Reconstruction of mandible with fibular flap and dental implant distractor: a new approach.

OBJECTIVE: To study the application of dental implant distractor (DID) in mandibular functional reconstruction. METHODS: We designed a new device named DID, which includes the permanent dental implant and the temporary distractor in itself. It is specially designed for fibula wider distraction in mandible reconstruction. Twenty-five sets of DID devices were put into 8 patients (6 men and 2 women) during operation. Two patients suffered from ameloblastoma of the mandible, 2 from odontogenic cyst of the mandible, 1 from fibrous dysplasia, and the other 3 from malignant tumor of the mandible. The age of 8 patients ranged from 19 to 67 (mean 46.8) years. RESULTS: During postoperative 2 - 15 months follow up, 7 patients were found to be successful. The clinical examination and X-ray film showed the normal shape of the mandible and the osteointegration of the implants were solid enough to withstand the denture force. Others had the DID removed because of inflammation. Two of the patients successfully worn the fixed dental prosthesis. The outcomes was satisfying. CONCLUSION: The DID device specially designed for mandibular reconstruction with fibular flap can help to simplify convenient procedures to a single surgery.

A fixed prosthodontic technique for mandibular osseointegrated titanium implants.

A detailed technique has been presented for the fabrication of a fixed prosthesis supported by osseointegrated titanium implants. The fixed prosthodontic technique is based on the original Swedish procedure. Several new dental products have been added, and some refinements in technique have been made in an effort to consistently produce a fixed prosthesis that fits, functions, and satisfies today's esthetically conscious patient. Branemark's tissue-integrated prosthesis (TIP, Bofors Nobelpharma, Inc.) is the only dental implant system with two decades of high-quality longitudinal clinical experience. A fixed prosthesis supported by osseointegrated implants offers a treatment modality that is predictably successful (Fig. 21).

Influence of the prosthetic restoration modality on bone loss around dental implants placed in vascularized iliac bone grafts for mandibular reconstruction.

OBJECTIVES: Is bone loss associated with implants placed in vascularized iliac crest bone grafts for mandibular reconstruction dependent on the prosthetic restoration modalities used? STUDY DESIGN: Eleven vascularized iliac crest bone grafts for mandibular reconstruction were examined after ablative tumor surgery. Forty implants loaded for at least 1 year were observed for up to 6 years. Horizontal and peri-implant bone loss was assessed for 2 groups: 6 patients with implant-supported bridges and 5 patients with implant-retained overdentures. RESULTS: Horizontal bone loss reached a steady state around 2 mm after an observation time of 2 years. Bony pockets had a depth of less than 1 mm. The described pattern was nearly independent of the prosthetic restoration modality used. The implant survival rate was 97.6%. CONCLUSIONS: Vascularized iliac crest bone grafts are safe implant-bearing areas. Implant-retained overdentures can be used with as good a longterm efficacy as the more complicated implant-supported bridges.

Case report: aesthetic management of a localised periodontal defect with a gingival veneer prosthesis.

Traditionally acrylic resin gingival veneer prostheses have been used to disguise aesthetic deficiencies of maxillary anterior teeth following successful periodontal therapy and have been retained by engaging horizontal undercuts distal to the canine teeth. They are, however, versatile prostheses with uses in fixed and removable prosthodontics and therapeutic treatment of gingival conditions. In the case presented a small acrylic resin gingival veneer prosthesis retained by a resilient lining material was used to manage a localised periodontal defect of the mandibular central incisor teeth.

[Follow-up of 5 cases of mandibular adamantinoma treated by osteotomy and mixed grafts].

The examination after fifteen years of five cases of mandibular adamantinoma treated with an operation performed in two stages (graft of metallic prosthesis and osseous autograft) has confirmed the success of this surgical method. In particular from the clinical and radiological examination it turns out that the essential requisites for a therapeutical success have been respected: the anatomical restoration and the aesthetical and function rehabilitation.

Vascularized free bone grafts for maxillary and mandibular reconstruction.

Vascularized osteocutaneous free grafts have been successfully applied to maxillofacial reconstruction. Careful planning allows for implant prosthesis fabrication in these cases. The scapula, fibula, and ilium are the donor sites that result in the most favorable application of implant prosthetics. Intraoral soft tissues, although compromised, are manageable with meticulous home oral hygiene practices and frequent recall examinations with professional cleaning. Many patients severely impaired by trauma, neoplasm, or congenital deformity can have restoration of form and function with the amalgamation of microsurgical and osteointegration techniques.

Effect of edentulousness on mandibular size and condyle-fossa position.

