Effect of thiol chelators on trace metal levels.

Influence of 2,3 dimercaptosuccinic acid, diethyldithiocarbamate and DL-penicillamine on the levels of essential metals in liver, kidney and serum was investigated. The hepatic zinc increased while renal copper decreased significantly upon treatment with these commonly used thiol metal chelators. On the other hand, hepatic copper decreased on treatment with DL-penicillamine. The renal zinc and serum copper and zinc remained practically unaffected on treatment with these metal chelators.

A new imaging agent for medullary carcinoma of the thyroid.

Thyroid scintigraphy, using 99mTc(V) dimercaptosuccinic acid, was performed in four patients with pathologically confirmed medullary thyroid carcinoma and elevated serum calcitonin values. Significant uptake of the tracer was found in the clinically palpable cervical tumor masses, metastatic sites, and residual tumor. This finding, probably specific for medullary thyroid carcinoma, could be of great use in the diagnosis and the surgical follow-up.

[Scintigraphy in chronic micturition disorders].

Partial functions of the kidney, i.e. renal blood flow, GFR etc, can be estimated with radionuclide techniques as safe and non-invasive methods. Many methods have been published to distinguish between obstructive and non-obstructive hydronephrosis. A simple and highly relevant technique is the diuretic radionuclide urography which should be done in all patients with questionable urographic or sonographic findings. The evaluation of split renal function is somewhat difficult because tissue absorption will influence uptake determination. In hydronephrosis this absorption cannot be predicted by sonographic measurement of the kidney's depth and mass. A solutions is to calculate the absorption in these patients by stepwise scintigraphy. Continuous quality control of the technique is emphasized.

[Quantification of separate renal function using Tc 99m DTPA and Tc 99m DMSA. Correlations between individual isotopic data and creatinine clearance].

The purpose of this prospective study (30 controls and 100 renal patients) is to compare one to each other the values of renal function measured with: 1--the DTPA 99mTc , 2--the DMSA 99mTc , and 3--the creatinine clearance. After the control values being established, correlations are done for the whole group (n = 130) between the creatinine clearance, the DTPA 99mTc clearance and the fixation rate of the DMSA 99mTc . The correlations obtained are:--Clcr Versus Cldtpa = 0,91.--Clcr Versus % Dmsa = 0,90. - Cldtpa Versus % Dmsa = 0,93. and indicate the two isotopic tests performed give similar quantitative results than creatinine clearance. Camera and computer allow to measure the separate renal function one by one, the two isotopic test giving similar values for each kidney. The results of these studies give a good concordance with the observed diseases, except for partial renal artery stenosis. The pyelic retention of DMSA 99mTc product, in condition of ureteral compression, gives a potential overestimation of this test and is a matter to debate. The extreme simplicity and facility of DMSA 99mTc procedures make this investigation accessible to any patient and give excellent morphological and functional tests. DTPA 99mTc clearances--global and separate kidney--give a more sensitive approach than DMSA 99mTc does because vascular captation , elimination rates could be focused. DTPA 99mTc procedure is to be used in patients with transplanted kidney, silent IVP kidney, contrast media contraindication or for exploring glomerular clearance without urine collection.

[Clinical study concerning upper urinary tract obstruction. 2. Relationship between 99mTc-DMSA uptake in obstructed kidneys and their functions].

To establish a method of detecting divided renal function of the obstructed kidneys noninvasively, comparative studies on 99mTc -DMSA uptake (60 minutes after injection), I 131-hippuran renal uptake (1-2 minutes after injection) and divided creatinine clearance immediately after relief of urinary obstruction were made on 24 cases of obstructive uropathy. A close significant correlation was obtained between 99mTc -DMSA and I 131-hippuran renal uptakes (gamma = 0.8809, P less than 0.001). Also, a significant correlation was noted between 99mTc -DMSA renal uptake and divided creatinine clearance (gamma = 0.7876, P less than 0.001), but the latter constantly gave lower values than the former. These observations led us to conclude that 99mTc -DMSA renal uptake is a reliable indicator of the renal plasma flow in the obstructed kidneys. Quantitative divided renal function of the obstructed kidneys can be estimated by the 99mTc -DMSA uptake method.

[Extracorporeal shockwave lithotripsy in children. Study of the effectiveness and renal consequences in a series of eighteen children].

17 children from 18 months to 16 years old were treated by ESWL for calculi of the upper urinary tract using a Sonolith 3000 lithotriptor. 31 calculi (5 to 60 mm) have been treated on 16 renal units and 3 pelvic ureters. The renal parenchyma of all treated kidneys was considered normal on preoperative ultrasound and IVU; 8 patients had a preoperative DMSA. The treatment required 1 to 4 sessions delivering 400 to 3000 shocks per session and 400 to 6000 shocks per treatment at 14 kV. Within three months post treatment, 14 patients (88.23%) were stone free and 3 patients had residual fragments (2 needed further surgical extraction). With a follow up of three months to five years, all patients have a normal blood pressure, serum creatinine and sterile urine. In no instance, IVU or ultrasound revealed a lesion imputable to ESWL. Seven patients had a DMSA six months or more after the last session; 4 of these were normal but the three other patients had a loss of function and an heterogeneous fixation on the treated side. These preliminary results confirm that ESWL is efficient for upper urinary tract calculi destruction in children. However ESWL seems to be susceptible to induce parenchymal damage, detected by DMSA, whose incidence and long term effects particularly on blood pressure need further evaluation.

