Comparison of derivative spectrophotometric and liquid chromatographic methods for the determination of rofecoxib.

Two different UV spectrophotometric methods were developed for the determination of rofecoxib in bulk form and in pharmaceutical formulations. The first method, an UV spectrophotometric procedure, was based on the linear relationship between the rofecoxib concentration and the lambdamax amplitude at 279 nm. The second one, the first derivative spectrophotometry, was based on the linear relationship between the rofecoxib concentration and the first derivative amplitude at 228, 256 and 308 nm. Calibration curves were linear in the concentration range using peak to zero 1.5-35.0 microg x ml(-1). HPLC was carried out at 225 nm with a partisil 5 ODS (3) column and a mobile phase constituted of acetonitrile and water (50 :50 v/v). A linear range was found to be 0.05-35.0 microg x ml(-1). The developed methods were successfully applied for the assay of pharmaceutical dosage form. The statistics of the analytical data is also presented. The results obtained by first derivative spectrophotometry were compared with HPLC and no significant difference was found.

[Application of M-RAPD technique to obtain the genomic fingerprints of various pathogenic microbials].

OBJECTIVE: To obtain genomic fingerprints of different pathogenic microbials and make certain whether their patterns can be used in the identification of microbials by means of multiplex random amplified polymorphic DNA analysis (M-RAPD). METHODS: Arbitrary primers of 10 oligonucleotides were randomly grouped, and various microbials chromosomal DNA were amplified with three combinatorial primers at a special higher annealing temperature. The products were detected by 15 g/L agarose electrophoresis and the patterns were analyzed by the software of Gelworks 1d Intermediate. RESULTS: Specific and resistant DNA fingerprints for different pathogenic microbials with combinatorial primers were gained. The profiles were clear, well-distributed and the number was great. The products of the three primers included most products of every two primers and would appear with no relation to their length, but small products had more opportunity; three primers could provide information contents half as many again as that two primers could provide for the same pathogenic microbials. There were obvious differences among different drug-resistant strains and between the drug-resistant strains and the corresponding reference strains, but different strains of the same microorganism had more similarity than discrepancy. The analytic data of the software of Gelworks 1d Intermediate also support our results. CONCLUSION: M-RAPD is a simple and rapid technique for the identification of different kinds of pathogenic microbials, and it can provide rich genetic information.

[Comparison of the cytotoxicity of chloroform on mouse L cells by measurement of proteins and chromium 51 release].

The authors describe a new cytotoxicity test based on the classical measurement of the residual proteins after a contact time of 4 days between the produit tested and the mouse L cells. The results are expressed as a cytotoxic dose 50 with a confidence limit for the 95% threshold after a statistical treatment of the analytical data by a computerized method. For chloroform, this method is more sensitive that the classical cytotoxicity test involving the chrom 51 release.

[Functional protein S in 77 patients treated with anticoagulants and in 32 probands with normal coagulation parameters].

We determined functional protein S with a new automated clotting assay, total protein S antigen with the Laurell technique and free protein S antigen with an ELISA test and calculated the ratio of total protein S antigen and prothrombin antigen (Laurell) of plasmas of 77 patients oral anticoagulant therapy and of 32 normals. The correlation of total protein S antigen with protein S activity was found to be r = 0.857. The normal range of the tests was calculated for orally anticoagulated patients and for normals and was compared with the analytical data from 4 orally anticoagulated, congenital protein S deficiency patients. We believe that the automated functional protein S clotting assay is a quick and easily performed test, which now allows even for protein S the use of a functional assay as the first step for the screening of coagulation inhibitor deficiencies.

[MIBI and parathyroid gland function in secondary hyperparathyroidism].

The role of the double phase parathyroid scintigraphy with MIBI in the primary hyperparathyroidism has been well studied. The present work analyzes the benefit of this image technique in the diagnosis and therapeutic approach to uremia associated hyperparathyroidism. We studied 18 dialysis patients with clinical and analytical data of hyperparathyroidism. All of them were receiving treatment with i.v. vitamin D. We carried out two scintigraphic studies, the first under basal conditions (without changes in treatment) and the second one, a week later after the stimulation of the parathyroid gland through the cessation of the treatment with vitamin D and phosphorus binders, and the use of a low-calcium dialysate. In the basal study, fourteen patients had a positive uptake test in at least one of the four glands. These patients had significantly higher PTHi levels than those with a negative scintigraphic study. After the stimulation test, we found a statistically significative rise in PHTi (644 vs 979 p < 0.001) and phosphorus levels (5.8 vs 7.3; p < 0.01), a slight fall in calcium levels (p = NS) and a statistically significative increment in the score of scintigraphic captation (5.5 +/- 4.5 vs 6.8 +/- 5; p < 0.05). We found a statistically significative correlation between the PTHi level and the score of scintigraphic uptake, in the basal study (r = 0.6, p < 0.01) and after stimulation (r = 0.6, p < 0.01). The only variable associated with the scintigraphic uptake was PTHi, justifying 31.1% of the variance in the first study and 32.7% in the second. In conclusion, the MIBI scintigraphic is a useful exploration in the uremic patient with hyperparathyroidism, as it predicts the functional state of the parathyroid glands according to uptake intensity. The stimulation test could be useful in planning therapy, but some of its characteristics need to be established, such as the precise time to reach the maximum level of parathyroid stimulation.

