Comparative carcinogenicities of 1-, 2-, and 4-nitropyrene and structurally related compounds in the female CD rat.

The comparative carcinogenicities of N-hydroxy-N-acetyl-1-aminopyrene, N-acetyl-1-aminopyrene, and 1-, 2-, and 4-nitropyrene were determined following i.p. injection into weaning female CD rats (67 mumol/kg body weight in dimethyl sulfoxide; 3 times/week for 4 weeks). At sacrifice 61 weeks after the first injection the incidences of malignant mammary tumors were increased significantly to 45 and 24% in the 4-nitropyrene- and N-hydroxy-N-acetyl-2-aminofluorene-treated groups, respectively. Cellular altered foci in the liver were increased significantly in the N-acetyl-1-aminopyrene-, N-hydroxy-N-acetyl-1-aminopyrene-, and N-hydroxy-N-acetyl-2-aminofluorene- treated groups; the latter two compounds also led to significantly increased formation of hyperplastic nodules in this organ. Significant increases in leukemia induction were observed in animals treated with 2-nitropyrene or N-hydroxy-N-acetyl-2-aminofluorene. In an experiment designed to compare the influence of the route of administration on the carcinogenic potential of this agent, 1-nitropyrene was injected i.p. or s.c. into weanling female CD rats (100 mumol/kg body weight; once a week for 4 weeks). The animals were sacrificed at 87 to 90 weeks after the first treatment. The incidences of mammary gland tumors in animals receiving injections of 1-nitropyrene by either route (59%) were significantly higher than in solvent-injected controls (37%). The incidences of adenocarcinoma in the i.p. 1-nitropyrene group (28%) and fibroadenoma in the s.c. 1-nitropyrene group (52%) were significantly higher than in the control animals (7 and 27%, respectively). These data suggest that the demonstration of the weak carcinogenicity of 1-nitropyrene is probably more a function of the length of the observation period than of the routes of administration used here. A further exploration of the effect of the route of administration involved treatment of weanling female CD rats by direct injection of 1-, 2-, or 4-nitropyrene into the mammary fat pads. A total of 2.04 mumol of the nitrocompound in dimethyl sulfoxide was injected into the mammary glands under each of the 6 left nipples. The right mammary glands were treated with the solvent only. Injections of the thoracic nipple areas were carried out on day 1; inguinal areas were treated on day 2. The animals were sacrificed after 77 weeks. The number of mammary tumor-bearing animals (23 of 28), the number with fibroadenoma (15 of 28), and the number with adenocarcinoma (19 of 28) were significantly increased in the 4-nitropyrene-treated group as compared with animals treated with only dimethyl sulfoxide.(ABSTRACT TRUNCATED AT 400 WORDS)

Carcinogenicity of diethylstilbestrol in the Wistar rat: effect of postnatal oral contraceptive steroids.

Diethylstilbestrol (DES) has been associated with vaginal neoplasia and malformations in humans. We have studied a test population of 504 female Wistar rats given diethylstilbestrol at from 0.0 to 0.5 mg/kg maternal body weight on days 18, 19, and 20 of gestation. Animals were euthanized in extremis, or at 2 years of age. The incidence of vaginal epithelial tumors was dose related. The types of epithelial tumors of the vagina were adenocarcinoma, squamous cell carcinoma, and mixed carcinoma, containing discrete adenomatous and squamous components. The incidence of vaginal epithelial tumors was determined to be dose related: rats exposed to 0 mg DES/kg maternal weight had an incidence of 0.6% (1 of 167 rats); 0.1 mg/kg, 4.1%; and 0.5 mg/kg, 4.3% (6 of 140); 25 mg/kg, 1.6% (1 of 63); and 50 mg/kg, 11.5% (3 of 26). Tumors of other reproductive tissues (mammary gland, ovary, oviduct, cervix, or uterus) demonstrated no discernible DES dose-response relationship. There was no oncogenic effect of postnatal administration of oral contraceptives (0 oral contraceptives, 31.25 micrograms/kg diet ethynylestradiol, and 31.25 micrograms/kg diet norethindrone or 104 micrograms/kg diet ethynylestradiol and 31.25 micrograms/kg diet norethindrone). Thus, vaginal tumors can be induced in a dose-related manner in the rat following in utero DES exposure. Oral contraceptive treatment did not increase the risk of neoplasia.

