[Fluorescein angiography in the diagnosis of periarteritis nodosa].

Periarteritis Nodosa (P.A.N.) is a systemic connective tissue disease with a variety of manifestations that includes ocular involvement in 20% of cases. The diagnosis of this condition is difficult due to the absence of any specific clinical signs or laboratory findings. However, histologic studies have demonstrated a segmental vasculitis that is often necrotic. Ocular findings frequently include choroidal involvement that is characteristic. Nevertheless, angiographic studies of this disease are extremely rare. The findings in three patients suspected of having P.A.N. are presented. Fluorescein angiography established the diagnosis of P.A.N. in two cases and ruled-out its presence in the third case. In the first case angiography demonstrated a retinal vasculitis with multiple arteriolar and capillary occlusions. There was also ischemic involvement of the choriocapillaris and a mild anterior optic nerve vasculitis. All findings resolved, leaving numerous Elschnig spots. In the second case the angiogram showed acute multifocal ischemia of the choriocapillaris. The ocular examination and fluorescein angiogram in the third case were entirely normal, thereby ruling-out P.A.N. on the basis of insufficient criteria. Acute multifocal choroidal ischemia is present in a variety of rare conditions: Toxemia of pregnancy, Disseminated Intravascular coagulopathy, Moskowitz Disease (T.T.P.), Leukemia and Malignant Hypertension. However, the presence of multifocal choroidal ischemia in the presence of a systemic connective tissue disorder strongly favors the diagnosis of P.A.N. The relative contributions of co-existent Malignant Hypertension and P.A.N. in producing choroidal ischemia are discussed. The spectrum of clinical manifestations and laboratory findings in P.A.N. as well as hypotheses concerning pathogenesis (immune-complex deposition) are described. Among all systemic vasculitis , only P.A.N., and rarely Scleroderma, feature choroidal involvement. This is possibly due to the fact that the degree of vasculitis in P.A.N. is sufficiently severe to cause clinically significant choroidal involvement.

Automatic detection of microaneurysms in diabetic fluorescein angiography.

A computerized method for the detection of microaneurysms (MA) in fluorescein angiograms is proposed, using the concepts of mathematical morphology. The MA which are almost circular particles, are extracted from the image by performing different "top-hat transformations". Some particles, however, may then be detected inside the nonhomogeneous vessels, and it is necessary to be able to extract the vasculary net. The MA which present fuzzy boundaries (due to the leakage of fluorescein) are the most difficult to extract. The algorithm has been tested on 25 angiograms, with 1 045 MA analysed. A comparison between the automatic counting procedure and three manual methods of counting has been made to prove the robustness of the proposed method.

Permeability of the corneal endothelium to fluorescein--a follow-up of keratoplasty cases.

The permeability of the corneal graft endothelium to fluorescein was determined, by the oral administration method, in 47 eyes with penetrating keratoplasty. The endothelial permeability was expressed as the transfer coefficient of the dye between the anterior chamber and the cornea. In the early postoperative period, an increase in the aqueous protein was noted and a correction was made for fluorescein binding to aqueous protein. The measurements of the graft thickness and morphometry of the endothelial cells by specular microscopy were also carried out concurrently. The transfer coefficient was high in the early postoperative period, and was reduced by time. Similarly the graft was thick in the early postoperative period but it thinned to a normal level over a period of several months. A significant correlation was found between the transfer coefficient and the corneal thickness. The mean cell size of the graft endothelium was significantly greater than that of the normal cornea. The mean cell size and the postoperative period were correlated using the present data and those reported by Sato previously. A significant positive correlation was found, indicating progressive enlargement of the graft endothelial cells over a long period of time. The mean cell size was not significantly correlated with the transfer coefficient nor with the graft thickness.

Fluress, fluorescein and benoxinate: recovery from bacterial contamination.

