The effect of anti-IFN-gamma antibody on the protective effect of Lyt-2+ immune T cells against toxoplasmosis in mice.

The effect of an IFN-gamma mAb on the protective activity of immune T cells against Toxoplasma infection was examined in a murine model of toxoplasmosis. Mice that received anti-IFN-gamma antibody and immune spleen cells all died of toxoplasmosis after challenge with Toxoplasma tachyzoites. In contrast, mice that received normal IgG and immune spleen cells all survived the infection. The protective activity of Lyt-2+ immune T cells, previously shown to be the principal mediators of resistance against Toxoplasma in mice was completely ablated by the anti-IFN-gamma mAb. These results suggest that IFN-gamma is the major mediator of the resistance against Toxoplasma infection in mice which is conferred by immune T cells.

[Prolonged fever revealing acquired toxoplasmosis in an adult].

Two cases of acquired toxoplasmosis are reported in 16 and 44 year old patients; prolonged fever was the dominating sign in both cases. It follows from these observations that toxoplasmosis has to be systematically considered as a possible etiology of prolonged fever of undetermined origin.

Study of congenital toxoplasmosis in Egyptian newborns.

Five hundred cord blood samples of normal, full term, apparently healthy newborns of both sexes and of different social classes were studied for Toxoplasma antibodies. Eighty nine cases (17.8%) showed a seropositivity for IgG antibodies while specific IgM antibodies were found in 3 cases. A significant higher percentage of seropositivity was found among newborns whose mothers had a previous history of abortion (p less than 0.01), still birth, prematurity or delivery of infants with congenital anomalies (p less than 0.001). The relation between seropositivity and maternal lymphadenopathy and/or fever during pregnancy was highly significant (p less than 0.001). Also, a highly significant (p 0.001) relationship was observed between mothers' contact with cats and seropositivity for toxoplasmosis. No statistical differences were found in the seropositivity between the 2 social classes studied, nor between the sexes. The study ensured that the problem of congenital toxoplasmosis in Egypt should be seriously considered and emphasized the importance of serological screening before and during pregnancy for those women who are at greater risk and of newborns.

Phenotypes and functions of lymphocytes in congenital toxoplasmosis.

Comparisons of lymphocyte surface phenotypes and functions were made among 25 pediatric patients with congenital toxoplasmosis (ages 1 month to 5 years) and 24 uninfected babies, a baby with postnatally acquired infection, and 6 uninfected, 7 recently infected, and 6 chronically infected adults. Percentages of lymphocytes with B1, T11, T3, T4, T8, T6, B4, Ia, NKH phenotypes and T4 to T8 ratios of infected and uninfected babies were the same (p greater than 0.05). Lymphocytes from congenitally infected babies had lower blastogenic responses (mean stimulation index [SI] = 5) to Toxoplasma lysate antigens (TLA) than lymphocytes from adult control groups with recent and chronic infection (Mean SIs = 32, 72; p less than 0.001). Compared to infected adults, congenitally infected children had similar or greater responses to concanavalin A (mean SIs = 94; 118, 76, p greater than 0.05) and in mixed leukocyte culture (mean SIs = 63; 22, 26, p greater than 0.05). For symptomatic babies, low blastogenic response to TLA correlated with more severe disease at presentation (p = 0.002). Lymphocyte blastogenic responses to TLA were increased for most children when they were older than 15 months, but responses remained less than adult levels for 9 of the 17 older children studied. There was no correlation between concomitant serum pyrimethamine levels and lymphocyte blastogenic responses to TLA. Pyrimethamine and sulfonamide treatment in vitro and in vivo did not alter lymphocyte response to TLA. Lymphocytes from congenitally infected babies failed to produce either gamma interferon or Interleukin 2 when cultured with TLA. In contrast, they produced these lymphokines when cultured with concanavalin A or phytohemagglutinin. Specific deficits in cell-mediated immune responses to TLA may account for the significant organ damage that occurs in infants and children with congenital toxoplasmosis.

[Diagnosis of cerebral toxoplasmosis in AIDS patients. Value of antibodies evaluation in cerebrospinal fluid].

