[Partial trisomy of chromosome 15 with new phenotypic manifestations].

A patient with a 15 partial trisomy and a 4 target chromosome in 100% of metaphases is presented. Phenotypic manifestations not previously described were observed such as macrocephally, long face, low implantation of ears, narrow forehead, epicanthal fold, copious eyebrows and synophrys, short nasolabial distance, convergent strabismus, delayed bucal eruption, long neck, hypertrophy of thenar and hypothenar bulging and articular hypermobility. The eyeground was degeneratively myopic. This case makes more extensive the variety of clinical manifestations of this disease.

[Biological function of morphologic peculiarities of the dromedary].

The dromedary is very well adapted to arid areas. It is able to survive for many days without drinking; the loss of water is negligible. Morphological characteristics hitherto interpreted as adaptations to dry areas now seem to have other reasons. In the natural habitat of the dromedary mostly thorny plants occur. Long eyelashes, eyebrows, lockable nostrils, caudal orifice of the prepuce and a relatively small vulva protect the animals against injury, especially during feeding.

[Clinico-radiometric studies of the eye in radioactive contamination].

The aim of the study was assessment of local and remote effects of radioactive substances on ocular media and development of measures to remove radionuclides from the eye. Radioactive substances getting into the eye in the form of aerosols, dust, or gaseous mixtures are unevenly distributed in it. The highest radioactivity was found on the skin of the eyelids and orbit, particularly so on the eyelashes and eyebrows, the least one on the cornea and in the conjunctival cavity. A follow-up of 8 to 17 years showed that timely removal of radionuclides from ocular surface prevented the development of specific symptoms and trophic changes of ocular media, of radiation cataracts among other things. The suggested method for washing the skin of the eyelids and orbital area from radioactive contaminants did not induce skin irritation, maceration, sense of discomfort, and is recommended for practice.

Objective measurement of normal facial movement with video microscaling.

This study examines variability in facial movements in normal subjects using a new objective method of measuring facial movement, video microscaling. Video microscaling superimposes a computer-generated measuring scale over a video recorded image of facial movements. Distances moved are determined digitally. Eleven subjects were tested raising the eyebrows and smiling five times on two separate days. Test-retest, day-to-day, side-to-side, and intersubject variability were evaluated. Average variability was relatively low; however, some normal subjects exhibited considerable variability, particularly from day-to-day and from side-to-side. Because of this, a facial nerve grading system based solely on objective measurements of facial movements may be invalid. Video microscaling is a promising research technique.

The "pluck and sew" technique of individual hair follicle placement.

A novel and simple approach to single hair follicle transplantation for alopecia is a hair threaded on a curved needle with the follicle attached. It is the best aesthetically acceptable procedure in certain areas, namely the eyebrow, the sideburn, the hair margin, areas requiring precise grafting following face lift procedures, and the eyelash. The presented technique simplifies the precise, oriented placement of single hair grafts and has been time-tested by the senior author for the last 20 years.

Clinical and histopathological features of secondary syphilis.

Fifty-four patients with secondary syphilis were studied in regard to the clinical manifestations, response to treatment and histopathology of the skin lesions. The correlation between the skin lesions and histopathology, between the duration of skin lesions and VDRL titer, between type of skin lesions and VDRL titer were also determined. The clinical manifestations varied from macular, maculopapular, papular, papulosquamous and urticarial lesions. The hair loss usually occurred on the scalp, but the eyebrows or even total body hair loss could occur. The response to treatment was good, only one patient relapsed. The histopathology was related to the clinical manifestations, there was sparse inflammatory cell infiltration in the dermis in macular lesions, but more dense infiltration as well as more epidermal change in papular and papulosquamous lesions. There were significant correlation between the duration of skin lesions and VDRL titer, but no correlation between types of skin lesions and VDRL titer.

Kabuki make-up syndrome and hearing impairment.

In 1981, Niikawa et al. and Kuroki et al. independently described patients with a new syndrome consisting of mental retardation, postnatal growth deficiency and unusual facial features such as long palpebral fissures with eversion of the lateral one-third of the lower eyelids, arched and laterally sparse eyebrows and large prominent ears among other malformations. The condition has been called Kabuki make-up syndrome because the facial features in affected individuals resemble the make-up of the actors in a Japanese play: Kabuki. After the initial reports on Japanese individuals, the condition has been observed in several other patients of different ethnic origins including a few patients from this country (Kaiser-Kupfer et al., 1986; Pagon et al., 1986). We describe an additional 13-year-old male patient with Kabuki make-up syndrome with possible implication of autosomal dominant inheritance from his mother. Interestingly, our patient also displayed hitherto unreported severe ossicular malformations resulting in significant hearing impairment.

