Associations between low levels of serum estradiol, bone density, and fractures among elderly women: the study of osteoporotic fractures.

To evaluate the skeletal effects of endogenous serum estradiol, we measured bone mineral density (BMD) at the calcaneus and radius (single photon absorptiometry) and at the hip and spine (dual x-ray absorptiometry) in 274 women aged 65 yr or more who participated in the Study of Osteoporotic Fractures. Lateral radiographs of the thoracic and lumbar spine were also taken, and serum was assayed for estradiol. Those who had estradiol levels from 10-25 pg/mL had 4.9%, 9.6%, 7.3%, and 6.8% greater BMD at total hip, calcaneus, proximal radius, and spine than those with levels below 5 pg/mL. After multiple adjustments, BMD differences remained statistically significant and corresponded to about 0.4 SD. Vertebral deformities were less prevalent among women whose estradiol level exceeded 5 pg/mL; the multiple adjusted odds ratio was 0.4 (95% confidence interval, 0.2-0.8). We conclude that physiologically low estradiol has a salutary effect on the skeleton in elderly women, possibly by reducing skeletal remodeling.

Ankle fractures: supination-eversion fractures of stage IV. Primary and late results of operative and non-operative treatment.

Supination-eversion fractures of stage IV, 60 treated operatively and 29 non-operatively, were seen at follow-up after 3-10 years. This disclosed a statistically significant difference between the results of the two methods. Operation which attempted to re-establish the anatomical position was apparently the better method in the presence of more than minimal displacement or a suspicion of deltoid ligament rupture.

Isolated fracture of the lateral malleolus requires no treatment. 49 prospective cases of supination-eversion type II ankle fractures.

54 patients with isolated SE II fractures through the lateral malleolus, with 2 mm maximum dislocation, were treated with an elastic bandage and immediate weight bearing. 2 patients suffered dislocation but both should not, on reassessment, have been included in the study. 49 of the remaining patients were assessed after an average of 1.5 years. All but 4 patients had very minor, if any, symptoms. The average sick leave was 6.3 weeks and the patients were back to normal activity in about 4 months. This virtual nontreatment was safe and beneficial to both the patients and the health service.

[Inline skating--high fracture risk. Two of three injured are boys and young men. Wrist fractures are most common].

In-line skating injuries have increased in recent years. Hospital based data from Umeå concerning 135 persons injured in in line skating collisions were analyzed. The highest yearly incidence of injury was found in males 10-19 years of age, at 1.7 per 1,000 inhabitants; the corresponding figure for females was 0.5. Two-thirds of incidents were caused by falls due to balance problems without the influence of any "external factor" such as rough road surfaces. No collisions with motor vehicles or other road users were registered. Nearly half of the injuries were fractures or dislocation injuries, most frequently of the upper extremities. Non-minor head injuries were rare. Protective gear for wrist and elbow may have the potential to reduce these injuries.

[Treatment of open fractures before Lister and the management of the fatal leg fracture of Admiral Michiel Adriaensz de Ruyter, 1676].

On 22 April 1676, Admiral De Ruyter suffered a fatal open fracture of the right lower leg during the naval battle near the Etna. The fleet doctor, Jan Mannart, and the two ship's surgeons treated the wound with brandy and prescribed rest, but fatal gangrene developed a few days later. Before Lister first reported his technique of antiseptic treatment in 1876, the possibilities for the treatment of open wounds were limited to cauterisation and application of hot oil. Until the middle of the 19th century, amputation of the wounded limb was the most obvious solution. Why amputation was not performed in the case of De Ruyter is unknown. Actually, however, amputation would probably have increased his chances of survival only to a limited extent.

Percutaneous reduction and Kirschner-wire fixation of impacted intra-articular fractures and volar lip fractures of the proximal interphalangeal joint.

We retrospectively review the cases of 5 men with closed, impacted intra-articular fractures of the middle phalanx at the proximal interphalangeal joint and describe a new technique-involving percutaneous reduction and Kirschner-wire fixation-that minimizes surgical trauma, allows fixation for early range of motion, and provides results as good as, if not superior to, those obtained with existing techniques.

Experimental tibial fractures in rabbits simulating proximal tibial metaphyseal fractures in children.