Modern imaging methods make possible the more precise examination of the complicated bony structure of the temporomandibular joint (TMJ) and the performance of cephalometric analysis. The aim of our study was to analyse the effect of edentulousness on the mandibular size and the condyle-fossa position using roentgencephalograms and axial computed tomography (CT) scans. The study group consisted of 20 edentulous patients (14 women, six men, mean age 60 years) whose mean period of edentulousness was 20 years (range 3-34 years). A CT examination of their TMJs was performed and roentgen-cephalograms in 16 of this group were taken after prosthetic treatment. Sixteen dental students were chosen according to sex as controls. Earlier CT scans of 49 dentate subjects of both sexes were used as controls for the analysis of bicondylar asymmetry. The position of the glenoid fossa was more anterior in edentulous subjects than in dentate ones and its anterior position correlated significantly (P < 0.02) with the period of edentulousness, a finding which has not been confirmed before. It can be concluded that the fossa is a remodelling unit as a part of the functional entity when the function is altered dramatically as in the case of edentulous patients.

Rehabilitation of a patient with severe dentoalveolar injuries: a case report with a 10-year follow-up.

This clinical report describes the emotional and physical rehabilitation of a young man. The impact of the injuries sustained and repeated failure of traditional dental treatment methods had caused the patient to become quite withdrawn. A successful outcome followed surgical placement of multiple titanium plasma-sprayed cylindrical fixtures in severely damaged dental supporting tissues to serve as intermediary abutments for complex maxillary and mandibular fixed prostheses. The loss of crestal bone during the postprosthetic years is determined. The advantages only implant dentistry could bring are identified.

Functional reconstruction in a case of osteoblastoma of the mandibular condyle.

Resection of the mandibular condyle inevitably leads to functional impairment even in cases of reconstruction with prosthesis or autogenous bone graft. Loss of function is caused by the loss of action of the lateral pterygoid muscle. Therefore, preservation and fixation of this muscle to a transplant should be performed whenever possible. A case of osteoblastoma of the condylar head and neck with functional reconstruction after ablative surgery is presented.

Immediate loading with mini dental implants in the fully edentulous mandible.

The use of immediate loaded prostheses is not recommended for at least 2 weeks to prevent implants from premature loading. In addition, immediate removable prostheses are negated for at least 4 weeks at the site of guided bone regeneration or bone graft. However, patients are often not pleased with limited diets and the unaesthetic appearance during the healing period without a denture between implant placement and final prosthesis. Mini dental implants provide stable and esthetic temporary prostheses immediately after implant placement and bone grafts. The use of mini dental implants is simple and cost effective because the patient's old denture is used as a provisional prosthesis. This article shows the success of 27 mini dental implants that were placed for 11 mandibular fully edentulous patients.

Speech and other oral functions. Clinical and experimental studies with special reference to maxillary rehabilitation on osseointegrated implants.

The aims of this thesis were first to longitudinally investigate patients' opinions of their speech and other oral functions, evaluate some parameters of oral functions and speech production after treatment with a maxillary fixed prosthesis on osseointegrated implants (FPOI) and secondly to analyse normal Swedish [s] production. 21 consecutive selected patients rehabilitated with maxillary FPOI were studied over a 3-year period. The parameters of masticatory functions were clenching force, chewing efficiency, oral stereognosis and oral motor ability. With the aid of perceptual, acoustic, and model analysis the speech production with special reference to [s] was also longitudinally evaluated. The occlusal thickness perception (OTP) was estimated. In order to evaluate the usefulness of electropalatographic (EPG) and optoelectronic equipments in analysing speech production a multiple analysis of [s] production in varying contexts were made at four sessions. All subjects were very pleased with the oral rehabilitation. The chewing efficiency over time improved. The replacement of a complete denture with an FPOI in the upper jaw, resulted in to a progressive increase of chewing efficiency and clenching force, and a subjective improvement of oral function. Concerning OTP, a threshold level of 20 microns was recorded in the subjects with natural dentition, at 50 microns in FPOI subjects and 100 microns subjects with complete dentures. Time for identification of small test pieces as an expression of oral stereognostic ability (OSA) as well as number of errors decreased immediately after insertion of an FPOI compared to the edentulous condition. Assembly time in oral motor ability test (OMA) also decreased. A further improvement was found at the final examination 3-6 months after rehabilitation. After initial phonetic problems, 92% of the patients considered themselves free from speech problems at the 3 year follow-up. The patients' experience corresponded well with the judgements of both the expert and the non expert groups. Difficulties in pronouncing [s] were associated with decreased bite-force, number of occluding contacts, frontal width of the fixed prosthesis and tenderness at palpation of the masticatory muscles. No correlation between open or closed interdental spaces and deteriorated speech was found. In general, EPG and mandibular movement pattern were similar between sessions and the sound quality normal. However, when subjects wore the artificial palate for the first time the [s] groove was wide, the mandibular movement smaller and the [s] quality some what deteriorated. [s] in varying context had different tongue groove width but a similar high average mandibular position.

Giant cell tumor in the mandible: surgical treatment and implant-supported reconstruction--a clinical report.

Giant cell tumors of the jaw are extremely rare. The malignant variants of these tumors are distinguished by their rapid growth, pain, or tumor paraesthesia. The lesions generally manifest as multilocular radiolucencies of bone with well differentiated margins. If not diagnosed and treated, the lesions may resorb the alveolar ridge and displace the roots of the teeth. The learning objective of this article is to present the diagnosis of the lesion, mandibular resection, bone graft augmentation of the surgical site, implant placement, and restoration of oral function by implant-supported prosthesis. Multidisciplinary approach and cooperation are credited for the successful final result.