The ability to recover from bacterial contaminations with Staphylococcus auresus and Pseudomonas aeruginosa was determined for three optometric DPAs: Fluress, fluorescein and benoxinate. Results show that Fluress recovers from contamination more rapidly than benoxinate or fluorescein. This ability to recover from contamination and the relative ease of use of Fluress may make it the DPA choice for a number of optometric procedures including applanation tonometry.

Early signs of diabetic retinopathy by fluorescein angiography.

The initial sign of diabetic retinopathy is considered to be microaneurysm, determined by ophthalmoscopy. To elucidate early vascular changes in diabetes, fluorescein angiography was performed in 272 eyes of 166 patients who had shown no evidence of diabetic retinopathy by ophthalmoscopy. Early retinal vascular changes shown by fluorescein angiography were seen in 181 eyes (66.5%) out of 272 eyes. These angiographic findings embraced dye leakage, dilatation of capillaries, filling defect of capillaries, microaneurysm-like spot dilatation and microaneurysm. Statistical analysis and follow-up studies for 1 to 5 years revealed that, with progression of the disease, the angiographic changes progressed in the following order: dye leakage, dilatation of capillaries, filling defect of capillaries, microaneurysm-like spot dilatation and microaneurysm.

Differential binding of fluorescein-labeled lectins to mouse thymocytes: subsets revealed by flow microfluorometry.

Fluorescein-labeled lectins bound to mouse thymocytes were analyzed by flow microfluorometry. This technique has identified several lectins that bind differentially to thymocyte subsets. The most complex fluorescence distributions were obtained using lectins with nominal specificities for galactose or N-acetylglucosamine. Inhibition of binding by sugars confirmed that the fluoresceinated lectins were bound to cells at their carbohydrate binding site. Simultaneous analyses of lectin fluorescence and forward light scatter intensity showed that cell subpopulations of different sizes can exhibit marked differences in the level of binding such that the amount of lectin bound per cell is often independent of cell size. A minor population of dull or unstained cells, delineated by several of these lectins, correlates with the subpopulation of medium-sized thymocytes resistant to in vivo cortisone treatment.

A comparative study of the use of fluorescein and the Doppler device in the determination of intestinal viability.

Fluorescein is a reliable indicator of mucosal viability in dogs. This fact is probably applicable to humans. The Doppler device was not a reliable predictor of intestinal nonviability, and in the instance of mesenteric venous occlusion, with ischemic intestine, it was totally inaccurate in assessing intestinal viability. Short segments of intestine failing to take up fluorescein stain will retain viability, owing to the lesser nutrient requirements of resting smooth muscle. However, segments should not be used for anastomoses, since the mucosa will, to some degree be ischemic.

[Fluorescein angiographic studies of the perilimbal conjunctiva and limbus in hypertension].

Seventeen patients with vegetovascular dystonia with hypertensive reactions, 19 patients with essential hypertension at IB-IIA stages, and 26 hypertensive patients at IIB stage of the disease underwent fluorescein-angiographic study of the perilimbal conjunctiva and limb. The advantage of this method as compared to the biomicroscopic one, is that it allows one to obtain objectively documented information on the state of vessels and velocity of blood flow in them. It was established that by the method of fluorescein-angiography of the perilimbal conjunctiva region and limb there may be detected pathologic changes in the microcirculation of the field under study not only at early stages of essential hypertension but also in vegetovascular dystonia patients with hypertensive reactions, when changes on the fundus of the eye were absent.

Altered skin flap survival and fluorescein kinetics with hemodilution.