[Pathogenetic mechanisms in phenylketonuria: disorders affecting the metabolism of neurotransmitters and the antioxidant system].

AIM: To review the clinical and biochemical changes in neurotransmission and antioxidant system in phenylketonuric patients under dietary treatment. DEVELOPMENT: Phenylketonuria (PKU) is an inborn error of metabolism caused by decreased activity of the enzyme L-phenylalanine-4-mono-oxigenase that synthesizes tyrosine from phenylalanine. According to analytical data from PKU patients and to experimental studies in animal models, high phenylalanine values in plasma and tissues seem to be related with defective biosynthesis of neurotransmitter (mainly serotonin and dopamine) and impairment of antioxidant system. Despite dietary treatment, PKU patients usually present moderate hyperphenylalaninemia over the evolution of the disease that might cause clinical and biochemical abnormalities. CONCLUSIONS: Increased plasma phenylalanine concentrations and dietary treatment might be related with neurotransmitter and antioxidant system abnormalities in human phenylketonuria. These biochemical alterations might be involved in the physiopathology of PKU.

[Analysis of contents of berberine in Coptis chinensis of Lichuan].

OBJECTIVE: To determine the contents of berberine in Coptis chinensis of Lichuan and establish its best cultivation scheme. METHOD: We used high performance liquid chromatography (HPLC) to measure the contents of berberine at different altitude, growth age, and leaves. RESULTS: Analytic data showed that the growth age and firry woods shading did not affect the contents of berberine in rhizome of Coptis chinensis. Low altitude was more suitable for Coptis chinensis to synthesize berberine. The contents of berberine in rhizome of Coptis chinensis with floral leaf were higher than those with lusterless and lustrous leaves, but no significant difference. CONCLUSION: The synthesis of berberine is closely correlated with shading conditions. It is recommended that the shading ratio should be reduced or the sheds removed in the middle growth age (2-3 years), and then the herb should be reshaped at the last year to enhance the synthesis of berberine, so as to obtain high-quality Coptis chinensis in the harvest.

[The incidence of the inappropriate ADH secretion syndrome (SIADH) in major otorhinolaryngologic surgery].

There are few works dealing with the presentation of the inadequate secretion of antidiuretic hormone in major ENT procedures. In this prospective study done by the AA., based in analytic data, they found out the presence of the syndrome in as many as 39 percent of those operations. Consequently the suggestion is made that major ENT-surgery must be considered among the potential factors of the syndrome.

Studies on the syntheses of 1-substituted benzimidazole derivatives.

Several novel 1-heteroarylalkyl substituted benzimidazoles were synthesized in the reaction of various halogenoalkylheterocycles with benzimidazoles. The structures of the products were confirmed by their spectral and analytical data. The potential biological properties of the new compounds were evaluated using the PASS programme.

[Medico-social legislation of Ukraine: organizational-legal aspects of management in the sphere of community health care (current state, problems and perspectives)].

The authors analyzed organizational-legal aspects of management in the sphere of community health care. The foundation of medical-legal base of public health is substantiated in the article. It enables to form new mechanisms aimed at improvement of medical care, creation of data-analytical system, unification of health care and diagnostics standards.

[Application of three-dimension luminescence method to the identification of petroleum pollutants].

As three-dimension luminescence method can provide abundant sample information, it is applied to the identification of petroleum pollutants. In this paper, the whole analytical process of three-dimension luminescence method is described, including sample disposal, analytical conditions, analytical process, especially the automation in acquisition, disposal and differentiation of analytical data.

[Fistulae or catheter for elderly who start hemodialysis without permanent vascular access?].

