Basic trauma life support: knowledge of medical students.

BACKGROUND: The first hour of management (golden hour) of any trauma patient might be the determining factor if he/she will survive. The first contact most patients have in an emergency room is with a young medical graduate. The knowledge of basic trauma life support (BTLS) by all doctors therefore becomes important. AIMS AND OBJECTIVES: This study set out to assess how much knowledge medical students had about BTLS in their final year compared with their knowledge at the beginning of clinical postings. METHOD: A cross-sectional study of fourth and sixth (final) year medical students to evaluate their knowledge of BTLS using a test of 30 questions under examination conditions. One hundred and twenty-three fourth year students were given a test after a lecture on the care of injured patients. The same test was administered on 41 sixth year students without prior lecture on day of test, having had a similar lecture 2 years before. Data analysis was performed using the SPSS 11.0 for Windows statistical software and EPI INFO version 6.04. RESULT: Mean scores were 25.8 +/- SD1.91 for the 4th year students and 21.0 +/- SD2.93 for the 6th year students. There was a statistically significant difference between scores of the 4th year and the 6th year (P < 0.000001). CONCLUSION: Knowledge was imparted in the 4th year but was forgotten by the 6th year. It is essential that courses in BTLS be given periodically to all medical graduates and all personnel involved in care of the traumatised patient.

[Population structure and its dynamics of rare and endangered plant Alsophila spinulosa].

This paper analyzed the population structure of Alsophila spinulosa in seven plots by the method of spatial sere substituting for time sere, and measured its spatial distribution pattern and dynamics by applying five aggregate indices, C, K, m*, m*/m and Idelta. The spatial distribution pattern with different quadrat scale was examined, according to the map of individual location. The results indicated that the population structure of Alsophila spinulosa in the seven plots presented growing, stable, adult or senescent type, and the spatial distribution pattern was changed from clump to random with the development of Alsophila spinulosa population. The values of dispersion index Idelta of the population decreased with increasing quadrat scale.

Indicators of clinically significant red cell antibodies produced by sensitized lymphocytes in liver transplant patients.

It has been documented that transplanted livers can carry sensitized lymphocytes that subsequently produce red cell antibodies. We evaluated immunohematological variables in liver donors and recipients for indicators that might be predictive of serological red blood cell (RBC) destruction mediated by the passenger lymphocytes. Organ donor sera with antibody scores greater than ( > ) 60 correlated with a positive direct antiglobulin test (DAT) and need for increased RBC transfusion in liver transplant recipients. We therefore propose an antibody screen on all potential liver donors and titration of unexpected alloantibodies; titration of ABO antibodies of liver donors who demonstrate minor ABO-incompatibilities with their recipients; and, when needed, transfusion of group O RBCs to recipients of livers from donors with minor ABO-incompatibility who have antibody scores >60.

Thermal injuries in under-4-year-old children: the Lesotho experience.

The Queen Elizabeth II Hospital is the apex of the health care services in the Kingdom of Lesotho but has no designated burns unit. A review of patients managed during a 5-year period was done to document our experience so as to show what can be achieved even without a standard burns unit. We reviewed the clinical course of children who were admitted for care of acute thermal injuries during a 5-year period, May 1997 to April, 2002 inclusive. All were treated in the children surgical ward in accordance with a protocol of care emphasizing urgent care of shock, accurate fluid therapy, open wound care and early enteral feeding. Ninety-eight children (51 males and 47 females) aged 4 months to 43 months (mean 21.66 +/- 11.30 months) were admitted on account of acute thermal injuries during the study period. The injuries included scalding (46%) involving predominantly the upper part of the body and full thickness burns (87%). Skin grafting was needed in 82% of the patients. The common complications were infection, fluid imbalance, respiratory problems, and contracture deformities. The mean duration of hospital stay was 40 +/- 4.43 days and the case fatality rate was 9%. The case fatality rate in this series is considered high and could be improved if there were a well staffed specialized burns unit.

