Encapsulated fat necrosis is a rare condition with clinical features that closely resemble a lipoma. The aetiology and pathogenesis are not fully understood but the condition has a distinctive histological appearance. We describe a case of encapsulated fat necrosis that exhibited unusual mobility. The literature is also reviewed.

We present a case report of acute bilateral excercise-induced compartment syndrome in the adductor longus muscles, which was treated with bilateral medial fasciotomies. Postoperatively, the healing process of the adductor muscles was followed up by repeated MR imagings over six months. Myonecrosis was found in peroperative muscle biopsies. Pain and muscle swelling subsided soon after the fasciotomy, correlating with the early postoperative MR findings. Four months postoperatively, the signal intensity of the adductor muscles was normal in T1- and T2-weighted images, but the normal fibre structure of the adductor muscles could only be seen 6 months postoperatively. At six month's control checkup there was no subjective weakness of the adductors, and hyperesthesia had disappeared and the patient was capable of normal activities.
The purpose of this study was to compare pure eccentric and concentric strength training regarding possible specific effects of muscle action type on neuromuscular parameters, such as a decreased inhibition during maximal voluntary eccentric actions. Two groups of young healthy adult men performed 10 weeks of either eccentric or concentric unilateral isokinetic knee extensor training at 90 degrees.s(-1), 4 sets of 10 maximal efforts, 3 days a week. Knee extensor torque and surface EMG from the quadriceps and hamstring muscle groups were collected and quantified in a window between 30 and 70 degrees knee angle (range of motion 90-5 degrees ) during maximal voluntary eccentric and concentric knee extensor actions at 30, 90, and 270 degrees.s(-1). Changes in strength of the trained legs revealed more signs of specificity related to velocity and contraction type after eccentric than concentric training. No major training effects were present in eccentric to concentric ratios of agonist EMG or in relative antagonist (hamstring) activation. Thus, for the trained leg, the muscle action type and speed specific changes in maximal voluntary eccentric strength could not be related to any effects on neural mechanisms, such as a selective increase in muscle activation during eccentric actions. Interestingly, with both types of training there were specific cross-education effects, that is, action type and velocity specific increases in strength occurred in the contralateral, untrained, leg, accompanied by a specific increase in eccentric to concentric EMG ratio after eccentric training.
The value of colour-coded Doppler sonography to evaluate results of percutaneous transluminal angioplasty of the superficial femoral artery was tested in 26 patients. Because of good accessibility, morphology may be demonstrated similar to intravenous DSA. However, accurate evaluation of dilatation success also requires haemodynamic measurements. Numerous parameters were determined one day before, one day after and one month after angioplasty at various points of the extremity (dilated segment, popliteal artery, lower leg arteries). Results were compared with those of normal individuals. Because of the large range of scatter and overlapping between normal and pathological values, only a few parameters can be used as valuable indicators for successful PTA. Spectral wave form becomes bi- or triphasic pattern and due to this the Pulsatility Index rises distal to the dilated area. An increasing systolic acceleration rate, peak systolic velocity and prestenotic Pulsatility Index have excellent predictive value. Using a Damping Factor, the later walking range may be predicted.
Although free vascularized iliac bone graft has been successfully used for the reconstruction of large bone defect with microvascular surgery, there is a serious problem of how to repair in one-stage, those cases having a large bone defect with a very wide skin defect. A free combined anterolateral flap and vascularized iliac bone graft with double vascular pedicles seems to be a most suitable method for cases having both large bone and skin defects. Two case reports are presented in which this flap was used. Based on the authors' cases, the advantages of this flap are its thinness and the extreme wideness of the skin territory. The anatomy of the pedicle vessels is large and long, and the donor scar can be made in an unexposed area. This flap can be considered for use in one-stage reconstructions of both large bone and skin defects in the oral and leg regions.

The muscle atrophy of the quadriceps muscle was investigated in patients after knee operations and immobilization in a long leg plaster cast. Muscle biopsies were taken before operation and after 5 weeks immobilization. The activity of oxidative (SDH) and glycolytic (PFK) enzymes was determined. The fiber area of the different fiber types were measured and enzyme activities in single characterized fibers were analyzed. A selective type 1 atrophy was found with a reduced cross-cut area of the type 1 fiber, and a reduced oxidative enzyme activity occurred in the slow-twitch type 1 fibers. The effect of the muscle tension in the immobilized leg and the inhibiting role of the postoperative pain is discussed.
Femoral nerve impairment subsequent to gynaecological operations is well known. Injury to the N. cutaneous femoris lateralis has not yet been reported either in gynaecological or neurological literature. --At the Department of Obstetrics and Gynaecology, Medical School Hannover, 8 cases of neuropathy of this nerve have been observed from 1973 to 1982. They occurred after vaginal as well as after abdominal operations. The cause of the injury may be a short, but extreme hyperextension of the leg while bringing the patient into lithotomy position. During abdominal operations continuous pressure of a self retaining retractor may lead to the damage. --A similar case however, where the causes mentioned above were absent, revealed that other possibilities must also be considered.
To objectify the muscle function of 5 patients ten years after implantation of a tumor endoprosthesis of the knee simultaneous muscle force and IEMG was recorded. The muscular activity in the operated leg was found to be reduced for the extension muscles (8-20%) and for the flexion muscles (28-52%) when comparing to the normal leg. Knee extension of the operated leg resulted from the function of the rectus femoris muscle. During normal exercise in daily life all patients were not handicapped und some of them did sport.
