Flow and image cytometric DNA ploidy, including 5c exceeding cells, of serous borderline malignant ovarian tumors. Correlation with clinicopathologic characteristics.

OBJECTIVE: To analyze DNA ploidy of serous borderline ovarian tumors by flow cytometry (FCM) and image cytometry (ICM), with 5c exceeding cells also analyzed, and to evaluate their correlation with clinicopathologic characteristics of patients and tumors. STUDY DESIGN: Cell suspensions were prepared according to a modified Hedley method from formalin-fixed, paraffin-embedded tissue blocks of 43 tumors. One part of the suspension was used for flow cytometric measurement; from the other part, filter slides were prepared for ICM. RESULTS: FCM and ICM found 2 aneuploid (peridiploid) serous borderline ovarian tumors, and FCM found 1. ICM found 3 tumors with 5c exceeding cells and 2 tumors with octaploid cells. There was no correlation between DNA aneuploidy and presence of 5c exceeding cells with tumor size, International Federation of Gynecology and Obstetrics stage or survival. CONCLUSION: The results confirm a good correlation between FCM and ICM DNA ploidy and the ability of ICM to detect 5c exceeding cells. The prognostic value of DNA ploidy and 5c exceeding cells in serous borderline malignant ovarian tumors warrant further evaluation.

Target organ status and cardiovascular risk in borderline hypertension.

In order to assess the prevalence of target organ damage 78 men with borderline hypertension, according to the World Health Organization criteria, and 67 normotensive controls underwent echocardiographic, electrocardiographic and fundoscopic examination, followed by ambulatory blood pressure monitoring for 24 hours. Echocardiographic left ventricular hypertrophy was found in 13 borderline hypertensive subjects (16.6%), while no electrocardiographic or fundoscopic abnormalities could be detected. Our data suggest that noninvasive assessment of cardiovascular status, including echocardiography, allows recognition of a subset of borderline subjects with an increased risk for subsequent cardiac morbid events, thereby improving prognostic stratification.

Total papillectomy for borderline malignant tumor of papilla of Vater.

The therapies for treating tumors of the papilla of Vater remain controversial because accurate preoperative diagnosis is difficult. Treatments include endoscopic resection, pancreaticoduodenectomy and total papillectomy. We report the case of a 69-year-old man who underwent total papillectomy for a borderline malignant tumor of the papilla of Vater. In our institution, the decision to perform a total papillectomy for borderline malignant adenoma is based on whether it is intestinal type or pancreaticobiliary type. Carcinoma of the papilla of Vater is classified into two types: an intestinal type and a pancreaticobiliary type and the prognosis of the intestinal type is much better than that of the pancreaticobiliary type. We suggest that total papillectomy can be performed for an intestinal, borderline malignant tumor.

Systolic time intervals and echocardiographic findings in borderline hypertension.

Systolic time intervals, echocardiographic dimensions and hemodynamic parameters were determined in 42 borderline hypertensive patients with high cardiac output and 33 normal subjects. In borderline hypertensive patients, the preejection period was significantly reduced (P less than 0.001) and was negatively correlated to cardiac index (P less than 0.001). The interventricular septum thickness (IVS) was significantly increased (P less than 0.001) while the posterior wall thickness (PW) was within normal values. The IVS/PW ratio was significantly elevated (P less than 0.001) and was correlated negatively with the cardiac index (P less than 0.01) and positively with the preejection period (P less than 0.01). These findings suggest that (i) myocardial hypertrophy and increased left ventricular performance exist in borderline hypertension, (ii) myocardial contractility is reduced as myocardial hypertrophy increases.

[Fertility of women after conservative operation for borderline ovarian tumors].

