Training program focuses on developing managers' skills.

The movement toward skill development and behavior modification in the training and staff development field is just beginning to affe(t management training in hospitals. The training model described in this article is adaptable to a variety of tasks involving the management of people.

New industrial training program for interns and postdoctoral fellows.

Pharmaceutical industry research managers consistently have difficulty finding job candidates trained to address industrial research projects. The current situation is even worse than usual because of high demand for research scientists, particularly those trained in solid phase synthesis, computational chemistry, and informatics. Network Science Corp., Chemical Computing Group, and pharmaceutical research groups are creating a website to assist students in gaining experience as industrial interns or postdoctoral fellows.

Evaluation from start to finish: Incorporating comprehensive assessment into a training program for public health professionals.

This article examines the role of a comprehensive evaluation process in the context of a program to enhance use of the Internet by public health professionals. The Public Health Information Link for Nevada incorporated training and Web site development into a program to raise levels of awareness and competence of the public health professional. Evaluation played a key and ongoing role in the planning and implementation phases and made a marked contribution to the overall effectiveness of the project. Evaluation contributed to the ability to make midprogram adjustments, thereby enhancing user satisfaction and confidence. Participant evaluations indicated significant improvement in self-perceived competence in utilizing the Internet for job-related activities. The authors conclude that the effort to integrate evaluation in an ongoing manner is balanced by the value of continuous feedback. A Web site review checklist, which will be useful to public health professionals who are developing new Web sites, is appended.

The impact of a transitional training program for young adults with learning disabilities.

A study was conducted in 1988-1990 at Lesley College's (Cambridge, Massachusetts) Threshold Program, one of the few campus-based, comprehensive transition programs in the country for young adults with learning disabilities functioning in the low-average intellectual range. The goal of the study was to assess the ability of program graduates from the classes of 1984 to 1987 to successfully manage the demands of independent living and competitive employment after intensive training in Threshold's 2- to 3-year program. From a total of 71 graduates contacted, 45 participated in the study, including 34 females and 11 males. The subjects ranged in age from 21 to 31 with the average age being 24. Information was gathered from all subjects through written questionnaires and personal or telephone contact. Small-group interviews were conducted with a sample of eight subjects. Factors contributing to the stability of competitive employment and independent living, support systems most commonly used, and specific impacts of the Threshold program on the lives of its graduates were studied. The data revealed that 61% of the respondents were currently employed in one of Threshold's fields of training, 52% had held their jobs for at least 1 year, and 42% (excluding cohort '87) held their jobs for at least 2 years. Seventy-five percent were living independently and 84% of those had maintained an apartment for more than 1 year. Subjects found compatibility with roommates and money management to be the biggest challenges. They perceived themselves as being quite independent and felt they would not be as successful without the opportunity of this program model.

Experience with diagnostic laparoscopy in a hepatology training program.

Laparoscopy is an underutilized endoscopic procedure in the United States even though it has proven to be an important diagnostic modality. Among factors that may account for the unpopularity of this procedure are lack of experience and proper training in many gastroenterology fellowship programs, a perceived notion that there is a relatively high complication rate, and the preference for radiologic imaging techniques for establishing a diagnosis. We reviewed, over an 11-month period, our experience with laparoscopy in a large teaching center to better assess its safety when performed by trainees. The major and minor complication rates were 0.9% and 2.9%, respectively, with no fatalities. These findings compare favorably with the general experience.

Effects of a short-term circuit weight training program on glycaemic control in NIDDM.

This study assessed the effects of short-term circuit weight training (CWT) on glycaemic control in NIDDM. Twenty-seven untrained, sedentary subjects (mean age, 51) with NIDDM participated in an 8-week randomised, controlled study, involving either CWT 3 days/week (n = 15) or no formal exercise (control) (n = 12). All subjects performed regular self-blood glucose monitoring throughout. Fasting serum glucose and insulin were measured following a 12-h fast and during an oral glucose tolerance test (75 g) before and after 8 weeks. Twenty-one subjects completed the study (CWT, n = 11) (Control, n = 10). Strength for all exercises improved significantly after CWT. Pooled time-series analysis, using a random effects model, revealed an overall decrease in self-monitored glucose levels with CWT compared to controls. Significant reductions from baseline values were observed in both the glucose (-213 mmol l-1 per 120 min, P < 0.05) and insulin (-6130 pmol l-1 per 120 min, P < 0.05) area under the curve following CWT relative to controls. After adjustment for body mass changes, the change in self-monitored glucose levels and insulin area under the curve, but not glucose area under the curve, remained significant. Short-term CWT therefore may provide a practical exercise alternative in the lifestyle management of this condition.

Survey of infection control training program graduates: long-term care facility and small hospital practitioners.

Graduates of a 2-day basic training course in infection control were surveyed. Respondents were generally from Midwestern long-term care facilities and small hospitals. These infection control practitioners had multiple roles in addition to infection control, most notably employee health and quality assurance. Infection control practitioners demonstrated significant job stability. The vast majority of institutions where survey respondents were employed followed recommended infection control practices.

Psychiatric residents' experience and career interest in geriatric psychiatry: implications for training programs.

A survey of residents' experience and career interest in geriatric psychiatry was carried out in Canadian training centres. Residents' exposure to specific training experiences in geriatric psychiatry has been steadily increasing, although a large proportion of residents continue to report insufficient amount of experience and supervision in this area. Residents viewed geriatric psychiatry as involving special skills and knowledge and half were in favour of a compulsory rotation in this area. Geriatric psychiatry was an infrequent career choice along with other areas of psychiatry that have had difficulty in recruiting. Alternative approaches to service provision and recommendations for meeting the manpower needs are discussed.

Epidemiology of injuries and illnesses during the United States Air Force Academy 2002 Basic Cadet Training program: documenting the need for prevention.

The rapid physical and emotional conditioning that occurs during Basic Cadet Training (BCT) creates ample opportunity for injuries and illnesses. The goal of this analysis will be to document the magnitude of injury and illness problems during BCT at the Air Force Academy and to identify potential risk factors for targeted prevention efforts. Data used in this study were collected on the basic cadet population from various sources during BCT 2002. Data analysis for this retrospective observational study included cumulative incidences, chi2 analysis, and logistic regression. Risk factors for injuries included being of the female gender, being an intercollegiate athlete, being overweight, and having an obese body mass index (BMI). Female gender, military preparatory school attendance, and obese BMI were risk factors for illnesses. Future BCT medical surveillance should focus on females, recruited athletes, and those with high BMIs. Prevention efforts should focus on reducing ankle sprains and respiratory infections to limit the amount of lost training time.

[Well-designed training programs crucial for dialysis patients].

For many reasons patients on hemodialysis treatment have a low physical capacity. In this study the physical capacity was tested in 41 patients with chronic renal failure during their first weeks on hemodialysis and after 18 months of treatment. Compared to reference values the test result showed a significantly decreased physical strength, endurance and balance capacity already at the start of the dialysis. The patients were tested and got individually tailored information at the kidney unit and received some physical training outside the hospital. After 18 months the result for the group was not improved in spite of the fact that the 11 most frail patients had died. Diabetic and elderly patients had a significantly lower result than the rest of the patients. Future research will show, if more regular exercise (predialytic) individually programmed by a specially trained physiotherapist who can obtain up-to-date medical information as working in close contact to a dialysis unit, will further improve the condition of these patients.
