Salsalate, morphine, and postoperative ileus.

BACKGROUND: Previously, we demonstrated that ketorolac, a nonsteroidal antiinflammatory drug (NSAID), prevented postoperative small bowel ileus in a rodent model. The aim of this study was to evaluate the effect of salsalate, an NSAID without antiplatelet effect, on postoperative ileus alone or in combination with morphine. METHODS: Forty-eight rats underwent placement of duodenal catheters and were then randomly assigned to one of eight groups (n = 6). Four groups had standardized laparotomy following drug administration, whereas 4 groups underwent the same treatment without laparotomy: control and morphine animals received 0.1 mL alcohol via the catheter, whereas salsalate and salsalate-plus-morphine animals received salsalate (15 mg/kg) dissolved in 0.1 mL alcohol. The animals also received 0.5 mg/kg morphine (morphine and salsalate plus morphine) or the same volume of saline (control and salsalate) subcutaneously. Transit was measured following the injection of a nonabsorbed marker via the duodenal catheter and is defined as the geometric center (GC) of distribution. An additional 20 rats had serosal electrodes placed on the jejunum, and were assigned to one of four treatment groups (control, salsalate, morphine, and salsalate plus morphine; n = 5 each group). Myoelectric activity was recorded until the reappearance of the migrating myoelectric complex (MMC) following laparotomy. RESULTS: Laparotomy and morphine independently reduced small bowel transit (P = 0.0006 and 0.006, respectively, by three-way analysis of variance [ANOVA]; GC 4.3 +/- 0.2 control versus 2.2 +/- 0.3 laparotomy versus 3.6 +/- 0.4 morphine), but morphine did not further worsen postoperative transit (GC 2.4 +/- 0.4; P = 0.42). Although salsalate did not alter baseline transit, pretreatment improved postoperative transit (P = 0.0002; GC 3.6 +/- 0.4). This effect was lost with the addition of morphine (GC 2.7 +/- 0.2; P = 0.21). The MMCs returned earlier after laparotomy in salsalate-pretreated rats (63 +/- 18 minutes salsalate versus 160 +/- 12 minutes laparotomy; P < 0.01, one-way ANOVA). However, this effect was also lost in animals receiving morphine (106 +/- 16 min; P > 0.05). CONCLUSION: Salsalate improves postoperative small bowel motility in a rodent model; however, this effect is masked by morphine.

Inflammatory responses induced in mice by lectin from Talisia esculenta seeds.

A novel lectin from Talisia esculenta seeds (TEL) has recently been purified and characterized. In this study we investigated the proinflammatory activity of TEL in mice using both the air-pouch and peritoneal cavity as well as paw oedema models. TEL (10-40 microg) induced significant neutrophil and mononuclear cell recruitment when injected into either mouse air-pouch or peritoneal cavity. The neutrophil accumulation into the air-pouch was dose- and time-dependent with a maximal response at 16 h, returning to control levels at 72 h whereas maximal mononuclear cell accumulation was observed at 24 h after TEL injection. The same profile of neutrophil accumulation was observed when this lectin was injected into mouse peritoneal cavity, although the maximal mononuclear cell recruitment was observed 48 h after TEL injection. Additionally, TEL (12.5-200 microg/paw) caused a dose-dependent mice paw, as evaluated at 4 h after the lectin injection. D-mannose, better than D-glucose, significantly inhibited TEL-induced neutrophil migration into the peritoneal cavity or air-pouch. D-galactose had no effect on TEL-induced neutrophil migration in either cavity studied. On the other hand, D-mannose slightly inhibited the TEL-induced paw oedema, whereas neither D-glucose nor D-galactose affected this phenomenon. In conclusion, our data show that TEL induces neutrophil and mononuclear cell accumulation by a mechanism related to their specific sugar-binding properties.

Matrix metalloproteinase expression in the olfactory epithelium.

