[The diagnosis of poisonings by toxic fungi].

Investigations carried out by the author resulted in elaboration of a probability differential diagnostic algorithm of intoxications with poisonous fungi based on a mathematical evaluation of the informative value of clinico-laboratory signs of intoxication. This method made it possible within a short time to determine the group of poisonous fungi that caused intoxication and to real differential diagnosis and institute timely adequate treatment.

Renal angiomyolipoma. Ultrasonographic, CT, angiographic, and histologic correlation.

The findings at ultrasonography (US), CT, and angiography in 13 patients with renal angiomyolipomas were compared with histologically determined proportions of fatty, myogenic, and vascular components. The angiomyolipoma was composed mainly of fat in 4 cases, half of fat in 2, mainly of myogenic tissue in 4, and was hemorrhagic in 3. US showed high echogenicity in 10 patients, while CT demonstrated attenuation values of fat in 3 tumors with mainly fat tissue but gave non-specific findings in the other cases. The angiographic findings were considered specific for angiomyolipoma in 2 out of 11 cases (18%). Low fat content and hemorrhagic areas caused diagnostic difficulties at both US and CT. The diagnostic algorithm for renal angiomyolipoma is discussed.

[Magnetic resonance tomography, computed tomography and ultrasonic diagnosis of hemangiomas of the liver].

The paper is concerned with the potentialities of modern radiation methods in the diagnosis of liver hemangiomas. USI, CT, angiography and MR-tomography (MRT) were used for investigation of 64 patients with liver hemangiomas. The diagnostic potentialities and the role of each of the above methods in the diagnostic algorithm in liver hemangiomas were studied. The results of the investigation have shown that MRT possesses a higher informative value than USI and CT in the diagnosis of liver hemangiomas and can even replace angiography in the verification of diagnosis.

[An automated thermographic complex: new possibilities of the diagnosis of ORL disease].

The Kiev Research Institute of Otorhinolaryngology has built an automatic diagnostic system that includes a Raduga thermograph and a CM-1403 computer. The diagnostic algorithms are based on the thermosemiotics of ENT inflammatory diseases developed by the authors. The advantages of the system are: production of a hard copy of thermograms, registration and long storage of information, mathematical treatment of thermal images, data base and rapid search for the thermogram needed, screening diagnosis based on the "healthy-sick" principle.

[Renal hydatid cyst: a nuclear magnetic resonance study].

A case of renal hydatid cyst (R.H.C.) in which study both traditional diagnostic methods (such as serology, radiology, ecoraphia, C.A.T., etc.) and the newer ones, including Nuclear Magnetic Resonance (N.M.R.) were used, is presented. The diagnostic characteristics of this type of cystic pathology were reviewed through N.M.R. study, as well as current criteria to outline a correct differential diagnosis with simple Renal Cysts (R.C.). Finally, a theoretical place for each of these methods within the diagnostic algorithm of renal masses is suggested.

[Experience in the surgical treatment of combined atherosclerotic lesions of the coronary arteries, abdominal aorta and arteries of the lower limbs].

A comprehensive clinicoangiographic and functional examination was made of 180 patients with combined coronary and vascular abnormalities. With this, a diagnostic algorithm was developed, which determined the authors' approach to evaluation of the severity of patients and the choice of surgical policy. A total of 150 patients were operated on, the total mortality being 7.3%. The examination of operative and hospital mortality rates in patients with combined abnormalities showed that pre-aortocoronary bypass surgery before a reconstructive vascular operation in patients with severe coronary diseases made it possible to avoid serious coronary events in the following postoperative period.

[The role of digital subtraction angiography in the diagnosis of pulmonary anomalies].

The authors evaluate the importance of pulmonary digital subtraction angiography (DSA) in anomalies of pulmonary tissue, the bronchial tree and vascular pulmonary malformations. In a group of eight patients, comprising unilateral pulmonary hypoplasia, pulmonary cysts, vascular malformations and pulmonary sequestrations, they analyze the asset of DSA and the place of this method in the diagnostic algorithm. Reduction of the invasive and risky character of the examination, lower costs, the possibility of immediate evaluation and subsequent processing of the picture and modification of the length of the record is one of the main advantages, as compared with classical angiography.

[Comparative evaluation of invasive methods of examination of pulmonary coin lesions].

The results of examination of 575 solitary spherical structures in the lungs are analysed. Comparative evaluation of various methods of examination showed that transthoracic aspiration biopsy conducted under the control of computered tomography by means of two thin needles has advantages over radiological examination, cytological examination of sputum, and bronchofibroscopic biopsy. The authors developed a diagnostic algorithm used in a patient with a solitary spherical structure in the lung.

[Methodologic provisions for developing models of individual prognoses of pregnancy outcome].

