Superior dislocation of the fibular head associated with a tibial shaft fracture.

Superior dislocation of the proximal tibiofibular joint occurred in a 34-year-old woman who sustained a Grade 3B open left tibial fracture. An external fixator was applied to the tibia, and the tibiofibular joint was treated by closed reduction. Two bone graftings were performed, and the patient ultimately required a fibular osteotomy to allow tibial impaction. At 10 months posttrauma, the patient is fully weight-bearing and has no pain either at the tibia or the tibiofibular joint.

[Management of recurrent patellar dislocation by semitendinous muscle transfer].

After review of the pertinent literature authors report on a new tendon transfer technique not having been found in the available literature. The method has been considered suitable for the treatment of patellar dislocations due to erroneous stretching direction occurring before the end of growth. The semitendinous muscle was separated and transfosed in the tunnel drilled into the patella. By means of this technique the function of the muscle was used against the lateralization tendency of the patella. The paper presents the follow-up of 9 cases.

[Lower extremity nerves injuries in bone fractures and joint dislocations--etiopathogenesis, treatment].

Lower extremity nerve injuries accompanying bone fractures and joint dislocations lead to serious disturbances at the lower limbs and caused significant prolongation of the treatment time. Clinical material consisted of 73 patients treated surgically with lower extremity nerve injuries due to bone fractures and joint dislocation. Based on this material we analysed the most frequent localisations of the nerves lesions and the mechanism of injury. The indications to surgical treatment were also described.

[MRI evaluation of the patellar articular cartilage in patients with subluxation of the patella].

In patients with subluxation of the patella, injury of the patellar articular cartilage is frequently observed and correct evaluation is important to manage these patients. We examined 11 patients with subluxation of the patella and five normal volunteers. In 12 patellofemoral joints of seven patients with subluxation of the patella, the abnormalities observed on MRI were compared with those on arthroscopy and/or at operation. MRI was performed with a Magnetom 1.5 T (Siemens) using the round surface coil. Pulse sequences were SE (TR 400 ms/TE 19 ms), FLASH (TR 320 ms/TE 15 ms FA 90 degrees and 40 degrees), and SE (TR 2000 ms/TE 26, 70 ms). We analysed MR findings of the 12 abnormal joints and 10 normal joints according to the following classification of abnormalities observed on arthroscopy. (1) normal appearance (n = 3 joints), (2) softening and fibrillation (n = 6), (3) fragmentation (n = 3), and (4) erosion to bone (n = 0). In only one of the six cases with softening and fibrillation observed on arthroscopy, MRI could visualize the thickening of patellar articular cartilage, but in all three cases with fragmentation observed on arthroscopy, MRI could visualize the thin inhomogenous cartilage with irregular surface. The combination of SE (TR 400 ms/TE 19 ms) and FLASH (TR 320 ms/TE 15 ms FA 90 degrees) are extremely effective pulse sequence to detect the abnormalities of patellar articular cartilage. We conclude that MRI is a useful noninvasive method of detecting advanced changes in patellar articular cartilage.

Patellofemoral pain in children.

The primary inadequacy in the treatment of patellofemoral pain in children has been the inability to translate distinct problems recognized during physical examination into a specific clinical classification. Using a classification system based on etiology rather than symptoms allows the clinician to proceed with treatment based on a specific diagnosis. The majority of patellofemoral pain in children is caused by trauma and malalignment syndromes (or a combination of both) and can usually be managed successfully with nonoperative methods. Success in a rehabilitation program depends on a precise understanding of the original or underlying cause of the child's patellofemoral pain.

[Comparative evaluation of various surgical technics in the treatment of dislocated patellar fractures].

From the point of view of the different operative methods 137 patellar fractures operated on in the 3 years' material (1969--1971) of the Central Casualty Rehabilitation Institute have been analysed by the authors. 116 fractures could be re-examined 2--4 years after patients were back at work. If the operation was followed by adequate functional management, satisfactory results were found after all operations. When the patients were back at work, further improvement of the results has been observed. In the time the patients got again to work, the results of partial patellectomy seemed better,--however, after 2--4 years osteosynthesis yielded better final results. Traction loop proved more effective than cerclage. If possible, conservation and exact unionof the patella is of great importance in both juveniles and manual workers. Early functional treatment improves the late results of the operation.

[Displacement of the tibial tuberosity in retropatellar cartilage damage and recurrent patellar dislocation].

In our hospital in the years 1980 till 1986 37 displacements of the tuberositas tibiae were performed in chondropathia patellae and recurrent luxation of the patella. The follow-up examination took into consideration the radiological and clinical findings, the subjective complaints and the ability of going in for sports. These clinical experiences and pressure measurements in the femoro-patellar joint show the best results after the displacement of the tuberositas tibiae according to Blauth. Additionally we recommend an accurate indication for chondropathia patellae and recurrent luxation of the patella and accompanying surgical measures.