Prediction methods for babies' birth weight using linear and nonlinear regression analysis.

The aim of this study is to determine more accurate prediction methods between linear and non-linear methods for prediction of babies' birth weight among maternal demographic characteristics. Three hundred pregnant women were included in the study. Blood glucose level before and after ingestion of glucose load, age, body mass index, % of change in weight during pregnancy, height, gestational age, parity, and fetal sex were collected as independent variables and baby birth weight as dependent variable. In linear regression, least squares estimation method was used to estimate parameters. Non-linear regression method was performed using neural network model with multilayer perceptrons, back propagation method was preferred as learning algorithm. Coefficient of determination, R2, of the linear regression equation was found 59.8% and the standard error of the estimate was calculated as 325.69 gr. In non-linear regression method R2 value was also found 59.8% and standard error of estimate was calculated as 320.30 gr. According to the results of the present study, one method is not significantly better than the other. When "accuracy in prediction" is aimed, it is better to use the two methods and compare the results, and then decide on the selection of the favourable method.

Biliary tract reconstruction with autologous rectus sheath graft--an experimental study.

BACKGROUND/AIMS: Common bile duct injuries which cause significant morbidity and mortality are one of the most serious complications of cholecystectomies. They can be repaired by various approaches. METHODOLOGY: We used an autologous graft obtained from the posterior sheath of the rectus muscle in biliary tract reconstruction for common bile duct defect. This experimental study was carried out on six dogs. A tissue fragment was excised from the posterior sheath of the rectus muscle together with the fascia transversalis and peritoneum. A channel from the fascial graft was formed around a T-tube. The autologous rectus sheath graft was sutured to the common bile duct as a segmental conduit graft. Afterwards, blood biochemistry values and liver histopathology were investigated. Dogs were studied by T-tube cholangiography 4 months postoperatively just prior to removing the T-tube and again 2 months later at relaparotomy. The hepatic parenchyma, intra- and extrahepatic bile ducts were detected by ultrasonography during the last 2 months, one time in fifteen days. RESULTS: There was no anastomotic insufficiency. The fascial graft gained an appearance similar to bile duct in dogs. Postoperatively, no change in blood biochemical parameters was observed, and in the preoperative and postoperative periods no histopathological change in the liver was found. CONCLUSIONS: These findings indicate that the use of an autologous fascial graft to repair common bile duct injuries as a conduit graft for segmental bile duct loss may be a feasible and alternative method of treatment.

The effects of compost prepared from waste material of banana on the growth, yield and quality properties of banana plants.

Three doses (15-30-45 kg plant(-1)) of the compost prepared from the clone of Dwarf cavendish banana were compared with farmyard manure (50 kg plant(-1)), mineral fertilizers (180 g N+ 150 g P +335 g K plant(-1)) and farmyard manure+ mineral fertilizers (25 kg FM+ 180 g N+ 150 g P +335 g K plant(-1)) which determined positive effects on the growth, yield and fruit quality characteristics of banana plants. The banana plants were grown under a heated glasshouse and in a soil with physical and chemical properties are suitable for banana growing. The contents of N, P, K and Mg in compost and in farmyard manure were found to be similar. Farmyard manure, farmyard manure + mineral fertilizers and 45 kg plant(-1) of compost increased growth, yield and fruit quality characteristics. According to obtained results, 45 kg plant(-1) of compost was determined more suitable in terms of economical production and organic farming than the other fertilizer types.

Injection Safety Practice among Health Workers in Static Immunisation Centres in an Urban Community of Nigeria.