Isovolemic hemodilution is known to reduce blood viscosity. Reduction of either the blood cell concentration or the serum total protein has been found to improve blood flow and ultimate survival of skin flaps. Nutritive blood flow and tissue survival were examined in dorsal pedicle skin flaps in three groups of rats at three hematocrit levels--44% (controls), 31%, and 19%. After hemodilution by phlebotomy and crystalloid replacement, the dye kinetics of intravenously administered fluorescein were measured with the fiberoptic perfusion fluorometer. Skin flap survival at the different hematocrits were also examined. Hematocrits and serum total protein were measured before and after hemodilution and the elevation of the flap. The rate of fluorescein elimination and ultimate flap survival were significantly augmented in the two hemodiluted groups. The maximum benefit was seen in the group of animals hemodiluted to 19%. However, significant improvement in blood flow and skin flap survival was noted in the group of animals that were hemodiluted to a more clinically acceptable level (31%). The decreases in serum total protein following flap elevation did differ among the groups. This study supports the hypothesis that isovolemic hemodilution may be a valuable technique for salvaging marginally ischemic tissues.

Low incidence of side effects following intravenous fluorescein angiography.

The complications that developed in 1,800 patients who underwent 2,631 fluorescein angiographies (FAs) were reviewed. In 5.4% of the patients, complications developed during 4.4% of the FAs. The patients with complications were ten years younger than the average patient undergoing the test. The rate of complications was twice as high in the male patients. Skin testing seems useless in attempting to predict allergic reactions in susceptible patients. Only one life-threatening situation, acute pulmonary edema, was found. Cases of numbness of the thumb, thrombophlebitis, pruritus of the soles and hands, and "hay-fever"-like reactions are also reported. A case of a patient in whom myocardial infarction developed one hour following the test is also discussed, but it is not believed to have been directly caused by the fluorescein injection.

[Fluorescein angiography. Why not in color? (author's transl)].

The Author reminds the possibility to record fluorescein angiography with normal filters in use for black and white angiography on color slide films of high sensibility (400 ASA). The main advantage of this method is that the real fluorescein may be separated from the autofluoresceine and that whole registered frames can be immediately projected on the screen, for example during a laser therapy.

[Diagnosis of pancreatic insufficiency with fluorescein dilaurate in patients with cystic fibrosis (author's transl)].

The tubeless pancreas-function test with fluorescein dilaurate, which is based on the saponification of the test substance by pancreas-specific arylesterases, and which has proved itself in adults, was given to children for the first time. After reduction of the size of the test dose and the amount of liquid to be drunk, there were no difficulties in carrying out the test. Elimination of the fluorescein released from the flourescein dilaurate and excreted via the kidneys was clearly less in patients with cystic fibrosis than in normal subjects. Thus, the test in its modified form for children can provide adequate differentiation between normal and pathological pancreas function.

[Diagnosis of retinal vascular changes in diabetic children by means of fluorescein-angiography (author's transl)].

By means of fluorescein-angiography diabetic microangiopathy can be earlier revealed than by means of ophthalmoscopy. 36 children with diabetes (aged 6,25-15,5 years, x 11,6 years) with a duration of the diabetes between 0 and 12,75 years (x 3,9) were submitted to fluorescein - angiography, in 20 cases a follow up study was performed. Pathological changes were found in 21 cases, in 10 of 15 children with normal angiograms the duration of the disease was less than 5 years. More than 50% of all patients followed up showed an increase of diabetic microangiopathy.

Discrepancy between indocyanine green and fluorescein angiography in occult choroidal neovascularization.

Six eyes without or with minimal evidence of choroidal neovascularization on fluorescein angiography are presented. All six eyes have marked retinal pigment epithelial alterations. Indocyanine green angiography reveals extensive hyperfluorescence, indicative of choroidal neovascularization undermining the macula in these six eyes. It is surprising that three eyes are asymptomatic and two others have a relative good visual acuity. It is suggested that hyperfluorescence observed by indocyanine green angiography without or with minimal late dye diffusion on fluorescein angiography could indicate a slow progression and thus a better prognosis of the maculopathy.

Local connected fractal dimensions and lacunarity analyses of 60 degrees fluorescein angiograms.