[Retroperitoneal teratoma associated with primary neoplasm of the ovary. Presentation of a case].

The retroperitoneal teratomas are rare neoplasias that represent only 3.5% of all extragonadal teratomas. They generally appear in the first decade of life, and the incidence in adults is rare. We describe a rare case of extragodonal teratoma, not only because the patient is in the 7th decade of life, but also because of the localization and association of two epithelial cellular tumors of the ovary. We also describe the surgical and histopathological findings as well as an analysis of the frequence, clinical characteristics, diagnosis and treatment for this neoplasia.

[Cell kinetic investigations in fibrocystic mastopathies and invasive breast carcinomas].

The thymidine labeling index (TLI) was measured in tissues from 64 women with fibrocystic changes, fibroadenomas and invasive breast carcinomas by an in vitro method. The lesions were classified according to Prechtel and Hutter. Per patient a total of about 5000 cells were counted from five different regions. The TLI in Mastopathia (MP) I was 0.23 +/- 0.16 (mean +/- SD), in MP I-II 0.43 +/- 0.40, in MP II 0.77 +/- 0.41, in MP III 0.99 +/- 0.67, in fibroadenomas 0.87 +/- 0.49 and in carcinomas 3.44 +/- 2.04. These results underline the proliferative heterogeneity of different forms of fibrocystic conditions. Their relative risk for the subsequent development of invasive breast cancer seems to be low but different. The variable labeling indices of breast cancer express the different growth rate of the tumors. Labeling indices, flowcytometric data and immunohistochemically detected Ki-67-growth fraction show a similar behaviour. Results of cell kinetic studies could contribute for characterization of human breast carcinoma and their preneoplasias.

Benign breast tumours in an African population.

A detailed prospective clinical study of 238 women presenting with benign breast swellings in the South Eastern equatorial rainforest of Nigeria clearly shows that 94% of lesions were due to two disorders, fibroadenomas and bacterial infections, and that they affected mostly young women in the peak of their reproductive life. Fibroadenoma which accounted for disease in no less than 69% of the women was multiple in 8%, bilateral in 11%, and was considered giant in size in 5% of the cases; by contrast, fibroadenosis was rare, accounting for swellings in only eight patients. Bacterial infections, chronic pyogenic mastitis and tuberculosis together caused breast swellings in 25% of the women studied. In tropical Africa, multiple chronic discharging sinuses suggest tuberculosis of the breast.

[Pyruvate oxidation and S35-lipoic acid fixation by breast tumor homogenates].

The rate of pyruvic acid oxidation by human mammary gland tumor homogenates is several times lower compared with that by homogenates of the adjacent non-involved tissues. This dependence is especially manifest with malignant tumors, although in this case both in the involved and non-involved tissues the pyruvate oxidation is slower than in benign tumors. In the in vitro experiments the rate of S35 lipoic acid binding by the precipitated in 10000 g fractions of the homogenates in question was the highest for malignant tumors and 3.36 times lower for the adjacent tissues. In benign tumors these values and their correlation were much more lower.

Occurrence of antibody against intracytoplasmic A-particles of mouse mammary tumor virus in sera from breast cancer patients.

Acetone-fixed smears of DBA/2 mouse leukemia cells that produce clusters of intracytoplasmic A-particles (pronucleocapsids of mouse mammary tumor virus) were employed as an indirect immunofluorescence system to detect the antibody to A-particles in human sera. With positive test sera, specific fluorescence was easily detectable as discrete cytoplasmic granules at the site of A-particle clusters. The antibody was found in 26 (60%) out of 43 breast cancer patient sera and 4 (25%) of 16 mammary fibroadenoma patient sera, while only 4 (11%) out of 37 control woman sera were antibody-positive. In the case of breast cancer patients, occurrence of tha antibody was not specifically related to a particular type of tumor histology. In a considerable number of positive cases, the antibody tended to disappear within various lengths of time after surgical operation of the breast cancer.

[Color Doppler in the echographic evaluation of breast nodules].

The authors report on their experience with color Doppler sonography in the diagnosis of solid breast masses. Twenty-two patients were examined; breast masses were studied with B-mode US first, and then with color Doppler US to evaluate eventual tumor vascularization. Color Doppler US demonstrated only one vascular pole in histologically confirmed benign masses. On the contrary, in 92.8% of histologically confirmed malignant masses, color Doppler easily depicted 2 or more groups of nutritional arteries. Color Doppler makes the diagnosis of malignant masses easier, thus allowing, in the author's opinion, a reduction in the number of biopsies of solid breast masses clinically/mammographically detected. The use of color Doppler US is therefore suggested: the technique is noninvasive, fast, and easy and its widespread use would translate into advantages for both the patient and the clinician.