[Trichorhinophalangeal syndrome].

A girl with tricho-rhino-phalangeal syndrome is described. Besides clinical and radiologic symptoms of type I of this syndrome (short stature, pear-shaped nose, prominent and elongated philtrum, small carious teeth, thin and sparse hair, badly shaped laterally protruding ears, deep voice, cone-shaped phalangeal epiphyses as well as shortening of metacarpal and metatarsal bones) the patient exhibits a group of abnormalities characteristic for type II (broad nasal bridge, thicker septum, laterally postured nostrils, heavier eyebrows, mild deafness and epilepsy). The girl also has the symptoms of primary hypothyroidism because of the ectopic thyroid gland, the first recorded instance up to now, so it is probably a coincidence.

[Relapse of multiple myeloma in extramedullary sites].

Multiple myeloma was diagnosed in a 69-year-old man with pathological hip fracture, who had increased plasma cells in the bone marrow, osteolytic lesions in bones, low level of monoclonal protein (IgAK) in serum and urine, and anemia, hypercalcemia and renal insufficiency. He was treated for 1 year with chemotherapy with good results. 6 months after cessation of treatment the disease relapsed with multiple extramedullary plasmacytomas in skin and subcutaneous areas, right eyebrow, right knee, sternum and right axilla, but repeated bone marrow examinations were without evidence of disease activity. Chemotherapy was resumed but the patient died 1.5 years after the relapse with severe jaundice due to multiple liver plasmacytomas and pelvic masses.

[A case report of acute death caused by hyperextension injury of neck].

We report an autopsy case of acute death from an upper cervical spinal cord injury caused by hyperextension of the neck. A 59-year-old drunken man who had been suffering from cervical spondylosis was slapped in the face, fell flat on his face, and remained as such for 4 hours. The victim was carried to a hospital, he had already been dead on arrival. Although he received an intensive care, he remained unconscious for 12 hours until a cardiac arrest. The findings of an autopsy were as follows: Externally, excoriation on the forehead and the left eyebrow and slight subcutaneous hemorrhage in the dorsum of the nose were observed. The intervertebral disk and the anterior longitudinal ligament between the 5th and 6th cervical vertebrae were severed, and a lot of bleeding in the region of the injuries was observed. The 6th cervical spinous process was fractured in the direction to a major axis. A part of the posterior root filaments was torned and the bleeding points could be seen on the front of the 6th cervical spinal cord. In the cross section of the 6th cervical spinal cord, the typical hemorrhagic necrosis could be recognized surrounding the central canal, whereas the similar finding was observed at the second cervical spinal cord. On the histopathological examinations, in C2, C5-C6 regions marked ischemic changes and hemorrhagic necrosis could be observed. From these post-mortem results, we diagnosed that the victim died from an upper spinal cord damage due to a stretching consequence of the hyperextentional injury.

The East Asian eyelid-open technique.

Creation of a dynamic levator aponeurosis fold or double eye adds to the beauty of the East Asian. The eyes are larger and less puffy, and the eyelashes appear longer and curve up and out instead of down. The distance from the ciliary border to the eyebrow is decreased and more in proportion. The Mongolian trait is preserved.

Assessment of the preferred vertical position of the ear.

To determine the most desirable ear level (vertical position), a set of four drawings was assessed by 40 observers. Two levels were almost equally preferred when viewed in profile, one with the upper edge of the ear at the level of the most lateral point of the eyebrow and the other with the upper edge at the height of the upper eyelid. Various factors affecting the visual impression of auricular level and their clinical significance are discussed.

[1984-1994: Ten years of skin flaps. Prefabricated flaps].

