexpensive treatment with a good chance of success (given appropriate case selection), it should always be considered as an option before molar extraction.

Dens invaginatus: complications and treatment of non-vital infected tooth.

A case of non-vital infected dens invaginatus of the maxillary right lateral incisor with open apex, which presented with pain and swelling is presented. Although root growth and apical closure was achieved using calcium hydroxide, the periapical infection persisted and resolution was only achieved after apical curettage and apicectomy.