UNLABELLED: Autologous access is the best vascular access for dialysis also in older patients and it should be mature when patient needs hemodialysis. It is not always possible. Surgeon availability and demographic characteristics of patients (age, diabetes, vascular disease...) are factors that determine primary vascular access. AIM: To analyse outcome and vascular access complications in elderly who start hemodialysis without vascular access. PATIENTS AND METHODS: All patients older than 75 years who initiated hemodialysis without vascular access between January 2000 and June 2002 were included, They were divided en two groups depending on primary vascular access. GI: arterio-venous fistulae. GIIl: Tunnelled cuffed catheter. Epidemiological and analytical data, vascular access complications related, as well as patient and first permanent vascular access survival from their inclusion in dialysis up to December 2002 were analysed and compared in both groups. RESULTS: 32 patients were studied. GI: n = 17 (4 men) and GIIl: n =1 5 (8 men), age: 79.9 +/- 3.8 and 81.7 +/- 4 years respectively (ns). There were no differences in sex and comorbidity (diabetes, ischemic heart disease, peripheral vascular disease and hypertension). It took GI 3 months to get a permanent vascular access suitable for using, while it took GIIl 1.3 months (p < 0.005) The number of temporary untunnelled catheters was higher in GI (3.35 vs 1.87 p < 0.05). Vascular access complications: 70.6% of infections occur in GI (incidence (I) = 48 infections/100 patients-year) while only 29.4% were detected in GII (I = 25 infections/100 patients-year). 70% of central venous thrombosis happen in GI (I: 25 CVT/100 patients-year) vs 30% in GIIl (I = 14.4/100 patients-year) (ns). No significant differences neither in bleeding (66.7% vs 33.3%) nor ischemia (75% vs 25%) were found. Dialysis dose (Kt/V) as well as anaemia degree were similar in both groups. Permanent vascular access survival after 2 years was 45.8% in GI and 24% in GII (ns). Patient survival was similar in GI and GII (72% vs 51% ns). CONCLUSIONS: Elderly who start hemodialysis without vascular access took longer to get a suitable permanent vascular access when arterio-venous fistulae is placed than with a tunnelled cuffed hemodialysis catheter. As a consequence, vascular access complications are larger, infection ones are the most common. In these patients a tunnelled catheter should be inserted at the time a peripheral arterio-venous access is created, in order to avoid temporary untunnelled catheters.

[Mycophenolate mofetil in high risk IgA glomerulonephritis].

Mesangial IgA glomerulonephritis (MIgAGn) is the most common biopsied primary glomerulonephritis worldwide, with a poor long-term prognosis for renal function in over a third of all patients. No proven therapy currently exists for MIgAGn. Recent studies have suggested some benefit with mycophenolate mofetil (MMF), especially in hypertensive patients with kidney failure and proteinuria, though other studies have failed to corroborate these findings. We report eight adult patients with biopsy proven MIgAGn followed in a single hospital. They all received angiotensin-converting enzyme inhibitors or angiotensin II receptor blockers. Compassionate use of MMF was based on the presence of clinical and analytical data suggesting a high risk of short- to medium-term progression to chronic renal failure. MMF treatment was stopped after two and three months in two patients who had advanced renal failure at the start of therapy because of disease progression and greater fluid retention. Several months later they both required dialysis and kidney transplantation. The mean duration of MMF therapy in the other six patients was 15 (range: 10-18) months. The mean serum creatinine concentration fell from 1.82 +/- 0.47 to 1.55 +/- 0.41 mg/dl (p = 0.04). Protein loss in 24-hour urine collection fell from 1.95 +/- 1.35 to 0.77 +/- 0.58 g/day (p = 0.02). These results in this low number of patients showed that treatment with MMF in high-riks patients with MIgAGn and early stage kidney failure generally stabilized the disease and reduced proteinuria. MMF was well tolerated and may be of some benefit in a subgroup of patients with MIgAGn and a poor prognosis.

Carboxyhemoglobin and blood cyanide concentrations in relation to aviation accidents.

INTRODUCTION: It is important in aviation accident investigations to determine if a fire occurred during flight or after the crash and to establish the source(s) of the toxic gases. METHODS: Bio-specimens from aviation accident fatalities are submitted to CAMI for analyses. In blood, CO is analyzed as carboxyhemoglobin (COHb) and hydrogen cyanide as cyanide (CN-). Analytical data were stored in a database, and this database was searched for the period of 1990-2002 for the presence of COHb and CN in the submitted cases. RESULTS: Out of 5945 cases, there were 223 (4%) cases wherein COHb was > or = 10%. Of the 223 cases, fire was reported with 201, no fire with 21, and undetermined fire status with 1. CN concentrations were at or above 0.25 microg x ml(-1) in 103 of the 201 fire-related cases. None of the 21 non-fire cases had CN-, but nicotine was detected in 9 of the cases. All non-fire cases with COHb > 30% (four cases) were associated with exhaust leaks. Of the 223 cases, COHb-CN- fractional toxic concentration (FTC) was lethal only in 31 cases with elevated CN levels. CONCLUSIONS: The presence of COHb and CN in elevated concentrations in the blood of victims found by autopsy to have died on impact would indicate an in-flight fire. In the absence of fire and CN-, the elevated COHb concentrations would suggest an exhaust leak, particularly at COHb > 30%. The findings of this study also suggest that, in addition to COHb, CN plays a detrimental role in fire-associated aviation accident fatalities.

