[Mandatory or voluntary vaccinations? Juridical and medico-legal aspects].

The Italian juridical and legislative aspects of vaccination based on a compulsory system are presented considering the medico-legal questions. The prospective of a voluntary system, as anticipated by many normatives and other official documents, is examined. The experience of some local health authorities on cases of in observance of the vaccinal obligation are detailed Finally professional risks for vaccinal operators are examined and the need of specific guidelines for parents' consent to compulsory vaccination during infancy is prospected.

Drug treatment court opens in British Columbia.

British Columbia has launched its first drug treatment court. Designed as a mandated addiction treatment alternative to incarceration, the court will attempt to steer drug users charged with drug-related offences away from jail and into rigorous supervised addiction treatment. While some have applauded this recent development, local experts have criticized the move, citing the lack of evidence supporting the efficacy of drug courts and the need for more voluntary treatment programs.

Voluntary counselling, HIV testing and adjunctive cotrimoxazole are associated with improved TB treatment outcomes under routine conditions in Thyolo District, Malawi.

SETTING: Two rural districts in Malawi: Thyolo, where voluntary counselling and human immunodeficiency virus (HIV) testing (VCT) is offered to all tuberculosis (TB) patients and adjunctive cotrimoxazole to HIV positives, and Mulanje, where no such interventions are offered. OBJECTIVES: For all TB patients registered in 2001: 1) to determine the uptake of VCT and cotrimoxazole in Thyolo, and 2) to compare treatment outcomes between Thyolo and Mulanje. DESIGN: A cohort study using routinely collected programme data. RESULTS: There were 1239 TB patients in Mulanje and 1103 in Thyolo. In Thylo, 1064 (97%) patients consented to VCT, 1006 were HIV tested (91%) and 761 (69%) were started on cotrimoxazole a median of 4 days from registration; 77% of patients tested in Thyolo were HIV-positive. For all TB patients, in Thyolo and Mulanje, treatment success was respectively 75% and 61% (P < 0.001); death was 21% and 25% (P = 0.026); and other outcomes were 4% and 14% (P < 0.001). The adjusted relative risks of treatment success (1.23), death (0.84) and other outcomes (0.26) in Thyolo were significantly different from those in Mulanje (P < 0.001). CONCLUSION: VCT and adjunctive cotrimoxazole is well accepted by TB patients in Thyolo and, with other HIV care and support services, is associated with good treatment outcome indicators for the National Tuberculosis Programme. This intervention is being expanded to other districts in Malawi, and other African countries should consider a similar approach to the dual HIV-TB epidemic.

Characteristics of volunteers and non-volunteers for voluntary counseling and HIV testing among unmarried male undergraduates.

The 2001 HIV sero-prevalence survey in Nigeria revealed a rate of 5.8 percent with those under the age of 25 years having the highest prevalence rate. Most University students fall within this age group. This study is part of a larger study on the sexual behavior of youths and young adults and was designed to compare the characteristics of volunteers and non-volunteers for voluntary confidential counseling and HIV testing (VCT) among males. Six hundred and nine male undergraduate students were randomly selected and enrolled for the study. Data were collected using a pre-tested questionnaire. Of the 609, 51 (8.3%) volunteered to have their blood screened for HIV. All volunteers who received pre-test counseling went for the HIV test. Volunteers were significantly older than the non-volunteers (P<0.0001), and were more likely to be sexually experienced (P=0.002). Among the sexually experienced, the volunteers were older at first sexual intercourse (FSI) (P<0.0001), and were more likely to have used a condom at FSI (P=0.001). Volunteers had significantly higher knowledge scores for HIV/AIDS (P=0.006), and the attitude to HIV/AIDS in both groups was positive. The marriage pattern of their parents with regard to polygyny was similar, and fewer volunteers had fathers in the higher socio-economic class and mothers who had completed secondary education (P<0.00001, (P=0.02). Among the 51 volunteers, 8 (15.7%) tested positive. Those who tested positive were less likely to have lived with parents, and were all sexually experienced. Those who screened positive were also more likely to be currently sexually active and to have fathers with low level of education. Three (5.9%) of volunteers did not return for results and posttest counseling. One of the three was positive for HIV. Of those who tested positive, 3 (37.5%) reported not using the condom at all, while the rest were using it only occasionally. VCT among the youths is possible however, small numbers encountered in the study is a limitation and there is a need to replicate this study using larger numbers. Tertiary institutions should provide VCT services for the students where they can be counseled appropriately and continuously throughout their stay in the institution. This hopefully will reduce the number of new HIV cases seen.

Acceptance rate of HIV testing among women seeking induced abortion in Benin City, Nigeria.

Although the campaign against HIV has been intensive, it has been focused mainly on the public health implications. Little or no attention is specifically directed at encouraging the public to know their HIV serostatus. The actual public health burden due to this infection is therefore not known. Accurate data on the health burden is critical for national planning on preventive and other interventional measures. Using a semi-structured self-administered questionnaire, we interviewed 1051 women seeking induced abortion in four randomly selected private clinics in Benin City between January and September 2002. The blood samples of those who accepted HIV testing were collected and screened at the University of Benin Teaching Hospital. The women were aged between 16 and 46 years (mean 31.1+/-6.7 years). One hundred and thirty seven women (13%) accepted HIV testing. Of the 1051 women, 1001 (95.2%) had multiple sexual partners and 722 (68.8%) regularly practiced unprotected sex. All were aware of HIV/AIDS. The older women were more likely to accept voluntary HIV testing than the younger ones. Acceptance rate for voluntary HIV testing was low in both the employed and unemployed and much lower among full time housewives (p = 0.01). We therefore recommend that HIV campaign programmes should include strategies aimed at arousing public interest in voluntary HIV testing.