The purpose of this study was to evaluate the diagnostic value of search for intrathecally synthesized specific antibodies in cerebral toxoplasmosis. Intrathecal synthesis of antibodies was measured by calculating the immunity load coefficient (ILC) in both serum and cerebrospinal fluid. In this retrospective study the records of 42 AIDS patients with clinically on the basis of computerized tomography and therapeutic results, and was excluded in 31 patients (control group). Specific immunoglobulins G were found in the cerebrospinal fluid of 9 out of the 11 patients with toxoplasmosis, but also in 14 of the 31 controls. Since ILC measurements in serum and cerebrospinal fluid identified 7/11 cases other hand, since the ILC value was higher than the positivity threshold in controls, the specificity of the test was assessed at 68 percent.

Acquired Immunodeficiency Syndrome↗

Current diagnosis and management of toxoplasmosis in cancer patients.

Toxoplasmosis should be considered as a potential pathogen in cancer patients who have clinical syndromes, especially lymphadenopathy, for which the cause is not clear. Patients with reticuloendothelial malignancies or solid cancers treated with systemic chemotherapy or with progressive disease appear to be at particular risk for life-threatening toxoplasmosis. There has also been an explosion in the number of cases of toxoplasmic encephalitis in patients infected with HIV. Diagnosis rests on demonstration of trophozoites in tissue or body fluids, demonstration of characteristic serologic test results, and, in cases of lymphadenopathy, characteristic histologic features.

[Epidemiology of toxoplasmosis in Chile. VI. Prevalence of human infection, investigated by means of an indirect hemagglutination test, in regions VII, VIII and IX].

In presecuting the investigations on the epidemiology of toxoplasmosis in Chile, a new series of serological surveys has been performed during 1982-1989 in 10 urban and 25 periurban-rural localities from the regions VII, VIII and IX of the country (34 degrees 41'-39 degrees 38' South lat.). In 9,758, randomly selected apparently healthy persons, and indirect hemagglutination test (IHAT) for toxoplasmosis was carried out. The age of these individuals (4,203 males and 5,555 females) varied between 4 and 84 years. The examined persons represent a 0.33% of the total population of the three studied regions. IHAT titers of 1: > or = 16 were regarded as positive. The global prevalence for positive IHAT was 45.5% (50.5% in men and 41.7% in women). A higher proportion of positive tests was observed in urban areas (47.0%) than in periurban-rural sections (33.3%). An increasing prevalence with age was also observed. Only 5 (0.05%) persons had IHAT titers higher than 1:1000.

[Epidemiology of toxoplasmosis in Chile. VII. Prevalence of human infection investigated by means of indirect hemagglutination reaction in the regions X, XI and XII].

In pursuing the studies on the epidemiology of toxoplasmosis in Chile, a recent series of surveys have been undertaken in 1983-1990 in 4 urban and 22 periurban-rural localities from regions X, XI and XII of the country (39 degrees 38'-56 degrees 00' South lat.). An indirect hemagglutination test (IHAT) for toxoplasmosis was performed to 6,438 randomly selected apparently healthy persons. The age of the surveyed persons (2,779 males and 3,659 females) varied between 1 and 93 years. The number of examined persons represents a 0.61% of the total population of the three studied regions. IHAT titers of 1: > or = 16 were considered as positive. The general prevalence for positive IHAT was 46.3% (47.4% in men and 45.4% in women). Similarly to other regions, a higher proportion of positive reactors was detected in urban sections (49.0%) than in periurban-rural ones (40.3%). An increase of prevalence with age was also observed. Only 8 (0.12%) persons presented IHAT titers higher than 1:1000.

[Toxoplasmosis in the Republic of Senegal. Sero-epidemiological survey].

The presence of Toxoplasmosis in Senegal was investigated by the detection of IgG antibodies. 415 samples were obtained from healthy subjects from different regions of the country. In subjects of over 30, a greater number were found positive for IgG antibodies if they lived in a humid climate (25% in Northern Senegal, 57.7% in eastern Senegal). In Dakar 33.3% of the pregnant women studied had antibodies. This study confirms the presence of Toxoplasmosis in Senegal.

Laboratory diagnosis of acute toxoplasmosis by immunosorbent agglutination assay (IgM ISA).

A group of 401 patients suspected for toxoplasmosis was examined by the indirect immunofluorescent antibody test (IFAT). All patients positive in IFAT (176) were examined by the immunosorbent agglutination assay (IgM ISA). In the IgM ISA 154 of them were negative, 10 temporarily and 12 high-positive. Some of high-positive patients were examined repeatedly; decrease of high levels of specific IgM antibodies occurred 2-9 months after the first examination. For the IgM ISA antigen prepared from peritoneal exudate of experimentally infected mice was used. The antigen was stable at 4 degrees C or in liquid nitrogen at least 1 year. The IgM ISA combined with IFAT and IgM IFAT was proved satisfactory for the diagnosis of acquired acute toxoplasmosis and can be recommended for laboratories with lower capacity.