The olfactory epithelium contains neuronal progenitor cells capable of continuous neurogenesis and is a unique model for studying neural degeneration, regeneration, axon outgrowth and recovery from injury. Matrix metalloproteinases (MMPs), and tissue inhibitors of metalloproteinases (TIMPs), have been implicated in cell turnover, development, migration, and metastatic processes. We used Western blot and immunohistochemistry to determine whether MMP-2 and associated proteins TIMP-2 and membrane type 1 matrix metalloproteinase (MT1-MMP) are present in the olfactory epithelium of mice. We found MMP-2 expression localized to the olfactory basal cells and immature neurons. After injury-induced neural degeneration, MMP-2 and MT1-MMP levels decreased while TIMP-2 levels increased. However, following 35 days of neurogenesis and cell replacement TIMP-2 and MT1-MMP returned to control levels. The results show a correlation between MMP and TIMP levels and the stages of neural degeneration, regeneration and recovery of the olfactory epithelium following injury.

Changes in the intragastric distribution of Helicobacter pylori during treatment with omeprazole.

Omeprazole is a powerful inhibitor of gastric acid and may suppress Helicobacter pylori by effecting the pKa of H pylori urease, by altering the pattern of infection, or by promoting overgrowth of other bacteria. At routine endoscopy H pylori was detected by histology and culture before and after four weeks' treatment with omeprazole, 40 mg each morning. A 13C-urea breath test was also done at t = 0, 2, 4, and 6 weeks. Thirty nine patients with duodenal ulcer (n = 25) or reflux oesophagitis (n = 14) were studied, of whom 29 of 39 had H pylori infection. During omeprazole treatment, 13C-urea breath test values fell significantly--mean (SEM) values before treatment and at four weeks were 23.0 (2.1) and 15.5 (2.7) per mil respectively, p < 0.001. Before treatment H pylori was seen in 28 of 29 antral, 29 of 29 corpus, and 28 of 29 fundic biopsy specimens. After four weeks of omeprazole treatment, the histological density of H pylori in the antrum and corpus was reduced (p < 0.001), while that in the fundus was increased. The migration of H pylori from the antrum to the fundus was associated with a corresponding decrease in the activity of antral gastritis. H pylori was not seen in antral biopsy specimens from 12 of 29 patients whose median excess delta 13CO2 excretion fell from 23.0 to 9.9 per mil. In the body mucosa, 26 of 29 specimens were still positive for H pylori and there was no significant change in the gastritis type. Two weeks after finishing treatment, the mean (SEM) excess delta 13CO2 excretion returned to levels before treatment. Omeprazole decreases antral H pylori colonisation but increases that in the fundus. The changes in the intragastric distribution of the organism are associated with concomitant changes in the activity of gastritis and are matched by a progressive fall in the excretion of delta 13CO2.

[Study of behavioral and histological change in mice following olfactory bulbectomy].

Behavioral and histological changes in mice following bilateral olfactory bulbectomy were studied. Mice were trained to discriminate between a 0.01% Cycloheximide solution and distilled water. After olfactory bulbs were removed, discrimination was lost, and had not returned 300 days after bulbectomy. The histological changes observed by light microscope were as follow. Degeneration of olfactory epithelium was observed immediately after the bulbectomy, followed by decrement of the epithelial thickness and the number of olfactory cells. An increase in both epithelial thickness and the number of olfactory cells was observed 14 days after the bulbectomy, and epithelial thickness 300 days after the bulbectomy was similar to that of the sham-operation group. At 300 days after the bulbectomy, axons of olfactory cells migrated through the lamina cribrosa, but didn't contact the forebrain. In this study, the olfactory bulb was considered to have played a role in functional recovery of olfactory behavior, and that olfactory cells were continuously renewed under conditions of target organ, i.e. olfactory bulb, loss.

[The brain drain: theoretical framework and hypotheses].

