Design, content and financing of an essential national package of health services.

A minimum package of public health and clinical interventions, which are highly cost-effective and deal with major sources of disease burden, could be provided in low-income countries for about US$ 12 per person per year, and in middle-income countries for about $22. Properly delivered, this package could eliminate 21% to 38% of the burden of premature mortality and disability in children under 15 years and 10-18% of the burden in adults. The cost would exceed what governments now spend on health in the poorest countries but would be easily affordable in middle-income countries. Governments should ensure that, at the least, poor populations have access to these services. Additional public expenditure should then go either to extending coverage to the non-poor or to expansion beyond the minimum collection of services to an essential national package of health care, including somewhat less cost-effective interventions against a larger number of diseases and conditions.

Determination of volatile chemicals released from microwave-heat-susceptor food packaging.

Microwave heat susceptors that convert electromagnetic energy to heat attain high temperatures that make it possible to cook some foods to golden crispness in a microwave oven. Susceptors are typically packaged with foods intended for microwave use, e.g., waffles, pizzas, and french fries. The high temperatures > 302 degrees F used to cook some foods release trace levels of volatile chemicals from metalized polyester film, adhesive, and paper packaging materials; these volatile chemicals may be absorbed by the food. We simulated microwave susceptor cooking conditions and developed protocols by using headspace concentration capillary gas chromatography and mass spectrometry to identify volatile chemicals released from heated susceptors. We purchased a limited, cross-sectional sample of local retail microwave food products packaged with susceptors and used our protocol to analyze 10 different susceptor products. Although more than 140 unique chromatographic peaks were tabulated, only 44 volatile chemicals were identified, including 1,1,1-trichloroethane, benzene, and 2-(2-butoxyethoxy)ethanol, which were derived primarily from the paper and adhesive susceptor components. No one susceptor contained all the identified substances. The standard additions technique was the preferred method for quantitation. Trichloroethane and 2-(2-butoxyethoxy)ethanol were present in several products at 75-122 micrograms/in.2 of susceptor surface area. Benzene was found in 3 susceptors at < or = 0.22 microgram/in.2 levels. Examination indicates that adhesives used in more recent susceptor products were reformulated to remove even this trace level of benzene.

Bacteriophage T4 expression-packaging-processing vector that encapsidates HIV-type I GP120-V3 fusion protein inside the head.

Bacteriophage T4 as an expression-packaging-processing vector has recently been developed by using the T4 non-essential capsid scaffold protein IPIII(1-3). The IPIII gene was expressed at high level in E. coli from plasmid and was truncated at its C-terminus to permit construction of gene fusion in three different reading frames of EP-16 vector. Regulated higher-expression PPL reading frame vectors were also constructed. Infection of the plasmid-containing bacteria with bacteriophage mutants deleted for the IPIII gene showed that viable phage encapsidated and proteolytically processed the IPIII fusion proteins. An IPIII gene fused to human immunodeficiency virus type I (HIV-1) gp120 loop3 domain (V3) IPIII-V3 fusion gene products has been constructed and was packaged and processed within viable phage particle. The packaged fusion protein Gp120-V3 may be used for production of vaccine in the future.

Replication-competent retrovirus produced by a 'split-function' third generation amphotropic packaging cell line.

We report the discovery, on routine screening, of a replication-competent retrovirus (RCR) produced from a third generation amphotropic packaging cell line, GP + envAM12. In this line, the gag-pol and env helper genes are located on separate plasmids to minimise the chances of recombination events that may lead to RCR formation. Plasmid pBabeNeo was transfected into GP + envAM12 to obtain a producer line. Supernatant from this line was used to infect two lines, B16 and 1735-puro, and stable colonies which were obtained were pooled. Supernatants from both these lines were able to transfer the neo gene to fresh cells through several passages, indicating the presence of helper virus. Rescue of puro from 1735-puro cells, which contain the pBabePuro provirus, was also achieved. Mobilisation of lacZ from TELCeB6 cells, which contain the MFGnlslacZ provirus, was also possible. The RCR was able to transfer lacZ to ampli-GPE cells, an ecotropic packaging cell line, but transfer to fresh GP + envAM12 cells was minimal, demonstrating receptor interference of the RCR by the amphotropic envelope. These observations strongly suggest that a RCR with an amphotropic envelope arose in the GP + envAM12-pBabeNeo producer line. To our knowledge, this is the first report of RCR arising from routine use of GP + envAM12 cells or any similar third generation packaging line and has important implications for the use of such lines in human gene therapy protocols.