OBJECTIVES: The aim of the study was to assess procreation in group of patients who were treated by conservative operation for borderline tumors of the ovary. DESIGN: The analysis included 42 patients conservatively operated for ovarian tumor of borderline malignancy in Department of Gynecology Medical University of Gdansk between 1978-2000. The incidence of pregnancy, age of patients, tumor pathology, type of conservative surgery and the course of pregnancy and labour were evaluated in this study. RESULTS: In the analysis group were 36 (85.7%) stage IA, 2 (4.8%) stage IB, 3 (7.1%) stage IC and 1 (2.4%) stage III C patients. Unilateral adnexectomy was performed in 36 (85.7%) patients, 4 (9.5%) unilateral cystectomy, 2 (4.8%) bilateral cystectomy with omentectomy in one case. After conservative operation 10 (23.8%) patients were pregnant and delivered healthy children but 2 patients delivered twice and 1 third. Recurrence was observed in 2 patients in period of 27 and 50 months after operation. 5 years survival was 97.6%. CONCLUSIONS: Percentage of pregnancy after conservative treatment for borderline ovarian tumors was high (23,8%) and number of recurrences was low so conservative surgery allows young women to retain procreational potential without increasing risk of recurrence.

[Changes of profile prominence in borderline cases with extraction and non-extraction orthodontic treatment].

OBJECTIVE: To compare the treatment effect on the prominence of profile in borderline cases between extraction and non-extraction treatment. METHODS: The sample consisting of 33 borderline cases based on the judgment of 5 orthodontic specialists was divided into three groups according to the treatment way selected by the doctor in charge of the case. Three groups comprised 12 non-extraction cases, 13 four first premolars extraction cases and 8 four second premolars extraction cases. Structure superimposition was used to measure landmarks displacements which reflect the change of profile prominence before and after orthodontic treatment using pretreatment FH plane as a frame of reference. RESULTS: Only the prominence of upper and lower incisors showed statistically significance between the extraction and non-extraction treatments. There was no statistically significant difference between the extraction of four first premolars and second premolars. CONCLUSION: The main effect of extraction vs. non-extraction on profile of borderline cases is the prominence of upper and lower incisors, while their influences on upper and lower base bone and soft-tissue profile are not obvious.

[Interobserver reproducibility in the pathologic diagnosis of borderline ductal proliferative breast diseases].

OBJECTIVE: The aim of this study involving general pathologists was to assess interobserver reproducibility in the pathologic diagnosis of borderline ductal proliferative breast diseases. METHODS: Ten general pathologists independently reviewed 43 specimens chosen to represent the spectrum of borderline ductal proliferative breast lesions. All slides were blindly reviewed without given standardized criteria, and were classified as either mild usual hyperplasia, moderate-severe usual hyperplasia, mild atypical hyperplasia, moderate-severe atypical hyperplasia, ductal carcinoma in situ, or ductal carcinoma in situ with invasion. According to the years of training, these 10 general pathologists were divided into the experienced group and the less trained group. Interobserver agreement was statistically analyzed using Kappa statistic. Then, by comparing all the diagnoses of individual pathologist with the consensus opinion confirmed by two breast pathologists in terms of Page standard, we acquired the diagnostic accuracy and ascertained the undue diagnosis. RESULTS: The ten general pathologists' interobserver reproducibility in the diagnosis of borderline ductal proliferative breast diseases was rather low, especially that in their diagnosis of atypical hyperplasia. The reproducibility and accuracy were slightly higher in the experienced pathologists than in the less trained pathologists. Some pathologists made over-diagnosis or under-diagnosis of the lesions to different degrees. However, when the categories of diagnostic terms were simplified, the interobserver reproducibility increased. CONCLUSION: The use of standardized criteria is an important approach to increasing the diagnostic reproducibility and accuracy.

[Cephalometric comparison of soft-tissue morphology between extraction and non-extraction orthodontic treatment in borderline cases].