"This article studies the 'brain drain' issue from the sociological perspective. It analyzes the various reasons why students from developing countries go to North American and European universities and why they decide either to return or not to return to their own country after graduation. The author...concludes that, although there are political, socioeconomic and academic factors, one's decision to return or not to return to one's country depends mainly on individual aspirations (personal and cultural development) and collective aspirations (perceived possibilities of playing a constructive role, political or otherwise, in one's native country)." [Percutaneous electrogastrography in the perioperative period in laparoscopic gastric banding at the First Surgical Clinic of the General Faculty Hospital in Prague].

Percutaneous electrogastrography (EGG) is a method which makes non-invasive follow-up of the myoelectric gastric activity possible. The authors present the results of assessment of the myoelectric gastric activity during the perioperative period in patients undergoing, at the First Surgical Clinic General Faculty Hospital, laparoscopic non-adjustable gastric banding. The data were recorded using a Microdigittrapper and evaluated using spectral analysis and Fourier's transformation. The authors mention also briefly conditions which must be met to ensure the study, and the physiological and pathophysiological background of the method. In 14 patients the ECG record was made 24 hours prior to surgery and then 5 hours, 24 and 48 hours after surgery on fasting and after a fluid bolus. The assembled results of the dynamics of perioperative changes on the EGG were compared with results obtained in a group of healthy volunteers. The most frequent finding during the early postoperative period was bradygastria, the return to preoperative values occurred on average 48 hours after surgery. Percutaneous electrogastrography is a non-invasive method which can provide the surgeon during the early postoperative period with information on myoelectric gastric activity.

Emigration and wages in an open economy: some evidence from Pakistan.

"This paper examines the impact of labour emigration on the wages of both the skilled and unskilled workers....Using... Pakistani data, it is found that unskilled labour is used extremely intensively in the agriculture sector (exportable), skilled labour is used extremely intensively in the manufacturing sector (importable), and capital is used as the middle factor in both the traded goods sectors. Moreover, capital is used significantly less intensively in the construction (non-traded) sector relative to both the traded sectors.... The results suggest that the higher wages to both the skilled and unskilled workers must be compensated by a reduction in the rate of returns to capital if export-oriented and import-competing sectors in Pakistan are to remain internationally competitive."

The role of nonsteroidal anti-inflammatory drugs in the treatment of acute soft tissue injuries.

OBJECTIVE: Nonsteroidal anti-inflammatory drugs (NSAIDs) are frequently used to hasten the return of injured athletes to competition after injury. Evidence demonstrates that while NSAIDs may speed recovery after acute soft tissue injuries, long-term healing may be compromised. This review aims to assess the effects of NSAIDs on inflammation and healing associated with acute soft tissue injury. DATA SOURCES: CINAHL (1982 to 1997), SPORT Discus (1977 to 1997), and MEDLINE (1993 to 1997) were searched using the keywords "NSAIDs," "musculoskeletal," "acute," "sprain," and ";strain." DATA SYNTHESIS: NSAIDs exhibit anti-inflammatory effects via prostaglandin inhibition, neutrophil migration suppression, and oxygen free-radical inhibition. Retardation of inflammatory processes after acute injury may limit the area of secondary tissue damage but may also retard healing. Animal models have demonstrated short-term benefits with NSAIDs after acute injury, along with long-term adverse effects on tissue structure and function. NSAIDs have exhibited few benefits in the treatment of delayed-onset muscle soreness. Clinical trials of NSAIDs in the treatment of acute soft tissue injuries have shown conflicting results and have been highly criticized. CONCLUSIONS/RECOMMENDATIONS: Based on the research literature, the short-term benefits of NSAIDs in the treatment of acute soft tissue injuries must be weighed against the potential long-term adverse effects on tissue healing, structure, and function.

The role of avian macrophages in the production of avian lymphokines.