The etiology of valgus deformity after a seemingly innocuous fracture of the proximal tibial metaphysis in children is unknown. The purpose of this work was to identify the etiology of this deformity using a rabbit model. Twenty-two eight-week-old rabbits were divided into two groups. In Group I, the medial periosteum was excised from the left proximal tibial metaphysis. A partial osteotomy was created 5 mm distal to the epiphyseal plate, involving the medial one-half of the tibial metaphysis. Two 0.7-mm Kirschner wires were placed as markers on each side of the osteotomy. In Group II, the identical procedure was performed from the lateral side. Standard roentgenograms were obtained postoperatively and weekly until death by sodium pentobarbital overdose at six weeks. The angular deformity, medial growth, and lateral growth were calculated from the markers on the roentgenograms. All tibias were decalcified and analyzed under light microscopy. In Group I, all 11 rabbits developed valgus deformity averaging 12.2 degrees; asymmetric growth was statistically significant. In Group II, ten of 11 rabbits developed varus deformity averaging 9.8 degrees; asymmetric growth was not statistically significant. Despite obvious asymmetric growth in both groups, light microscopy failed to reveal any asymmetry of the physis. Valgus and varus deformities are secondary to asymmetric growth, which is not demonstrable by light microscopy.

Team physician #4. Avulsion fracture of the distal tibial epiphysis in skeletally immature athletes (juvenile Tillaux fracture).

Adolescent athletic injuries are special in that they differ from those of the skeletally mature athlete in several respects. The physis is weaker than the ligaments, so that the typical mechanism of injury, which might be expected to result in a syndesmosis sprain of the ankle in adults with subsequent tearing of the ligaments, frequently causes an epiphyseal avulsion in children. The potential for leg length discrepancy and growth aberrations exists only if a significant amount of growth remains in the unfused epiphysis. The juvenile Tillaux fracture of the anterolateral distal tibial epiphysis is rare because it requires a specific mechanism of injury of dorsiflexion and external rotation at a time when the tibial epiphysis is fused medially but open laterally. We have also noted in these injuries a tear of the interosseous ligament, which has not previously been stressed in the literature. Intraoperative findings have helped to clarify the pathoanatomy and mechanism of injury. Anatomic reduction is necessary to prevent posttraumatic sequelae.

[Fracture-separation of the articular process of the lower cervical spine. Its relation to fracture-dislocation in hyperextension].

The authors have seen 13 cases of fracture-separation of the articular processes in the lower cervical spine with horizontal facets. This type of lesion was caused by trauma in hyperextension, compression and rotation. The mechanism appears to be similar to that of dislocations in hyperextension. Four types are defined according to the displacement and the association of disc and ligamentous lesions. Most of the cases were unstable and had to be fixed surgically. Eleven cases were treated in this way, twice by a posterior approach, 6 times by an anterior approach and 3 times by a combined anterior and posterior approach.

[Epidemiology, treatment and long-term results of spinal fractures at a non-university affiliated central hospital with special reference to fractures of the lumbar spine].

In a retrospective study epidemiology, treatment, clinical and radiological results of 283 patients with spine fractures in a five-year period are presented. The operation rate ranges from 42% of the cervical to 9% of the thoracical and 24% of the lumbar spine. We found good radiological results concerning the correction of the wedge compression and the collapse of the lumbar vertebral body by fixation with an internal fixator. After a follow-up of 2-5 years nearly 80% of conservative as well as operative treated patients had more or less residual back pain. Because there is a correlation between the quantity of pain and the radiological dislocation, exact initial diagnosis, without overlooking an instability, and adequate conservative or operative treatment is very important.

Treatment of unstable fractures, dislocations and fracture-dislocations of the cervical spine with Senegas plate fixation.

The results of the anterior approach to the cervical spine for the treatment of fractures and dislocations by arthrodesis and Senegas plate fixation are described. Twenty-two patients underwent a one- or two-level arthrodesis of the cervical spine. Their mean age was 42 years. The injuries were subdivided using the radiological classification described by Harris. In the group of patients who presented with a complete neurological deficit below the level of injury, there was only one patient who deteriorated. X ray examination 1 year after surgery showed fusion in 17 patients (100%). In contrast to some recent cadaveric and animal studies in which the anterior approach and fixation were found to be less stable than posterior fusion, our results obtained with this method are excellent, despite the fact that post-operative immobilization was limited. Problems with the anterior approach did not arise in these 22 patients. Alignment was always acceptable and fusion was achieved within one year in all cases.