PURPOSE: The retinal vascular tree exhibits fractal characteristics. These findings relate to the mechanisms involved in the vascularization process and to the objective morphologic characterization of retinal vessels using fractal analysis. Although normal retinas show uniform patterns of blood vessels, in pathologic retinas with central vein or artery occlusions, the patterns are irregular. Because the generalized box fractal dimension fails to differentiate successfully between normal and abnormal retinal vessels in 60 degrees fluorescein angiograms, the authors have further investigated this problem using the local connected fractal dimension (alpha). METHODS: The authors studied 24 digitized 60 degrees fluorescein angiograms of patients with normal retinas and 5 angiograms of patients with central retinal vein or artery occlusion. The pointwise method estimated the local complexity of the angiogram within a finite window centered on those pixels that belong to the retinal vessels. Color-coded dimensional images of the angiograms were constructed by plotting the pixels forming the object with a color that corresponded to specific values of alpha +/- delta alpha. RESULTS: The color-coded representation allowed recognition of areas with increased or decreased local angiogram complexity. The alpha distributions showed differences between normal and pathologic retinas, which overcomes problems encountered when using the methods of calculating the generalized fractal dimensions. A multivariate linear discriminant function using parameters from the alpha distribution and a further fractal parameter--lacunarity--reclassified 23 of the 24 normal and 4 of the 5 pathologic angiograms in their original groups (total: 92.1% correct). CONCLUSIONS: This methodology may be used for automatic detection and objective characterization of local retinal vessel abnormalities.

Fluorescein angiographic features of neovascular maculopathy in angioid streaks.

Among the 46 eyes of 23 patients diagnosed with angioid streaks, macular complications (subretinal neovascularization) were found in 20 eyes of 11 patients (43.5%): the 14 eyes of 7 males, and 6 eyes of 4 females. Of these patients, 81.8% were in their fifties to sixties. The patients with and without macular complications did not differ in the incidence of pseudoxanthoma elasticum and other ocular findings, including peau d'orange and salmon spots. In 17 eyes of 10 patients (85%) with macular complications, fluorescein angiography demonstrated multiple small, round, hypofluorescent dots in the area temporal to the macula and around the optic disc. The extent of this lesion varied from simple hypofluorescent dots to dots each surrounded by a hyperfluorescent area. These changes could be detected not by ophthalmoscopic fundus examination but only by fluorescein angiography.

[Fundus fluorescein angiographic findings of sympathetic ophthalmia].

Eight cases of sympathetic ophthalmia diagnosed by histopathologic examinations of the enucleated exciting eyes were collected and the fundus fluorescein angiographic (FFA) findings of the sympathized eyes in acute stages were analyzed. The manifestations of the FFA were divided into three types. We suggest that if one eye has a history of trauma and the other eye loses visual acuity rapidly accompanied with uveitis signs after certain incubation period, the fundus fluorescein angiography should be examined to achieve a clinically objective diagnostic basis so that the patients are treated in time.

Identification of the ATP binding site in tyrocidine synthetase 1 by selective modification with fluorescein 5'-isothiocyanate.

Identification of the nucleotide binding site in peptide synthetases has been approached by affinity labeling of tyrocidine synthetase 1 with fluorescein 5'-isothiocyanate. Binding was accompanied by irreversible inhibition of the ATP-dependent phenylalanine activation reaction and was prevented in the presence of MgATP2-. The reaction obeyed pseudo first-order rate kinetics and was accelerated by Mg2+. Complete inhibition corresponded to incorporation of 2.3 mol of fluorescein 5'-isothiocyanate (FITC)/mol of protein. Upon protection by MgATP2-, about 1 mol of FITC is still incorporated; however, this does not affect activity. The modified synthetase was extensively fragmented by tryptic digestion and the labeled fragments isolated by reverse-phase high performance liquid chromatography. Two peptides, DHQVKIR and LDKMPLTPNDKIDR, have been identified by sequencing, and the FITC conjugate of the former peptide has been detected by laser desorption mass spectrometry. The labeled residues, Lys-422 and Lys-505, are located within highly conserved segments of this new class of synthetases.