Lymphocyte inhibitory factor (LyIF) and lymphocyte chemotactic factor (LCF) were produced by sensitized thymic (T) and bursal (B) cells and thymic (T) cells, respectively. The passage of sensitized T and B cells through a Sephadex G-10 column abrogated the ability of T cells to produce LyIF and LCF while B cells remained capable of producing LyIF. The return of macrophages to the adherent cell depleted population reconstituted the T cells' ability to produce LyIF and LCF. These data suggest a greater macrophage dependency of thymic cells than bursal cells in the production of lymphokines.

Effects of changes in oxygen tension on lipoxygenase metabolites. Serum 15-HETE is increased in kittens exposed to hyperoxia.

The chronic phase of O2-induced retinopathy is characterized by retinal neovascularization. We have previously demonstrated that 15-hydroxy-5,8,11,13-eicosatetraenoic acid (15-HETE), a product of white cells, is proangiogenic. In this study, kittens exposed to in vivo hyperoxia produced increased amounts of 15-HETE. Nine litters of 30 kittens (aged 6-8 days) were used. Control kittens were left in room air; hyperoxic kittens were placed in 80% oxygen for 48 h; recovery kittens were returned to room air for 24 h following hyperoxic exposure. Following treatments, the animals were sacrified, and blood was evaluated for 15-HETE. Stimulated serum 15-HETE levels were assayed by high-performance liquid chromatography and GC-selected ion monitoring. While controls produced 0.48 +/- 0.16 (SE) nmol/ml of 15-HETE, values in the hyperoxic and recovery animals were increased at 0.7 +/0 0.2 and 0.68 +/- 0.15 nmol/ml (p less than 0.05 and p = 0.05, respectively). Increased production of this proangiogenic metabolite by WBCs (which can migrate out of blood vessels to set up extravascular angiogenic foci) may play a role in the genesis of the neovascularization process that occurs in response to oxygen-induced injury.

Schistosomiasis mansoni in Yemeni in California: duration of infection, presence of disease, therapeutic management.

Investigations of schistosomiasis mansoni in 218 Yemeni agricultural workers in the San Joaquin Valley of California revealed a prevalence of 56%. In those infected, quantitative egg counts performed by the Kato thick smear method revealed that 57% had light infections (1-100 eggs/g), 27% moderate infections (101-400 eggs/g), and 16% heavy infections (greater than 400 eggs/g; mean--918 eggs/g). The Yemeni had been migrating to the USA for the past 20 years, a period in which the prevalence of schistosomiasis had remained constant in the Yemen. The prevalence of schistosomiasis in those who had been away from Yemen for less than 5 years was 59% with a mean egg output of 236 eggs/g, but in those away for more than 5 years (up to 20 years) it was 32% with a mean egg output of 75% eggs/g. This is in spite of the fact that 75% of the latter had returned to Yemen for short visits. Statistical analysis by the Fisher's exact probability test revealed a significantly lower egg output in those away from Yemen more than 5 years. On the basis of these findings the mean life span of the Yemen strain of Schistosoma mansoni in man was estimated to be between 5 and 10 years. The presence of disease was assessed in this population by traditional medical means without prior knowledge of the status of the infection in the individuals examined. Under these circumstances, there were no differences in symptoms such as weakness, diarrhea and abdominal pain among the uninfected proportion of the population, total infected group and a small subgroup of those most heavily infected. None of the patients had hepatomegaly and only two had splenomegaly--one lightly and the other heavily infected. Because of the toxicity of antischistosomal drugs and the lack of treatment facilities only the 22 most heavily infected (greater than 200 eggs/g) of the 122 individuals with schistosomiasis mansoni were treated with niridazole. In addition the two individuals with splenomegaly were treated with antimony dimercaptosuccinate. Side effects, though common, were not severe. Although follow-up in this migrant population was poor the nine patients examined 3 to 7 months after treatment showed a mean decrease in egg output of 97%.

Earnings by language group in Quebec in 1980 and emigration from Quebec between 1976 and 1981.