[Toxoplasmosis in the heart transplant patient].

Toxoplasmosis is a well-known problem under immunosuppressive conditions in cardiac transplant patients. In our series 12% of the patients (8 of 65) had clinical and/or serological signs of active toxoplasmosis. Due to its serious prognosis once the clinical process has started we favor a generous diagnostic interpretation of the serological results and a broad indication to drug therapy.

Surveillance of toxoplasmosis in different groups.

Two hundred and twenty three serum samples were screened for the antitoxoplasma antibodies of IgG type by a direct agglutination method, of these 78 (34.9%) were sero-positive. In high risk groups 71 (40.1%) of 177 cases were positive, while in the low risk groups 7 (15.2%) out of 46 cases were positive. Sero-positive males were more than females, and the disease increased steadily with age. Of the high risk group the prevalence of toxoplasmosis was 53.7% in immunocompromised group, 44.7% in animal handlers, 39.8% in pregnancy wastage group and 25.8% in a congenitally abnormal children. A high percentage prevalence of toxoplasmosis was noticed in high risk groups as compared with low risk group.

Association of antibody titer and chorioretinal scarring in toxoplasmosis retinochoroiditis.

Thirty subjects with clinically diagnosed toxoplasmosis retinal lesions, and 30 control subjects matched for age, sex, and race, were evaluated for the presence of IgG antitoxoplasmosis antibodies by enzyme-linked immunosorbent assay (ELISA) methodology. Positive titer was found in 56% of subjects and 20% of controls. Positivity of titer was not correlated to location of lesion, laterality, presence of satellites, sex, or race. Increasing age in both subjects and controls was weakly correlated to positive titer. Study results indicate that a relative overdiagnosis of toxoplasmosis occurs if positive antibody titer is accepted as criteria for positive diagnosis. Currently employed criteria based on physical findings appear to be of little value in predicting positive titer.

Economic and public health considerations of congenital toxoplasmosis in lambs.

Congenital toxoplasmosis was diagnosed in a flock of Hampshire sheep in South Dakota. The 80 ewes produced 144 lambs, 30 of which were born dead; toxoplasmosis was diagnosed in 11 of the dead lambs. The remaining 114 lambs grew normally, but 68 (40.3%) had agglutinating Toxoplasma gondii antibodies. Modified agglutination test T gondii antibody titers for 114 lambs were: less than 100 (n = 46), 64 (n = 2), 256 (n = 1), 1,024 (n = 12), and greater than or equal to 4,096 (n = 53). Tissues of 8 seropositive lambs were bioassayed for T gondii tissue cysts. Toxoplasma gondii was isolated from the tongue and lamb chops of 7, heart of 3, and legs of lamb of all 8 lambs.

Seroepidemiology of toxoplasmosis on Taiwan and some of the offshore islands.

Over 4,000 serum specimens from various population groups on Taiwan, Lan Yu, and the Pescadores Islands were tested for antibodies to Toxoplasma gondii by use of the indirect hemagglutination test. Only 74 sera were considered to be positive at titers of 1:256 or greater. The highest prevalence rates of positive antibody titers were obtained from 335 people from Ian Yu Island; 59 (18%) were considered positive. A few (11 soldiers) positive titers were detected in sera from 2,487 Chinese Marine recruits on first bleedings and 4 found positive on a second bleeding 12-18 months later. Only a few suspected cases of toxoplasmosis have been reported from humans, yet a number of animals have been found infected with the parasite and other animals found serologically positive. These and other aspects of toxoplasmosis on Taiwan are discussed.

[A sero-epidemiological survey on toxoplasmosis in Yunan province].

From 1983 to 1987, we carried on a seroepidemiological survey on Toxoplasmosis using Indirect Haemagglutination Test (IHA) in seven prefectures of Yunna province. A total of 14577 human serum samples and 1427 animal serum samples were tested, 1159 positive in 14577 human sera (7.95%), and 163 positive in 1427 animal sera (11.42%). It is clear that toxoplasmosis infection is rather widely prevalent in Yunnan province.