DNA distribution pattern and comparison of quantitative data in breast tumours.

Aspiration cytology smears from 24 breast alterations were subjected to cytophotometry and TV image analysis. The smears were grouped according to histological diagnosis and cytological pattern. Cell cycle parameters and DNA indices were calculated from the decomposition of the DNA histograms. Based on quantitative morphometric features the TV image analyser distinguished the benign cells from the malignant ones. In benign cases its diagnostic accuracy is of 80%. Cytological Grades 1, 2 and 3 are also identifiable by this system. Cytophotometric measurements were also performed with 22 invasive breast cancers with known 5-year survival. The cell cycle parameters were related to the survival data. From the point of view of survival only the ratio of the S plus G2 phase cells of all the parameters examined proved to be significant.

[Parasternal lymphoscintigraphy in planning the radiation therapy of breast cancer].

Altogether 82 breast cancer patients were investigated to determine a possibility of the use of parasternal lymphoscintigraphy with colloid finely divided radiopharmaceuticals 99mTc and 198Au to form individual irradiation fields; 43 patients with fibroadenomatosis were taken as controls. Variants of the localization of parasternal lymph nodes were defined, standard and "optimum" (individual) parasternal irradiation field values were compared. 54.9% of the patients were shown to be in need of correction by increasing a standard irradiation field (5 cm wide).

[Intracytoplasmic lumina of benign and malignant breast diseases--a light and electron microscopic study].

Intracytoplasmic lumina (ICLs) of 70 cases with breast carcinoma and 29 cases with benign breast diseases were observed by light and electron microscopy. ICLs were morphologically divided into two types. Type A was characterized by the presence of secretory materials stained with eosin in the lumen and Type B by the cytoplasmic vacuoles under light microscope. Electron microscopic observation on Type A ICLs showed numerous filiform microvilli projecting towards the lumen and various amounts of secretory materials in the lumen. Type B of ICLs only had scanty and short microvilli and rarely secretory materials in the lumen. The results indicated that: 1. The frequency of ICLs in breast cancer was significantly higher than that in benign breast disease (P less than 0.01). 2. The frequency of ICLs in breast cancer showed strong negative correlation with its histological grades but not with its histological types. 3. ICLs had similar frequency under both light and electron microscopes. As a relatively specific structure in breast carcinoma cells, ICLs may be helpful in the diagnosis of breast carcinoma and establishment of the breast origin for metastatic carcinoma.

[Sonography in breast diagnosis].

In a prospective study of 235 tissue masses verified histologically, we examined the diagnostic contribution of ultrasound in comparison to mammography. When the two methods were combined, 96.8% of all malignant neoplasms could be diagnosed correctly. Most (90.5%) were found by mammography alone and 4.8% were detectable by ultrasonography but not by mammography. Despite relatively good results in the evaluation of solid masses, it is not advisable to omit tissue examination. The superiority of ultrasound over mammography in the diagnosis of cystic lesions could lead to a significant reduction in needle aspirations and biopsies.

Intramammary granular cell myoblastoma.

Two female patients, age 15 and 21 years, with intramammary granular cell myoblastoma who were without recurrence of the development of additional lesions after one year and eight years, nine months respectively, are reported. Electron microscopic examination of one of our cases disclosed the typical osomphilic granules but no features to support a particular cell of origin. Review of the 52 previously reported cases indicates that this unusual intramammary neoplasm has been encountered with relatively few exceptions in the 20 to 59 year age group and in the upper quadrants. It clinically mimics fibroadenoma or carcinoma and on frozen section examination may be misinterpreted as malignant. None have recurred following adequate excision.

[Oncological cytodiagnosis of the mammary gland].

Oncological mammary cytodiagnosis concerns a secretion, imprint, and punction. It is an integral and unsubstitutable part of the team cooperation in the counselling and prophylactic medical activity. Its pros are made use of in the field of prebioptic selection. Together with clinical and X-ray examination it forms a diagnostic whole which can be used in active prophylaxis as a method of screening. The arbiter of suspect and positive findings is biopsy.