OBJECTIVE: To compare soft-tissue morphology changes by cephalometric measurements between extraction and non-extraction orthodontic treatment in borderline cases. METHODS: The samples consisted of 33 cases selected as borderline cases by 5 orthodontic specialists. They were divided into 21 extraction cases (including 13 four first premolar extraction cases and 8 four second premolar extraction cases) and 12 non-extraction cases by checking patients' treatment records. Conventional cephalometric analysis was made to compare soft tissue structures before and after orthodontic treatments and the same comparison was made between two different extraction patterns. RESULTS: No statistical difference was found in pretreatment soft-tissue morphology between extraction and non-extraction groups divided from borderline cases. The PosBs/FH of the four first premolars extraction group was smaller than that of non-extraction group, and the Ns-Sn-Pos of the four first premolars extraction group was smaller than that of four second premolar extraction group. None of the post-treatment soft-tissue measures showed significant statistical differences between four first premolars extraction group and non-extraction group, but there were 6 items showed significant statistical differences between four second premolars extraction group and non-extraction group. Compared with extraction and non-extraction treatments, the most significant soft-tissue changes were: PosBs/FH, LL-SnPos, and Bs-EP . CONCLUSION: Although pre-treatment soft-tissue morphology of second premolar extraction group was close to that of non-extraction group, the post-treatment soft-tissue morphology of first premolar extraction group became closer to that of non-extraction group. Compared with non-extraction treatment, the more significant changes caused by extraction treatment were located in the lower lip and chin, but not the upper lips.

[Radical surgical therapy and role of adjuvant chemotherapy in ovarian tumors of borderline malignancy].

OBJECTIVE: Evaluation of the benefit of the radical surgical therapy in the assessment of prognostic factors and indication of adjuvant therapy in patients with borderline tumors of ovary. DESIGN: Retrospective study. SETTING: Department of Obstetrics and Gynecology, University Hospital, Ostrava. METHODS: The group consists of 26 patients operated on our department, 12 of which underwent restaging operation after insufficient primary surgery in different hospital and 14 underwent primary surgery in our department. The average age was 42.9 years. RESULTS: Radical surgery with regional lymphadenectomy was performed in 18 cases (69%) and non-radical in 8 cases (31%). In 7 patients (27%) fertility preservation operation was done. According to histological results 19 tumors (73%) were of serous type, 7 being of mucinous type (27%). There were no other histological types in our group. The lymphnodes were positive in 4 cases (22%) out of 18. Eight patients were indicated for adjuvant chemotherapy (31%), 4 of them for regional lymphnodes positivity, 2 for peritoneal implants, 1 patient for tumor residuum. The last case was a patient with tumor duplicity of a borderline tumor of ovary stage FIGO IIC and corporal carcinoma stage FIGO IB. We had one case of recurrence after fertility preservation surgery. There was no case of pregnancy yet. CONCLUSIONS: Borderline tumors of ovary have good prognosis and low recurrence rate. In spite of that, there exists a group of women with higher risk of recurrence and disease progression which can be fatal. These patients may benefit from adjuvant therapy, which is indicated after evaluation of risk factors ideally acquired from the radical surgery.

[Clinical analysis of patients with recurrences of borderline ovarian tumors including surgical treatment].

OBJECTIVES: Risk of recurrences of borderline ovarian tumors is low (7-10%) but the cases with incidence of recurrence have worse prognosis. The time between primary operation and the first diagnosis of recurrence and treatment is different. DESIGN: The purpose of the current study was to evaluate frequency and the methods of treatment patients with recurrences of borderline ovarian tumors. MATERIALS AND METHODS: The analyzing group consists of 129 patients with borderline tumors of the ovary (BTO) diagnosed and treated in Department of Gynecology between years 1978-2000. RESULTS: In our recurrences developed in 7% of patients. The recurrences in BTO were connected with stage of BTO, types of primary operations and residual disease. Recurrences developed in 5,9% patients with stage I and in 18,1% patients with stage II/III. Similarly recurrences developed in 5,8% patients with no residual disease and in 20% patient with residual disease. A higher relapse rate was observed in patients treated conservatively but recurrences was quickly recognized and efficaciously treated. 5 years survival of patients without recurrences was 91,7% and with recurrences was 72,9%. CONCLUSIONS: 1. Risk of recurrences of BTO is low (7%) and depends on stage and residual disease. 2. Risk of recurrences is higher after primary conservative operation but long follow-up and regular gynecological examinations allows to early diagnosis and treatment of recurrences. 3. Prognosis of patients with recurrences of BTO is good. 5-years survival was 90,6% of group without and 72,9% of group with recurrences of BTO.