The term prefabrication of flaps corresponds to completely different technical modalities, often complementary and associated, which share the common feature of extending the indications and potential of pedicle or free flaps. Current methods of prefabrication of flaps can be considered to be based on one or several basic principles of reconstructive surgery. The authors discuss and analyse various modalities: a free flap can be made autonomous or can be expanded prior to transfer. A transfer can be initially prefabricated at its donor site by performing several operative steps in situ. A free transfer can be performed on the main vascular axis of a flap which is subsequently transferred, for example: free scalp transfer to reconstruct an eyebrow on a donor site reconstructing half of the face, but the first in situ operative phase usually consists of performing cutaneous, bone, cartilaginous or even alloplastic material transplants. The transfer is only performed after a sufficiently satisfactory complex anatomical, functional or esthetic unit has been achieved, in order to avoid difficult or dangerous remodeling operations after transfer. The most revolutionary appearance in the area of prefabrications is certainly induction of a pedicle or free neoflap in a so-called random territory. The addition of an arteriovenous pedicle associated with fascia underneath a muscle, underneath a skin cover, in contact with an osteoperiosteal segment allows a perfectly transferable neovascularized unit to be obtained after several weeks. All of the procedures described: autonomization before transfer, expansion before transfer, in situ prefabrication of a complex unit by addition of several tissue units (autologous or alloplastic) in one or several stages and finally vascular induction of a muscular, cutaneous or bone unit, etc. by transfer of a vascular axis in contact with this anatomical structure and combinations of these various modalities obviously open up a new approach which will considerably extend the already remarkable possibilities offered by free tissue transfers.

Keratosis follicularis spinulosa decalvans: report of a new pedigree.

Keratosis follicularis spinulosa decalvans is a rare, X-linked genodermatosis characterized by follicular hyperkeratosis, scarring alopecia of the scalp, eyebrows and eyelashes, corneal dystrophy and photophobia. We describe two cases from a large family, the first with keratosis follicularis spinulosa decalvans to be reported in the U.K.

Focal facial dermal dysplasia: report of a case with associated cardiac defects.

Focal facial dermal dysplasia is characterized by congenital scar-like depressions on both temples and a wide spectrum of associated facial abnormalities. We report a 2-month-old white boy who presented with bitemporal defects, laterally deficient eyebrows, absent lower eyelashes, periorbital puffiness, a bulbous nasal tip with prominent sebaceous hyperplasia, and down-turned corners of the mouth. In addition, he had tetralogy of Fallot, an association which previously has not been described.

Head for business.

No matter how you look at it, "the business of nursing remains patient care," writes Marjorie Beyers in her preface to The Business of Nursing, a recent book from American Hospital Publishing. Still, in the book with potential authors and reviewers, the idea of putting the word "business" in front of "nursing" raised a few eyebrows, says Beyers, executive director of the American Association of Nurse Executives. "In corporate thinking, any service of value has a price. Although certainly nurse executives agree that nursing care is a valued human science, there is lively debate about whether it should have a price." To that end, the book's six essays reflect the complexity of changes in health care, the effects on patients and their families, and the challenges facing nursing executives today. The chapter excerpted here--"Seizing Opportunities in the New Health Care Delivery System, " by consultant and former nursing executive Richard Brock--lays a foundation for changing both nursing practice and attitudes. Brock also offers advice for working with staff nurses to guide them through the changes ahead.

Microcephaly, macrotia, unusual mimics and mental retardation syndrome: new syndrome or variant of De Lange type 2 syndrome.

We report a boy who shows a severe microcephaly, with mild mental retardation and hypotonia, and a dysmorphic facies: (flat profile, arched eyebrows, mild ptosis, short nose with raised basis, large tip and anteverted nares, long, smooth philtrum, narrow mouth with down turned corners, very large, backward tilted ears, with a prominent lobule, retrognathism and very small and widely spaced, although normally shaped teeth. Vesicoureteral reflux was present. The mother showed similar aspect, large ears, and a grinning smile. This appear to represent an undescribed phenotype which share some resemblance to mild Cornelia de Lange and Kabuki syndromes.

[Endoscopic forehead lift].

Indications for the endoscopic frontal lift are the same as for the conventional open procedure: (a) ptosis of the eyebrow; (b) laxation of the frontal skin; (c) frowning of the glabella in the frontal area. While the conventional open procedure requires a coronal incision from ear to ear, the endoscopic procedure requires only four to six 1-cm incisions in the hair area. From there we lift the frontal skin subperiosteally up to the orbital rim where we incise the periosteum. Here the corrugator and procerus muscle are incised and the necessary portion of frontal skin can be elevated. Technical details, as well as the instruments needed are described in over 30 procedures.
