[Comparative evaluation of results of intertrochanteric femur osteotomy and conservative hip joint releaving treatment in Perthes-Legg-Calvé disease].

In this paper the comparative evaluation of the results of the surgical and conservative treatment applied in patients with Perthes-Legg-Calvé disease is given. The results obtained with intertrochanteric femur osteotomy performed in 39 patients and those obtained in 66 patients with conservative treatment are reported. The results are evaluated from the point of view of the patients' age, the localization of the process and the stages of the disease. The surgical treatment brought about in 1/3 of the cases excellent-in 1/3 good - and in 1/3 unsatisfactory results. Int the group of patients who received conservative treatment 1/3 of the cases showed excellent - 1/5 of the cases good - and 3/5 of the cases unsatisfactory results. The advantage of the surgicat treatment is that the patients are not confined to bed for long time and they must nol use walking splint releasing the hip-joint.

A double-blind comparative study of 150 mg flurbiprofen daily and 75 mg indomethacin daily in the treatment of osteoarthrosis of the hip joint.

Thirty patients with osteoarthrosis of the hip were studied in a double-blind crossover comparison of 150 mg flurbiprofen daily and 75 mg indomethacin daily, each drug being given for 2 weeks separated by a wash-out period of 1 week on placebo. Statistical analysis of the results from 26 patients with complete records showed that both drugs produced statistically significant improvements over baseline after 1 and 2 weeks in assessments of severity of pain, night pain, duration of morning stiffness, and intermalleolar straddle. No statistical differences, however, were found between the two drugs. There were no reports of side-effects with flurbiprofen, but 6 reports from 3 patients whilst receiving indomethacin.

Late prosthetic hip joint infection with Actinomyces israelii in an intravenous drug user: case report and literature review.

Late infections with Actinomyces israelii have been described for prosthetic hip joints but not in association with intravenous drug use. We present a case of a 43-year-old intravenous drug user who developed A. israelii infection in connection with a hip prosthesis 11 years after implantation, and we review four previously reported cases of Actinomyces prosthetic joint infections.

Determination of the femoral and pelvic geometrical parameters that are important for the hip joint contact stress: differences between female and male.

The difference between male and female femoral and pelvic geometry was studied by considering some geometrical parameters such as interhip distance, inclination of the crista iliaca, diameter of the femoral head and centre-edge angle of Wiberg. The values of these parameters were determined for 79 healthy female and 21 healthy male subjects. Standard anterior-posterior radiographs were used and processed by the computer-aided system. The results show some important sex differences in femoral and pelvic geometrical parameters which determine the hip joint contact stress.

Use of explicit criteria for total hip joint replacement fixation techniques.

We developed a tool to judge the appropriateness of indications and fixation mechanisms for total hip joint replacement (THJR) and applied it to a sample of patients. Criteria were developed using a modified Delphi panel judgment process, following the RAND methodology (RAM). We recruited, during 1 year, patients with a diagnosis of osteoarthritis undergoing THJR in five public hospitals. The appropriateness of the THJR intervention and the fixation mechanism was judged by the explicit criteria developed by a panel of experts. Of the 216 scenarios scored by the panel for the use of each of three fixation mechanisms, the cemented fixation was considered inappropriate in 69.5%, versus just 33.3% for the non cemented. Of those scenarios considered appropriate, the most appropriate mechanism of fixation was considered to be non cemented (74.4%), while cemented (17.8%) and hybrid (7.8%) scenarios were scored as appropriate less often. The previous explicit criteria were applied to 583 real patients. After evaluation of the interventions, 30% of the fixation mechanisms used were considered appropriate, while 21.8% inappropriate. Appropriate use of fixation mechanisms varied among hospitals. RAM can provide explicit criteria to help in clinical decision making and evaluating indications for a THJR intervention. Nevertheless, in the case of the appropriateness of fixation mechanisms, due to the lack of evidence, the panel criteria were biased towards the non cemented technique, which had important implications for the evaluation of some hospitals.

Elastohydrodynamic lubrication analysis of ultra-high molecular weight polyethylene hip joint replacements under squeeze-film motion.

Elastohydrodynamic lubrication was analysed under squeeze-film or normal approach motion for artificial hip joint replacements consisting of an ultra-high molecular weight polyethylene (UHMWPE) acetabular cup and a metallic or ceramic femoral head. A simple ball-in-socket configuration was adopted to represent the hip prosthesis for the lubrication analysis. Both the Reynolds equation and the elasticity equations were solved simultaneously for the lubricant film thickness and hydrodynamic pressure distribution as a function of the squeeze-film time was solved using the Newton-Raphson method. The elastic deformation of the UHMWPE cup was calculated by both the finite element method and a simple equation based upon the constrained column model. Good agreement of the predicted film thickness and pressure distribution was found between these two methods. A simple analytical method based upon the Grubin-Ertel-type approximation developed by Higginson in 1978 [1] was also applied to the present squeeze-film lubrication problem. The predicted squeeze-film thickness from this simple method was found to be remarkably close to that from the full numerical solution. The main design parameters were the femoral head radius, the radial clearance between the femoral head and the acetabular cup, and the thickness and elastic modulus for the UHMWPE cup; the effects of these parameters on the squeeze-film thickness generated in current hip prostheses were investigated.