Management of borderline personality disorder: a review of psychotherapeutic approaches.

There are currently three major psychotherapeutic approaches to the management of borderline personality disorder (BPD): the psychodynamic, the cognitive-behavioral, and the supportive. There are special varieties within each: e.g., transference-focused psychotherapy (psychodynamic) or dialectic behavioral therapy (cognitive-behavioral). Though differing in basic conceptions and in methodology, all approaches aim at the amelioration of both the symptom-aspects that dominate the clinical picture at the outset, and the personality difficulties that remain apparent after the symptoms have been alleviated. The term "management" implies a focus on the more serious aspects of the borderline picture. These can be pictured hierarchically as to their level of seriousness, and there is universal agreement about the nature of this hierarchy. Therapists must pay attention first to suicidal and self-mutilative behaviors. Next, one deals with any threats to interrupt therapy prematurely. Third in order of seriousness: non-suicidal symptoms such as (mild to moderate) depression, substance abuse, panic and other anxiety manifestations, or dissociation. Psychopharmacological treatment will often be used adjunctively to help control any target symptoms, which usually fall into such categories as cognitive-perceptual, affect dysregulation, or impulsive/ behavioral dyscontrol. Therapists must then be alert to any signs of withholding, dishonesty, or antisocial tendencies, since these have an adverse effect on prognosis. When all these disruptive influences are (to the extent possible) dealt with, therapists will next take up milder symptoms such as social anxiety or lability of mood. Throughout this initial process, the personality-disorder attributes of BPD will become more apparent, and will usually emerge with greater clarity, once the serious symptoms have been dealt with. The management issues will gradually be supplanted with the overlapping and enduring personality issues: inappropriate anger, abrasiveness, manipulativeness, demandingness, jealousy, "all-or-none" thinking and the extreme attitudes (idealization/devaluation) that accompany such thinking, masochistic traits, etc. Under ideal circumstances, the borderline patient will have "graduated" toward a higher level of function, where (acute) management issues have been adequately dealt with or have receded into the background. Psychotherapy, individual and group, becomes the dominant intervention, with such goals as psychic integration, skills training, and the fostering of long-range ambitions relating to friendships, partner choice, and work.

Effect of treatment of borderline hypertension on microalbuminuria in non-insulin-dependent diabetes mellitus.

The advantage of treatment for borderline hypertension has been a debate. We studied two of the commonly used antihypertensive drugs, i.e., trichlormethiazide (Fluitran 2 mg/tab) and enalapril (Renitec 5 mg/tab) on urinary albumin excretion in seven NIDDM subjects with borderline hypertension, who had never been treated with antihypertensive drugs before entry into this study. The observation period was 2 months, and the treatment period was 6 months. Trichlormethiazide (1 tab qd) or enalapril (1 tab qd) were randomly assigned for the first 3 months and then patients were switched to the other drug for the following 3 months. During the treatment period, blood pressure, body weight, blood chemistry including renal function tests, and urinary albumin excretion rate were regularly followed up every 1 to 3 months. The results showed that both of the regimens significantly lowered blood pressure and the urinary albumin excretion rate [12.72 (2.56-25.95) micrograms/min at baseline, to 5.11 (3.0-13.73) micrograms/min during trichlormethiazide treatment and 4.96 (1.38-11.14) micrograms/min during enalapril treatment, p less than 0.008]. However, no significant difference was noted between the two drugs. The magnitude of change in the urinary albumin excretion rate did not correlate with the changes in blood pressure. Renal function, glycemic control, and lipid profiles did not change significantly during the treatment period. In conclusion, both trichlormethiazide and enalapril are effective in lowering the urinary albumin excretion rate in NIDDM subjects with borderline hypertension.

