Studies on hearing loss in a community with endemic cretinism in Central Java, Indonesia.

An audiometric study was carried out in a community of 642 subjects severly affected by endemic goitre and cretinism. Hearing loss was measured in 34 out of 41 subjects diagnosed as cretins, 92 normal subjects aged 5-20 years from the same community and 54 subjects (also of 5-20 years) living in a nearby control area without endemic goitre. The excess number of hearing defects in the endemic area seems to be entirely due to the process that leads to cretinism. There is no reason to describe deafness and deafmutism in an area with severe endemic goitre as a separate entity. The hearing defect showed a definite greater loss in the higher frequencies than in the lower frequencies and was found in 92% of the cretins. Deafmutism was present in 5, a loss of more than 60 db in 8, a loss of 40-60 db in 10 cretins. A loss of 20-30 db was found in 2.2% of normal subjects in the endemic area and 1.8% of those living in the control area. It is concluded that audiometry is a simple and significant test to establish the presence of the neurological form of endemic cretinism, which is the most prevalent form in most endemias. The differential diagnosis and pathogenesis of the described hearing defect are discussed.

Influence of socioeconomic status on childhood acute lymphoblastic leukemia treatment in Indonesia.

OBJECTIVE: A major reason for poor survival of childhood acute lymphoblastic leukemia in developing countries is treatment refusal or abandonment. This can be associated with parental socioeconomic status and attitudes of health care providers. Our study examined the influence of 2 socioeconomic status determinants, parental income and education, on treatment in an Indonesian academic hospital. METHODS: Medical charts of 164 patients who received a diagnosis of acute lymphoblastic leukemia between 1997 and 2002 were abstracted retrospectively. Data on treatment results and parental financial and educational background were collected. Open interviews were conducted with parents and health care providers. RESULTS: Of all patients, 35% refused or abandoned treatment, 23% experienced treatment-related death, 22% had progressive or relapsed leukemia, and 20% had an overall event-free survival. Treatment results differed significantly between patients with different socioeconomic status; 47% of poor and 2% of prosperous patients refused or abandoned treatment. Although poor and prosperous patients used the same protocol, the provided treatment differed. Poor patients received less individualized attention from oncologists and less structured parental education. Strong social hierarchical structures hindered communication with doctors, resulting in a lack of parental understanding of the necessity to continue treatment. Most poor patients could not afford treatment. Access to donated chemotherapy also was inadequate. Treatment refusal or abandonment frequently resulted. There was no follow-up system to detect and contact dropouts. Health care providers were not fully aware that their own attitude and communication skills were important for ensuring compliance of patients and parents. CONCLUSIONS: Children's survival of acute lymphoblastic leukemia in developing countries could improve if problems that are associated with parental financial and educational background and medical teams' attitudes to treatment and follow-up could be addressed better.

Dental cast study of adult patients with untreated unilateral cleft lip or cleft lip and palate in indonesia compared with surgically treated patients in The Netherlands.

To determine differences in maxillary and dentoalveolar relationships between untreated and treated patients having unilateral clefts of the lip and alveolus (UCLA) or lip and palate (UCLP), dental cast assessments were done on 70 untreated adult Indonesian patients (UCLA-I, UCLP-I) and 67 Dutch patients, surgically treated in infancy (UCLA-D, UCLP-D). The Indonesian group consisted of 44 UCLA-I and 26 UCLP-I patients, and the Dutch group of 24 UCLA-D and 43 UCLP-D patients. In the UCLA-I patients, deformities occurred in that part of the dentoalveolar complex that surrounds the cleft. Lip repair in the UCLA-D group more frequently caused deformities in the incisor and buccal areas on the cleft side. In the UCLP-I patients, deformities were present in the incisor and cuspid areas on the cleft side. The buccal segments showed collapse both on the cleft and noncleft sides. Lip and palate repair in the UCLP-D group caused significantly more deformities in the incisor, cuspid, and buccal areas up to the level of the first molars, both on the cleft and noncleft sides. Surgical treatment seems to cause maxillary and dentoalveolar deformities up to the first molars more frequently, but these are not as pronounced as one would expect: following the practiced surgical regimen, the deformities were usually mild. Negative effects of surgical intervention seem to be antagonized by the restored integrity of the lip and palate leading to orientation of maxillary parts and correction of tongue position, which in turn has a molding effect on the maxilla and mandible.

Diabetes mellitus in an urban population in Jakarta, Indonesia.

The prevalence of diabetes in an urban population in Koja Utara subdistrict of Jakarta is 1.63%. The prevalence of diabetes increases with age. The tendency toward bimodality in the 55-64 age group of our study lends support to the usefulness of the WHO criteria 1980. There is no difference between the sexes. Male to female ratio was 1.2:1. There is a higher prevalence of diabetes among the obese, and the obese and overweight comprise 65.9% of diabetics. Obesity seems to be an important risk factor in the development of diabetes. There was a higher prevalence of diabetes in the high socio-economic level group than in the low one. The group of Chinese origin showed a higher prevalence of diabetes than the other groups. A positive family history of diabetes was found in 27% of our diabetic patients. Dietary recall analysis revealed no significant difference in the percentage of carbohydrate, protein, fat, or in the percentage of refined carbohydrate. However, total caloric intake was significantly higher in the high socio-economic-diabetic group. Total caloric intake seemed to be more important than quantitative dietary factors.