[Coxiella burnetii osteoarthritis].

A case of a 25-year-old female characterized by febrile osteoarthritis and arthromyalgias as the only clinical manifestation of a Q fever is discussed. Analytical data were compatible with those of an acute inflammation and serologic findings reached diagnostic levels. Response to Doxycycline treatment was totally satisfactory. Even though associated arthromyalgias are described in some cases of Q fever as well as sporadic cases of osteoarthal infection due to Coxiella burnetii, there are no oligosymptomatic cases published as the one discussed. Some physiopathologic considerations referred to this peculiar onset are suggested.

[Inherited thrombophilia in pregnancy: a systematic review].

The purpose of this study was to conduct a systematic review of the literature of studies that examined the association between inherited thrombophilias and adverse pregnancy outcomes. We have evaluated developments in this area published since 1998. The published studies differ widely in design, mostly in patients and controls selection and in data analytic approach. Despite the growing evidence in the literature, there are still gaps in our knowledge of thrombophilia in pregnancy, specially regarding less prevalent thrombophilic defects, such as deficiencies in antithrombin, protein C, and S. Several studies on the association on factor V Leiden showed that it may play a role not only in second trimester losses, but also in pre-eclampsia, intrauterine growth-retardation, and placental abruption. Studies on the prothrombin gene mutation yielded conflicting results. Further large prospective studies are needed to asses the relative clinical and cost effectiveness of anticoagulant therapies in the prevention of pregnancy adverse outcomes.

[Chemical composition of one source of sodium chloride mineral water from Neamţ county].

This paper describes the analytical study of a sodium chloride mineral water from Bălţăteşti Spa--Neamţ county. From the obtained analytical data it was observed that the dominant ions are those of Cl- (48.11 mEq%) and Na+ (43.07 mEq%), respectively. Also, the water contains a significant proportion of Mg2+ ions (6.12 mEq%) compared with the one of Ca2+ ions (0.339 mEq%). It can be remarked that the analysed source consists of a very concentrated, hypertonic water with a high content in chlorine, sodium and magnesium. This kind of mineral water is recommended in external treatments.

[Late nephrology referral influences on morbidity and mortality of hemodialysis patients. A provincial study].

OBJECTIVE: To evaluate the influence of late referral to nephrology of the patients with chronic renal failure in the morbimortality of the patients who start hemodialysis. SUBJETS AND METHODS: There were included in the study the patients who started hemodialysis (HD) as first form of treatment, and that survived at least three months in both hospitals of reference of the province of Huesca from january 1990 to december 2001. Patients who started HD after acute renal failure were excluded. Clinical and analytical data were determined for each patient at the start of HD and during the follow-up. Early (ER) and late referral (LR) were defined by the time of first nephrology encounter greather than or less than 4 months respectively, before HD initiation. Morbidity analysis (using multiple linear regression with rate of days of hospitalization as dependent variable) and global and anual during the first three years of follow-up survival analysis (using Cox proportional hazards regression) were carried out. RESULTS: A total of 139 patients (78%) started HD in the ER group and 39 (22%) in LR group. Mean follow-up was similar in both (ER = 34.43 +/- 25.5 months; LR = 34.42 +/- 28.37 months). At the start of dialysis LR was associated to higher proportion of temporary catheters, lower level of hematocrit and albumin, higher comorbidity and higher levels of urea and creatinine. Risk factors selected by the model in the morbidity analysis were index of comorbidity (CI), late referral, serum albumin, urea reduction ratio (URR) and hematocrit (R2 = 0.334, F = 16.97, p < 0.005). The final equation of regression was: Rate of hospitalization's days = 101.12 + (2.45 x CI) - (12. 11 x LR) - (11.57 x Alb.) - (0.43 x PRU) - (0.83 x Hto). Variables selected by Cox's regression model that were associated with survival throughout complete follow-up were hematocrit (RR = -0,207, CI 95% 0.726-0.910, p < 0.0005), index of comorbidity (RR = 0,265, CI 95% 1.066-1.594, p = 0.007), PRU (RR = - 0,059, CI 95% 0.893-0.996, p = 0.038) and type of dialysis membrane (RR = 0,771, Cl 95% 0.260-0.822, p = 0.007). Nevertheless, in successive models fitting after 12, 24 and 36 months of follow-up the variable LR influenced in an independent way survival first two years, losing his significance later. CONCLUSION: In our study patients of the group LR presented a worse clinical and metabolic situation at the beginning of the HD. Later there was demonstrated in this group a higher long-term morbidity and a lower survival the first two years.