[Case in which periodontal surgery was performed and prostheses were designed to match condylar movement].

PATIENT: A patient presented with malocclusion caused by generalized periodontal disease, subgingival caries, and poorly designed restorations and prostheses. Occlusal reconstruction was carried out in this case according to occlusal guidance that was determined based on mandibular head movement, following periodontal surgery. The emergence profile of the prostheses was made to match the periodontium. DISCUSSION: By achieving an occlusal form, where the maxillary and mandibular molar teeth could be instantly separated through proper anterior guidance corresponding to the internal structure of the mandibular joint, it was possible to attain long-term stability while maintaining harmony with the temporomandibular joint without damaging the periodontium. CONCLUSION: Occlusal guidance corresponding to the internal structure of the temporomandibular joint is important for maintaining the stability of the periodontium, teeth and temporomandibular joint.

[Titanium magnets on implants as an aid in rehabilitation after tumor treatment].

After the resection of a tumor, many patients need a reconstruction with hard and soft tissue and also with sufficient dentures. Often, only little space is available for the implantation. Implant length and diameter have to be reduced, and the result is a change in the biomechanics with a possible mechanical overloading of the implant. We examined 52 tumor patients undergoing reconstruction with 189 implants. A new concept involving attachment with the help of magnets is presented, offering a satisfactory solution in these difficult cases after tumor resection and reconstruction.

A modified method of immediate loading using Brånemark implants in edentulous mandibles.

According to the original protocol, Brånemark implants require a two-step surgical procedure in order to become osseointegrated. The two-stage surgery is sometimes uncomfortable for the patient, and recent research has shown that osseointegration can also be achieved with a one-stage technique with early or immediate loading of the implants in good quality bone, which simplifies and shortens treatment for the patient's benefit. The aim of this study was to evaluate the immediate loading of Brånemark implants with a screw-retained prosthesis in edentulous mandibles. Forty-three patients each had three Brånemark implants of varying lengths and diameters installed in the inter-foramina mandibular area. The implant installation procedures were performed according to the standard protocol with avoidance of over-preparation of the implant sites. The prosthesis framework was prefabricated in advance and was cast and separated into three parts, followed by adjustment in the mouth, with a soldering, impression made in situ; finally, the framework was sent to the laboratory for completion of the bridge. The fixed implant bridge was usually delivered to the patient later the same day. Three of the 129 implants failed to osseointegrate, yielding a success rate of 97.6%, with a follow-up period ranging from 3 to 49 months. The results obtained in this study were encouraging, and thus far this modified method of immediate loading in the anterior edentulous mandible appears to be predictable.

Bone tissue engineering using polycaprolactone scaffolds fabricated via selective laser sintering.

Polycaprolactone (PCL) is a bioresorbable polymer with potential applications for bone and cartilage repair. In this work, porous PCL scaffolds were computationally designed and then fabricated via selective laser sintering (SLS), a rapid prototyping technique. The microstructure and mechanical properties of the fabricated scaffolds were assessed and compared to the designed porous architectures and computationally predicted properties. Scaffolds were then seeded with bone morphogenetic protein-7 (BMP-7) transduced fibroblasts and implanted subcutaneously to evaluate biological properties and to demonstrate tissue in-growth. The work done illustrates the ability to design and fabricate PCL scaffolds with porous architecture that have sufficient mechanical properties for bone tissue engineering applications using SLS. Compressive modulus and yield strength values ranged from 52 to 67 MPa and 2.0 to 3.2 Mpa, respectively, lying within the lower range of properties reported for human trabecular bone. Finite element analysis (FEA) results showed that mechanical properties of scaffold designs and of fabricated scaffolds can be computationally predicted. Histological evaluation and micro-computed tomography (microCT) analysis of implanted scaffolds showed that bone can be generated in vivo. Finally, to demonstrate the clinical application of this technology, we designed and fabricated a prototype mandibular condyle scaffold based on an actual pig condyle. The integration of scaffold computational design and free-form fabrication techniques presented here could prove highly useful for the construction of scaffolds that have anatomy specific exterior architecture derived from patient CT or MRI data and an interior porous architecture derived from computational design optimization.

The mandibular complete overdenture.

Mandibular complete overdenture treatment has been available for decades, but its use was limited when the treatment relied on retained teeth as overdenture abutments. This treatment, however, is currently experiencing more popularity than ever before. In fact, dentistry may be experiencing a philosophical shift, in which mandibular implant overdenture treatment may become the new standard of care for the treatment of the edentulous mandible. Practitioners are looking for simplified treatments that can provide cost-effective alternatives to more complex implant prosthodontic procedures. Implant overdentures provide a strong return for the investment in treatment time and expense and are a treatment suited to the lower socioeconomic status of many edentulous patients. The clinical outcome of this treatment is significantly better than that achieved with conventional mandibular dentures, especially when patients are experiencing technical problems because of compromised prosthesis retention or stability.