The method of individual prognosis of spontaneous abortion in clinically indefinite conditions is considered. The diagnostic algorithm is based on the use of the optimal dendrite method to identify classes at the first stage and to classify a female as a healthy subject or a patient further (at the second stage). The algorithm is realized as a programme module operating in the FORTRAN language on a dialogue for a personal computer.

The radiological investigation of urinary tract infections in childhood.

Children with urinary tract infection require imaging of the entire urinary tract to search for a correctable underlying cause and to find those with or at risk of renal parenchymal damage. In this paper, we discuss the causes of urinary tract infection and assess the merits of each imaging technique. A diagnostic algorithm is proposed.

Diagnostic procedures for pleural disease.

Using a systematic approach based upon findings obtained from the patient's history, physical examination, routine laboratory studies, and chest roentgenograms, the clinician will establish the presence and location of pleural effusion in the majority of cases. The selective use of tests based upon the clinician's initial diagnostic impressions will allow accurate separation of transudative from exudative effusions; exudative effusions will be diagnosed in over 85 per cent of patients. There remain many unanswered questions regarding the diagnosis of pleural effusion. Prospective studies are needed to establish diagnostic algorithms that minimize under- and overutilization of tests. The introduction of Medicare Prospective Reimbursement under Diagnosis-Related Groups (DRGs) on October 1, 1983, may provide the appropriate incentives to hospitals and clinicians to carry out these important studies.

[The instrumental diagnosis of disseminated lesions of the lungs].

A study is presented of 52 patients with disseminated pulmonary lesions. Among them: histiocytosis X--17, alveolar microlithiasis--5, pulmonary phospholipoproteinosis--7, idiopathic hemosiderosis--14, primary pulmonary amyloidosis--1, primary diffuse lung tumours--8. All patients were subjected to complex bronchological examination including use of different biopsy transbronchial methods as well as open biopsy of the lung. The diagnostic value of the employed bronchial biopsies is discussed. A diagnostic algorithm of rarely met pulmonary disseminations has been developed. Recommendations on the use of different instrumental methods are formulated.

[Vaginal ultrasonography: a new testing method in obstetrics and gynecology].

Transvaginal sonography represents a new tool for the gynecological diagnosis and decision making. The authors present theoretical and practical aspects of transvaginal sonography with special emphasis on the 6.5 MHz vaginal probe. Based on one thousand one hundred and eight examinations it is obvious that this procedure is valuable in diagnostic algorithm of ectopic pregnancy and to follow follicular growth, as well as ultrasound guided oocyte retrieval.

[Radiodiagnosis of liver hemangiomas].

A combined use of radiation methods in the diagnosis of liver hemangiomas is considered. Twenty-two hemangiomas were detected in 18 patients using ultrasound investigation (USI), computerized tomography (CT) and angiography. The most characteristic signs of hemangiomas were identified. USI and CT were shown quite reliable in the detection of hemangiomas. They can be used for solving diagnostic problems depending on a situation. A diagnostic algorithm envisaging the most optimum use of USI, CT and angiography in liver hemangiomas, was developed.

[Medical, social, economic value of the rational use of non-invasive studies for diagnosing phlebitis. Experience of the Regional Hospital Center of Nimes from 1984 to 1988].

Continuous wave Doppler, and B-Mode ultrasound allow the diagnosis of deep venous thrombosis with a 96% sensitivity, and a 98% specificity versus venography (1). Therefore, these methods can be extensively used for the detection of deep venous thrombosis, following a new diagnostic algorithm: X ray venography is used only for the assessment of topographic extent of iliac and caval thrombosis, and when non invasive methods give equivocal results. As a result, the number of X ray venographies significantly decreased since 1984 (7) in Nîmes University Hospital. About 75% of patients with a deep venous thrombosis (diagnosed by non invasive methods) were treated without necessity of X ray venography.

Detection of new mutation disease in man and mouse.

Recent developments in molecular biology technology have greatly facilitated the methods for detection of mutations responsible for genetic disorders in both humans and animals. In this article we review some of these new methods and present a diagnostic algorithm that facilitates the routine and rapid diagnosis of any genetic disorder for which the defective gene has been isolated.

[The use of iridoscopy in prophylactic examinations].

To study the method of iridodiagnosis, clinical and iridoscopic examinations were conducted in 1104 patients and healthy persons, of them 516 men and 588 women, aged from 18 to 65 years. The contingent of the subjects represented 6 groups of patients with the most frequently observed pathology and a control group. The results obtained allow to consider that iridoscopy is indeed a sufficiently informative method for diagnostics of organ pathology without differentiation, in most of cases, between nosologic units, and achieves the highest effect in combination with detailed collected complaints and the anamnesis. The method can be used in prophylactic examinations of big contingents. The attempt to formalize iridoscopic data has shown that it is possible, in principle, to draw up a diagnostic algorithm, to describe it in one of programming languages and to work out an iridodiagnostic complex.