Computer-assisted learning package for frequency distribution of physiological variables.

There are a variety of ways of picturing a frequency distribution type, viz, the histogram, the frequency polygon, the smoothed curve and the ogive. We have developed a computer package to demonstrate gradual change of a histogram into a curve. For a given set of data on frequency distribution of, say blood pressure levels in specified categories, this package helps the computer to draw bars which gradually rise to the level of the frequencies, and lateron are replaced by the polygon and finally by the frequency curve of the Gaussian type on the computer screen pixel by pixel. This thus demonstrates the meaning and genesis of frequency curves. This package could be very useful in learning the concept of frequency curves particularly the Gaussian form.

Management of children with potential anaphylactic reactions in the community: a training package and proposal for good practice.

BACKGROUND: The increase in potentially dangerous food allergic reactions (e.g. to peanuts and nuts) in children had lead to an increase in the prescription of adrenaline, including adrenaline for self- or parent-injection. This raises problems in the management of these children in schools. Our experience in response to this is described. OBJECTIVE: To develop a training package to ensure that each child can continue at school but receive optimum emergency treatment in a safe environment. To provide education, training and support to school staff, and to support parents. METHODS AND RESULTS: This paper describes a model of good practice in the support of children with potentially fatal anaphylactic reactions. The following points are emphasized. Children with a history of anaphylactic reactions should be expertly assessed prior to the prescription of adrenaline injection. A written treatment plan is essential. Parents need expert support in managing the child. Schools need training and support. Children should lead a normal life, and not be stigmatized or labelled by their problem. Close liaison between primary care, acute services, community services and education services are essential for the successful management of this problem. Data from a questionnaire showed the training package was of benefit, with considerable reduction in anxiety in school staff and parents. CONCLUSION: A broad training package is essential: training in administration of adrenaline is only one aspect, and emphasis is placed on education, recognition of allergic reactions and prevention. The presence of children in school needing access to adrenaline is no longer perceived as a problem in our area. We recommend other health authorities provide a similar service.

The effect of a physical activity intervention package on the self-esteem of pre-adolescent and adolescent females.

The purpose of this study was to examine the effects of a physical activity intervention package on the self-esteem of pre-adolescent and adolescent females. The package involved three components: physical activity, education and self-report. Subjects (N = 181) were pre-, early-, and middle-adolescent girls ranging in age from 9 to 16 who were enrolled in an independent school. An experimental pre-test/post-test design which involved two independent variables (intervention package and age group), each with three levels, was used. Self-esteem was measured with the Self Description Questionnaire I and II (Marsh, 1988). Results indicated that low self-esteem individuals benefitted from the intervention. However, statistically significant results were limited to the younger age group.

Packaging principle: how DNA methylation and histone acetylation control the transcriptional activity of chromatin.

Chromatin contains DNA, the transcriptional machinery, and structural proteins such as histones. All these components together are necessary for the physiologic control of transcription. A consideration of recent advances leads to a packaging principle for gene regulation. This packaging principle states that the transcriptional activity of a gene is largely controlled by the packaging of the template within chromatin. This assumption follows from experiments that establish the covalent modification of chromatin as a major causal influence on gene activity. DNA methylation and histone acetylation alter the nucleosomal infrastructure to repress or activate transcription. These covalent modifications can have roles in both promoter-specific events and the global control of chromosomal activity. These regulatory functions are essential for vertebrate development.