"Between 1976 and 1981 many pople moved out of Quebec, especially anglophones. To the extent that a person's decision to stay or to move may be related to earnings prospects, earnings comparisons with individuals who lived in Quebec in 1981 may suffer from a selection bias. This paper analyses emigration from Quebec and earnings with a two-equation model, using a sample of men drawn from the from the 1981 [Canadian] census. It is found that a selection bias is indeed present for anglophones and that the returns to learning French may be underestimated when that bias is ignored."

[Studies on changes of lysozyme activity during pre- and postoperative periods in alimentary tract surgery].

From the clinical and experimental studies, the following results were obtained. The serum lysozyme activity decreased postoperatively and returned to the preoperative level 2 weeks later. Postoperative changes of the activity reflected the state of surgical stress inflicted on the subjects, namely, the changes were parallel to the degree of the stress and their postoperative courses. The postoperative elevation of lysozyme activity in anastomosed colonic tissues or abdominal walls, in which the activity was not determined preoperatively was supposed to be a reasonable phenomenon and closely related to tissue regeneration or protection from bacterial infection. The mechanism of this local elevation should be due to migration from the peripheral blood and movement of the lysozyme-producing or -secreting cells to the wounds. The phagocytic activity of peritoneal macrophage hyperfunctioned postoperatively. This might suggest lysozyme synthesis or secretion by peritoneal macrophage and participate in the protective action of the subjects. The postoperative serum lysozyme activity was maintained at the normal level by preoperative peroral administration of egg-white lysozyme preparation, therefore, the administration was thought to be very effective to the acceleration of protective action of the subjects on whom the surgical stress was imposed.

[Functional morphology of the spleen].

The spleen was till the first half of the twentieth century considered to represent an organ of no special significance. Nowadays its clearance function and position in the immunity system is thoroughly investigated. Informations on its cellular composition and compartments are sufficient. The problems coinciding with circulation in the spleen are solved. The study of interactions of T and B lymphocytes regarding the development of humoral immunity do not provide clear results. Unclear remain also the relations of extracellular and cellular structures. The localisation of the spleen in blood circulation and large amount of migrating lymphocytes justify its significant position in the protection of host against infections spreading haematogenously. It is not inevitable for survival, but its removal increases the risk of unmanageable infections carried by bacteria with polysaccharide capsules. The microenvironment in the spleen enables intercellular relations and determines the development of immune reactions. Microcirculation enables it to function as a biological filter without the necessity of specialized filtrating structures being present. In consequence of blood filtration everything leaks into the extravasal space. Subsequently all components normally constituting the blood return back into the blood within vessels. That is why no special structures are formed in order to filter individual components from the blood. All components that should not be present in the blood are excluded extravasally. The substances which are not let through the wall of sinuses into the blood, remain in the marginal zone and in the red pulp. The macrophages of the marginal zone and red pulp destroy and degrade the unrequired substances. (Ref. 26).

Laparoscopic inguinal hernia repair using an anatomically contoured three-dimensional mesh.

BACKGROUND: Laparoscopic inguinal hernia repair frequently is performed with mechanical fixation of a flat polypropylene mesh. Mechanical fixation is associated with pain syndromes, and mesh migration may occur without fixation of flat prostheses. An anatomically contoured mesh (3D Max; Davol, Cranston, RI, USA) using no or minimal fixation would avoid these problems. METHODS: A retrospective case study reviewed 212 transabdominal preperitoneal herniaplasties with 11 x 16-cm 3D Max mesh in 146 patients. Fixation with three helical tacks at the most was used early or with very large defects. RESULTS: Fixation was used in 19% of the cases, but only for 1 of the last 98 patients. As reported, 94% of the patients returned to normal activities by 3 weeks, 97% returned to unrestricted sports by 6 weeks, and 92% complete recovery from surgery by 9 weeks. Fixation or bilateral repair did not alter recovery. Four patients had minor pain or numbness. Symptomatic recurrence was 0%. One asymptomatic indirect recurrence was noted on examination, during a mean follow-up period of 23 months, yielding a 0.55% hernia rate and a 0.42% patient-year recurrence risk. CONCLUSIONS: An anatomically contoured mesh for transabdominal preperitoneal hernia repair often requires no fixation, with minimal risk of neuropathy and less than a 0.5% patient-year recurrence rate. Recovery is excellent even with bilateral repair or some fixation.

