Summarizing the logistics of liberation from mechanical ventilation.

A goal of critical care clinicians should be to liberate patients from ventilators on the first possible day to avoid the multitude of complications associated with prolonged mechanical ventilation. The past decade has been marked by a deluge of scientific studies that have illuminated methods to expedite successful liberation from mechanical ventilation. This article summarizes the principle themes for this issue of Respiratory Care Clinics of North America and distills the available data to two simple weaning algorithms.

Predicting contraceptive use in Bangladesh: a logistic regression analysis.

Family planning knowledge, attitude and practice surveys typically assess respondents' reproductive attitudes and intentions to use contraception. Longitudinal observation of individual respondents nevertheless shows that such questions are not strongly predictive of subsequent behaviour. This study examines 3 years' data which show that a set of such responses to questions are nevertheless substantially superior in predicting behaviour than any single indicator. Thus statistical techniques which bring into account the apparent multidimensionality of contraceptive motivation can greatly improve upon the estimation of future practice of family planning in a population.

The logistics of broader pre-clinical evaluation of potential anti-cancer agents with reference to anti-tumour activity and toxicity of mitozolomide.

Anti-tumour responses with CCRG81010, M & B 39565, NSC 353451, 8-carbamoyl-3-(2-chloroethyl)imidazo [5,1-d]-1,2,3,5-tetrazin-4(3H)-one (Mitozolomide) in a panel of 4 murine colon tumours of varying growth characteristics and chemosensitivity and a spontaneous murine lymphoma are similar to those seen with standard nitrosoureas. The moderately well differentiated colon adenocarcinoma MAC 16 is nonresponsive to mitozolomide and methylCCNU. Responses in the other 4 lines studied are only achieved near to maximum tolerated dose and at this level there is severe host toxicity. Haemopoietic toxicity is clearly demonstrated by analysis of peripheral blood counts and by CFU-S assays and severe testicular and ovarian toxicity was also seen at dose levels necessary to achieve anti-tumour effects. Using mitozolomide as an example, the study has demonstrated the feasibility of conducting simple but thorough toxicity evaluation for the determination of the therapeutic index. This approach would provide invaluable guidelines for the selection for clinical trial of the most appropriate members of a series of new cytotoxic compounds.

Logistics of platelet production: platelet concentrate from one-day-old blood stored at 10 degrees C.

Platelet concentrates were prepared from blood stored at 18 to 20 hours at either 10 +/- 2 degrees C or 16 +/- 2 degrees C. Platelets prepared from blood stored at 10 degrees C had good in vitro properties. The results were not distinguishable from those obtained with freshly-isolated platelets. The in vivo effectiveness of platelets prepared in this manner must be established.

Logistics and quality control for DNA sampling in large multicenter studies.

To study associations between genetic variation and disease, large bio-banks need to be created in multicenter studies. Therefore, we studied the effects of storage time and temperature on DNA quality and quantity in a simulation experiment with storage up to 28 days frozen, at 4 degrees C and at room temperature. In the simulation experiment, the conditions did not influence the amount or quality of DNA to an unsatisfactory level. However, the amount of extracted DNA was decreased in frozen samples and in samples that were stored for > 7 days at room temperature. In a sample of patients from 24 countries of the EUROPA trial obtained by mail with transport times up to 1 month DNA yield and quality were adequate. From these results we conclude that transport of non-frozen blood by ordinary mail is usable and practical for DNA isolation for polymerase chain reaction in clinical and epidemiological studies.

Logistics of radioimmunotherapy with yttrium 90 ibritumomab tiuxetan (Zevalin).

Radioimmunotherapy is a promising new therapeutic option for the treatment of B-cell non-Hodgkin's lymphoma. Several monoclonal antibody and radionuclide conjugates, including I-131 tositumomab and Y-90 ibritumomab, have been investigated in clinical trials. Yttrium 90 ibritumomab tiuxetan (Zevalin; Biogen Idec Inc, Cambridge, MA) is indicated for the treatment of patients with relapsed or refractory low-grade, follicular, or transformed B-cell non-Hodgkin's lymphoma, including patients with rituximab-refractory follicular non-Hodgkin's lymphoma. The ibritumomab tiuxetan regimen requires coordination of a multidisciplinary team on which the nuclear medicine physician (or radiation oncologist) plays a major role. The nuclear medicine physician (or radiation oncologist) is responsible for overseeing the administration of the radioimmunoconjugates and for interpreting the imaging scans to determine the expected or altered biodistribution of the radioimmunoconjugate.