Commentary: Patient package inserts. I. Nature, notions, and needs.

A total of 137 completed questionnaires were obtained from several population samples in an attempt to assess drug knowledge, information sources, and views on drug information with particular attention to the possible role of a patient-oriented package insert. The results indicated that the study population was variably informed and could make fairly accurate assessments by this knowledge. Responders placed great importance on the doctor and pharmacist as preferred and actual drug information sources. The mass media were accorded little prominence. Great emphasis was placed on the adequate provision of information, and this appears to be a crucial function in an adequate doctor-patient relationship. The results appear to indicate a favorable climate for establishing a patient-oriented package insert that should have summarized as well as detailed information to complement the doctor and pharmacist as primary information sources.

A novel radiotracer method for the precise determination of water vapor transmission rate characteristics of packaging systems.

A novel analytical method is described for sensitive determination of moisture transmission characteristics of packaging systems. In this method, moisture uptake is measured by a radiotracer technique that utilizes tritium-labeled water (HTO). This technique is specific for actual water ingress, unlike weight gain methods, which require blanks to correct for water absorbed by the packaging materials. Because the amount of water transmission necessary for detection decreases with increasing activity of the water in the incubation chamber, the method can be made much more sensitive than gravimetric methods. The sensitivity of the radiotracer method permitted analysis of moisture entry modes of container systems. Also described herein is the applicability of various desiccants for use in the radiotracer method, discussed in terms of isotope effects and proton exchange.

Effects of modified atmosphere and vacuum packaging on the growth of spoilage and inoculated pathogenic bacteria on fresh poultry.

Fresh chicken breast meats inoculated with Yersinia enterocolitica and Aeromonas hydrophila were packaged in glass jars either containing different compositions of modified atmospheres (MA) (100% CO2; 80% CO2/20% N2), or in vacuo or containing air, and were stored at 3 +/- 1 degrees C and 8 +/- 1 degrees C. The changes in gas composition as well as Y. enterocolitica, A. hydrophila, total aerobic bacterial, total psychotropic, Lactobacilli and Enterobacteriaceae counts were determined after 0, 1, 3, 7, 9, 11 and 14 days of storage. The results show that while the growth of Y. enterocolitica and A. hydrophila were retarded following MA storage, the pathogens were capable of growth in MA and vacuum storage at both temperatures, for the inoculation levels studied. For total aerobic bacterial counts, there were no differences between the values for chicken breast meats kept in different atmospheres. Being packaged in CO2 had the greatest inhibitory effect on the growth of psychotropic aerobic bacteria during the first 3 days. Lactic acid bacteria levels of samples stored in MA conditions and in vacuo increased rapidly when compared to those levels of samples stored in air. It was also found that the effect of MA storage increased at 3 +/- 1 degrees C.

PVC as pharmaceutical packaging material. A literature survey with special emphasis on plasticized PVC bags.

In this report the state of the art with respect to PVC as pharmaceutical packaging material is described. A general introduction into the applications of PVC is followed by a description of its production process. The metabolic effects of the monomer of PVC, vinyl chloride and of the most commonly used plasticizer diethylhexylphthalate are mentioned. Special attention is given to the pharmaceutical properties of plasticized PVC bags in comparison to other plastics and the environmental aspects of waste PVC disposal. Although there are emotional and political queries regarding the future use of PVC as a (pharmaceutical) packaging material, we conclude that there is no scientific justification for a total or partial ban of PVC. PVC will remain a fact of life as a cheap, versatile, high-performance and well-investigated plastic material for medical and pharmaceutical applications, to be replaced by newer plastics only for certain well-defined indications where the requirements of the plastic to be used are so specific that it will economically and technically be justified to use another polymer. Community and hospital pharmacists have to be prepared for a role in intake of waste plastic disposables, probably against deposit money, in order to fulfil the logistics needed for recycling.