Two Plasmodium species of the crocodile skink Tribolonotus gracilis from Irian Jaya, Indonesia.

Two undescribed Plasmodium species were present in 1 of 8 New Guinea skinks, Tribolonotus gracilis. Plasmodium tribolonoti n. sp. is characterized by rounded or oblong schizonts 6.1 x 5.3 microm (5-7 x 4-7) that produce on average 14.3 merozoites (10-21), in no particular arrangement of nuclei. All parasitized proerythrocytes and had no effect upon dimensions of host cells or their nuclei. Gametocytes, mostly erythrocytic, averaged 7.2 x 6.3 (6.5-9.0 x 5.5-7.5), with length x width (LW) 45.5 microm2 (38-63) and L/W 1.15 (1.0-1.5). Gametocytes are not sexually dimorphic in size or shape and had no effect upon dimensions of host erythrocytes or their nuclei. Schizonts of P. gracilis n. sp. averaged 4.3 x 3.5 (3-6 x 3-5), with 4.9 merozoites (3-8) usually arranged as a fan, and had no effect upon dimensions of host erythrocytes or their nuclei. Gametocytes averaged 5.9 x 5.5 (5.0-6.6 x 5.0-6.0), with LW 31.9 microm2 (25-40) and L/W 1.06 (1.0-1.2). Gametocytes are not sexually dimorphic in size or shape and had no effect upon dimensions of host erythrocytes or their nuclei.

Evaluation of gemeprost vaginal pessaries in the termination of abnormal intra-uterine pregnancies at twelve teaching hospitals in Indonesia.

Vaginal pessaries, up to a maximum of 5, each containing 1 mg of 16,16-dimethyl-trans-delta squared-prostaglandin E1, ester (Gemeprost) were inserted as high as possible into the posterior fornix of the vagina at 3 hourly intervals in 459 women with abnormal intrauterine pregnancies presenting for termination of the pregnancy. There were 145 missed abortions (31.6%), 207 intrauterine fetal deaths (45.1%), 86 molar pregnancies (18.7%), 5 anencephalics (1.1%), and 16 others (3.5%). The study was conducted from July 1981-March 1982. 402 women (87.6%) underwent abortion within the 30 hour observation period, including 139 women (30.3%) who required additional procedures to complete evacuation of the uterus. Expulsion of the uterine contents failed to occur within 30 hours in 57 women (12.4%) and alternative procedures were employed. These failures consisted of 37 missed abortions, 17 intrauterine fetal deaths, 2 molar pregnancies, and 1 pregnancy with an IUD in situ. Side effects caused by the drug were basically tolerable. 234 women (51.0%) had 1 or more side effects, gastrointestinal tract disturbances (nausea, vomiting, and diarrhea) being the most frequently observed, and moderate fever the next most frequent. There was 1 ruptured uterus.

Relationships between soil-transmitted helminthiases and growth in urban slum schoolchildren in Ujung Pandang, Indonesia.

This study examined the relationship between intestinal helminthiases and growth in urban slum schoolchildren. Children (n = 330) who received single doses of either pyrantel, albendazole, or placebo at baseline and again at 6 months were followed and examined fully at 0, 3, 6, and 12 months for helminth infections and anthropometry. Results of treatments indicated that all groups reduced their prevalence and intensity toward the period of the study. Reductions in intensity of both infections in the treatment groups were observed large in 3 and 12 months, whereas at 6 months reached mostly the initial level of infection. There was no significant difference in growth changes between the dewormed (once and twice) and the placebo groups. However, after controlling for some potential confounding factors, there was a significant relationship between reduction of A. lumbricoides infection and height gain at 3 and 12 months (both P < 0.05). In addition, there was a significant association between reduction in T. trichiura infection and increased midarm circumference at 3 and 12 months (P < 0.002 and P < 0.08, respectively). We conclude that treatment of helminth infections in school-age children may improve growth in areas where malnutrition and helminth infections are prevalent.

Effect of air pollution on respiratory health in Indonesia and its economic cost.

A total of 16,663 pairs of junior high school students and their mothers in Indonesian cities were surveyed, using a self-administered questionnaire, to measure the effect of air pollution on respiratory health and the cost of associated illness. Multiple regression analysis showed that the prevalence rates of the symptoms of cough, phlegm, persistent cough, wheezing without a cold, and asthma, in the student and mother groups, were significantly correlated with the nitrogen dioxide (NO2) emitted along large roads near their residences, and to a lesser extent with smoking. In Central Jakarta and Tangerang, where the average NO2 concentrations were highest (37 ppb and 31 ppb, respectively), reduction of NO2 to a proposed level of 25 ppb could yield savings in mean direct out-of-pocket expense per capita for treatment of the above symptom(s) of 15,639-18,165 Indonesian rupiah (6.80-7.90 US dollars), and reduce average work/school days lost per capita by 3.1-5.5 days.