BACKGROUND: Immunisation injection accounts for less than one-tenth of the global total injections, and it is believed to be safer than therapeutic injections. However, reports have it that about one-third of immunisation injections are unsafe in many countries of the world including Africa. OBJECTIVES: This study was conducted to determine the practice of sage immunisation injection among health workers in static immunisation centres in a urban community in Nigeria with a view to assessing the safety of immunisation injection services in the area. STUDY DESIGN/METHODS: Pre-tested semi-structured questionnaires were administered to 102 consenting staff and observation checklist was completed for each of the 13 fixed immunisation centres in the study area. RESULTS: Most of the subjects 82(80.4%) had poor knowledge of safe immunisation injection and about the same number were not aware of any policy on injection safety. Injection abscess was the main health hazard of unsafe injection reported by majority 71(69.9) of the subjects while as few as 10(9.8%) mentioned needle prick as a health hazard. The common infections associated with unsafe injection listed by the subjects were abscess, HIV and Hepatitis in that order of frequency. Also, most of them 69(67.5%) felt that open burning is the ideal method of disposal of injection waste, while the actual method of disposal practised in most health facilities were mainly open burning (48%) and open dumping into an insecure pit (24%). Up to 83% of the subjects preferred the use of standard disposable syringes and needles for vaccination, but most of them (75(73.5%) confirmed that they use both sterilisable and disposable syringes and needles for vaccination in their health centres. BCG vaccine was exclusively administered using sterilised needles and syringes, while tetanus toxoid was given using disposable syringes and needles in virtually all the centres. Re-use of syringe for vaccine withdrawal and re-capping of used needles before discard were common practices observed while accidental needle stick injury was reported by about half (49%) of the subjects. CONCLUSION: Staff re-training, adequate supplies of injection equipment and provision of appropriate facilities for disposal of injection waste would go a long way to enhance safe immunisation injection practice in the area. Key words: immunisation, injection practice, injection safety.

Case Report: Acute Limb Ischaemia and Gangrene associated with Peripartum Cardiomyopathy.

UNLABELLED: Thromboembolic complications are frequent in patients with peripartum cardiomyopathy (PPCM). Peripheral artery occlusions are however rare. This case report describes a 30-year-old grand multiparous woman with PPCM complicated by acute lower limb ischaemia and gangrene. This followed left intraventricular thrombi despite maintenance of sinus rhythm. We suspect that the thrombosis is caused by the hypercoagulable state of peripartum period, ventricular dilatation and hypokinesis. The impacts of economic and sociocultural factors in the management of this rare complication of PPCM in a resource depleted nation are highlighted. KEYWORDS: peripartum cardiomyopathy, limb ischaemia.

Acute pancreatitis possibly caused by allergy to bananas.

We are reporting a forty-seven year old female who had three attacks of acute pancreatitis after having; eaten bananas. She was treated with a light diet and intravenous fluids during each of her admissions. The treatment concluded with the disappearance of clinical symptoms such as epigastralgia, nausea, vomiting and diarrhea within a few days. In addition, elevated serum and urine amylase levels returned to normal values in parallel with the clinical symptoms. The data during her three attacks of acute pancreatitis were as follows: serum total IgE level = 644 IU/mL, specific IgE to bananas = 2.18 UA/ml. No remarkable abnormalities were present in sonography, computed tomography, magnetic resonance imaging of the abdomen, and magnetic resonance cholangiopancreatography. Endoscopic examination of the upper digestive tract showed the ampulla of Vater swollen and edematous, thus a biopsy was performed. Toluidine blue staining and immunohistochemical staining against human mast cell tryptase with the biopsy specimens showed mast cells accumulating in mucosa and submucosa. By avoiding consumption of bananas, she has not suffered from any additional attacks of pancreatitis since the third attack in the last thirty-four months up to this day of December 10, 2004.

Comparative lipid profile studies in cardiac and diabetic conditions.

Lipid profile in cardiac patients (myocardial infarction, angina pectoris, coronary heart disease, ischaemic heart disease), diabetic patients and normal humans was investigated. Total serum cholesterol, high density lipoprotein cholesterol (HDL-C), low density lipoprotein cholesterol (LDL-C), triglycerides, blood glucose, calcium, potassium and sodium were determined, employing established methods and procedures. Higher level of total cholesterol, LDL-C and triglyceride were found in both cardiac and diabetic patients, however, cardiac patients had much lower level of HDL-C as compared to normal humans. Age wise comparison revealed that level of total cholesterol; triglyceride and LDL-C were elevating while the level of HDL-C were decreasing with the age in cardiac and diabetic patients. Sex wise comparison showed that females had higher HDL-C level than males and therefore had fewer incidences of heart diseases. The level of Ca++, K+ and Na+ were similar in all age groups and sexes. No significant elevation in the level of these electrolytes was discovered.