Rearthroplasty of the hip joint.

Forty patients were reviewed 24 to 52 months after their second, third and fourth arthroplasty of the same hip. The protocol assessed pain, range of motion, leg shortening, gait performance, radiological appearance of the endoprosthesis and included the patient's own evaluation. Results of rearthroplasty were inferior to those of primary procedure, but still good enough, both in the surgeon's and the patient's opinion, to be the method of choice for revision of a failed total hip replacement.

[Indication and results of an alloarthroplastic hip joint replacement with the "crutch" endoprosthesis].

After discussing the relevant literature concerning indication, results and complications of total-hip-replacement by special prothesis of the upper end of the femur in cases of tumors or serious traumatic bone destructions of the intertrochanteric region of elder patients we representate our own 30 patients (from the years 1973 to 1982). Often, this special prothesis could be only a palliative regional solution in the cases of local removal of the tumor because of the advanced disease. But this treatment is important for temporary reconstruction; even in cases of aged patients with serious fragmented fractures of the pertrochanteric and subtrochanteric region we could reach an early remobilisation. This operation represents--in certain indications--an essential improvement of therapeutic facilities.

[Limits of prosthetic restoration of the hip joint and possibilities of recovery in failures].

The Authors report on treatment of patients with total prosthesis of the hip, in whom mobilisation of the prosthesis or one or both its components has occurred. In particular, stress is put on the results of the operation of removal of the prosthesis, which finds its major indications in cases of acute or remote, deep sepsis, and in case of aseptic mobilisation when the theoretical presuppositions allowing its substitution do not exist.

[Pre- and postoperative radiotherapy to prevent heterotopic ossification of the hip joint].

BACKGROUND: In-vivo experimental data indicate that both pre- and postoperative radiotherapy can prevent heterotopic ossification after hip surgery. This comparison was clinically tested in a randomized study. PATIENTS AND METHODS: From June 1992 to September 1993, 84 patients with high risk for the development of heterotopic ossification were randomized. The treatment concept consisted either of preoperative radiotherapy within four hours prior to surgery (arm A) or postoperative radiotherapy within 72 hours following hip surgery (arm B). Preoperative radiotherapy was given in one fraction of 7 Gy, while the postoperative radiotherapy was delivered in five fractions of each 3.5 Gy (total 17.5 Gy). All patient variables (age, sex, prior surgery) and predisposing risk factors were comparable in both treatment arms. For the radiological assessment of heterotopic ossification according to (Brooker-Score) X-rays of the pelvis or hip were evaluated which had been taken immediately pre- and postoperatively as well as at least six months following surgery and prophylactic irradiation. The functional hip status was evaluated pre- and postoperatively using the Harris-Score. Cases in which the Brooker- and/or Harris-Score worsened during the postoperative follow-up as compared to the pre- and immediate postoperative situations were considered as treatment failures. RESULTS: Of 44 patients with at least six months follow-up 41 (93%) experienced a successful prophylaxis. Two failures were observed in the preoperative and one in the postoperative group. The prophylactic efficacy was not influenced if the pre- or postoperative interval was longer than prescribed. All intra- and postoperative complications were comparable for both treatment groups. The mean interval to partial strain (50% body weight) of the operated hip was longer in the preoperative group (mean 19 +/- 27 days) as compared to the postoperative group (mean 8 +/- 13 days). With respect to full strain (100% body weight), the results were equal in both groups. The functional hip status decreased in two patients. Again the mean overall improvement in the postoperative group was larger (mean 42.7 +/- 17.1 points) as compared to the preoperative group (mean 34.3 +/- 13.7 points). CONCLUSIONS: Preoperative and postoperative radiotherapy have equal prophylactic efficacy to prevent heterotopic ossification following hip surgery. The main advantage of preoperative radiotherapy are the simple management of the patient, the reduction of possible complications associated with transport and positioning of the patient in the postoperative period as well as excellent acceptance of this treatment concept by patients, nurses and staff.

Anatomic basis of the transgluteal approach to the hip-joint by anterior hemimyotomy of the gluteus medius.

The authors present a study of the intrinsic anatomy of the gluteus medius m, and of its innervation through the caudal branch of the superior gluteal n. The existence of an intramuscular tendon in the thickness of the gluteus medius was constantly prooved in 40 muscles. The relations of the intrinsic fibrous structure of the muscle and its innervation were studied. The authors deduce from that the topography of a gluteus medius incision, with respect to a safety area towards its innervation, which leads to an exposure of the acetabulum that is satisfying and gives opportunities of a sound repair after the surgery of the hip joint through the transgluteal approach. They propose the "anterior hemimyotomy of the gluteus medius m" designation.