Compostable packaging materials--test methods and limit values for biodegradation.

In order to classify packaging materials as capable of organic recovery according to the packaging regulation of the European Union, the intended materials must be tested for their compostability. An important prerequisite is the determination of biodegradability by standardized test methods. Details of the intended tests are not comprehensible without further knowledge of the problems of biodegradation testing. The test methods and the limit values used to evaluate the biodegradation results are presented and discussed against the background of established test methods and limit values for chemicals. Furthermore the problem of ecotoxicity tests in connection with compost quality is discussed and an overview of European test laboratories is given.

Pediatric acepromazine poisoning: the importance of child-resistant packaging for veterinary drugs.

The first reported case of acepromazine ingestion in a pediatric patient is presented. It is an aliphatic phenothiazine that is structurally similar to chlorpromazine (thorazine). In our case, a 2 1/2-year-old male ingested 3 to 4 of his 80-pound Doberman's 25-mg tablets. The pills were stored in a vial without a child-resistant closure. The child displayed signs and symptoms of phenothiazine toxicity. There is no federal law which requires veterinary medicines to be dispensed in special packaging. All prescription medications should require child-resistant packaging.

Displacement imprinted polymer receptor analysis (DIPRA) for chlorophenolic contaminants in drinking water and packaging materials.

The preparation of a molecularly imprinted polymer (MIP) for pentachlorophenol is described together with two alternative reporter derivatives for use in a displacement imprinted polymer receptor analysis (DIPRA) format procedure. In this procedure, alternative reporter molecules were rebound to the synthetic receptor sites and their displacement by the target analyte was employed as the basis of a simple procedure for the measurement of chlorophenols in water and packaging material samples. Water samples were extracted using the standard procedure (EPA 528) and a detection limit of 0.5 microg l(-1) was achieved using the DIPRA detection method, with good agreement between the displacement technique and GC-ECD analysis. A variety of packaging materials, extracted using a buffered detergent solution were also analysed using the DIPRA procedure and showed good agreement with GC results. In addition, investigation of the cross-reactivity of a range of pesticides and materials commonly encountered in environmental analysis indicated the procedure gave good discrimination between pesticides bearing a chlorophenolic moiety and other materials. The procedure is considered highly suitable for use as a rapid field-test method or for incorporation into a test kit device.

Migration of lubricants from food packagings. Screening for lipid classes and quantitative estimation using normal-phase liquid chromatographic separation with evaporative light scattering detection.

A normal-phase high-performance liquid chromatography (NP-HPLC) method is introduced for the identification and quantitative estimation of 12 lipid classes (paraffin, wax esters, cholesterol esters, fatty acid methyl esters, triacyl glycerols, fatty alcohols, free fatty acids, cholesterol, 1,3-diacyl glycerols, 1,2-diacyl glycerols, monoacyl glycerols and fatty acid amide) used as lubricantsin food packaging materials. The HPLC separation is carried out on a LiChrospher Diol (100 A, 5 microm, 125 mm x 3 mm) column with gradient elution (isooctane/0.1% acetic acid in tert-butyl methyl ether) and evaporative light scattering detection (ELSD). The method has been calibrated with representatives of each class in working ranges of about 5-150 mg/l, depending on the lipid class. Intra-day variance for all representatives range from 1.9 to 5.1%, inter-day variances from 7.0 to 26.5% and the limits of detection from 0.79 to 3.65 mg/l (except for two classes). A simple sample preparation could be established for the determination of migrating lubricants obtained from packaging materials containing external or internal lubricants. Since the detector response depends on the chain length and the degree of saturation, the quantification of a lipid class with unknown composition is only semi-quantitative. The amount of migrating lubricants from an epoxy-based can coating could be estimated with 0.3 mg/dm2 and from a light weight container with 5.5 mg/dm2.