Antimony-induced cerebellar ataxia.

Visceral leishmaniasis VL, caused by Leishmania donovani is endemic over several parts of Sudan. The disease is fatal if not treated. Although sodium stibogluconate Pentostam, a pentavalent antimonial is not free from toxicity, it has been in use for treatment of VL for the last 50 years. Like other infectious diseases, neurological manifestations of VL and sodium stibogluconate have been documented. In this report, we present 2 cases of cerebellar ataxia most likely induced by Pentostam, and explain the probable cause.

Growth and amylase production by Aspergillus oryzae during solid state fermentation using banana waste as substrate.

Aspergillus oryzae, isolated from the starch rich litter soil, produced amylase when banana fruit stalk was used as substrate in a solid state fermentation system. The effects of incubation period, pH, temperature and various carbon sources on the production of amylase were studied. A maximum yield of 380U/mg was recorded on 96th hour of incubation. The amylase is tolerant to wide range of initial culture pH values (3 to 8) and temperature (25 to 35 degrees C), with an optimum pH of 5 and temperature of 35 degrees C. In SSF addition of starch (1%) increased the amylase production, when compared with other carbon sources used.

Procalcitonin and conventional markers of inflammation in peritoneal dialysis patients and peritonitis.

OBJECTIVES: To determine the significance of a newly described marker of inflammation procalcitonin (PCT), and to investigate its relationship to conventional markers of inflammation, such as C-reactive protein (CRP), fibrinogen, and erythrocyte sedimentation rate (ESR), in patients on peritoneal dialysis (PD) and with peritonitis. DESIGN: A prospective, observational clinical study. SETTING: The Nephrology Division of a University-affiliated teaching hospital. PATIENTS AND METHODS: 51 consecutive patients on PD were included in the study. Of this number, 16 developed peritonitis during the observational period. Baseline PCT, CRP, and fibrinogen concentrations and ESR of 51 PD patients were determined at a time point (TB) prior to any evidence of infection. These results were compared with laboratory values from 74 hemodialysis patients and 34 nonuremic control subjects. All PD patients then were followed prospectively for evidence of peritonitis. In addition to routine blood tests, including hemoglobin and leukocyte count, and routine biochemical tests, blood samples were taken to measure PCT, CRP, and fibrinogen concentrations and ESR at the time (T0) when patients first were diagnosed with PD peritonitis and also on the 4th (T4) and the 14th (T14) days after treatment for peritonitis was initiated. PCT was assayed by immunoluminometry. RESULTS: No significant difference was observed between baseline median serum PCT concentrations in PD and hemodialysis patients; however, in both groups, baseline median PCT concentrations were significantly higher than those of nonuremic controls (p < 0.05). The 16 patients on PD who developed peritonitis had 21 PD peritonitis episodes during the study period. The increased PCT concentration observed at T0 in PD peritonitis episodes decreased with therapy, and this change was statistically significant (p < 0.05). In a receiver operating characteristic curve analysis for peritonitis, the area under the curve (AUC) for PCT was 0.80, which was significantly lower than the AUC for CRP and greater than the AUCs for fibrinogen and ESR. The sensitivity of PCT for peritonitis was lower than the sensitivity of conventional markers of inflammation; however, the specificity of PCT was higher. CONCLUSIONS: Median serum PCT concentration in PD patients was significantly higher than in nonuremic controls but not hemodialysis patients. Serum PCT concentrations may serve as a useful adjunct to traditional markers of inflammation in detecting and monitoring inflammation and peritonitis in PD patients.