[Impact of surgical access on staging of early borderline and invasive tumors of the ovary].

The objectives were to evaluate the impact of the surgical approach on the staging of borderline tumors and early-stage malignancies of the ovary. We retrospectively reviewed cases of borderline and invasive ovarian tumors stages Ia through Ic treated surgically between January 1, 1985 and December 31, 2001. We compared the rates of potentially harmful procedures according to the surgical approach. The quality of surgical staging was assessed by examining each required procedure and by determining a score. The influence of variables related to patient characteristics, preoperative workup, and initial surgery on staging quality was tested by univariate analysis. Variables independently associated with staging quality were entered in a logistic regression model. SPPS 7.5 and STATA 8 software was used for statistical tests. Of 118 patients with borderline tumors, 48 (41 %) underwent laparoscopic surgery, 54 (45 %) laparotomy, and 16 (14 %) laparoscopy converted to laparotomy. Surgery was conservative in 57 % of cases overall ; this proportion was significantly greater with laparoscopy than with laparotomy (p < 0.05) and in younger women (p < 0.001). Intraoperative tumor rupture occurred in 9 % of patients, with no significant difference across surgical approaches (p = 0.1). Bag extraction was used in 19 (40 %) of 48 laparoscopically treated patients. Staging was incomplete in 73 % of patients. By univariate analysis, bilateral adnexectomy and, to a lesser extent, age > 44 years, laparotomy, hysterectomy, and treatment after 1995 predicted at least partial staging. Factors independently associated with at least partial staging in the multivariate model were treatment after 1995, bilateral adnexectomy, and hysterectomy. Of 178 patients with invasive tumors, 34 underwent laparoscopic surgery, 114 laparotomy, and 30 conversion from laparoscopy to laparotomy. The laparotomy group was characterized by significantly older patient age and larger tumors, compared to the laparoscopy group. Staging was often inadequate after initial surgery, most notably with low rates of paraaortic lymphadenectomy (0 % in the laparoscopy group, 18 % in the laparotomy group, and 33 % in the conversion group). Staging is often less complete with laparoscopy than with laparotomy. In patients with invasive cancer, inadequate initial staging is common, most notably when laparoscopy is used. The surgeons training seems of major importance.

Plasma alpha-L-fucosidase activity and the risk of borderline epithelial ovarian tumors.

The authors obtained venous blood samples from 106 female residents of western Washington in whom serous and mucinous borderline ovarian tumors had been diagnosed between 1980 and 1985 ("cases") and from a randomly selected telephone sample of 134 control women from the same counties. Mean activity in plasma of alpha-L-fucosidase was slightly higher in cases (334 units/ml) compared to controls (306 units/ml). The risk for a serous borderline tumor was virtually the same for women in all four quartiles of alpha-L-fucosidase activity. Women in the three lowest quartiles of activity were at 50-70% the risk of women in the highest quartile for the development of a mucinous tumor, but within these three quartiles there was no trend of decreasing risk with decreasing activity. These results suggest that for most women plasma alpha-L-fucosidase activity has little or no bearing on the risk of borderline ovarian tumors. Whether such an association exists in women genetically predisposed to these tumors could not be assessed.

Home blood pressure readings in borderline hypertensive patients.

Home blood pressure monitoring can provide valuable information for physicians managing borderline hypertensive patients. This study was conducted to compare office and home blood pressures in 36 borderline hypertensive subjects, and to determine the accuracy of the home monitoring unit used. The patients were very willing to record their home blood pressures for an extended period of time. The home blood pressure monitors were found to be quite accurate, and blood pressures measured at home were significantly lower than readings obtained in the office. In 39% (14 of 36) of the subjects studied, the average home blood pressures were more than 10 mmHg lower than their office readings. These findings support the hypothesis that home blood pressure monitoring can be useful in the management of borderline hypertensive patients.