Expression and regulation of cell adhesion molecules by hepatic stellate cells (HSC) of rat liver: involvement of HSC in recruitment of inflammatory cells during hepatic tissue repair.

Hepatic stellate cells (HSC), a pericyte-like nonparenchymal liver cell population, are regarded as the principal matrix-synthesizing cells of fibrotic liver. They might also play a role during liver inflammation. The present study analyzed (i) expression of cell adhesion molecules (CAMs) mediating cell infiltration, like intercellular adhesion molecule-1 (I-CAM-1) and vascular cell adhesion molecule-1 (V-CAM-1), by HSC, (ii) CAM regulation in HSC by growth factors and inflammatory cytokines, and (iii) CAM expression in situ during liver inflammation, using immunochemistry and Northern blot analysis. I-CAM-1 and V-CAM-1 expression was present in HSC in vitro and in cells located in the sinusoidal/perisinusoidal area of normal liver. Growth factors, eg, transforming growth factor-beta1, down-regulated I-CAM-1- and V-CAM-1-coding mRNAs and stimulated N-CAM expression of HSC. In contrast, inflammatory cytokines like tumor necrosis factor-alpha reduced N-CAM-coding mRNAs, whereas induction of I-CAM-1- and V-CAM-1-specific transcripts increased several fold. In situ, messengers specific for I-CAM-1 and V-CAM-1 were induced 3 hours after CCl4 treatment (thereby preceding mononuclear cell infiltration starting at 12 hours), were expressed at maximal levels 9-12 hours after CCl4 application, and decreased afterwards. I-CAM-1 and V-CAM-1 immunoreactivity increased in a linear fashion starting 3 hours after CCl4-induced liver injury, was detected in highest amounts at 24-48 hours characterized by maximal cell infiltration, and returned to baseline values at 96 hours. Interestingly, the induction/repression of CAM-specific messengers paralleled the time kinetics of tumor necrosis factor-alpha transforming growth factor-beta1 expression in injured liver. HSC might be important during the onset of hepatic tissue injury as proinflammatory elements and might interact with I-CAM-1 and V-CAM-1 ligand-bearing cells, namely lymphocyte function-associated antigen-1- or Mac-1/very late activation antigen-4-positive inflammatory cells, thereby modulating the recruitment and migration of mononuclear cells within the perisinusoidal space of diseased livers.

Lymphocyte homing to the gut: attraction, adhesion, and commitment.

Lymphocytes continuously migrate from the blood into the intestine. Naive lymphocytes leave the blood through high endothelial venules in Peyer's patches. During the multistep extravasation cascade, they sequentially roll on, firmly adhere to, and transmigrate through the endothelial layer using multiple adhesion molecules and chemotactic signals. In the organized lymphoid tissues of the gut, lymphocytes can become activated, if they meet their cognate antigens transported to Peyer's patches through the gut epithelium. During activation and proliferation, the lymphocytes become imprinted by the local dendritic cells, so that after returning to systemic circulation via the efferent lymphatic vasculature, they preferentially home to lamina propria of the gut to execute their effector functions. In inflammation, the recirculation routes of lymphocytes are altered, and these may explain the pathogenesis of certain extra-intestinal manifestations of gut infections and inflammatory bowel diseases. The increased knowledge on the mechanisms that regulate lymphocyte homing and imprinting has clear applicability in designing more effective vaccination regimens. A detailed understanding of the mucosal homing has recently led to the development of the first successful anti-adhesive therapeutics in human.
