Active area-covering action to prevent possible complications by early selection of potentially endangered women from among the entire female population of the area concerned, which should, desirably, be under fullest possible medical observation, this has come to be a major characteristic of modern obstetric medicine in the "most developed countries". Appropriate methods have to be devised for high-continuity optimisation of all organisational facilities herefore required. In this paper, methodical approaches are submitted for discussion, as a result of a comprehensive long-term analysis of patients in a big hospital. "Criteria of optimum" are covered in this first communication. Regional weight class rates, weight-class-standardised mortality and early morbidity (5' Apgar score) are proposed as criteria of universal applicability.
In medical attention to patients, achievement and risk are in close correlation. Coherent definition of the term of "risk", therefore, is a primary demand which must be met for systematic optimisation of systems for perinatal care. The definition introduced in this paper will enable critical appraisal of undifferentiated risk control, calculation of an upper limit value of theoretical effectiveness, and assessment of effectiveness in terms of health policy. It is also an uninvolved approach to reduction of characteristics. The suggestions made are backed up by a large bulk of results.

Our intention was to develop a high accuracy procedure in high dose rate intracavitary remote afterloading therapy of carcinoma of the uterine cervix similar to the accurate dosimetry used in external irradiation. Semi-conductor in-vivo-dosimetry was carried out in order to determine the extent of errors in the calculated dose at reference points and in the definition of point A. The use of a rapid processing computerized system before the treatment was developed in order to decrease the difference between the dose delivered at point A dose and the planned dose. Errors in calculation of within +/- 10% seemed to be unavoidable in the hgh dose rate intracavitary afterloading therapy of carcinoma of the uterine cervix. Two definitions should be used in patients to compute the point A dose according to the ovoid position. Using the rapid processing system, wo could deliver a point A dose within +/- 5% level of the planned dose. The remotely controlled afterloader was safely operated by means of in-vivo-dosimetry which was backed up in routine clinical practice by a pretreatment calculated dose.

We present a new class of autoantibodies--ANCA (antineutrophil cytoplasmic autoantibodies) which recognize as target antigen different enzyme constituents of primary granules of neutrophil granulocytes. These autoantibodies are present in a large number of diseases, such as: systemic vasculitides, idiopathic crescentic glomerulonephritis, inflammatory bowel diseases and in some other conditions, including rheumatoid arthritis, SLE, Felty syndrome, acute and chronic infections. ANCA not only facilitate the diagnosis but also have a pathophysiological role for some of the idiopathic vasculitides. In our study, including 110 patients referred to the laboratory for ANCA testing by indirect immunofluorescence technique, only 25 patients were positive with a diffuse cytoplasmic (c ANCA) or perinuclear (p-ANCA) pattern on alcohol fixed slides. Some relevant cases are presented in order to emphasize that ANCA antibodies are a useful marker in the early diagnosis of systemic vasculitides, allowing effective therapeutic handling.

OBJECTIVE: This research used a study-control group design and examined data collected from five hospitals to evaluate the effectiveness of retrospective peer-review systems on reducing utilization of red blood cells (RBCs). DESIGN: The effects of retrospective peer-review systems were studied in three parts: (1) trends of RBC utilization were compared by the slopes of linear regression lines that assessed the effect of time on RBC utilization among four study hospitals and one control hospital, (2) diagnosis-specific RBC utilization was compared between the control hospital and one matched study hospital, and (3) the effect of the retrospective review system of one study hospital was assessed by linear regression using data accumulated 1 year before and 2 years after implementation of the program. RESULTS: Three study hospitals showed no significant changes in RBC utilization during the 10-month study period. One study hospital and the control hospital demonstrated trends of reduced RBC use with negative slopes of regression lines; however, there was no difference in the degree of the two slopes, and the diagnosis-specific RBC utilization was not lower at the study hospital than at the control hospital. Furthermore, implementation of the retrospective peer-review system at one study hospital demonstrated no effect on RBC utilization. CONCLUSIONS: We conclude that the retrospective peer-review systems implemented at these four hospitals had no effect on reducing red blood cell utilization.