[A serous cystadenoma of the ovary of borderline malignancy with a fifteen-year history. A case report].

A case of a serous cystadenoma of a ovary of borderline malignancy is reported. Sixteen years earlier, the patient had undergone an exploratory laparotomy because of ovarian tumor, and the histologic diagnosis had been a serous cystadenocarcinoma. Postoperative chemotherapy was not effective and drainage of the tumor fluid had been performed for 15 years, with the estimated drainage volume estimated to have reached, 1,000 1. Gradual malnutrition and marked tumor growth then become apparent. A reevaluation of the initial histologic slides and her clinical course strongly suggested a serous cystadenoma of borderline malignancy. Thus a tumor resection, a bilateral salpingo-oophorectomy, and a hysterectomy was performed. A histologic diagnosis of a resected specimen confirmed a serous cystadenoma of borderline malignancy and the histologic features were quite similar to those of the initial biopsy specimens. The patient is living well postoperatively for 8 months without postoperative chemotherapy.

[Increased vasodilation response to the stimulation of low pressure mechanoreceptors in borderline hypertension].

Passive lifting of the lower limbs (antitilt) is a simple manoeuvre which stimulates low-pressure mechanoreceptors. Patients with borderline hypertension have an alteration of the arterial baroreflex modulated by an increased inhibitory effect of cardiopulmonary mechanoreceptors on vasomotor centres. The repercussions of antitilt on mean arterial pressure (MAP), heart rate (HR) and blood flow rate in the forearm (FRFA) were studied in 30 male subjects aged 18 to 25 years: 14 with untreated borderline hypertension ("patients") and 16 controls. FRFA was measured with a plethysmograph by means of a mercury constraint gauge. MAP was recorded during exploration with a DINAMAP instrument. The parameters were measured in recumbent position, then within minutes of the antitilt manoeuvre. In recumbent position the differences between patients and controls were: MAP 94.7 +/- 6 vs 84.7 +/- 7 mmHg (p less than 0.001); HR 76 +/- 13 vs 63 +/- 9 beats/min (p less than 0.01); FRFA 4.6 +/- 1.5 vs 3.5 +/- 1.4 ml/min/100 ml (p less than 0.05). Following antitilt, MAP and HR remained unchanged in the two groups, but there was a significant increase of FRFA in both controls (+ 0.6 +/- 0.2 ml/min/100 ml; p less than 0.02) and patients (+ 1.3 +/- 0.2 ml/min/100 ml; p less than 0.001). This increase was significantly greater in patients than in controls (p less than 0.05). This study shows that antitilt produces an increase of blood flow rate in the forearm that is more pronounced in patients with borderline hypertension than in controls. The stability of MAP and HR is in favour of non-actuation of the high-pressure arterial baroreflex.(ABSTRACT TRUNCATED AT 250 WORDS)

[Results of blood pressure measurements depending on the place and method of measurement in individuals with normal blood pressure and borderline hypertension].

The analysis of systolic (SBP) and diastolic (DBP) blood pressure measurements in 2 positions (sitting, standing) X 2 subsequent measurements (order) X 2 places (right, left arm) was performed on the basis of the Pol-Monica I screening conducted in 1984 yr. There were 1309 examined men and 1327 women aged 35-64 yrs. The mean values of 2 measurements taken in the sitting position on the right arm were used for division of the total sample into 7 categories: 1. normotension, 2. borderline systolic hypertension, 3. borderline diastolic hypertension, 4. borderline systolic-diastolic hypertension, 5. systolic hypertension, 6. diastolic hypertension, 7. systolic-diastolic hypertension. For each sex X blood pressure category the 4-way mixed ANOVA was calculated: persons as random effect and position, order and place as fixed effects. The effects and variance components were estimated and tested. The effects of position, order and persons and interaction position X persons, places X persons and position X persons X places were significant for both sexes, all BP categories for SBP as well as DBP measurements.