The effects of some drugs--pentamidine, stibocaptate, Hoechst 33258, F 151, compound 'E' and Nifurtimox--on Onchocerca volvulus in chimpanzees.

Several drugs were investigated for action against Onchocerca volvulus in chimpanzees. Pentamidine at 4 mg/kg daily for 16 days had no filaricidal action; nor did stibocaptate at 10 mg/kg monthly for 8 months. Hoechst 33258, a substituted bis-benzamidazole, showed micro- and macrofilaricidal activity; F151 (an arsenical compound) showed a purely macrofilaricidal action; and Compound E (a reaction product of Hoechst 33258 and F151) was micro- and macrofilaricidal. The toxicity of the three last-mentioned compounds was high, and further trials were not made. Nifurtimox, at doses of up to 40 mg/kg for 10 days, was shown not be microfilaricidal, but there was a suggestion that this drug may have some macrofilaricidal action.

[A clinical analysis of 117 cases of acute arsenic poisoning].

117 cases of acute arsenic poisoning, caused by ingestion of food contaminated by As2O2, presented with abdominal pain, vomiting, nausea and diarrhea. The average level of urinary arsenic was 3.926 mg/L. The incidence of neuritis, poisoning hepatopathy and abdominal ECG was respectively 7.7%, 32.54%, 35.9%. All the cases recovered after oral or parenteral administration of dimecapto succinate (DMS) in six weeks. DMS is the drug of choice in the treatment of arsenic poisoning.

[Tubulointerstitial nephropathy--radionuclide imaging diagnosis].

There are many spectrums in renal diseases which induce tubulointerstitial nephropathy (TIN). Primarily, the renal tubule and interstitium are involved but glomerular and vascular lesions are found in the chronic course of TIN. Renal scintigraphy is poor for evaluating characteristic aspects of tubular function of TIN but demonstrate individual renal function as a whole. Particularly, measurement of the 99m-Tc-DMSA renal uptake rate is useful for evaluating a functioning tubular mass.

Short-term efficacy of oral dimercaptosuccinic acid in children with low to moderate lead intoxication.

OBJECTIVE: To determine the short-term efficacy of meso-2,3-dimercaptosuccinic acid (DMSA) in mild to moderately lead poisoned children. METHODS: Medical records of all pediatric patients receiving 19 days of DMSA between June 1991 and May 1993 were reviewed retrospectively. Patients were included if their pretreatment blood lead concentration (BPb) was 1.21 to 2.36 mumol/L (25 to 49 micrograms/dL) and excluded if they: received DMSA through participation in a pharmaceutical company-sponsored drug study; underwent chelation therapy in the previous 28 days; or received another chelating agent concomitantly with DMSA; or if noncompliance was documented. Homes were inspected and abated of major hazards before chelation therapy. BPb and blood zinc protoporphyrin concentration (ZnP) were obtained at baseline. DMSA was administered in a dose approximating 10 mg/kg per dose every 8 hours for 5 days, followed by 10 mg/kg per dose every 12 hours for 14 days. Baseline laboratory studies were repeated weekly while the patients were receiving therapy and for 2 weeks after therapy, then monthly unless chelated again. RESULTS: Of the 46 children who were treated with DMSA, 18 were excluded from the analysis. In the remaining 28 children, the mean +/- SD pretreatment BPb and ZnP were 1.79 +/- 0.33 mumol/L (37 +/- 6.9 micrograms/dL) and 1.26 +/- 0.64 mumol/L (71 +/- 36.1 micrograms/dL), respectively. The percent reduction (mean +/- SD) in BPb compared with baseline was -43% +/- 20.8%, -26% +/- 16.9%, and -31% +/- 20.2% on mean days 18, 30, and 80, respectively, whereas the changes in ZnP were -12% +/- 21.7%, -20% +/- 18.1%, and -31% +/- 21.9%, respectively. Eighty percent of patients had 20% or more reduction in their pretreatment BPb and/or ZnP after completion of DMSA therapy (95% confidence interval, 61, 92%). No significant adverse effects were observed except for neutropenia (absolute neutrophil count of 0.752 x 10(9)/L) in one patient. CONCLUSION: Our findings support the short-term efficacy of DMSA in children with BPb of 2.36 mumol/L (49 micrograms/dL) or less.

Chelation of organoarsenate with dimercaptosuccinic acid.

Alkane arsenate herbicides are available commercially, and their acute toxicity has been well documented in previous studies. Animal studies have indicated that dimercaptosuccinic acid (DMSA) can be used as an oral chelating agent. A 20-y-old white male cocaine addict attempted suicide by drinking approximately 500 ml of a 16% monosodium methanearsenate solution. He vomited 10 or more times and was admitted to the intensive care unit with impending shock and early liver and renal involvement. Four 5-day courses of 30 mg DMSA/kg/24 h were given. This brought the serum arsenic level from 2,871 micrograms/L to 6 micrograms/L, and his urine arsenic level from 78,920 micrograms/L to 21 micrograms/L in 30 d. Renal function tests returned to normal, with normal renal creatinine clearance, normal blood urea nitrogen and serum creatinine. However liver functions were abnormal, with elevation of serum transaminases, which later proved secondary to chronic hepatitis. No side effects of DMSA was encountered during the therapy. DMSA was successfully used to detoxify acute organoarsenate poisoning in a clinical setting, supporting experimental reports in the literature.
